How to start a juice business from home.

What do successful juice brands like Suja, Sol-ti, JRINK, and Naked all have in common? They all started in someone’s home kitchen!

If you don’t feel ready to take the risk of spending hundreds of thousands of dollars and signing a commercial lease, starting a juice business from home is a great way to start and can get you to a point where you feel comfortable making the investment for a real store front or commercial kitchen.

Two reasons why starting from home can be a good idea:

  • Financial security: Get a good sense (ie MVP = minimum viable product) of whether your business is viable without having to invest or raise money to build out an entire kitchen.
  • Emotional certainty: Starting a business is never easy. By starting at home you’ll build some confidence that your product has traction before going “all in”.

And some general advice she would give to someone going through the process:

“Google it: Don’t blatantly copy anyone’s recipe, but get a head start by googling juice recipes so you have an idea where to start. Sometimes just doing it (especially at home!) is half the battle of starting. So much information is available nowadays – don’t waste your time on things that are already out there and focus on how you’ll innovate differently / differentiate from your competitors. Chances are it won’t be your recipe that will be your differentiator – more so sales strategy, brand, speed to market, etc. What makes you unique. “

Some advice to people starting from home:

“Life certainly is full of surprises – if you would have told me that in just a few short years our brand would go from operating out of my parents basement and selling at local farmers markets to now having a 6,000 sq ft. fully automated production facility with the capacity to produce up to 65,000 bottles of fresh juice per day I would have said you’re absolutely crazy. But the wild thing is if you’re willing to put in the work, face your fears, and chase your dreams anything is possible. So start now. Start today. Whether it’s in your kitchen or in your parents basement – we all have to start somewhere!”

Before we get into the step-by-step process, it’s worth pointing out that it may not be totally legal under FDA regulations to run a raw juice business from home. Many people have done it successfully and continue to do so, but just be aware that the FDA may tell you to stop and make you move to a real commercial kitchen at some point.

There are “cottage laws” in some areas that may allow you to run a fully-approved commercial juice business from home, but you will have to do your research to find out for sure what the regulations are in your area.

Now that that’s out of the way, on to the fun stuff!

Before you start selling juice, make sure you have a handful of recipes that are tried and enjoyed by your closest friends and family. Ari Sexner, juice business consultant recommends starting with a small menu of 5-8 core recipes.

This gives you enough to offer a full juice cleanse package, but not so much that your menu is hard to manage. Starting with too many options is burdensome and is difficult to manage. Ari also recommends having two wellness shot recipes. They are a great add-on and very popular.

Pro Tip: In nearly every cold-pressed juice business, their best selling recipe is a sweet green juice! Usually a recipe with apple juice as a base.

Cost of Goods Calculations

You will need to make sure your recipes are not only delicious, but also make you money. That means keeping the food cost down to about 25% of the final price. Other costs (bottle, cap, label) can add another 5% – 15%, bringing your total COGS to 40%.

In the above example, the Cost of Goods is about 30%. A business can make a profit at this margin. Even if you can justify a COGS of 50% – 60% running a business from home, you will  not be able to run a profitable business this way if you ever move into a commercial kitchen and hire staff. A juice priced at $10 – $12 is fairly common if it’s organic in a glass bottle, which allows a COGS of about $3-$4 at a comfortable margin.

Commercial Juicer

Equipment will be the biggest investment you will make for your business. Most importantly, you need to invest in a commercial juicer. If you choose to purchase a juicer that is not rated for commercial (continuous) use, you will run the risk of  having to spend the same or even more money with multiple machines, replacement parts and labor costs, not to mention the hassle of downtime, being frustrated and having to convert your recipes and processes once you do make the upgrade to a commercial machine. When evaluating a juicer make sure it checks all of these boxes:

  • It’s a genuine juice press. Juice made in an actual press tastes better, lasts longer, and will give you higher yield than other juice technologies. To be clear – some equipment brands that claim to be a “cold-press” aren’t actually a press. All the following are not real juice presses: Masticating juicers, “Slow” juicers, twin-gear / double auger juicers, centrifugal juicers. A real press must have a moving platen that presses the produce between two plates. For more information, check out our article Types of Juicers . 
  • It’s NSF certified. In the US, all food service equipment used to make commercial food and beverage products (including juice) must be NSF certified. The last thing you want is for the health department to tell you you can’t make juice because the equipment isn’t NSF certified.  Pro Tip: If you are evaluating a product and it says “conforms to NSF standards”, that may not mean the machine is actually NSF certified and contains the NSF mark that health inspectors look for. Make sure the equipment is legitimately NSF certified. If in doubt, check the NSF website .
  • It can grow with your business. If you’re investing thousands of dollars into a machine, make sure it can scale with your business. What this means essentially, is that you should make sure the machine is built to withstand continuous use and can last for years. Most juicers over-heat if they are used for a long time. Juicers that overheat or break down after long periods of use are not a good choice.

The most suitable entry-level commercial machine that contains all of the above features is the Goodnature M-1 commercial cold-press juicer. The M-1 is built for juice bars and other real food and beverage businesses, so it will withstand any level of use you can throw at it in your home juice business.

Other Equipment

You will also want to pick up the following equipment:

  • Portion scale. A digital kitchen scale that is used to weigh ingredients for recipes is a necessity. When making juice commercially, it’s not realistic to measure ingredients by whole fruit. For example, a recipe that contains “1 apple” or “3 medium carrots” will yield very different results depending on the size of the produce. So you will want to convert all of your recipes to weights. E.g. “8 oz. red apple” or “200 grams carrot”. We recommend a scale like this one: Digital Portion Scale
  • A set of “cambro” containers . Having a nice set of containers to store juice, produce, and other things is great. You will find yourself using them for many things throughout a juice production cycle. I recommend a few of these with lids: 6″ Deep Cambro Pans . And some of these: 2 qt Round Storage Containers 
  • A good knife! With a nice, sharp knife you can easily chop produce, peel citrus, and work efficiently. If you have a bunch of old, dull knives at home, now is the time to get one nice chef knife. A good 6 inch – 8 inch knife will do the trick.
  • Cutting boards and other small-wares. Get some large spoons, spatulas, a strainer, and some measuring cups.

Taking Orders

If you feel you’re ready to start taking orders, you will actually need a way to do that. The best way is through an online store. These days, there are plenty of options for launching an online store. Shopify is the most used and probably has the smallest lift to get going. In order to make a good impression you will need some nice content. Spend a few hundred dollars and pay a professional photographer / food stylist to photograph your juices for you. It is well worth it and will generate sales for you. The last thing you want is for a customer to get all the way to your website then think “this doesn’t look very professional, I’m not going to buy juice from them.”

Production Schedule

Most home juice businesses don’t generally stock juice. They take orders ahead of time and make the juice available the next day, or at a regular schedule. For example, any orders taken Mon – Wed will be available or delivered on Thursday, and any orders placed Thur – Sun will be available or delivered on Monday.

This method is generally acceptable if you’re using a Goodnature juice press, since the juice usually lasts 3-5 days if following our shelf life guidelines for cold pressed juice . So, customers who receive the juice on Monday can consume it Mon – Wed, then get their next batch on Thursday.

Producing on a schedule like this allows you to plan your production ahead of time without needing to estimate how much produce you have to buy and how much juice you have to make. It will reduce waste and decrease your production costs.

Bottles and Packaging

Glass vs plastic bottles are an ongoing debate, and most people agree that glass bottles are preferred, both for environmental reasons and for aesthetics – glass bottles definitely feel more premium and some even insist that juice tastes better out of a glass bottle (it does keep it colder which might be the actual reason for this). As for size, most companies have moved from 16oz bottles to 12 oz. The lower price point is more attractive to customers, and most people don’t necessarily want to consume a whole 16 ounces of juice. For more on this, check out my article here: Why Juices are Getting Smaller

Juice shots are also a growing trend, and are a great way for customers to have access to high quality raw juice without spending $8-$12. A 2oz – 4oz juice shot is also a great add-on to a juice sale.

If you plan on offering delivery, make sure to only accept sales in a certain area or you can find yourself quickly becoming a full time driver! In dense urban areas, you can find a lot of customers in a small area so delivery makes a lot of sense. In more rural areas, you might want to stick to pickup only. Or pickup and direct shipping.

Hi, I live in Fort Wort, Texas have been calling around and googling laws for selling juice from home, it seems that the Texas Cottage Food Law does not include fruit/veggies juices. Does that mean there is no way for me to sell juice without investing in a food truck or retail space for the commercial kitchen? I am so frustrated. Hi Jessica, Most likely the health department may not approve you running a juice business from home. This is nothing new - in general, the health department treats juice like it's toxic for some reason. it's very frustrating. You can either find a commissary kitchen that will rent you space, or even find another restaurant or other type of company that will let you share their kitchen when they're not busy. Many start this way. Or... you could start from home anyway, as many have done, even though it's not approved. Usually people can get away with it for long enough to get a little business going, then move into a commercial kitchen later. As a disclaimer - I recommend against doing this, as it may violate health code.

I loved the article. Thank you for the information. I currently buy plastic "wh square pet bottles" 16oz, 8oz and 2oz bottles from US Plastic Corp. They are slightly cheaper than captiva after shipping, however, they are not true to size. They have that gap that you are talking about in the milk bottles. Are the captiva bottles true to size? How can I tell if a bottle is true to size before ordering, is there a specific design I should be looking for or something in the description that tells me. Thank you so much.

I believe they have a bit of extra space in them. I have yet to find any bottles that don't! It's because most commercial beverage brands you find on the shelf in the grocery store have a bit of air gap in them, so for the companies to be able to say "12 ounces" the bottle has to be a bit larger than needed.

People ask me all the time about freezing juice. The truth is there is not much information out there about what happens to the living enzymes once it's frozen. I always try to drink fresh, raw juice. If I can't find raw juice, I would prefer to drink juice that had been frozen as opposed to HPP.

Supermarket beverage aisles are starting to look a lot more like a pharmacy.

There are sodas made with mushrooms that supposedly improve mental clarity and juices packed with bacteria that claim to enhance digestive health. Water infused with collagen carries the promise of better skin, and energy drinks offer to help burn body fat.

Welcome to the frenzy of functional beverages – drinks designed to do more than just taste good or hydrate. What started in the late 1980s with caffeine- and vitamin-laced energy drinks like Red Bull has grown into a multi-billion-dollar industry. Hundreds of brands are vying for consumers’ attention with increasingly exotic ingredients and wellness-focused marketing.

Feeling stressed? Try a drink with ashwagandha, a shrub long used in herbal medicine. Want to enhance your workout? There are drinks containing chromium, a mineral that may boost metabolism . Want to get in a party mood without alcohol? Multiple companies are making non-alcoholic spirits and beers infused with ingredients like guayusa, a leaf containing caffeine and antioxidants.

Consumer intelligence company NielsenIQ counted 53,000 UPC symbols in the U.S. functional beverage category last year, including all of the different flavors of energy drinks, sports drinks, sodas, waters, shakes and teas that are sold on the premise of enhancing mental or physical health.

Nutritionists say the general trend of consumers seeking out healthier beverages is a good one. But experts also say people should be cautious and read ingredient labels, especially if they are pregnant, taking medication or have other health issues. And they should avoid empty calories and sugars that they're not going to burn off. A 16-ounce Monster energy drink has nearly as much sugar as a regular Coke , for example.

“Someone who’s running a marathon has different needs than someone who’s commuting to work,” said Martha Field, an assistant professor in the division of nutritional sciences at Cornell University.

The U.S. Food and Drug Administration regulates ingredients and requires drink labels to be truthful, and the Federal Trade Commission can step in if companies make false claims. In 2013, the FTC determined that Pom Wonderful pomegranate juice was deceptively advertised as clinically proven to treat, prevent or reduce the risk of heart disease and prostate cancer.

But functional beverage makers generally make less specific claims, and the science behind them is sometimes inconclusive. SkinTe, a sparkling tea, says it “supports skin hydration and elasticity” with 3,000 milligrams of collagen in a 12-ounce can. But last year, Harvard Medical School researchers said there’s not yet solid evidence that collagen drinks or supplements enhance skin, hair or nail growth.

Negative reactions can also happen. Panera Bread faces at least two lawsuits claiming its highly caffeinated Charged Lemonade led to the deaths of people with heart conditions.

“It’s important to remember that everything has the potential to be both toxic and safe, depending on the amounts. The dose makes the poison,” said Joe Zagorski, a toxicologist for the Center of Research on Ingredient Safety at Michigan State University. “Since it’s difficult to determine the amount of specific compounds in many of these beverages, it’s better to proceed cautiously than to over-consume.”

In some ways, there’s nothing new about humans seeking added benefits from their drinks. Ancient Egyptians and Romans sipped chamomile tea for its perceived healing effects. In 1935, a Japanese microbiologist introduced Yakult, a fermented milk drink now sold around the world as a way to improve gut health.

In the last half-century, beverages ranging from Ovaltine to wheatgrass juice had their moments as praised nutritional supplements. What's different now is social media, which allows the speedy spread of information about less familiar ingredients, Cornell's Field said.

Two of the latest to generate a buzz are adaptogens, which are plants and mushrooms that have been shown to help the body respond to stress and fatigue, and nootropics, which are natural or synthetic cognitive enhancers like caffeine, gingko and amino acids.

Trends ebb and flow within the category. U.S. sales of prebiotic and probiotic drinks more than tripled last year, while sales in the more mature kombucha category rose just 8%, according to data compiled by consulting firm AlixPartners.

“Consumer savviness on functionality of ingredients has really increased,” said Sherry Frey, vice president of wellness at NielsenIQ.

For some, the health claims in functional drinks are secondary. Amy Cassels, a health and fitness coach from Magnolia, Texas, said functional drinks appeal to her because they typically contain natural ingredients, like fruit juice as a sweetener. She enjoys Poppi, a prebiotic soda, as a once-a-day treat.

“When I drink something like that, I do not believe that I am nurturing my health by drinking that. But I’m not harming my health either,” Cassels said. “It’s a guilt-free indulgence.”

A sizeable share of the beverages-with-benefits market is geared toward people who want to curb or cut their alcohol intake. London-based Three Spirit makes non-alcoholic drinks with more than 60 ingredients, including guayusa and schisandra, an Asian berry, that it says mimic the sense of relaxation and social lubrication that drinkers get from beer, wine and spirits.

“Fundamentally, alcohol is the ultimate functional drink,” Three Spirit Co-Founder Dash Lilley said. “People don’t just drink for flavor. It helps people socialize, helps people unwind, helps people perk up. So we thought we could do that really well by coming at it from a new angle.”

Randy Burt, a managing director at AlixPartners who studies food and beverage, said functional drinks align with a decades-long shift consumers have been making toward healthier diets and habits. He doesn't see demand for the drinks slowing down. Euromonitor, a market research company, expects global sales of functional beverages to grow 7% each year through 2027.

In the U.S., sales of functional beverages jumped 54% to $9.2 billion between March 2020 and March 2024, according to NielsenIQ. That was faster than the 43% growth for the non-alcoholic beverage market overall. Functional beverages now make up about 10% of the total U.S. non-alcoholic beverage market.

Frey said sales slowed a little in the U.S. last year, which she partly attributed to the relatively high cost of functional beverages. Twelve 12-ounce cans of Olipop, a prebiotic soda, sell for $35.99 on Amazon.com; a 12-pack of Dr. Pepper costs $6.48.

“Consumers are making trades, saying ‘Maybe I’ll still purchase this, but I won’t purchase it as frequently,’” she said.

Henry Chen, the founder and CEO of South San Francisco, California-based ALO Drink, a line of beverages made with aloe vera plants, said the increasing number of brands and promised health benefits is overwhelming for consumers. Chen suspects shoppers eventually will tire of purpose-driven drinks in favor of simpler libations.

“There are just too many narrowly specific functional needs that brands are claiming to address, too many esoteric ingredients that you need a science degree to understand being added to foods and beverages,” he said.

Even better might be bypassing the functional beverage aisle altogether, said Corrie Whisner, an associate professor at Arizona State University's College of Health Solutions.

“At the end of the day, if someone would ask me, ‘Should I be drinking these for my health?’ I would probably say no and stick to whole foods as much as possible,” Whisner said. “Just eat real food. Then you know what you’re getting.”

NEW YORK (AP) — For many people, tax season isn’t only about gathering W-2 forms or calling an accountant. It can also bring intense feelings of stress or anxiety about dealing with finances.

Financial stress during tax season can manifest in different ways, whether that’s procrastinating on your tax return until the last minute or experiencing intense stress about filing incorrectly. If you’re dealing with financial stress right now, you’re not alone.

“Many Americans experience high stress during tax times, and many Americans are dealing with financial stress,” financial wellness expert Joyce Marter said.

You might think emotions and money don’t go together, but they often affect each other, said Lindsay Bryan-Podvin, financial therapist and founder of Mind Money Balance.

“Our money and mental health intersect because they’re two parts of our overall wellness,” said Bryan-Podvin.

Here are recommendations from experts to reduce financial stress during tax season while still getting your return done by the deadline:

Don’t avoid, plan

Financial stress can happen all year long. While tax season is only a window of time, it comes with something that can be daunting: a deadline. Some might find a deadline motivates them to get things done, while others can feel paralyzed by it, said Dr. Tanya Farber, psychologist at McLean Hospital, a mental health facility in Massachusetts.

“If we’re overwhelmed by our anxiety, that’s where it may lead us to avoid thinking about finances or trying to avoid thinking about taxes,” Farber said.

Although you do have an option of filing for an extension, Farber doesn’t recommend prolonging the period when you have to worry about taxes. Instead, a good first step is to make a detailed plan to tackle them.

Farber recommends you break down all of the steps and start completing them one by one. A key step is to start gathering all of your documents, such as your W-2 or 1099 forms, savings and investment records, eligible deductions and tax credits , ahead of sitting down to file your taxes. Once you have a list of steps, scheduling times to complete the tasks will make it easier to build momentum.

While the required documents might depend on your individual case, here is a general list of what everyone needs:

Face your fears

For many, anxiety over filing taxes comes from fear, Farber said. Whether it’s fear of making a mistake in your return or unexpectedly owing thousands of dollars, these fears can overwhelm you to the point of avoiding even looking at your taxes.

If you identify that fear is what’s stopping you from filing, Farber recommends that you ask yourself if whatever you’re afraid of is likely to happen.

“When we have fears, they’re possible, probable or definite,” Farber said. “And oftentimes anxiety is the highest when we’re assuming the worst-case scenario.”

If thinking about your fears by yourself is not helping, Bryan-Podvin recommends you discuss them with a friend or a family member. In many cases, talking with someone else might help you focus on what might actually happen rather than focusing on the worst-case scenarios.

Focus on self care

If tax season brings a lot of financial stress for you, Bryan-Podvin recommends that you increase the amount of self-care activities you’re doing. Activities such as taking a walk, spending time with your dog or getting enough sleep can help soothe stress.

“We are going to be doing things that are difficult and depleting and anxiety-provoking so doing more things that are restorative can help us balance that,” Farber said.

Doing restorative activities can work as a toolbox of coping skills when you’re in the middle of a stressful situation, like filing taxes.

Ask for support

You might feel like you’ll be judged if you talk about money, but that’s often not the case. Talking about finances with your friends or family can be a moment to receive support, she said.

“Talking with others is going to give you access to more information and resources and also helps remove the shame and stigma because you’re not alone,” Marter said.

Whether it’s talking with a tax professional or reaching out to your most tax-savvy family member, proactively seeking support will help you avoid being stressed if you’re filing very close to the deadline.

Additionally, if you are experiencing mental health struggles, there are several resources you can use to find professional help.

Michael Pollan's deliciously simple meal plan to avoid ultra-processed foods — and where it falls short

Michael Pollan is probably best known for the seven simple words of diet advice he first used to open a New York Times essay in 2007, which later became the backbone of a bestselling book :

"Eat food. Not too much. Mostly plants ."

He knows it's really not so simple.

"We all make compromises," Pollan told Business Insider ahead of the release of his latest documentary, Food, Inc. 2 , out April 12. "We do the best we can, and people pick out the issues they want to deal with, and can deal with."

At home, Pollan tries his best to avoid eating industrial meat, eggs, or any other product from big factory farms.

He and his wife have set up their life to make healthy eating easy and affordable. They keep a stash of well-sharpened knives, ready to chop and cook a rainbow of veggies for dinner most nights. Lately, his table's featured stir-frys and pasta dishes, as well as some wild salmon, since they happen to be running in Alaska this season. Pollan also enjoys cooking a firmer, more protein-rich form of tofu I'd never heard of called yuba, which is essentially a soymilk skin. Hearty stews , perhaps with Indian or Moroccan-inspired spices, are some of the other fan favorites simmering around his kitchen island in Berkeley, California.

It sounds very healthy, very delicious, very aspirational … and pretty impossible. Because it is.

The reality is that Pollan does participate in America's industrialized food system. He's spent the better part of two decades investigating the way food corporations combine chemicals, plants, and animals in some very toxic ways, and as a result he feels "really uncomfortable participating in a system that was so brutal, not just to the animals, but to the workers in it," he told BI. But he knows that, ultimately, if you want to participate in US society, some amount of toxic and unethical food is unavoidable.

"If you had me over to your house and you were cooking a pork shoulder, I wouldn't be rude I would eat it," he said. "I'm not so zealous about it."

Still, he might have some intrusive thoughts while eating store-bought sliced white bread or fruit salad. He worries about the glyphosate that's doused on industrial wheat right before harvest and the harsh pesticides often sprayed on thin-skinned fruits like strawberries (he opts for organic whenever he can, but even that's not a guarantee of safety). "I think that the more you know about food, the more it shapes your diet," he said.

'Plant-based' is just a marketing trick from Big Food

Pollan's new documentary (in select theaters now, and streaming on Amazon Prime) is a follow-up to his Academy Award-nominated 2008 release "Food, Inc." "Food, Inc. 2" focuses on the few mega-companies that dominate our grocery stores and often end up deciding what we put in our mouths.

The film explores what happened at a Tyson meat processing plant in Waterloo, Iowa, during the first days of the COVID-19 outbreak (spoiler alert: it wasn't great.) It also features the voices of tomato pickers in Florida fighting for fair pay, Taco Bell employees asking for the same, one inventive farmer bringing a regenerative gizmo called the "clustercluck " to his cornfields, and Democratic Senator Jon Tester, who (when he's not in Washington) works an organic farm in Montana.

Over 90 minutes, the documentary takes you on a rather bleak journey through time, highlighting the many ways our food system has actually gotten worse since the release of the first Food Inc. 16 years ago.

And yet, it ends on a head-spinningly optimistic note. The shift is so stark it's almost funny. Just before the credits roll, the film encourages viewers to join the "movement" on its website, which encourages us to (surprise, surprise) eat fewer ultra-processed foods, support our local farmers by shopping at their markets when we can, reduce meat and dairy consumption, and then in our spare time, lobby for more antitrust law enforcement against the handful of big food companies that dominate the market for baby formula , meat, cereal, and other staples. Simple.

"The industry has a strong interest in complicating our relationship to food, creating problems that it can then solve — but it's a lot simpler than people think," Pollan says.

Is it though? Speaking to Pollan made me feel even more strongly than before that, even in a best-case scenario , for the thin slice of Americans who have the time, money, plus institutional and cultural support to adopt all of these grand ideas, some of the worst parts about our big, bad food system cannot be avoided. The emulsifiers that may mess with our gut bacteria in weird and poorly understood ways are sorely needed to keep food shelf-stable. And even if we stick to organics, the soil quality ain't what it used to be around here, meaning we derive less nutrients from the foods we eat.

"If you do eat food, not too much, mostly plants, you're going to be fine," Pollan tells me, doubling down on his old phrase.

But even he admits some caveats with his next breath. He's frustrated by the misleading "aura of health" that has sprung up around everything plant-based. That's a whole new game of whack-a-mole we have to play in the grocery store aisles. After all, sugar cane is a plant, and so is the corn that so many of our stabilizers, artificial sweeteners, flavors, and added ingredients come from. Plant-based diets aren't necessarily a beacon of health.

"I don't regard a lot of the stuff in the supermarket as food," he tells me.

But it's what we have.

Health Insurers’ Lucrative, Little-Known Alliance: 5 Takeaways

Large health insurers are working with a little-known data company to boost their profits, often at the expense of patients and doctors, a New York Times investigation found. A private-equity-backed firm called MultiPlan has helped drive down payments to medical providers and drive up patients’ bills, while earning billions of dollars in fees for itself and insurers.

To investigate this largely hidden facet of the health care industry, The Times interviewed more than 100 patients, doctors, billing specialists, health plan advisers and former MultiPlan employees, and reviewed more than 50,000 pages of documents, including confidential records made public by two federal judges after petitions from The Times.

Here are five takeaways.

The smaller the payout to doctors, the bigger the fees for insurers and MultiPlan

When patients see medical providers outside their plans’ networks, UnitedHealthcare, Cigna, Aetna and other insurers often send the bills to MultiPlan to recommend a payment amount.

MultiPlan and the insurers have a powerful incentive to keep the payments low because their fees get bigger as the payments get smaller.

Here’s how it works.

The most common way Americans get health coverage is through an employer that pays for workers’ medical care itself and uses an insurance company to administer the plan. Providers in the plan’s network have agreed-upon rates, but out-of-network providers often must negotiate payments.

By using MultiPlan’s frugal recommendations, insurers say they are saving employers money. But insurers and MultiPlan also benefit because their fees are typically based on the size of the declared “savings” or “discount” — the difference between the original bill and the amount actually paid.

In some instances, insurers and MultiPlan have collected more for processing a claim than the provider received for treating the patient.

UnitedHealthcare, the largest U.S. insurer by revenue, has reaped about $1 billion in fees annually in recent years from out-of-network savings programs, including its work with MultiPlan, according to legal testimony.

Patients could be on the hook for the unpaid bills

Patients have seen their bills rise after their insurers began routing claims to MultiPlan, as providers charge them for the unpaid balance.

Some patients said they have scaled back or ceased long-term treatment as a result. The predicament can be especially punishing for people who depend on out-of-network specialists, including for mental health or substance abuse treatment.

Patients have limited recourse. If they want to sue, they usually must first complete an administrative appeals process, and even if the case goes forward, they stand to collect relatively modest amounts.

Self-funded plans are mostly exempt from state regulation, and the responsible federal agency says it has just one investigator for every 8,800 health plans.

Some medical providers face big pay cuts

MultiPlan and insurers say they are combating rampant overbilling by some doctors and hospitals, a chronic problem that research has linked to rising health care costs and regulators are examining. But low payments also squeeze small medical practices.

Kelsey Toney, who provides behavioral therapy for children with autism in rural Virginia, saw her pay cut in half for two patients. She has not billed the parents of those children, but said she would not accept new patients with similar insurance.

Other providers said they have begun requiring patients to pay upfront because appealing for higher insurance payments can be time-consuming, infuriating and futile.

Former MultiPlan employees said they had an incentive to lock in unreasonably low amounts: Their bonuses were tied to the size of the reductions.

Employers are charged hefty fees

Insurance companies pitch MultiPlan as a way to keep costs down, but some employers have complained about large and unanticipated fees.

For a New Jersey trucking company called New England Motor Freight, UnitedHealthcare used MultiPlan to reduce a hospital bill from $152,594 to $7,879, then charged the company a $50,650 processing fee.

In the Phoenix area, trustees managing an electricians’ union health plan were surprised to learn that the fees charged by Cigna had risen from around $550,000 in 2016 to $2.6 million in 2019, according to a lawsuit the trustees later filed.

Employers trying to verify the accuracy of insurers’ charges have sometimes faced challenges getting access to their own employees’ data.

Private equity is playing both sides

For years, insurance companies have blamed private-equity-backed hospitals and physician groups for hiking bills and making health care more expensive. But MultiPlan is also backed by private equity.

MultiPlan’s annual revenues have climbed to about $1 billion thanks to its embrace of more aggressive approaches to reducing costs. Its premier offering is an algorithm-driven tool called Data iSight , which consistently recommends the lowest payments to doctors — typically resulting in the highest processing fees.

MultiPlan became publicly traded in 2020, and its largest shareholders include the private equity firm Hellman & Friedman and the Saudi Arabian government’s sovereign wealth fund, regulatory documents show.
