Organization of the Field

There is no simple or complete way to organize the field of health communication, though several sub-fields have existed depending on one’s research interests, as well as adventitious and historical circumstances. At the individual level, the focus is twofold: (1) how health cognitions affect, and behaviors influence and are influenced by, health communications; and (2) how interpersonal interactions between patients, family members, and providers, and with members of their social network, influence health outcomes. At the organizational level, some have studied the role of communication within health-care systems and how organization of the media and the practices of media professionals may influence population and individual health.

Finally, at the societal level, the focus is on large-scale social changes and the role of communication with such changes. For example, one might examine how strategic communications as well as natural diffusion of information impact individual and population health; or how communication mediates and is influenced by social determinants such as social class, neighborhood, social cohesion and conflict, social and economic policies, and how that impacts individual and population health.

Even as these levels provide a useful organizing framework, two caveats are warranted. First, policymaking and research related to health may affect more than one level. Second, interest in a level of analysis and pursuit of work at one level is not inconsequential. Locating a problem at one level, and studying it at that level, have implications for the kind of policy or practice that is likely to emerge from that research.

Interpersonal Communication

Extensive attention has been given to understanding the consequences of communication between physicians and patients on patient satisfaction, adherence, and quality of life. One theme is who controls the interaction between providers and patients, known as ‘relational control.’ A second theme focuses on the outcomes of patient– provider interactions. Extensive research has documented that patient–provider communication influences patient satisfaction which, in turn, is related to patient adherence and compliance to treatment regimens, ease of distress, physiological response, length of stay in the hospital, quality of life, and health status, among others. Third, researchers have documented stark differences in patient preparation and access, and in care received and health outcomes, between social classes as well as racial and ethnic groups.

The implications of interpersonal interaction in the context of families, friends, co-workers, and voluntary associations on health outcomes have become one of the most dynamic areas of research in health communication. This topic has been pursued from diverse theoretical viewpoints by researchers focusing on social networks, social support, family communications, and social capital based on the researcher’s disciplinary origins and research interests. In addition to social support, social networks can accelerate or decelerate diffusion of new information, and also influence how it is interpreted (Himelboim & Han 2014). Members within networks can serve as role models for lifestyle behaviors such as smoking and obesity. The emergence and spread of the Internet have broadened the scope of interpersonal interaction and its influence in health communication by moderating the limits of geography.

Mass Media and Health

The incidental and routine use of media for news and entertainment serves four functions in health. (1) The informational function is served when casual use of media for news or other purposes may expose the audience to developments on new treatments or new drugs, alert them to risk factors, or warn them of impending threats such as avian flu; (2) media serve an instrumental function by providing information that facilitates action; e.g., in times of natural disasters the audience may learn about places where they should take shelter, and information of this kind allows for practical action; (3) media defines what is acceptable and legitimate, performing a social control function; (4) the communal function is served when media provide social support, generate social capital, and connect people to social institutions and groups.

Information seeking, as a construct, has gained greater currency in recent times as more information on health has become routinely available because of greater coverage of health in the media, the spread of health-related content on the world wide web, or the consumerist movement in health that promotes informed or shared decision-making. It is widely assumed that under certain conditions some people actively look for health information to seek a second opinion, make a more informed choice on treatments, and learn in greater depth about a health problem that afflicts them or their friends or family members.

The most visible and popular means of strategic communications is through health campaigns which have become a critical arsenal in health promotion. A typical health campaign attempts to promote change by increasing the amount of information on the health topic, and by defining the issue of interest in such a way as to promote health or prevent disease. Recent reviews of the vast literature on health campaigns have identified conditions under which health campaigns can be successful (e.g. Noar 2006; Randolph & Viswanath 2004).

Emerging Challenges/Dimensions

First, the combined impact of computers and telecommunications on society has been transformative, impinging on almost every facet of human life including art, culture, science, and education. Consumer informatics integrates consumer information needs and preferences with clinical systems to empower patients to take charge of their healthcare, bring down costs, and improve quality of care. For example, the integration of electronic medical records with communications should facilitate communications between patients and providers, send automatic reminders to patients to stay on schedule, and help patients navigate the health-care system. Second, technological developments are coinciding with the consumerist movement in health-care. The paternalistic model that characterized the physician–patient relationship is slowly being complemented by alternative models such as shared/informed decisionmaking models (SDM/IDM) or patient-centered communication (PCC). Third, the significant investments in biomedical research enterprise in the developing world, and movement toward more evidence- based medicine, have led to calls for translation of the knowledge from the laboratory to the clinic and the community. Lastly, an urgent and a moral imperative in health is addressing the profound inequities in access to health-care and the disproportionate burden of disease faced by certain groups.

Have you ever felt reluctant to share ideas during a meeting because you feared judgment from senior colleagues? You’re not alone. Research has shown this pervasive issue can lead to a lack of diversity in public discourse, especially when junior members of a community don’t speak up because they feel intimidated.

Anonymous communication can alleviate that fear and empower individuals to speak their minds, but anonymity also eliminates important social context and can quickly skew too far in the other direction, leading to toxic or hateful speech.

MIT researchers addressed these issues by designing a framework for identity disclosure in public conversations that falls somewhere in the middle, using a concept called “meronymity.”

Meronymity (from the Greek words for “partial” and “name”) allows people in a public discussion space to selectively reveal only relevant, verified aspects of their identity.

The researchers implemented meronymity in a communication system they built called LiTweeture, which is aimed at helping junior scholars use social media to ask research questions.

In LiTweeture, users can reveal a few professional facts, such as their academic affiliation or expertise in a certain field, which lends credibility to their questions or answers while shielding their exact identity.

Users have the flexibility to choose what they reveal about themselves each time they compose a social media post. They can also leverage existing relationships for endorsements that help queries reach experts they otherwise might be reluctant to contact.

During a monthlong study, junior academics who tested LiTweeture said meronymous communication made them feel more comfortable asking questions and encouraged them to engage with senior scholars on social media.

And while this study focused on academia, meronymous communication could be applied to any community or discussion space, says electrical engineering and computer science graduate student Nouran Soliman.

“With meronymity, we wanted to strike a balance between credibility and social inhibition. How can we make people feel more comfortable contributing and leveraging this rich community while still having some accountability?” says Soliman, lead author of a paper on meronymity .

Soliman wrote the paper with her advisor and senior author David Karger, professor in the Department of Electrical Engineering and Computer Science and a member of the Computer Science and Artificial Intelligence Laboratory (CSAIL), as well as others at the Semantic Scholar Team at Allen Institute for AI, the University of Washington, and Carnegie Mellon University. The research will be presented at the ACM Conference on Human Factors in Computing Systems.

Breaking down social barriers

The researchers began by conducting an initial study with 20 scholars to better understand the motivations and social barriers they face when engaging online with other academics.

They found that, while academics find X (formerly called Twitter) and Mastodon to be key resources when seeking help with research, they were often reluctant to ask for, discuss, or share recommendations.

Many respondents worried asking for help would make them appear to be unknowledgeable about a certain subject or feared public embarrassment if their posts were ignored.

The researchers developed LiTweeture to enable scholars to selectively present relevant facets of their identity when using social media to ask for research help.

But such identity markers, or “meronyms,” only give someone credibility if they are verified. So the researchers connected LiTweeture to Semantic Scholar, a web service which creates verified academic profiles for scholars detailing their education, affiliations, and publication history.

LiTweeture uses someone’s Semantic Scholar profile to automatically generate a set of meronyms they can choose to include with each social media post they compose. A meronym might be something like, “third-year graduate student at a research institution who has five publications at computer science conferences.”

A user writes a query and chooses the meronyms to appear with this specific post. LiTweeture then posts the query and meryonyms to X and Mastodon.

The user can also identify desired responders — perhaps certain researchers with relevant expertise — who will receive the query through a direct social media message or email. Users can personalize their meronyms for these experts, perhaps mentioning common colleagues or similar research projects.

Sharing social capital

They can also leverage connections by sharing their full identity with individuals who serve as public endorsers, such as an academic advisor or lab mate. Endorsements can encourage experts to respond to the asker’s query.

“The endorsement lets a senior figure donate some of their social capital to people who don’t have as much of it,” Karger says.

In addition, users can recruit close colleagues and peers to be helpers who are willing to repost their query so it reaches a wider audience.

Responders can answer queries using meronyms, which encourages potentially shy academics to offer their expertise, Soliman says.

The researchers tested LiTweeture during a field study with 13 junior academics who were tasked with writing and responding to queries. Participants said meronymous interactions gave them confidence when asking for help and provided high-quality recommendations.

Participants also used meronyms to seek a certain kind of answer. For instance, a user might disclose their publication history to signal that they are not seeking the most basic recommendations. When responding, individuals used identity signals to reflect their level of confidence in a recommendation, for example by disclosing their expertise.

“That implicit signaling was really interesting to see. I was also very excited to see that people wanted to connect with others based on their identity signals. This sense of relation also motivated some responders to make more effort when answering questions,” Soliman says.

Now that they have built a framework around academia, the researchers want to apply meronymity to other online communities and general social media conversations, especially those around issues where there is a lot of conflict, like politics. But to do that, they will need to find an effective, scalable way for people to present verified aspects of their identities.

“I think this is a tool that could be very helpful in many communities. But we have to figure out how to thread the needle on social inhibition. How can we create an environment where everyone feels safe speaking up, but also preserve enough accountability to discourage bad behavior? says Karger.

“Meronymity is not just a concept; it's a novel technique that subtly blends aspects of identity and anonymity, creating a platform where credibility and privacy coexist. It changes digital communications by allowing safe engagement without full exposure, addressing the traditional anonymity-accountability trade-off. Its impact reaches beyond academia, fostering inclusivity and trust in digital interactions,” says Saiph Savage, assistant professor and director of the Civic A.I. Lab in the Khoury College of Computer Science at Northeastern University, and who was not involved with this work.

A research team has delved into the Target of Rapamycin (TOR) signaling pathway in plant grafting, emphasizing its role in regulating seedling vigor, graft junction healing, and shoot-to-root communications. Exploring TOR's involvement offers a promising avenue for improving grafting techniques and understanding plant communication, with the potential to significantly impact agricultural productivity and sustainability.

Grafting, an ancient horticultural practice crucial for plant breeding and cultivation, merges the traits of different species or cultivars to overcome various agricultural challenges. The technique's success depends on factors like seedling strength and effective graft healing.

Recent research highlights the significant role of the TOR signaling pathway in plant growth and development, including its influence on grafting outcomes. Recent studies have also shown that the TOR signaling pathway integrates growth, nutrient and stress response , and has the potential to improve grafting by regulating pre-grafting growth, promoting graft junction healing and enhancing shoot-root communication in grafted plants. In-depth studies on the role of TOR signaling in grafting may provide new avenues for crop improvement and agricultural innovation.

A study published in Vegetable Research elucidates the contributions of TOR signaling to grafting processes.

The research team explores the TOR signaling pathway in grafting, focusing on focusing on its role in the regulation of seedling vigor prior to grafting, healing of graft junction and shoot-to-root communication.

In greenhouses, factors like insufficient light and inadequate nutrient supply can lead to weakened seedlings, adversely affecting graft success. The TOR pathway, sensitive to light and other environmental signals , plays a central role in plant development by regulating growth in response to these signals. Specifically, the pathway is involved in skotomorphogenesis and photomorphogenesis through light-mediated regulatory mechanisms and regulates key processes such as hypocotyl elongation.

The research highlights the dynamic interactions between TOR signaling and environmental factors , including temperature and light, suggesting that these interactions significantly influence seedling vigor and grafting efficacy.

In addition, the study highlighted the role of gibberellins and m5C modifications in the TOR regulatory network. However, further research is needed to reveal the exact molecular mechanisms of TOR involvement in grafting and to optimize its application to improve grafting outcomes.

According to the study's senior researcher, Prof. Yihan Dong, "This perspective paper seeks to illuminate the latent role of TOR in the grafting process, with the overarching goal of directing heightened research focus towards the TOR signaling pathway within the realm of vegetable crops."

The research also underscores the need for further investigation into TOR's mechanisms in regulating pre-grafting growth and its potential applications in improving grafting outcomes and overall crop improvement strategies, pointing towards a systematic approach to leveraging environmental conditions to optimize seedling strength for successful grafting.
