Update: Due to changes by Google in 2020, you will now only be able to have star ratings in search engine results for individual product pages. If you have individual product pages, these should have product page schema markup applied to them to allow Google to read them as such. Review platforms, when used and optimised correctly, have a huge influence in providing traffic and conversions to your brand.

A good review (or, even better, many good reviews!) acts as a stamp of approval for a company’s service or product, telling potential customers that the product is one to purchase. This is crucial for online retailers as a validation of the product or service – particularly when optimising appearances in the SERPs (search engine results pages). 

Just how much of an influence will reviews have on the customer? A study from Podium and published in The Drum found that 93% of customers say online reviews impact their purchasing decision, making it crucial for a company to engage with online review platforms. 

Going beyond the customer influence that reviews have and looking at the SEO implications for reviews, we need to look at how Google uses them, what platforms to focus on and what impact these have.

The impact that reviews have on customers being driven to your business’s website is shown in a study carried out by Richard Baxter, measuring click through rates. The test found that the meta description with a review-rich snippet included (ie. with stars shown in the description) had a click through rate of 26.32%, while the other descriptions tested (with sales-driven wording and informative wording) had click through rates of 21.52% and 14.93% respectively.  

So reviews in your brands search results are needed. Here’s how to go about it.

One within the results as a rich snippet from Facebook’s reviews, and two sections within the Google My Business profile – Google reviews at the top and rankings from independent review platforms that Google has pulled; Facebook, Houzz, and Maclaren Electrical Services (which, incidentally, seems to be a mistakenly associated with BoConcept).

Anyone who was to search for this shop would have their eyeline drawn to each of these, influencing their immediate impression of the store, even before visiting the website.

Besides Google reviews and Facebook, which will be standards for most businesses to encourage customer reviews on, Houzz is a more tailored result for the homeware and furniture niche that BoConcept sits in. Be aware of similar platforms which cater to the specific niche of your business, as Google will likely know this and provide its listing as a variation from the standards.

In a post on the SEM Rush blog written by Jason Barnard, the top review sites for SEO visibility were highlighted measuring the number of keywords that the platform ranks for, the traffic this generates to the site and their SEO rank (SEM Rush measure)

Using reviews for eCommerce product page optimisation

For an eCommerce site, reviews for individual product pages can also be pulled through to appear on their listings on the search engine results page. This is a great way to help your page stand out from competitors in the organic search results.

  • Product schema markup – this tells Google that the page is a product page, so it will treat it as such in the Google search results

Maybe I'm gullible but overall I trust Trustpilot a lot. Where it fails is not breaking down companies into their divisions. I had pet insurance with a company that covered many other categories. Overall the score was very good. When sometime later I drilled down and filtered out as much as I could to their pet insurance the score was abysmal. I have since been very wary of this and found quite a few other examples.

It should not be like this, despite 1 customer experience they should not be able to leave 5 comments. Otherwise you create a very good opportunity to denigrate brands. This also needs to be corrected. But there is no action on your part.

Full of scam reviews, they also block genuine reviews. Our competitors reputation in here is basically build on fake reviews. Can easily spot them by repetitive quotes without sense in the industry. Just got an email from Trustpilot saying that I needed to give a reviewed company (Youfoodz) 'more information' 'or my review may be taken offline'. Clicking through, it wanted me - forced me - to give the company my phone number. Give over my private information or have my legitimate review, which the company already contacted me to resolve? Not cool.

....I love Trustpilot! ....So far , the only honest decent consumer review platform that defends the 1st amendment of the bill of rights of the U.S. Constitution! ....Their based in Denmark..........but they defend free speech....which is a basic human right!

I thought you were brilliant but FOUR days ago you told me that Monzo had responded to me and I had TEN days to respond to them Two issues: 1. Yesterday you told me that I only have TWO days to respond so somewhere 3/4 days disappeared. 2. I cannot find any reference to Monzo asking me for details of my long standing problem with them and you tell me that if I don’t respond to them my complaints will be deleted. I have made multiple comments about another business, yet Trust Pilot emails me and informs me that my review may be removed if I don't provide more details about myself to the company I reviewed! There are two major reasons why customer reviews are important.

1. Customer reviews are a learning opportunity for your business.

Negative or neutral reviews provide your company with an opportunity to learn and develop. You can see exactly where your business is failing to meet customer needs and remove common roadblocks in the customer journey .

For particularly poor reviews, reps can follow up with these customers and prevent potential churn. They can highlight specific pain points and craft personalized responses geared to specific problems. This makes your customer service team appear more invested in your customers' goals, which results in fewer negative reviews and increased customer retention.

2. Customer reviews are marketing and sales tools for your business.

The other major reason for obtaining reviews is the opportunity to share them with potential customers. Positive reviews can be used as customer testimonials during lead acquisition and can help you add more contacts to your CRM . In fact, 91% of consumers read at least one review before purchasing a product. So, don't just pat yourself on the back the next time you get a five-star review; Make sure everyone knows how good a job your company is doing.

With that in mind, it's not safe to assume we all know where to track down those positive customer reviews. I mean, we might all be able to rattle off one or two sites ("Yelp! And I think you can get them to show up in Google Maps, too?"), but we don't exactly have a laundry list of options at our disposal

Amazon was one of the first online stores to allow consumers to post reviews of products in 1995 , and it remains one of the most important resources for consumers looking to make informed purchase decisions. Products are rated on a five-star rating scale, which is broken down by the percentage of reviews per star, followed by most helpful customer reviews and most recent customer reviews.

Part of the New York Times, Wirecutter is an excellent resource for customers looking for honest, independent product reviews. All products go through rigorous testing by the Wirecutter team so readers can rest assured that their recommendations are legit.

From a business perspective, there isn't much you can do to influence ratings on this site, but it should encourage your brand to make its products the best they can be.

  • Although Wirecutter earns commission through affiliate links, they do not earn if readers return a product they recommended that they are dissatisfied with — keeping reviewers honest.

3. Angi (Formerly Angie's List)

Geared toward U.S.-based service businesses, Angi is a "higher-end" review site, because users actually have to pay for membership. But you get what you pay for. The reviews, given on an A–F scale, are typically very well-thought-out — not a lot of that ranting and raving that's more common on free review sites. The reviews can't be anonymous, which helps cut down on fake or misrepresentative reviews, and companies are allowed to respond to the reviews posted about them, too.

It's free to set up a page for your company. Once you have yours, encourage your customers who are on Angie's List to leave reviews there — members are the only ones allowed to do it!

Benefits of Angie's List:

  • Reviews on Angi are typically of high-quality since users need to pay to use the site and can't post anonymously.
  • Even though this is a paid subscription service, it's free to list your company on Angi.
  • If a customer has a bad experience with a recommended provider, Angi will work with both the customer and the company to resolve the issue.

Have customers located in Australia? CHOICE is an independent, member-funded consumer advocacy website based in Australia. It independently evaluates products and provides detailed reviews to its members.

The bulk of its work involves consumer rights, exposing shady industry tactics, as well as providing product reviews. In addition, it allow members to rate and discuss specific products and services with other CHOICE members. If you have customers in Australia, we recommend encouraging those who are on CHOICE to leave reviews there.

  • Since Choice evaluates products and services independently, its reviews are unbiased.

5. Trustpilot

Trustpilot is a fast-growing, community-driven consumer review platform based in Denmark, although it has expanded out of Europe into 65 countries, including the U.S. The platform for businesses helps companies from all over the world proactively collect customer reviews. It's the customers who write both product and seller reviews, keeping it third-party verified.

While there is a basic version for free (this lets you create a profile page and collect customer reviews), the paid versions let you create customizable review invitations, share ratings and reviews on social media, and link review data to your internal business systems.

  • Over 500,000 websites are rated on Trustpilot.
  • Trustpilot uses its own review system to rate its website. Based on about 136,000 reviews, Trustpilot is rated four stars out of five.
  • Trustpilot has advanced SEO features that help your website rank higher on SERPs when customers leave reviews for your company.
  • Trustpilot can be integrated with Shopify, Magento, and MailChimp.

6. TestFreaks

Similar to TrustPilot, the Swedish-based TestFreaks helps companies proactively collect customer reviews and write seller reviews to complement them.

Another cool addition? Its question and answer feature, which lets prospective customers post questions and receive answers directly from your customer service team.

  • TestFreaks has 100 million crawled reviews from over 15,000 sources.
  • TestFreaks provides a content moderation team that analyzes content in over 20 languages.
  • TestFreaks has an import feature where you can upload external customer reviews into its website.

Which? is an independent consumer review organization that tests and reviews products and then writes about them. Unlike Angie's List, it's the folks at Which? who write the reviews — not the customers. They don't accept submissions for product testing or survey inclusion, but they do encourage people to let them know about their products and services by emailing [email protected] .

While this leaves less room for you to influence whether your products end up on their site, it's still worth knowing about and checking in on this popular site, especially if you have customers in the U.K. The website has product reviews for everything from dishwashers and tablets to cars and credit cards. It tests and reviews all these products, and then writes about both its methodologies and results. It also surveys customers for things like the best and worst firms for customer service .

A lot of its content is free, but customers can become members for £10.75 per month to get access to a "Best Buys" and "Don't Buy" list, the latest reviews of products from its test labs, and access to its consumer legal advice service.

  • Which? offers more than 8,000 free reviews.
  • Which? tests and reviews more than 3,600 new products every year.
  • To prove that it's impartial, Which? provides a video that shows users how it tests and reviews each product.
  • Which? is 100% independent. It doesn't accept freebies from product manufactures or retailers so its reviews remain unbiased.
  • Which? offers a "Safety Alert" feature that notifies users when a product is deemed unsafe. There's also a "Brand Alert" feature that lets you know if a brand has a history of creating unsafe products.

8. Consumer Reports

A nonprofit organization, Consumer Reports is an independent product-testing organization that runs unbiased tests to rate and recommend products. It has reviewed over 7.7 million products, accept no advertising, and pay for all products that it tests. (Fun fact: The company buys and tests 80 cars each year! ) This is about as legitimate as it gets. As such, there's not much you can do here "except" if you sell a product, make sure it's really, really good.

If nothing else, you could take this website as a lesson in excellent content creation. For each product it reviews, it provides the review criteria, product overviews, a buying guide, and social sharing buttons. It's all quite comprehensive and, well, helpful. Pretty much the key to great content, am I right?

  • Consumer Reports has over 63 testing facilities — including its 327-acre auto test center.
  • Consumer Reports has spent over $30 million on testing and rating products.
  • The company plans to test more than 8,500 new products in 2021.
  • There are over eight product categories to choose from — each having several subcategories within them.
  • Consumer Reports provides an overall score for each product, so you know exactly how it compares to other products in its category.

9. Consumer Affairs

Consumer Affairs is a for-profit organization that reviews companies using an “overall satisfaction rating." This average is scored on a one to five-star scale that's updated every time a new review is submitted. Each company's page provides a list of all its reviews, which includes the consumer's star rating as well as a short explanation for its score.

Consumer Affairs also allows users to submit review recordings, which are two-minute videos where people talk about their experience with a brand. This makes the review much more compelling because you can actually see the person talk and express their genuine feelings about the company.

  • Consumer Affairs provides a demo for brands looking to buy a subscription to its service.
  • The "Review Feed" feature allows you to research all of your customer reviews in one, scrolling feed.
  • Consumer Affairs has a "Video Review" option where customers can record themselves giving a review.

10. TripAdvisor

If you're in the travel, hotel, airline, entertainment, or restaurant industries, you'll want to check out the reviews on the popular website, TripAdvisor. As the largest travel site in the world, it has over 225 million reviews, opinions, and photos taken by travelers. It also has some awesome content on low airfares, travel guides, rental listings, and advice forums for pretty much every location in the world you could possibly imagine. A lot of people look there before booking a trip.

The key to a successful profile on TripAdvisor is making it as close to the top of the popularity index as possible, so that people searching for information in a specific place see your listing. According to TripAdvisor , the popularity ranking algorithm is based on three key components: quantity, quality, and recency of reviews. Here's an excerpt of the advice they give businesses looking to improve their ranking:

"Quantity : Ask your guests to write reviews, and use our management center tools to remind them after they check out. Offering incentives for reviews is against the rules, though — take a look at our policy to make sure you understand what is and isn't okay.

Quality : Guests who enjoyed first class hospitality and a memorable experience are more likely to write positive reviews. Monitor what previous reviewers have written to see what worked and did not work best for your property so you can maintain and improve your service.

Recency : Recent reviews factor more strongly on your popularity rankings and older reviews have less impact on a hotel's ranking over time. Once again, encourage guests to write reviews to keep fresh content rolling in."

  • TripAdvisor Plus gives you access to discount hotel pricing for over 100,000 locations.
  • The average TripAdvisor Plus member saves about $350 the first time they use their membership.
  • TripAdvisor Business Advantage showcases your company's best reviews so customers see them first when reviewing your profile.

Yelp is a free review site that lets consumers rate businesses on a five-star scale. Any business can set up a profile on Yelp for free, and users can set up their own free profiles to review a business. You're free to respond to reviewers, too, but we recommend taking a balanced and polite approach to any negative reviews you receive, as Yelpers are in a pretty tight-knit community.

Yelp has also come under fire over the past few years for some slightly shady practices, like incentivizing businesses to advertise with them in exchange for gaming the search results for their business ("Pay us money and we'll push bad reviews down!"). Savvier consumers have learned to look at Yelp reviews as a whole and with the reviewer's clout in mind, instead of getting turned off by a business because of one bad listing.

That being said, it's still to your benefit to get a constant stream of positive online reviews coming to your business' Yelp account so happy customers are always at the top of your review feed — especially if you're a location-based business. Yelp profile information contains things like store hours and location information, so your profile will often turn up when people Google your business.

  • Yelp has roughly 184 million monthly users worldwide.
  • Anyone can sign up for Yelp and it's fairly easy for customers to leave a review — making it a good source of customer feedback.
  • Yelp users are a tight-knit community. Active users will provide a lot of detailed feedback for your business.
  • Research shows an increase of one star in your Yelp rating can increase revenue by 5-9%.

12. Google My Business

You know those reviews that show up when you search Google for a business? Yeah, those things are on this list in a big way.

Google's Pigeon algorithm update uses distance and location ranking parameters to deliver improved local search results. So, in order for your business' website to be properly optimized for search, you'll want to set up verified accounts with local directories — especially Google's, called "Google My Business." Getting reviews, comments, pictures, and so on, especially on Google, can give you a boost in search. Only verified local Google+ pages can respond to reviews.

An added bonus? Google Maps pulls that information and those reviews into the app, so having a lot of content in there will make your business look more reputable.

But in terms of search engine optimization (SEO), the search engines only care about whether a site displays suitably on mobile screen sizes. Which Website Builder Has the Best Social and Site Stats?

Many products offer SEO tools , but too often this is just a form for entering meta tags. You're mostly left to wrestle with that black magic known as SEO for yourself. One downside of most of these services is that, should you someday want to move to another web host, you'll likely be out of luck because of the custom code they use to display your site. Only a few of the services here let you take your site to another web hosting service: The most complete example of this is Weebly, which lets you download the standard site server folders. Squarespace offers some transferability by letting you output your site in standard WordPress format. As you might expect, the same transferability holds for WordPress.com.

The Best Website Builder Support Options

Support among the services varies widely, from free WordPress.com accounts only offering community support, to Jimdo's email-only service, to Wix's telephone callback service—even for free accounts! Many of the site builders offer rich online support knowledge bases and FAQs, so there's a good chance you won't even need to contact the company. We test each service's support as part of the review process by asking about some less common site-building procedures.

The Best Website Builders for Small Businesses

As you can see, there are quite a few factors to consider when choosing an easy online website builder. And you have a slew of provider choices—there are at least 20 more vendors than those included in this list. Hardly a week goes by when we don't get a pitch from a new one we've never heard of before. We've reviewed many of those, but they didn't make the cut, either because of outdated site designs, lack of site-building options, or inadequate ease of use.

Chatgpt-4o is wildly capable, but it could be a privacy nightmare.

OpenAI has launched ChatGPT-4o and it comes with impressive capabilities. ChatGPT-4o is much more human-like than previous iterations, able to solve equations, tell bedtime stories and identify emotions from visual expressions.

But as a privacy journalist, I find ChatGPT-4o concerning. AI needs vast amounts of data to operate and just using chatbots requires you to enter a bunch of information about yourself. This means you are relying on ChatGPT owner OpenAI to keep your data safe and protect your personal information.

ChatGPT maker OpenAI has launched ChatGPT-4o and it comes with impressive capabilities. But it also ... [+] poses risks to your privacy.

Yet ChatGPT has already made waves in Europe because its policies were previously contrary to the EU General Data Protection Regulation (GDPR). So much so that Italian regulators even banned the service temporarily last year.

Another future capability of ChatGPT-4o was demoed this week, seeing the new macOS app able to access a person’s screen. As you can imagine, this set off more alarm bells in the privacy community, especially considering Apple’s potential—although unconfirmed— partnership with Chat-GPT to power features in iOS 18 later this year.

I spoke to experts about the risks posed by services such as ChatGPT and its newest iteration GPT-4o. I’ve also included some tips to take into account when using this technology to protect your privacy.

The privacy implications of ChatGPT are two-pronged, says Oliver Willis, partner at BDB Pitmans. “From a user’s perspective, how does ChatGPT collect and use data about you when you are using it? From everyone else’s perspective, was ChatGPT trained on information about you and what will it tell users about you?”

In its privacy policy, OpenAI acknowledges that the information used to train ChatGPT includes personal data, says Willis. “They stress that the training information is not used to profile people, or to learn about them, but some people will see the use of this data as inherently intrusive.”

OpenAI also acknowledges that using personal data to train ChatGPT means responses sometimes includes information about individuals. “OpenAI offers a mechanism for restricting the use of their data to train ChatGPT, but it is less clear what OpenAI will do for someone who objects to it disclosing their personal data in a chat response,” says Willis.

ChatGPT collects all the data inputted by a user and will retain that information indefinitely to train its models unless you opt out—which isn’t easy to do, says Matthew Holman, partner at Cripps LLP.

“In practice it is really hard for individuals to exercise GDPR rights against large language models (LLMs) such as ChatGPT,” says Holman. For example, he says, it can “create inaccurate information or hallucinate; it is able to change information without explanation and it can be almost impossible to have your data erased once it is imputed into the LLM.”

The ChatGPT iOS App That Can Access Your Screen

Concerns are growing after a demo of a new macOS app —coming to iOS too—that can see everything that’s happening on your screen. “At launch, the ChatGPT Mac app supports manually capturing a screenshot, or uploading a file, to then query ChatGPT about it,” Apple-focused site 9to5Mac writes. “But in the future, you’ll be able to click a button that gives the app access to anything and everything on your display.”

This isn’t a new capability—as 9to5Mac points out, Apple provides APIs for developers to tap into a device’s screen contents if you consent to it, so it’s not a nefarious feature.

And the ChatGPT desktop app doesn't see your screen by default, an OpenAI spokesperson told me. In fact, when the user prompts ChatGPT with Vision, the firm only uses the screen recording permission to take screenshots when the user explicitly takes that action, they said.

In the demo during the Spring Update livestream, the team copied and pasted the code into ChatGPT and then used the Vision mode to see the plot on the screen. The firm is also considering privacy implications in its safety work, which builds on its previous work for GPT-4 with Vision, according to the OpenAI spokesperson.

However, experts are concerned about the amount of data people could end up providing to ChatGPT. “Its ability to provide answers based on audio input and screen content is a big shift from the current process of manually entering screenshots into ChatGPT for processing,” says Udo Schneider, governance, risk and compliance lead, Europe at Trend Micro.

There are “huge privacy implications” associated with any product or service that has access to your screen and content, says Simon Bain, AI expert and CEO of OmniIndex. This is especially true at work, he says. “There is the potential for regulated content to be on the screen—from personally identifiable information (PII) through to payment card information and other financial data.”

What Are The Privacy Implications If Apple Partners With OpenAI?

Apple’s iOS 18 comes with AI capabilities that will be in part processed on the device as well as on Apple’s own servers, resulting in a privacy-conscious offering. But the potential partnership with OpenAI poses questions about who would be responsible for looking after iPhone users’ information.

The most likely outcome is that they will be joint controllers of the data, meaning they are jointly responsible for looking after it, says Holman. “This will involve a lot of legal, technical and compliance oversight behind the scenes to make sure that both businesses are complying with the law.”

The Apple partnership comes with some privacy concerns, as both companies are US based, says Jeff Watkins, chief product and technology officer at xDesign. “The US has more sector specific data laws rather than them being at the federal level. This is generally less stringent than the GDPR and could pose some challenges in the use of the multi-modal elements of GPT-4o—especially when you consider it can use the camera on your device as well as see your screen.”

If it does partner with OpenAI, the main thing Apple will need to get right is transparency around what is being done with data, says Watkins. This should come alongside “judicious use of app permissions and enhanced privacy tools built into iOS.”

I have contacted Apple for comment and will update this article if the iPhone maker responds.

There’s no doubt ChatGPT-4o comes with some very cool features, but it’s important to be aware of how you use it and share your data.

When people use ChatGPT, they need to remember that OpenAI can use anything people tell it to improve the model, says Willis. “That includes letting some OpenAI staff and services providers view chats.”

Taking this into account, one thing all users should consider is opting out of use of data to train ChatGPT models, says Holman. “This will provide some comfort that data is not being used in unexpected ways.”

Tom Paton, founder of greensmartphones.com, advises learning how to delete information you would prefer not to share. He points out that deleting a chat does not erase information from ChatGPT's memory. “Instead, you need to go into settings and manage its memory from there."

If you care about your privacy, AI-based tools such as ChatGPT are a scary prospect. But AI is becoming integrated into many services—take iOS 18 for example—so we often don’t have a choice about whether we use them. With this in mind, it’s down to you to limit the data services collect, restrict app permissions and ensure you read the privacy policies.

The PGA Championship heads into its final few days, and it's proving to be a tough competition at the iconic Valhalla Golf Course. Keep reading to learn how to watch the PGA Championship live streams, including cable and recap options.

The tournament officially kicked off on Thursday with Round 1. Round 2 tee times were met with delays following an accident outside the club that resulted in a death. According to a statement on X from PGA of America, a worker with one of the PGA vendors was struck by a shuttle bus and killed. The accident led to some traffic buildup for both spectators and competitors.

Scottie Scheffler, the winner of the 2024 Masters Tournament and a favorite to win this year's PGA Championship, was arrested while trying to drive into the entrance of Valhalla after traffic had been stopped for the earlier collision. Scheffler was later released and has been able to participate in Round 2 of the tournament. 2023 PGA Championship winner Brooks Koepka is also participating in the competition this year, along with Tiger Woods, Rory McIlroy, and others.

Over the weekend, coverage will be split among three channels/streamers. On Saturday and Sunday, ESPN+ will live stream from 8 a.m. to 10 a.m. ET, followed by an ESPN broadcast from 10 a.m. to 1 p.m. ET. Then CBS will take over for the rest of the day from 1 p.m. ET.

Hilary Cass Says U.S. Doctors Are ‘Out of Date’ on Youth Gender Medicine

Dr. Hilary Cass published a landmark report that led to restrictions on youth gender care in Britain. U.S. health groups said it did not change their support of the care.

After 30 years as one of England’s top pediatricians, Dr. Hilary Cass was hoping to begin her retirement by learning to play the saxophone.

Instead, she took on a project that would throw her into an international fire: reviewing England’s treatment guidelines for the rapidly rising number of children with gender distress, known as dysphoria.

At the time, in 2020, England’s sole youth gender clinic was in disarray. The waiting list had swelled, leaving many young patients waiting years for an appointment. Staff members who said they felt pressure to approve children for puberty-blocking drugs had filed whistle-blower complaints that had spilled into public view. And a former patient had sued the clinic, claiming that she had transitioned as a teenager “after a series of superficial conversations with social workers.”

The National Health Service asked Dr. Cass, who had never treated children with gender dysphoria but had served as the president of the Royal College of Pediatrics and Child Health, to independently evaluate how the agency should proceed.

Over the next four years, Dr. Cass commissioned systematic reviews of scientific studies on youth gender treatments and international guidelines of care. She also met with young patients and their families, transgender adults, people who had detransitioned, advocacy groups and clinicians.

Her final report , published last month, concluded that the evidence supporting the use of puberty-blocking drugs and other hormonal medications in adolescents was “remarkably weak.” On her recommendation, the N.H.S. will no longer prescribe puberty blockers outside of clinical trials. Dr. Cass also recommended that testosterone and estrogen, which allow young people to develop the physical characteristics of the opposite sex, be prescribed with “extreme caution.”

Dr. Cass’s findings are in line with several European countries that have limited the treatments after scientific reviews . But in America, where nearly two dozen states have banned the care outright, medical groups have endorsed the treatments as evidence-based and necessary .

The American Academy of Pediatrics declined to comment on Dr. Cass’s specific findings, and condemned the state bans. “Politicians have inserted themselves into the exam room, which is dangerous for both physicians and for families,” Dr. Ben Hoffman, the organization’s president, said.

The Endocrine Society told The New York Times that Dr. Cass’s review “does not contain any new research” that would contradict its guidelines. The federal health department did not respond to requests for comment.

Dr. Cass spoke to The Times about her report and the response from the United States. This conversation has been edited and condensed for clarity.

What are your top takeaways from the report?

The most important concern for me is just how poor the evidence base is in this area. Some people have questioned, “Did we set a higher bar for this group of young people?” We absolutely didn’t. The real problem is that the evidence is very weak compared to many other areas of pediatric practice.

The second big takeaway for me is that we have to stop just seeing these young people through the lens of their gender and see them as whole people, and address the much broader range of challenges that they have, sometimes with their mental health, sometimes with undiagnosed neurodiversity. It’s really about helping them to thrive, not just saying “How do we address the gender?” in isolation.

You found that the quality of evidence in this space is “remarkably weak.” Can you explain what that means?

The assessment of studies looks at things like, do they follow up for long enough? Do they lose a lot of patients during the follow-up period? Do they have good comparison groups? All of those assessments are really objective. The reason the studies are weak is because they failed on one or more of those areas.

The most common criticism directed at your review is that it was in some way rigged because of the lack of randomized controlled trials, which compare two treatments or a treatment and a placebo, in this field. That, from the get-go, you knew you would find that there was low-quality evidence.

People were worried that we threw out anything that wasn’t a randomized controlled trial, which is the gold standard for study design. We didn’t, actually.

There weren’t any randomized controlled trials, but we still included about 58 percent of the studies that were identified, the ones that were high quality or moderate quality. The kinds of studies that aren’t R.C.T.s can give us some really good information, but they have to be well-conducted. The weakness was many were very poorly conducted.

There’s something I would like to say about the perception that this was rigged, as you say. We were really clear that this review was not about defining what trans means, negating anybody’s experiences or rolling back health care.

There are young people who absolutely benefit from a medical pathway, and we need to make sure that those young people have access — under a research protocol, because we need to improve the research — but not assume that that’s the right pathway for everyone.

Another criticism is that this field is being held to a higher standard than others, or being exceptionalized in some way. There are other areas of medicine, particularly in pediatrics, where doctors practice without high-quality evidence.

The University of York, which is kind of the home of systematic reviews, one of the key organizations that does them in this country, found that evidence in this field was strikingly lower than other areas — even in pediatrics.

I can’t think of any other situation where we give life-altering treatments and don’t have enough understanding about what’s happening to those young people in adulthood. I’ve spoken to young adults who are clearly thriving — a medical pathway has been the right thing for them. I’ve also spoken to young adults where it was the wrong decision, where they have regret, where they’ve detransitioned. The critical issue is trying to work out how we can best predict who’s going to thrive and who’s not going to do well.

In your report, you are also concerned about the rapid increase in numbers of teens who have sought out gender care over the last 10 years, most of whom were female at birth. I often hear two different explanations. On the one hand, there’s a positive story about social acceptance: that there have always been this many trans people, and kids today just feel freer to express who they are. The other story is a more fearful one: that this is a ‘contagion’ driven in large part by social media. How do you think about it?

There’s always two views because it’s never a simple answer. And probably elements of both of those things apply.

It doesn’t really make sense to have such a dramatic increase in numbers that has been exponential. This has happened in a really narrow time frame across the world. Social acceptance just doesn’t happen that way, so dramatically. So that doesn’t make sense as the full answer.

But equally, those who say this is just social contagion are also not taking account of how complex and nuanced this is.

Young people growing up now have a much more flexible view about gender — they’re not locked into gender stereotypes in the way my generation was. And that flexibility and fluidity are potentially beneficial because they break down barriers, combat misogyny, and so on. It only becomes a challenge if we’re medicalizing it, giving an irreversible treatment, for what might be just a normal range of gender expression.

What has the response to your report been like in Britain?

Both of our main parties have been supportive of the report, which has been great.

We have had a longstanding relationship with support and advocacy groups in the U.K. That’s not to say that they necessarily agree with all that we say. There’s much that they are less happy about. But we have had an open dialogue with them and have tried to address their questions throughout.

I think there is an appreciation that we are not about closing down health care for children. But there is fearfulness — about health care being shut down, and also about the report being weaponized to suggest that trans people don’t exist. And that’s really disappointing to me that that happens, because that’s absolutely not what we’re saying.

I’ve reached out to major medical groups in the United States about your findings. The American Academy of Pediatrics declined to comment on your report, citing its own research review that is underway . It said that its guidance, which it reaffirmed last year, was “grounded in evidence and science.”

The Endocrine Society said “we stand firm in our support of gender-affirming care,” which is “needed and often lifesaving.”

I think for a lot of people, this is kind of dizzying. We have medical groups in the United States and Britain looking at the same facts, the same scientific literature, and coming to very different conclusions. What do you make of those responses?

When I was president of the Royal College of Pediatrics and Child Health, we did some great work with the A.A.P. They are an organization that I have enormous respect for. But I respectfully disagree with them on holding on to a position that is now demonstrated to be out of date by multiple systematic reviews.

It wouldn’t be too much of a problem if people were saying “This is clinical consensus and we’re not sure.” But what some organizations are doing is doubling down on saying the evidence is good. And I think that’s where you’re misleading the public. You need to be honest about the strength of the evidence and say what you’re going to do to improve it.

I suspect that the A.A.P., which is an organization that does massive good for children worldwide, and I see as a fairly left-leaning organization, is fearful of making any moves that might jeopardize trans health care right now. And I wonder whether, if they weren’t feeling under such political duress, they would be able to be more nuanced, to say that multiple truths exist in this space — that there are children who are going to need medical treatment, and that there are other children who are going to resolve their distress in different ways.

Have you heard from the A.A.P. since your report was published?

They haven’t contacted us directly — no.

Have you heard from any other U.S. health bodies, like the Department of Health and Human Services, for example?

Have you heard from any U.S. lawmakers?

No. Not at all.

Pediatricians in the United States are in an incredibly tough position because of the political situation here. It affects what doctors feel comfortable saying publicly. Your report is now part of that evidence that they may fear will be weaponized. What would you say to American pediatricians about how to move forward?

Do what you’ve been trained to do. So that means that you approach any one of these young people as you would any other adolescent, taking a proper history, doing a proper assessment and maintaining a curiosity about what’s driving their distress. It may be about diagnosing autism, it may be about treating depression, it might be about treating an eating disorder.

What really worries me is that people just think: This is somebody who is trans, and the medical pathway is the right thing for them. They get put on a medical pathway, and then the problems that they think were going to be solved just don’t go away. And it’s because there’s this overshadowing of all the other problems.

So, yes, you can put someone on a medical pathway, but if at the end of it they can’t get out of their bedroom, they don’t have relationships, they’re not in school or ultimately in work, you haven’t done the right thing by them. So it really is about treating them as a whole person, taking a holistic approach, managing all of those things and not assuming they’ve all come about as a result of the gender distress.

I think some people get frustrated about the conclusion being, well, what these kids need is more holistic care and mental health support, when that system doesn’t exist. What do you say to that?

We’re failing these kids and we’re failing other kids in terms of the amount of mental health support we have available. That is a huge problem — not just for gender-questioning young people. And I think that’s partly a reflection of the fact that the system’s been caught out by a growth of demand that is completely outstripping the ability to provide it.

We don’t have a nationalized health care system here in the United States. We have a sprawling and fragmented system. Some people have reached the conclusion that, because of the realities of the American health care system, the only way forward is through political bans. What do you make of that argument?

Medicine should never be politically driven. It should be driven by evidence and ethics and shared decision-making with patients and listening to patients’ voices. Once it becomes politicized, then that’s seriously concerning, as you know well from the abortion situation in the United States.

So, what can I say, except that I’m glad that the U.K. system doesn’t work in the same way.

When asked after this interview about Dr. Cass’s comments, Dr. Hoffman, the A.A.P.’s president, said that the group had carefully reviewed her report and “added it to the evidence base undergoing a systematic review.” He also said that “Any suggestion the American Academy of Pediatrics is misleading families is false.”
