  • OvarCare Program (through Ovarcome) OvarCare is a financial, psychosocial, and informational support program offered by Ovarcome for ovarian cancer patients in active treatment. Eligible patients who meet Ovarcome’s financial eligibility guidelines can apply for an OvarCare grant, which is comprised of a $200 financial grant, $100 gas gift card, $100 grocery gift card, 6 telephonic psychosocial counseling sessions with a licensed mental health clinician (optional) , and a telephonic genetic counseling session with a licensed genetic counselor (optional) .
  • Patient Access Network (PAN) Foundation The PAN Foundation provides up to $3700 a year in co-payment assistance annually to ovarian cancer patients who are enrolled in Medicare and have incomes up to 500% of the federal poverty level. Patients must reside and receive treatment in the United States.
  • Patient Advocate Foundation (PAF) Case workers from the Patient Advocate Foundation work one-on-one with patients, caregivers or providers resolve healthcare issues. The Foundation also offers co-payment assistance to women with ovarian cancer.

Understanding Required Financial Assistance in Medical Care

Many consumers struggle to afford the costs of essential medical services. To bring more focus to the impact of medical bills and collections on consumers, the Consumer Financial Protection Bureau (CFPB) is releasing reports and blogs over the next several months discussing the key financial hurdles the health care system presents to patients and their families. In this research brief, we discuss required financial assistance (also known as charity care), one of the main forms of assistance hospitals offer to low-income consumers to help cover the cost of medical treatment. 1 In the following, we define required financial assistance, explain its sources of funding, who is eligible, and who its likely beneficiaries are. In forthcoming briefs, we intend to discuss the pitfalls of medical credit cards and installment payments in the medical payment space.

What is required financial assistance?

Who is eligible for required financial assistance, who receives required financial assistance, funding for financial assistance, enforcement of state and federal provisions.

Required financial assistance is a form of community benefit involving medical care that is provided for free, or at a discount, to patients who cannot afford to pay. Nonprofit hospitals are required by federal law and some state laws to meet a community benefit standard to maintain significant tax and other financial benefits afforded by their nonprofit status. This standard is met in part by the provision of financial assistance to those who are not able to pay. In addition, federal law and some state laws require hospitals to have written policies that outline information such as eligibility criteria and application requirements to receive financial assistance. Under the Affordable Care Act, nonprofit hospitals must:

  • Widely publicize these financial assistance policies
  • Provide a paper copy of a plain language summary of the policy as part of their patient intake or discharge process
  • Display financial assistance policies in public spaces at the hospital
  • Include website links and contact information about the financial assistance program on billing statements 2

Some state laws have additional requirements. For example, all Washington State hospitals are required to inform patients about financial assistance options verbally and in writing and must screen patients for eligibility before attempting to collect payments. 3

Federal law does not specify the criteria hospitals should use to determine who is eligible for financial assistance, but several states have laws and regulations that do. 4 State law eligibility requirements vary widely. For example, Washington State requires hospitals to develop financial assistance policies allowing patients with family incomes below 100% of the federal poverty level (FPL) to receive free care, and patients with family incomes up to 200% of the FPL to receive care at a discount. 5 In some states, such as New Jersey and Massachusetts, the income threshold for free care eligibility is 200% of the FPL. New Jersey law also specifies that financial assistance is available to low-income residents with no health insurance, or with health insurance that pays only part of a medical bill, or for patients ineligible for private or government-sponsored coverage. New Jersey law also specifies the fraction of the bill that patients are responsible for as a function of their incomes. 6

The available evidence indicates that not all patients eligible for financial assistance receive it. 7 Many patients, for example, do not receive legally mandated information about their potential eligibility for financial assistance. A 2015 study suggests that only 44% of hospitals reported that they notified patients about eligibility for financial assistance before attempting to collect unpaid medical bills. 8 This suggests that hospitals can do a lot more to inform patients about financial assistance options available to them.

Among those who do receive required financial assistance, there appears to be a gender imbalance. A Rutgers University study, for example, found that in New Jersey in the early 2000s women accounted for a larger share of financial assistance claims than men by a 2-to-1 margin, and that prenatal care and delivery were the most common financial assistance services provided by New Jersey hospitals. 9

State laws and regulations vary in important ways. Some states provide incentives to hospitals to have well-funded financial assistance initiatives and effective distribution of funds for financial assistance, while other states appear to do little to incentivize the provision of financial assistance. Some states provide property, income, and sales tax exemptions for nonprofit hospitals that provide financial assistance, and have clear requirements or rules related to the provision of financial assistance. 10 Other states provide some or all of these tax exemptions but do not have any additional requirements or rules to provide financial assistance beyond the Affordable Care Act requirements. And some states have no legislation governing the provision of financial assistance at all. These varying policies may, in turn, affect the number of patients who successfully access financial assistance. But there is scant systematic evidence on the impact of various state-level financial assistance policies, and it is difficult to determine whether these policies are a help or a hinderance to obtaining financial assistance by the eligible patients who need it.

Nonprofit hospitals receive significant federal and state tax exemptions. The Joint Committee on Taxation estimated that, in 2002, these tax exemptions amounted to $12.6 billion in tax relief.11 In 2011, the total amount of tax relief was estimated at $24.6 billion. 12 We are not aware of more recent estimates of the total tax relief that nonprofit hospitals receive. Financial assistance and other community benefits provided by hospitals would at least in part be funded from tax exemptions at both the federal, state and local levels. From the data available, it appears that the total amount of tax relief for nonprofit hospitals greatly exceeds the levels of financial assistance hospitals provide in any given year. 13

There are other ways for hospitals in certain states to be reimbursed for the levels of financial assistance they provide. Several states fund financial assistance through dedicated programs. For example, in New Jersey, the Health Care Subsidy Fund allocated $319 million in 2022 to hospitals for financial assistance. 14 In Massachusetts, payments from the Health Safety Net in 2019 amounted to $387 million. 15 These states’ funds have been fully expended each year and are provided in addition to the hospitals’ financial assistance obligations. New York and Pennsylvania also have programs that provide financial assistance to low-income individuals for the cost of medical care. 16

Generally, nonprofit hospitals spend a relatively small fraction of their net income on required financial assistance. A recent study by Johns Hopkins researchers analyzed financial assistance expenditures by nonprofit hospitals and found that in 2017 these hospitals provided $14.2 billion in financial assistance, while they generated approximately $49 billion in overall net income. 17 The study also found that the higher the net income of the hospital, the smaller the percentage of its net income was directed towards financial assistance. Another study, by the California Nurses Association, indicates that, in 2016, financial assistance represented only approximately 20% of California nonprofit hospitals' net income. So, there appears to be some variation in the share of operating expenses that nonprofit hospitals use for financial assistance. Indeed, there appears to be a large gap, at least for some hospitals, between the levels of financial assistance that would be justified by the tax benefits hospitals receive and the levels of financial assistance actually offered.

Financial assistance laws that require hospitals to commit to a certain required level of spending on financial assistance could help bridge the gap between the levels of financial assistance required by those eligible and the levels of financial assistance actually provided. For example, Texas provides three ways for nonprofit hospitals to meet their financial assistance obligations. 18 One of these ways is based on a hospital’s tax-exempt status and requires the nonprofit hospital to provide financial assistance at least equal to a hospital or hospital system’s tax-exempt benefits. 19 A study published in 2010 looked at the effects of requiring nonprofit hospitals in Texas to spend at least four percent of their net patient revenue on government-sponsored indigent care and financial assistance. 20 The study concluded that nonprofit hospitals that were spending less than four percent of net patient revenues on financial assistance increased their spending on financial assistance, whereas those with spending above four percent exhibited a marginally significant decrease in their spending.

Consistent with the general underutilization of financial assistance, we found little evidence that compliance with financial assistance rules is systematically monitored and enforced at the federal level. However, at the state level, many state attorneys general monitor and enforce compliance with state laws mandating the provision of financial assistance to qualifying patients. Some states’ monitoring and enforcement is non-public, while other states’ enforcement actions are public. For example, recently, the Washington State Attorney General filed a lawsuit alleging that 14 hospitals in the state failed to provide free or discounted medical care to low-income patients and pressured them to pay even if they were eligible for financial assistance. 21

Enactment of state laws by additional states and more enforcement actions could help more patients gain access to financial assistance hospitals are required to provide. The North Carolina State Health Plan and the Johns Hopkins Bloomberg School of Public Health released a report in 2021 that concluded that there is no public official or agency in North Carolina that is charged with enforcing the provision of financial assistance by nonprofit hospitals. The report indicated that this lack of oversight rewards hospitals that collect the state tax exemption without providing sufficient levels of financial assistance. 22

Access to required financial assistance can provide significant relief to the patients and families impacted by medical bills and collections, but financial assistance for medical care appears to be underused. More work by academic researchers and public policy specialists needs to be done in this area to understand how to improve access to financial assistance for the eligible families who are seeking it and ensure that the benefits flow to the intended beneficiaries.

What is RMC’s Financial Assistance Policy? RMC provides free or discounted care to patients who cannot afford to pay their medical bills. To qualify, patients must meet certain income guidelines and provide documentation of their financial situation. RMC’s Financial Counselors will work with you to determine if you qualify for assistance.

How is Financial Assistance Determined? Financial assistance is based on the Federal Poverty Level (FPL), which is updated each year by the Department of Health and Human Services. The FPL is used to determine if someone qualifies for government programs like Medicaid or the Children’s health insurance Program (CHIP). The FPL is also used to determine eligibility for RMC’s Financial Assistance Policy.

Some of the options we offer include: – Payment plans – Discounts for prompt payment – Financial assistance from outside agencies – Refunds for overpayments – Sliding scale fees based on income

Financial assistance is not the only way to pay for medical care. There are a number of alternatives that may be available to you, depending on your situation.

One option is to use insurance. If you have health insurance you may be able to use it to cover some or all of your medical expenses. However, you will likely have to pay deductibles and copayments.

Another option is to use credit cards. You can put your medical expenses on a credit card and then pay the bill over time. This can be a good option if you don’t have the money upfront but can afford to make monthly payments.

You could also take out a loan. You may be able to get a loan from a bank or other financial institution to pay for your medical expenses. This can be a good option if you have good credit and can afford the monthly payments.

The Impact of Financial Assistance Programs on Health Care Utilization

Most hospitals and managed care organizations have financial assistance programs that aim to reduce financial burdens and improve health care access for low-income patients. We use administrative data from Kaiser Permanente to study the effects of financial assistance on health care utilization. Using a regression discontinuity design based on an income threshold for program eligibility, we find that financial assistance significantly increases health care utilization initially, though effects dissipate three quarters after program receipt. Financial assistance also increases the detection of and medication refills for treatment-sensitive conditions, suggesting financial assistance may increase receipt of high-value care.

It’s important to have health insurance at all times, so that your medical appointments, prescriptions, and lab tests are covered. Every year, people who are covered by MassHealth need to renew their health insurance to keep their no-cost coverage

Boston Medical Center (the "Hospital" or "BMC"), in partnership with its licensed Community Health Centers, will provide high quality, accessible, medically necessary services, without cost or at a reduced cost, to eligible patients.

The Hospital will help uninsured and underinsured individuals apply for health coverage through a public assistance program or the Hospital's financial assistance program (including but not limited to MassHealth, the premium assistance payment program operated by the Health Connector, the Children's Medical Security Program, the Health Safety Net, and Medical Hardship). BMC will not charge patients deemed eligible under the financial assistance policy more than Amounts Generally Billed for emergency or other medically necessary care.

The Hospital provides financial assistance to low-income uninsured and underinsured patients who are Massachusetts residents and with incomes up to 300% of the federal poverty level. A Massachusetts resident of any income may qualify for Medical Hardship through the Health Safety Net if allowable medical expenses have so depleted his or her countable income that he or she is unable to pay for health services.

How to Apply:

Patients requesting financial assistance should complete the confidential Financial Assistance Program Application. All applications, supporting documentation, and communication will be treated as confidential.

Financial Assistance with a Hospital Bill

If you received a bill for eligible care at a Med Center Health Hospital, you may be entitled to several different forms of assistance.

Uninsured or Self-pay Discounts

All self-pay patients receiving care at a Med Center Health hospital are automatically entitled to a 40% discount. You may also be eligible for a 10% Quick Pay Discount. For additional details on these discounts, you can visit our Self Pay page .

Payment Plans

We offer payment plans with 0% interest and no fees to you. Please visit our Payment Plan page for additional details.

Financial Assistance

If your household income is less than or equal 400% of the federal poverty limit , you may be eligible for Financial Assistance. Those with household incomes less than or equal to 200% of this limit may be eligible for 100% discounts of their bill. Patients with household incomes between 201% and 300% of the federal poverty limit may be eligible for a 50% discount, and those with household incomes between 301% and 400% may be eligible for a 25% discount. Amount Generally Billed

Med Center Health uses the “look-back” method to calculate the amount generally billed (AGB) as required by federal law. The AGB is the maximum amount we will collect from a patient who is eligible for Financial Assistance under our Financial Assistance Policy. The AGB percentage is based on all claims allowed by Medicare fee for service and private health insurers over a 12-month period divided by the associated gross charges for those claims. The following is percentages were calculated on this basis for the time period of October 2022 through September 2023 for each of our hospitals. Financial Assistance

UChicago Medicine recognizes that patients and their families may need help paying for health services if they do not have health insurance or if their insurance does not cover their entire bill. We offer discounts based on financial need, which may cover all or part of a patient’s account balance(s).

All patients may apply for financial assistance. In some cases, such as for children, the individual responsible for paying the patient’s bills (known as the guarantor) can apply for financial assistance.

Financial Assistance Criteria

Proven financial need, household income, family size and other resources determine if a patient can get a discount on their balance due:

  • Patients with household incomes at 200 percent or less of the federal poverty level can receive a 100 percent discount.
  • Patients with household incomes above 200 percent but below 600 percent of the federal poverty level can receive an 80 percent discount.
  • In addition, patients whose UChicago Medicine bills for one year total 20 percent or more of their annual adjusted gross income will not have to pay more than 20 percent of their income for all of those bills.
  • Discounts only apply to emergency treatment and medically necessary health care services. Non-medically necessary services, such as cosmetic procedures, are not discounted.

Patients who receive financial assistance cannot be charged more than the amounts generally billed by UChicago Medicine.

How to apply for financial assistance.

The process for applying for financial assistance is as follows:

  • Download and fill out the application for financial assistance, which is available in English  or in Spanish .
  • Supply supporting documents listed on the application, including copies of tax returns and proof of income (e.g., pay stubs).
  • Sign the application and return it with required documentation as soon as possible to the following address:

Emergency medical care is provided free to all foreign nationals in case of life-threatening conditions that require immediate medical treatment. You will be given first aid and emergency surgery when necessary in all public health care facilities. Any further treatment will be free only to people with a Compulsory Medical Insurance, or you will need to pay for medical services. Public health care is provided in federal and local care facilities. These include 1. Urban polyclinics with specialists in different areas that offer general medical care. 2. Ambulatory and hospitals that provide a full range of services, including emergency care. 3. Emergency stations opened 24 hours a day, can be visited in a case of a non-life-threatening injury. It is often hard to find English-speaking staff in state facilities, except the largest city hospitals, so you will need a Russian-speaking interpreter to accompany your visit to a free doctor or hospital. If medical assistance is required, the insurance company should be contacted before visiting a medical facility for treatment, except emergency cases. Make sure that you have enough money to pay any necessary fees that may be charged.

Travelers need to arrange private travel insurance before the journey. You would need the insurance when applying for the Russian visa. If you arrange the insurance outside Russia, it is important to make sure the insurer is licensed in Russia. Only licensed companies may be accepted under Russian law. Holders of a temporary residence permit or permanent residence permit (valid for three and five years respectively) should apply for «Compulsory Medical Policy». It covers state healthcare only. An employer usually deals with this. The issued health card is shown whenever medical attention is required. Compulsory Medical Policyholders can get basic health care, such as emergencies, consultations with doctors, necessary scans and tests free. For more complex healthcare every person (both Russian and foreign nationals) must pay extra, or take out additional medical insurance. Clearly, you will have to be prepared to wait in a queue to see a specialist in a public health care facility (Compulsory Medical Policyholders can set an appointment using EMIAS site or ATM). In case you are a UK citizen, free, limited medical treatment in state hospitals will be provided as a part of a reciprocal agreement between Russia and UK.

Pharmacies can be found in many places around the city, many of them work 24 hours a day. Pharmaceutical kiosks operate in almost every big supermarket. However, only few have English-speaking staff, so it is advised that you know the generic (chemical) name of the medicines you think you are going to need. Dental care is usually paid separately by both Russian and expatriate patients, and fees are often quite high. Dentists are well trained and educated. In most places, dental care is available 24 hours a day.

It is standard practice for expats to visit private clinics and hospitals for check-ups, routine health care, and dental care, and only use public services in case of an emergency. Insurance companies can usually provide details of clinics and hospitals in the area speak English (or the language required) and would be the best to use. Investigate whether there are any emergency services or numbers, or any requirements to register with them. Providing copies of medical records is also advised.

Moscow hosts some Western medical clinics that can look after all of your family’s health needs. While most Russian state hospitals are not up to Western standards, Russian doctors are very good.

Several Russian hospitals in Moscow have special arrangements with GlavUPDK (foreign diplomatic corps administration in Moscow) and accept foreigners for checkups and treatments at more moderate prices that the Western medical clinics.

In a case of a medical emergency, dial 112 and ask for the ambulance service (skoraya pomoshch). Staff on these lines most certainly will speak English, still it is always better to ask a Russian speaker to explain the problem and the exact location.

Ambulances come with a doctor and, depending on the case, immediate first aid treatment may be provided. If necessary, the patient is taken to the nearest emergency room or hospital, or to a private hospital if the holder’s insurance policy requires it.
