Keynote speech to the world health summit 2021 – 24 october 2021, unicef executive director henrietta fore.

Excellencies, colleagues, friends … it is a pleasure to be with you here today for the World Health Summit.  

I am honoured and inspired by the spirit of collaboration among experts in science, politics, business, government and civil society represented at this Summit.   

On behalf of UNICEF, I am grateful for the opportunity to speak with you now at this critical moment in the global response to the COVID-19 pandemic – a pandemic which continues to impact so many aspects of our lives.   COVID-19 has hobbled economies, strained societies and undermined the prospects of the next generation. While children are not at greatest direct risk from the virus itself, they continue to suffer disproportionately from its socioeconomic consequences. Almost two years into the pandemic, a generation of children are enduring prolonged school closures and ongoing disruptions to health, protection and education services.  

That is why today I am here to discuss the health threats facing the 2.2 billion children around the world who UNICEF serves, and the opportunity we have to protect them.  

Driven by new variants of concern, the virus continues to spread. While successful vaccination campaigns in the wealthy world have driven down rates of hospitalization and death, millions in low income countries await their first dose, and fragile health systems – on which children rely – are in jeopardy.  

Yet the gap between those who have been offered vaccination against COVID-19 and those who have not is widening. While some countries have protected most of their populations, in others, less than 3 per cent of the population have had their first dose. Those going without vaccines include doctors, midwives, nurses, community health workers, teachers and social workers – the very people that children, mothers and families rely upon for the most essential services.  

This is unacceptable. As a community of global health leaders, we have a choice. We can choose to act to reach more people with vaccines. This will keep people safe AND help to sustain critical services and systems for children.  

Today, almost 7 billion doses of COVID-19 vaccine have been administered, less than a year since the first vaccine was approved. And we are now on track to produce enough vaccines to protect the majority of people around the world before the end of next year.  

But will we protect everyone?   

Will we send lifesaving, health-system-saving COVID-19 vaccines to the world’s doctors, nurses, and most at-risk populations?  

Will donors continue to fund ACT-A and COVAX sufficiently to procure and successfully deploy the tests, treatments and vaccines needed to end the pandemic? Or will the costs of in-country delivery fall on struggling economies so that they are forced to cut other lifesaving health programmes such as routine childhood vaccinations? 

Will we stand by as the lowest-income countries, with the most fragile health systems, carry on unprotected – risking high death rates due to shortages of tests, treatments and vaccines? Or will we invest so that community health systems everywhere can withstand further waves of the virus, and bounce back from future shocks?  

Will we allow new variants of the virus to flourish in countries with low vaccination rates? Or, will we reap the benefits of global cooperation to defeat this global problem, together?   

The world has learned that financing for prevention, preparedness and response is insufficient and not adequately coordinated. And that is a vital lesson.  

But even more fundamentally, we have learned that the underlying strength of the health sector in general is a critical factor in a country’s ability to weather a storm like COVID-19.  

After all, what good are vaccines if there is no functioning public health system to deliver them?  

How do we hope to contain outbreaks if there are not enough trained and paid healthcare workers?  

This pandemic has been crippling for high income countries where average spending on healthcare per capita exceeds $5,000. So, it is hardly surprising that it is causing critical strain in lower-income countries where the average per capita expenditure on healthcare each year is less than $100.  

The past 22 months have shown us that even as we battle immediate threats such as a pandemic, we must also ensure continuous access to essential health services. If we do not, there will be an indirect increase in morbidity and mortality.  

As COVID-19 took hold of the world, healthcare workers serving pregnant mothers, babies and children faced unthinkable choices. As COVID patients gasped for breath, desperate for oxygen, mothers and babies needed it too. As wards filled up with virus victims, staff were not free to help the very young. As health budgets were stretched to the breaking point, routine healthcare began to go by the wayside.   

These are some of the reasons why more than twice as many women and children have lost their lives for every COVID-19 death in many low and middle-income countries. Estimates from the Lancet suggest up to nearly 114,000 additional women and children died during this period.  

I greatly fear that the pandemic’s impact on children’s health is only starting to be seen.  

While the pandemic has underscored that vaccination is one of the most cost-effective public health interventions, we have already seen backsliding in routine immunization. In 2020, over 23 million children missed out on essential vaccines – an increase of nearly 4 million from 2019, with decades of progress tragically eroded.  

Of these 23 million, 17 million of them did not receive any vaccines at all. These are the so-called zero-dose children, most of whom live in communities with multiple deprivations.       

Here are some of the most urgent choices we could make to address these problems: 

Governments can share COVID-19 doses with COVAX as a matter of absolute urgency and resist the temptation to stockpile supplies more than necessary.  

Governments can also honour their commitments to equitable access and make space for COVAX and other parts of ACT-A at the front of the supply queue for tests, treatments, and vaccines as they roll off production lines.  

Manufacturers can be more transparent about their production schedules and make greater efforts to facilitate and accelerate equitable access to products. This will help to ensure that COVAX and ACT-A get supplies faster. 

Governments, development banks, business and philanthropy can target strategic, sustainable investments in building robust and resilient primary healthcare services – embedded in each and every community.  

We can and we must choose a path ahead that is equitable, sustainable and rooted in the principle that every human being, young and old, rich and poor, has the right to good health.  

And there is good reason to believe that now is the time to set ourselves upon that path.  

A look back at history shows us that global threats and crises that challenge multiple interests and equities have a way of pulling together diverse partners to solve shared problems. Indeed, it is out of some of the most tragic crises that the world has found some of the best solutions.  

I believe now is such a time. We have a historic opportunity to both end the COVID-19 pandemic and set out on the road towards eradicating preventable diseases, ending avoidable maternal, newborn and child deaths, and building a strong foundation for community health that will serve this generation and the next.  

We can and we must seize this moment together.

Adding at least half an hour of yoga/exercise to your daily routine will help you overcome these problems.
  • In today’s world, everybody is engrossed in their mobiles or laptops, and there’s very little time they spend without it. Sometimes, it is necessary to take a break from all these electronic and smart devices. This will help us maintain proper physical and mental health.
  • A good night’s sleep is an easy and effortless way to maintain your health. Your body works to repair all the cells and make sure that your body functions properly the next day in the time that you rest. A minimum of 8 hours of sleep is necessary for any adult being.
  • Try to avoid processed or fried foods and make sure that your plates include enough vegetables and fruits. A healthy platter does wonders for one’s health.
  • If you are feeling disturbed or upset about something, talk to your near and dear ones; you can also consult a specialist if you need. Just like physical health, mental health is also something that is to be considered important. When we fall ill, we have medicines; similarly, if someone is feeling mentally unwell, they should consult a specialist.

With the COVID19 pandemic going on for the past two years, one should take the following precautions to avoid getting affected by the virus. Here are some simple steps one can follow:

  • Always put on a mask whenever you step out of your houses. This is applicable to people of all ages.
  • Everyone should avoid going to crowded places as there is a high risk of getting affected.
  • If you feel you are affected by the virus, seclude yourself from the rest of your family members and keep in touch with your doctors so that everything is monitored and you stay out of danger.
  • Try to eat healthy food and fruits that’ll help you develop immunity.

Narrow Your Topic

Evaluate your topic, cite your sources.

Consider these questions:

  • What subjects or ideas interest you?
  • What kinds of life experience do you have?
  • What kinds of issues have affected you or people you care about?
  • Do you have a passion about an idea, a question, a subject? How can you explain or describe it such that others might be passionate about it as well?
  • Does your subject have an edge? Does the topic have passionate supporters and opponents as well as being logical and reasonable? Is it debatable? Is it an unsolved problem?

A good practice is to make a list of ideas. As an example, here is an imaginary student’s list of ideas:

With the topics on your list, ask yourself these questions:

  • Which topics are most worthy of your time?
  • Why is your topic significant?
  • Does it work with my assignment? (Is your speech informative, persuasive, etc.)

It is often beneficial (unless the topic is given or encouraged) to avoid heavily discussed topics. This helps to keep the speech interesting rather than giving an audience information they hear regularly.

Overused topics may include abortion, global warming, affirmative action, the death penalty, recycling, and sex and violence in the media. There is always a possibility to find an interesting angle or portion of the topic, but make sure you verify it with a professor first.

Let's take our student's list as an example. Our student might not want to write a speech on recycling, but maybe they have a a great way to reuse/remake something that is normally thrown out. This could work as a topic for a demonstration speech, though they would need to have the topic approved.

One way to develop these ideas is to make a concept map. Below is a sample of the student's concept map if they focused on knitting.

Now this particular student enjoys mysteries and crime shows, because they like to figure out who the culprit is. The student needs to write an informative speech, and decides that they could inform others on how to solve mysteries.

Since it is a very large topic, the student decides to focus on helping people solve mysteries by informing them on how to tell if someone is lying .

After narrowing the topic, evaluate your speech to see if it is a good fit for your assignment.

  • If it takes a while to explain your topic is either too complicated or too broad. Consider your time requirements and if you can adequately discuss the topic.
  • Begin to focus on what you want to say and why. Part of this will already be dictated by the type of speech you are assigned. Making a concept map can help provide you with ideas.

  • You will want to use solid, scholarly information on the topic. General information might be easy to find, but you will need facts and research to back up your claims and information.

In our example the student’s evaluation would look like this:

Write out your research question or thesis statement. Underline words that you believe best represent the main ideas.

You also need to credit the people who did the research you are using otherwise you will be claiming it is your own (even if unintentionally doing so). Plagiarism is a serious offense.

Just like in college writing, speeches should provide your audience with verbal cues to the information you have used: the SOURCE where you found your information. (This might be an interview, scholarly article, book, or website, etc.); the AUTHOR, when available, and the DATE when your source was published or accessed (for web sources and interviews).

Here are three ways to incorporate citations for your speech:

  • Use quotation marks to attribute words of another person on your note cards. You can express quotations in your speech in several ways.
  • Provide credit or citation such that the audience can trace back to the original source.
  • Paraphrasing the main ideas WITH correct attribution.  A paraphrase will replace some of the words while keeping the main idea of the original work.

We must have all come across the old adage, ‘Health is Wealth’ and it is indeed. As we all know, remaining Healthy is key in order to live a fulfilling life. When we speak about being Healthy, we imply the ability to function optimally both physically and mentally. As we explore the dynamics of Health and Fitness, we have to acknowledge the fact that mental Health is as important as physical Health. 

So why is it important to be Healthy and physically fit? Because, as human beings, we have limited time on earth and being fit is one of the key requirements that allow us to live and enjoy life. A fit person is more likely to make the most of his/her time in the journey of life. Over the centuries, people have stressed on the importance of good Health. A number of great discoveries and inventions, such as vaccinations and cutting edge technologies have been developed in order to ensure that human beings are able to live a Healthy life. Since the start of civilization, people have embraced various means to ensure that they remain Healthy.

Now, as we discuss the importance of being fit, we must also explore the key components that allow us to be so. When we think about Health and Fitness, the first thought that crosses our mind is exercise and rightly so. As a matter of fact, there is no denying that exercise is the first step towards ensuring that you remain fit. Exercise in any form allows the release of the hormone endorphin, which triggers a feeling of positivity and allows you to make the most of your day. 

However, it is important to remember that exercising is only one of the components of being physically fit. In addition to exercise, you must also maintain a Healthy diet. The inclusion of whole grain, proteins, fruits and green leafy vegetables in your diet will ensure that your digestive system is functioning optimally which is a must for maintaining good Health. In addition, you must also ensure to get 6-8 hours of sleep every day in order for your body to metabolize well. 

As we have already mentioned above that being fit refers to physical as well as mental Health, it is necessary that we give equal importance to both. Practices such as yoga, meditation can prove to be very relaxing and one often finds solace in them. They allow you to relax and take your mind off the feelings that wear you down. Moreover, one should not hesitate to seek professional help if they do not feel invigorated mentally. 

In conclusion, I would like to say that we endeavour all our lives to make sure that we have a better life. So, why not make each day more enjoyable by being Healthy and fit? Make time for yourself, get some exercise, practise meditation and live fully.

Since a Healthy individual is not only limited to being physically fit, I would also like to throw some light on mental Health as well. With our busy schedules, we often overburden ourselves with work and tend to ignore our mental Health, which can have devastating effects in the long run. Mental Health, if not more, is as important as physical Health. If we are engrossed with feelings of depression, anxiety, etc., we must make sure to meditate as it will help bring a sense of calm. 

To conclude, I would like to say that practices that harm our Health should be avoided and instead we should try to make our lives more fulfilling by focusing on our Health and Fitness.

There is increasing evidence that implicit bias – non-conscious attitudes toward specific groups – is a source of racial inequities in certain aspects of health care, and lawmakers are taking note.

Since the tragic murder of George Floyd in May 2020 , wherein a Black man was killed by police, several U.S. federal and state legislators have introduced proposals declaring racism as a public health crisis. In March 2024, four U.S. senators led a resolution calling out the “implicit racial and ethnic biases within the health care system, which have an explicit impact on the quality of care experienced by members of racial and ethnic minority groups.”

Following this reasoning, states like California and Massachusetts have enacted legislation mandating implicit bias training for health care providers. Health institutions have also focused on addressing implicit bias among the next generation of providers. For example, the American Medical Association’s guidelines to address systemic racism in medicine includes requiring training that covers various forms of racial bias.

But is implicit bias training improving care quality for Black patients? We are a social and health psychologist and a health economist who are investigating the role that provider implicit bias plays in racial health care disparities. Our ongoing review of the existing evidence suggests the answer is: not yet.

What is implicit bias?

The first thing to understand is that implicit bias isn’t just one thing. It involves multiple interconnected components that govern how someone interacts with specific groups or its members: affect, behavior and cognition.

Psychologists sometimes refer to those components as the ABCs.

The affective component of bias, also known as prejudice, is defined as having negative feelings towards a group or its members. The behavioral component of bias, or discrimination, involves negative or harmful actions towards a group or its members. Lastly, the cognitive component of bias, also known as stereotyping, refers to expectations and beliefs about a group.

One common misunderstanding is that implicit bias is inherently unconscious and people are unaware of their own negative feelings, beliefs and behaviors. In fact, research suggests that people are remarkably accurate in perceiving their own levels of implicit bias.

Each component of bias can operate at implicit and explicit levels. At the implicit level, the ABCs arise spontaneously and effortlessly, while ABCs operating at the explicit level are intentional and effortful. For example, the unease someone may feel when encountering a large Black man at night is an emotion triggered at the implicit level. Actively making an effort to replace those feelings of unease with neutral or positive feelings are emotions activated at the explicit level.

Why does implicit bias matter in health care?

Black and white people experience stark differences in treatment during medical interactions. A December 2023 survey from the Kaiser Family Foundation found that nearly 1 in 5 Black people reported experiencing unfair or disrespectful treatment from their health care providers in the past three years because of their race. Only 3% of white respondents reported similar treatment. Researchers have seen similar health inequities across race and ethnicity .

Extensive research over the past two decades indicates racial inequities in patient-provider communication stem largely from implicit prejudice among health care providers . This implicit prejudice manifests during medical interactions with Black patients through a wide range of communication behaviors . These include nonverbal behaviors, or how people move their bodies – such as eye contact and hand movements – and paraverbal behaviors, or how people deliver speech – such as their tone and volume. Both of these behaviors typically occur spontaneously.

For example, providers with higher levels of implicit prejudice tend to talk more and spend less time evaluating Black patients. They also display less positive and more negative affect and more frequently use anxiety-related words like “worry,” “afraid” and “nervous.”

Importantly, Black patients are adept at discerning these subtle negative communication behaviors. “It’s petty, little things,” a Black patient told the Kaiser Family Foundation. “When they call the nurse, they rush to come see the white people. They don’t rush to see the Black people. I think it’s racist.” Consequently, Black patients report lower levels of satisfaction after interacting with providers with higher levels of implicit prejudice.

A common misconception is that implicit prejudice is a key driver of racial disparities in medical treatment. However, current research does not support the idea that providers with higher levels of implicit prejudice treat Black patients worse than white patients. Additionally, more research is needed to determine whether implicit stereotyping from providers – such as automatically associating the idea of being “medically uncooperative” with Black people – would also lead to negative communication behaviors or sub-optimal treatment decisions for Black patients.

What’s wrong with implicit bias training?

Many researchers and clinicians see implicit bias training as an essential component of medical education. However, current programs have shortcomings that undermine their effectiveness.

To understand what typical implicit bias training is like, our ongoing systematic review looks at 77 studies on implicit bias training programs in U.S. health care institutions. Although the majority of the programs were designed to address implicit racial bias, a significant number also addressed other forms of bias including gender identity, sexual orientation and socioeconomic status. Most programs aim to educate health care workers and trainees on implicit bias and how it may affect their patient care, as well as increase awareness about their own biases. Most are single sessions that last about 5.5 hours on average.

However, the design of these training programs does not align with current scientific knowledge about implicit bias.

First, while awareness of one’s biases is a necessary first step to mitigating implicit bias, it alone is not sufficient . Providers must also be personally invested in and have the mental capacity to address their biases.

Second, mitigating implicit bias requires repeated and consistent practice . Implicit bias is like a habit: it is deeply ingrained and operates without intentional control, making it challenging to recognize and change.

Third, training effectiveness is more accurately assessed through patient outcomes, such as care satisfaction, rather than self-reflection or implicit bias scores. Because providers may be concerned about how program facilitators will judge them , they may not provide honest feedback. Furthermore, changes in implicit bias scores do not necessarily result in decreased discriminatory behaviors, making it unclear how these programs can change the quality of care that Black patients experience.

How can health care systems better address implicit bias?

Developing and implementing effective implicit bias training in health care is a scientific endeavor that requires a strong supporting structure.

For example, the clinical and translational science, or CTS, framework , originally designed to help translate discoveries in the lab into treatments in the clinic, could also be applied to implicit bias training. This framework guides scientific progress across incremental stages, starting from confirming the mechanism behind an illness to developing and testing a new treatment for use in the broader community.

This framework is particularly relevant to the development and implementation of evidence-based implicit bias training. Researchers first focus on confirming the mechanisms that underlie implicit bias. Then, after developing and testing implicit bias training programs, they examine its effectiveness across institutions and among diverse health care professionals.

Applying a rigorous scientific process to the development of implicit bias training requires an institution’s long-term commitment, robust support and substantial resources. We believe this investment is a small price to pay for the invaluable progress it promises in reshaping health care for the better for everyone.

Thank you Jill, for that warm welcome. It is such a pleasure seeing you on this stage. I’d like to credit your work, in your former life as Health Minister in Victoria, for all the frontiers you have pushed to improve the health of so many people.

And thank you Dr Nicholson, and Djirri Djirri  for the beautiful welcome to this country, and for setting the scene about how First Nations knowledge and cultures must inform this global meeting.

I also pay my respects to your people, the traditional custodians of this land, and to your elders past, present and emerging. I extend that respect to all First Nations people taking part in this event.

I also acknowledge the many distinguished leaders in health taking part in this opening ceremony, and this important regional meeting. Including Monash University Vice-Chancellor Sharon Pickering; President of the World Health Summit, Axel Pries; and by video – our Foreign Affairs Minister Penny Wong. I would also like to acknowledge WHO Director-General, Tedros Adhanom Ghebreyesus; Dr Saia Piukala , WHO Regional Director for the Western Pacific Region; Ministers and senior representatives of Pacific Island nations and our region. Special mention must also go to Christina Mitchell and Sophia Zounges, both of Monash, who are the 2024 International Co-Presidents of the World Health Summit. You’ve done an outstanding job in bringing this meeting to Melbourne - the first World Health Summit regional meeting to be held in this country.

While it’s an incredibly long list of acknowledgements to open my speech, it shows that both people and collaboration is what drives our health systems, our respective nations, and our communities.

Regional Health There are many challenges across our region that unite our experiences, while each nation also has their own bespoke set of barriers. But the flip side of the challenges, is the set of opportunities that faces each nation too.

From climate change to women’s health. From growing a workforce to fit the needs of a nation, to ballooning costs and economic pressures. The Summit brings together experts and leaders from all sectors of global health to consider the options in science, innovation and policy to address our region’s most critical health challenges. And to collaborate to seize the opportunities as well. I’m conscious that we in Australia have a lot that we can learn as well from the nations and leadership around this room today as well.

When running a health system, there are always competing demands. Duplication and inefficiencies. Difficulty obtaining the right health workforce. Areas where there a public appetite for change, and others where there is not. And in every nation, health inequities as the special needs of some population groups aren’t met. And governments don’t have bottomless pockets, and health care is increasingly expensive.

Australian Policy All of these challenges are felt by the Australian Government and we are working hard to address them. Access to primary health remains a stubborn issue in Australia and we are currently developing and implementing medium to long term reforms to strengthen this part of our system.

Australia’s Medicare system – founded by a Labor legend, Prime Minister Gough Whitlam, was introduced in its first form, in 1975. Just shy of 50 years ago it was forged with the ideals of universal and free healthcare. And these ideals still motivate me, and our Government.

Medicare has cared for generations of people and functioned as a safety net for vulnerable Australians. We know our Medicare system needs reform to strengthen and keep it fit for purpose. We know we need to shape it, in line with a greater focus on preventive health. Good health systems prevent healthcare issues up stream, to save people heartache and keep them healthy and safe, while taking pressure off hospitals and reducing very expensive care.

One of Australia’s great health success stories is cervical cancer. We are on track to be the first country in the world to eliminate cervical cancer, due to our screening and prevention programs. We know cervical cancer is preventable and treatable, but globally it remains one of the most common causes of cancer-related death in women. And through an important partnership across the Pacific region, for which the Australian Government has contributed $12.5 million, Australia is supporting health systems across the region, with co-designed programs that fit the needs of participating nations.  

First Nations Health We are also deeply committed to ending the greatest inequality in health in our nation, by providing better outcomes for First Nations people. Most targets of the National Closing the Gap Agreement continue to be elusive. We recognise how decades – and centuries – of racist policies have compounded disadvantage, reduced life expectancy, and thrown additional diseases at the feet of First Nations people. It’s not good enough.

As a Government, we are pursuing policies and programs which are led by First Nations organisations and people, to address key factors including heart and kidney disease, diabetes and low birth weight babies.

Climate Change Another challenge which faces us all is, of course, climate change. Climate change and health are closely entwined. We have climate change jeopardising the health of so many people in a multitude of new and unfolding ways. And also, globally, our health systems are estimated to contribute around 5 per cent of total greenhouse gas emissions.

The Australian Government recognises these issues.  We have seen tropical diseases shift further south, as heat patterns change. We have seen food insecurity increase as agriculture changes. And we have seen an increase in the number and fury of bushfires in Australia, which has led to more smoke and more respiratory issues.

Our National Health and Climate Strategy, which I was proud to release late last year in Dubai at COP, identifies action for the next five years as well as the coming decades.It also identifies how we can support our regional neighbours as they respond to climate change.

I’m pleased to inform you that we expect to make an announcement tomorrow regarding an international collaboration that will help to decarbonise our global health systems. We need to work together to embed environmental practices in supply chains.

Conclusion As I wrap up, I would like to, once again, emphasise the importance of collaboration at meetings such as this. The partnerships we build across nations – where everyone’s perspective, expertise and cultures are heard and respected – are the basis for regional change and collective improvement. These relationships are the bedrock of global reform and mutual empowerment.

I am a nurse and a unionist. And in the context of this World Health Organisation meeting, I see wisdom in the values of solidity, collectively and unity guiding these discussions and the decisions that come out of it.

By standing together, working together and sharing knowledge, we build a better future, a better community, and a better health system that serves those who need it most. I’m delighted that Australia is hosting this meeting to focus on regional issues and regional solutions.

Sunak to cite Britain’s ‘sicknote culture’ in bid to overhaul fit note system

Prime minister to express concern about ‘over-medicalising’ normal worries with diagnoses as mental health conditions

Rishi Sunak will today claim Britain is suffering from a “sicknote culture”, as he warns there is a risk of “over-medicalising” normal worries by diagnosing them as mental health conditions.

In a speech on how to reduce people being signed off sick from work, the prime minister will say the government is planning to trial getting “work and health professionals” to issue fit notes, shifting away from GPs carrying out this role.

Sunak will say he is concerned about the increase in long-term sickness since the pandemic, largely driven by mental health conditions with 2.8 million people now “economically inactive”.

In relation to mental health, he will say he would “never dismiss or downplay the illnesses people have”, but also argue that there is a need to be “more honest about the risk of over-medicalising the everyday challenges and worries of life”.

His language echoes that of Mel Stride, the work and pensions secretary, who has claimed that too often doctors “label or medicalise” conditions which in the past were seen as “the ups and downs of life”.

As part of a new drive to overhaul the system, Sunak will say one plank of the reforms will be testing whether responsibility for issuing sicknotes should be shifted from “overstretched” GPs to “specialist work and health professionals who have the dedicated time to provide an objective assessment of someone’s ability to work and the tailored support they need to do so”.

The law was changed last year to allow fit notes to be issued by any doctor, nurse, pharmacist, physiotherapist or occupational therapist in addition to GPs, who have traditionally overseen the system. No 10 last night would not clarify whether the government was planning to widen the criteria further to allow non-medical professionals to issue or change fit notes as well.

Sunak will suggest in his speech that GPs are signing people off sick for work “by default”, with 11m fit notes issued last year, of which 94% assessed people as “not fit for work”. No 10 claimed that the fit note system has “opened the floodgates for millions of people to be written off work and into welfare without getting the right support and treatment they might need to help them stay in work”.

However, extracts of the speech released to the media on Thursday night did not address the issue of NHS mental health capacity, with NHS bosses warning “overwhelmed” services have been unable to cope with a big post-Covid increase in people needing help.

Dr Sarah Hughes, chief executive of the mental health charity Mind, said: “We are deeply disappointed that the prime minister’s speech continues a trend in recent rhetoric which conjures up the image of a ‘mental health culture’ that has ‘gone too far’.

“This is harmful, inaccurate and contrary to the reality for people up and down the country. The truth is that mental health services are at breaking point following years of underinvestment, with many people getting increasingly unwell while they wait to receive support.”

She added: “To imply that it is easy both to be signed off work and then to access benefits is deeply damaging. It is insulting to the 1.9 million people on a waiting list to get mental health support, and to the GPs whose expert judgment is being called into question.”

James Taylor, the director of strategy at the disability charity Scope, said: “Much of the current record levels of inactivity are because our public services are crumbling, the quality of jobs is poor and the rate of poverty among disabled households is growing.”

Employment experts said the number of fit notes issued – 11m last year – has not risen since before the pandemic.

“Fit notes are not driving the increase in economic inactivity. They are not responsible for high levels of worklessness from ill health. Economic inactivity has been driven by people who are already out of work and have been for long periods,” said Tony Wilson, the director of the Institute for Employment Studies.

Plans to reform the fit note system were first flagged by Jeremy Hunt in the autumn statement. A new service called WorkWell is due to be launched this year in 15 areas , which is described as “early-intervention work and health support and assessment”. This will be targeted largely at people with mental health and musculoskeletal conditions, with people who have fit notes attending sessions with “work and health coaches” in an effort to get them to return to employment.

The last Labour government introduced “fit notes” rather than “sicknotes” in 2010 to put a new emphasis on doctors certifying what patients are able to do, rather than what they cannot.

Alison McGovern, Labour’s acting shadow work and pensions secretary, said: “We’ve had 14 Tory years, five Tory prime ministers, seven Tory chancellors, and the result is a record number of people locked out of work because they are sick – at terrible cost to them, to business and to the taxpayer paying billions more in spiralling benefits bills.”

Ruth Rankine, the director of the NHS Confederation’s primary care network, said its members had “long advocated for a review of the fit note process which could be more effectively managed through trained professionals that support people back into work”.

But she added: “The deeper problem isn’t the system – it’s that people are sicker than they were and they have more complex healthcare needs. This is why it is vital the government starts treating investment in the NHS as an explicit tool of economic development and also, that the prime minister should lead a national mission for health improvement to shift the focus from simply treating illness to promoting health and wellbeing.”

But when we are stressed, angry, or sad for a long time, our bodies can get sick. It’s like a garden. If you take care of it, it grows beautifully. But if you ignore it, it becomes messy and full of weeds. So, we also need to take care of our feelings and thoughts, just like we take care of our bodies.

In conclusion, our lifestyle, which includes our diet, exercise, sleep, and mental health, plays a significant role in our health. If we want to be healthy, we need to live in a way that is good for our bodies. It’s never too late to start living healthier. Remember, our bodies are our homes. Let’s take care of them. Thank you.

Columbia University has become the epicenter of a growing showdown between student protesters, college administrators and Congress over the war in Gaza and the limits of free speech.

Nicholas Fandos, who covers New York politics and government for The Times, walks us through the intense week at the university. And Isabella Ramírez, the editor in chief of Columbia’s undergraduate newspaper, explains what it has all looked like to a student on campus.
