Moderation in food habits, daily exercise, and balanced work-life can surely make a big difference to our health and body. When a person stays mentally and physically fit, his actions and decisions are more practical and logical and hence he is more successful in life. We need to have a good lifestyle by eating a balanced diet, engaging in daily exercise, breathing fresh air and drinking clean water, maintaining excellent posture, getting adequate sleep and relaxation, and keeping ourselves clean in order to preserve good health. If one is unhealthy, one will have problems with their health or other aspects of life.

Health was once defined as “the ability of an organism to function normally”. However, the definition of health has evolved over time. Being healthy does not necessarily mean physical fitness. Rather, it refers to a state of health, including emotional, mental and social stability. Health is the source of true wealth. A healthy body contains key elements that are essential for the proper functioning of the body. Physical health is the first important factor. Physical well-being is the state of having a healthy body free from disease. People tend to live longer when their bodies are healthy.

A balanced diet is the best way to keep your body healthy. Consuming the right amount of nutrients you need gives you better control over your physical health. Regular exercise is the next very important factor. Don't forget to exercise, even for 10 minutes, to keep your body in shape. Eating junk food all the time is not a good idea. He's been diagnosed with an aggressive form of prostate cancer, and he wrote all about it in an article published at The Washington Post. Dr. Collins, welcome back to ALL THINGS CONSIDERED.

FRANCIS COLLINS: Thanks, Scott. Nice to be with you.

DETROW: I just want to start off - I was really sorry to hear about this diagnosis. How are you doing?

COLLINS: I'm doing OK. Not exactly what I would have hoped to be experiencing right now, but I'm feeling pretty fortunate that, thanks to a lot of surveillance and early detection, I'm in a good position with the surgery that's now coming up soon to expect that this cancer can be cured. And that is a lot different than if I hadn't had all of this attention to make sure that we were watching this closely.

DETROW: What was your first response to the news that not only did you have cancer, but a pretty serious form of it?

COLLINS: It's one of those moments that I guess we all have once in a while, where the words come at you and you realize my life is now different than it was five seconds ago.

DETROW: Yeah.

COLLINS: When they said that this cancer - having been previously a very slow-growing, not-much-to-see-here kind of version - now in the latest biopsies had switched into high gear, something called a Gleason 9 - and it only goes to 10 on that scale - I knew that it was going to be time for action - serious action, and everything that I had maybe planned for the coming weeks, months and years was going to be affected.

DETROW: You mentioned a surgery. Can you tell us about how you're treating this?

COLLINS: So because the cancer does seem to be completely still contained within the prostate, after much searching to be sure of that, then the best way to avoid a downstream outcome, which I really would like to avoid - where the cancer spreads elsewhere, like to bones or to lymph nodes or to liver, lungs or brain - is to actually get the prostate removed. If you want to use an analogy, if the horse is the cancer and the barn is the prostate, we're not just going to get rid of the horse, we're going to get rid of the barn, too, because...

COLLINS: ...We're just not quite sure how big that horse is. So the surgery is what's called a radical prostatectomy. It's done using a robot as well as a surgeon to try to limit the amount of recovery time, but also to be very precise in terms of how one does this, trying not to injure the other structures nearby, like the nerves and the vessels that can be kind of important.

DETROW: Are you worried about the treatment at all? I mean, this is to be blunt, surgery in a pretty intimate part of your body. And I think more broadly with cancer, a lot of people worry about the treatment. How - my dad's around your age. He's battling cancer right now. The treatment's working, but I see how much it takes out of him.

COLLINS: No, Scott, you're quite right, and I am worried about that. The surgery has gotten a lot better, but there's still plenty of opportunities for some men, and maybe that will be me, to end up with incontinence or with impotence because this is, as you say, a rather sensitive part of the male anatomy. And as hard as the surgeons try to try to take out the prostate and the cancer, there's a lot of other things nearby that can be temporarily or sometimes even permanently affected. And one has to have a realistic expectation walking into this that that could happen.

DETROW: Yeah. What are you hoping to gain by being so public about your diagnosis?

COLLINS: You know, maybe it feels like it would be a good thing to have more of a discussion about two things. One is doing this kind of regular early detection for cancers of all sorts - not just prostate cancer, but breast cancer, colon cancer. We fell way behind on a lot of those screens during COVID, for obvious reasons. People just weren't able to necessarily go in and have them done. We need to kick back in there because this is how you detect cancer early and save lives from lots of different areas of the body. So that was one reason.

The other is prostate cancer is particularly one of those that makes men uncomfortable, and I get that. And talking about it openly has not always been easy. And we need to get over that. And particularly men who have reached the point of age 50 or so, really need to think about what they can do to take care of themselves with the kind of screening that I had. I honestly think if I hadn't been in this circumstance of detecting something five years ago that said, ooh, there's something happening, following it closely with what we call active surveillance, and then discovering rather quickly the cancer had taken a bad turn, I probably wouldn't have known about this for several more years, at which point it might have presented by metastatic disease, which could not then be cured. So there's a message here. This kind of surveillance - even though a lot of people put it off, this can be lifesaving. And if there's some little part of my story that gets somebody interested in doing that for themselves, then it's worth being out there in the public.

DETROW: I imagine you've had health problems before over the course of your life, but probably nothing this serious. I'm wondering, being in this position, being on the other side of the conversation with a medical professional, have you learned something about medicine that maybe you didn't realize when you were the expert, when you were the person walking somebody through their options, as opposed to being the person listening to what your options are?

COLLINS: Oh yeah. It's one thing to be imagining what somebody is experiencing as you're giving them news that might be really serious and maybe even life-threatening. It's another to be the person in that spot. As many times as I have thought about what that feels like to be given a diagnosis of cancer, it's just a little different when it is coming at you. And I realize I had some of those same difficulties at first about being able to absorb this and listen carefully and get my head around exactly what was being said and what the consequences would be. So yeah, I think I will - when I next have the chance to be involved in this kind of circumstance as a physician, I'll have a different perspective.

DETROW: You know, you mentioned a couple times in the article where you talked about this news - you mentioned gratitude. Can you tell me what exactly you're grateful for?

COLLINS: I'm grateful that I had the opportunity to take part in this kind of early detection, which should be now available to everybody but hasn't broadly been accomplished in a lot of instances. And we have health inequities in our country that contribute to that. Without that access, I would not be in the place of being able to anticipate that this disease can be cured. I'm grateful to have the expertise of people at the NIH. I'm part of a clinical trial there, and therefore I'm hoping that whatever is learned about me can be widely shared and can teach people about other ways to deal with this disease.

I'm grateful with all the people that are around me who have been so supportive. Scott, I can't tell you that when this came out publicly, I just had this deluge of information and outreach from people, some of whom I haven't been around for a while, people who I have had wonderful, friendly relationships with. But just in the last day or so, all of these people that I considered friends have somehow crossed a line in being very comfortable saying, Francis, I love you. The word love has just come forward in a dramatically positive way. Maybe that's what it takes - is for something like this to get people to say the word. That's pretty touching. That's pretty amazing. I have...

DETROW: Well...

COLLINS: (Laughter) My heart is full of gratitude for that.

DETROW: That's a nice feeling. But let me actually - let me ask about that because, at the same time, I know that it's also a widespread feeling of when you hear that somebody, you know, has gotten a really serious diagnosis or something has really taken a bad turn with their health, sometimes people feel like, gosh, I don't even know what to say. I want to reach out, but I don't know what to say. You're being reached out to right now. What would you tell people who kind of wrestle with that?

COLLINS: Oh, I think what some of the people are reaching out to me with is a great example - to say, you know, I'm really supportive of you. I know you're going through a tough time. I just want you to know I love you. That was just astounding. That's a whole lot better than my thoughts and prayers are with you, which was sort of the old way of saying it. These are very genuine statements of affection that I have very much cherished.

DETROW: Speaking of praying, though, I did want to ask about how you ended your essay. I'm going to read a quote from the end of it. You write...

(Reading) A little over a year ago, when I was praying for a dying friend, I had the experience of receiving a clear and unmistakable message. This has almost never happened to me. It was just this - don't waste your time. You may not have much left.

You are getting treated. You are moving forward. You feel like you have a clear path to beat this cancer, to keep going. But tell me how you're thinking about that sentiment now, what you want to do with the time you've got left.

COLLINS: Well, getting a cancer diagnosis does focus your mind on that a bit, doesn't it?

COLLINS: I am a person of faith, and even before this diagnosis, I had that particular revelation that you just read about that I don't know - quite know how to interpret. And in fact, of course, that's true of all of us. We don't know how much time we have left, and we shouldn't be wasting that time. But it feels more acutely true right now that if there are things I am doing that are not necessarily contributing much to bettering the people around me or the world, maybe those things ought to be deprioritized. And I had to really think about, how do I want to focus my time on giving the most I can to my family, to my friends, to my profession, to my faith, so that I won't look back and say, oh, I really kind of wasted that? That feels like a moment to pay attention to, especially when you have a significant cancer that's just landed in the middle of your life course.

DETROW: Yeah. That's Dr. Francis Collins. He was the director of the National Institutes of Health from 2009 to 2021. Thanks for talking to us, and we'll be thinking about you as you go through with this.

COLLINS: Well, thanks a lot, Scott. It's a pleasure to talk with you today.

Hey, Gen X, Z and millennials: Here's why you may miss out on the great wealth transfer

No one would have guessed that in retirement, Judi and David Koncak would be nearly out of money and unable to leave their kids much more than a pittance.

They’re both college graduates. David, 84, had a successful business that allowed Judi to stop teaching to stay home and raise their two children. They traveled, owned cars and a home, sent their two kids to college and saved for retirement.

“I thought we’d spend our golden years sitting on a beach in Hawaii with Mai Tais, even if in wheelchairs,” Judi Koncak, 83, said.

Instead, her husband had a stroke, surgeries, and prostate cancer – all of which helped deplete their savings and more.

Now she's back at work part time earning about $15 an hour, selling items on Marketplace “for pennies on the dollar,” and turning to the nonprofit, The Senior Source , in Dallas to find help to pay her bills.

With all their savings gone, she suspects their “house is all I’ll have left” for her kids, and she’s “not even sure until we get repairs done if they would even want it. Whatever they can get out of it, I don’t know.”

Health care or inheritance?

If the Koncaks' struggles with health care costs as older adults sound familiar, it’s because they are. Even with insurance, Americans struggle to pay for expenses like premiums, copayments, coinsurance, and uncovered health services.

As a result, the significant wealth transfer from baby boomers to younger generations that researchers have predicted may not be so great after all, as much of older Americans' money goes toward health care.

"One of the biggest factors that drives wealth depletion during retirement is health care costs, including rising out-of-pocket costs for medical treatment and the probability of needing long-term care later in life,” wrote George Schein , technical director for Advanced Consulting Group, in a Nationwide Retirement Institute research report. “The hoped-for transfer of accumulated wealth from boomer parents to their millennial and Gen-X children may ultimately end up in the medical system.”

One-third of Medicare beneficiaries, including more than half under age 65, said it was difficult to afford health care, according to a survey of 7,873 adults from April through July last year by the Commonwealth Fund, a nonprofit focused on health care issues. More than 20%, including over 40% of those under age 65, said health care expenses made it harder for them to afford food and utilities. And they’ve delayed or skipped necessary care .

Kathy Kiersted, 64, said she and her husband, Jose, 62, who live in Arizona, pay more than $1,500 monthly in health premiums. She was laid off from Amazon and had to buy COBRA (Consolidated Omnibus Budget Reconciliation Act) insurance because policies from the Health Insurance Marketplace were even more expensive.

"I even spoke to brokers, but costs were comparable to COBRA but with much higher deductibles," she said. "I really don’t know how people afford health care. It's almost better not to have anything and have Access health care" she said, referring to Arizona's Medicaid program, which pays for most everything if you are very low income.

Kiersted consults part time to help pay for health care. She hasn't been able to find full-time work, she believes, due to her age. Her daughter is getting married soon, but she won’t be able to help pay for the wedding.

“She knows, but as a mom, it’s heartbreaking,” she said.

“We’re the middle class and upper-middle class ,” Kiersted said. “We pay our bills and have a little extra, but we’re all spending a lot in health care (so) that we’re not in a position to help or leave money” to children.

Koncak, of Texas, points out the cumulative cost of small bills.

Copays , “even just at $20 each time, because we go to so many different things – occupational therapy, physical therapy, oncologist,” add up fast, too, she said.   Medicare also doesn’t cover dental or vision, so those are out-of-pocket expenses.

“Every senior needs glasses,” she said. “And dental.”

Dispelling the myth of the “great wealth transfer”

With all baby boomers (born 1946 to 1964) becoming at least 65 by 2030 and owning 52.8% of the wealth in this country, researchers expect a great generational wealth transfer of up to $84 trillion in assets over the next 20 years.

Generation X (1965-1980), millennials (1981-1996) and Gen Z (1997-2012) are expected to inherit $72 trillion of that amount. About $12 trillion is expected to go to charities.

Most millennials expect to inherit at least $350,000 from their parents or other family members, Schein, of the Advanced Consulting Group, wrote in the Nationwide Retirement Institute research report.

But more than 25% of Americans believe paying for long-term care will diminish their children's inheritance, a 2023 Nationwide Retirement Institute survey of 1,439 boomers, Gen Xers and millennials showed.

How expensive is health care?

Since 2000, the inflation-adjusted price of medical care , including services provided, insurance, drugs, and medical equipment, has increased by over 114%, outpacing the 81% rise in overall prices, nonprofit healthcare researcher KFF said .

On average, the annual per-person cost of home care in 2021 was roughly $42,000 (for 30 hours of weekly care at $27 per hour), more than 20% higher than in 2019, AARP said . The average annual cost of nursing home care is more than $108,000 for a private room, more than twice the typical annual income for people 65 or older.

And with longer life expectanc y, an even higher amount of retiree assets will go toward paying for medical care and insurance premiums.

Sixty percent of health care costs come after the age of 65, and if you’re over 85, you use three times more health care services than people between 65 and 75, said Josh Gordon, director of health policy for the Committee for a Responsible Federal Budget, a nonprofit public policy organization.

Can people avoid going broke to leave a legacy?

If you’re still in your 50s and pretty healthy, you might be able to buy long-term care insurance to help mitigate the costliest health care expense, said Justin Stivers, financial adviser and founding attorney at Stivers Law.  If you’re older than that, it may be too expensive.

About six to 10 years before retirement, you could explore guaranteed income using an annuity , said Chad Druvenga, chief executive of insurance brokerage firm CBS Brokerage. An annuity could provide extra monthly income during retirement.

From there, you’d only have Medicare or self-pay options, which is where many boomers like Koncak and Kiersted have ended up already.

Medicaid for low-income people would cover most everything, including long-term care and in some cases, even dental and vision. Each state has its own rules for eligibility and coverage.

“It is so sad... unless you’re in a place where everything is free for you or you have money to pay,” Kiersted said. “But we’re stuck in the middle. There’s no middle class, either you’re upper class or lower class.”

Children from high-net wealth families will come out of this generational wealth transfer with money, but “that’s an upper-class thing, not afforded to every class anymore,” she said.

What are people left to do if they’re already strapped?

In addition to part-time work, the Koncak's 24-year-old grandson moved in, pays a modest rent and helps with his grandfather's care . Koncak already sold her car and has been selling old dishes from her mother and things from her husband's shuttered business. She hasn’t ruled out that she may have to sell her jewelry.

“I don’t want to sell my jewelry,” she said. “I never dreamed I would have to do that, but I might have to. We only have so much money coming in, and we’ve used up our nest egg .”

Kiersted, meanwhile, said “I can’t even plan that far out because it’s so depressing. Health care is the biggest stress on finances for both of us. We’re grateful we can take care now, but the future scares me.”

How isospec plans to change human health with rapid diagnostics.

Isospec Analytics founders Thomas Rizzo, Ahmed Ben Faleh and Stephan Warnke

Any given human blood sample contains as many as 15,000 small biological molecules that can offer invaluable clues to the person’s health – yet fewer than 5% of these molecules can currently be identified. That’s where the Swiss-based start-up Isospec Analytics hopes to make a difference – and save lives in the process.

Isospec, which is today announcing it has raised $1.9 million of pre-seed funding, operates in the field of biomolecular analysis technology. “Understanding disease mechanisms at the earliest possible stage is the most important work scientists can do to develop new treatments,” explains co-founder Ahmed Ben Faleh. “The small molecules in your blood are crucial if we want to do exactly that.”

Ben Faleh met his co-founders, Stephan Warnke and Thomas Rizzo, seven years ago at EPFL, the Swiss technology institute in Lausanne, and the trio have spent much of the intervening period developing the techniques that Isospec now uses. The business itself launched in 2022 as a spin-out from EFPL as the trio began to recognise the commercial applications of their work. “We already have an operational lab and are providing molecular identification services to paying customers across Europe, the US and Japan,” Ben Faleh says of early commercialisation efforts.

Critically, Isospec’s technology enables it to take a blood sample and to identify a much larger number of the molecules it contains – and to do so in hours, rather than in weeks or months, which conventional techniques often require. The technology depends on insights from multiple disciplines, including analytical chemistry, photonics and cryogenic materials initially designed for space applications.

Importantly, however, this is only the first stage in Isospec’s work. To be useful in a clinical or research setting, it also needs to understand what the presence of the newly-identified molecules in a blood sample actually mean for the patient. The company is already working with hospitals and research institutes to examine existing samples and map other data so that it can develop diagnostic tests.

Once Isospec knows what molecules someone suffering from, say, lung cancer will have in their blood, it can test specifically for those molecules. At this point, the technology will be able to provide rapid diagnoses of health problems and diseases at a very early moment in their progression. “We need to find the biomarkers that indicate people are at sick,” Ben Faleh explains.

Getting to a position where its tests operate on a standalone basis will take a number of years – particularly given the need for regulatory approvals – but Isospec expects to be able to offer its technology as a complementary diagnostic tool within the next year or so. It is also exploring alternative commercial models – either working independently as a diagnostic service in its own right, or licensing its intellectual property to pharmaceutical and biotechnology companies building tests of their own.

The goal is not simply to speed up diagnosis, adds Ben Faleh. “When you can see how a disease progresses and the mechanisms for it doing so at this very early moment, that will be the key to developing new drugs and treatments,” he says.

Accelerating Isospec’s own evolution is therefore a priority, with the business now investing in automation technologies and software engineering in order to expand its testing capacity and the speed at which it can work. Hence the need for investment – and today’s pre-seed round.

The money is coming from Founderful, with additional participation from specialist investors Tiny.vc, another.vc and Venture Kick. “Isospec’s value proposition, which offers early access to its cutting-edge research in biomolecular analysis as a service, convinced us of the potential of the technology and the team,” explains Alex Stöckl, a partner at Founderful, who has taken a seat on the company’s board.

As for the founders themselves, they see an opportunity to do much more than create a successful healthcare business. “This is something that goes beyond product development or commercial objectives,” says Ben Faleh. “If this works and we can identify those critical biomarkers for diseases such as cancer, Alzheimer’s or diabetes, say, we really can change the lives of millions of people.”

Many Patients Don’t Survive End-Stage Poverty

Dr. Ryan is an associate physician at the University of California, San Francisco, department of medicine.

He has an easy smile, blue eyes and a life-threatening bone infection in one arm. Grateful for treatment, he jokes with the medical intern each morning. A friend, a fellow doctor, is supervising the man’s care. We both work as internists at a public hospital in the medical safety net , a loose term for institutions that disproportionately serve patients on Medicaid or without insurance. You could describe the safety net in another way, too, as a place that holds up a mirror to our nation.

What is reflected can be difficult to face. It’s this: After learning that antibiotics aren’t eradicating his infection and amputation is the only chance for cure, the man withdraws, says barely a word to the intern. When she asks what he’s thinking, his reply is so tentative that she has to prompt him to repeat himself. Now with a clear voice, he tells her that if his arm must be amputated, he doesn’t want to live. She doesn’t understand what it’s like to survive on the streets, he continues. With a disability, he’ll be a target — robbed, assaulted. He’d rather die, unless, he says later, someone can find him a permanent apartment. In that case, he’ll proceed with the amputation.

The psychiatrists evaluate him. He’s not suicidal. His reasoning is logical. The social workers search for rooms, but in San Francisco far more people need long-term rehousing than the available units can accommodate. That the medical care the patient is receiving exceeds the cost of a year’s rent makes no practical difference. Eventually, the palliative care doctors see him. He transitions to hospice and dies.

A death certificate would say he died of sepsis from a bone infection, but my friend and I have a term for the illness that killed him: end-stage poverty. We needed to coin a phrase because so many of our patients die of the same thing.

Safety-net hospitals and clinics care for a population heavily skewed toward the poor, recent immigrants and people of color. The budgets of these places are forever tight . And anyone who works in them could tell you that illness in our patients isn’t just a biological phenomenon. It’s the manifestation of social inequality in people’s bodies.

Neglecting this fact can make otherwise meticulous care fail. That’s why, on one busy night, a medical student on my team is scouring websites and LinkedIn. She’s not shirking her duties. In fact, she’s one of the best students I’ve ever taught.

This week she’s caring for a retired low-wage worker with strokes and likely early dementia who was found sleeping in the street. He abandoned his rent-controlled apartment when electrolyte and kidney problems triggered a period of severe confusion that has since been resolved. Now, with little savings, he has nowhere to go. A respite center can receive patients like him when it has vacancies. The alternative is a shelter bed. He’s nearly 90 years old.

Medical textbooks usually don’t discuss fixing your patient’s housing. They seldom include making sure your patient has enough food and some way to get to a clinic. But textbooks miss what my med students don’t: that people die for lack of these basics.

People struggle to keep wounds clean. Their medications get stolen. They sicken from poor diet, undervaccination and repeated psychological trauma. Forced to focus on short-term survival and often lacking cellphones, they miss appointments for everything from Pap smears to chemotherapy. They fall ill in myriad ways — and fall through the cracks in just as many.

Early in his hospitalization, our retired patient mentions a daughter, from whom he’s been estranged for years. He doesn’t know any contact details, just her name. It’s a long shot, but we wonder if she can take him in.

The med student has one mission: find her.

I love reading about medical advances. I’m blown away that with a brain implant, a person who’s paralyzed can move a robotic arm and that surgeons recently transplanted a genetically modified pig kidney into a man on dialysis. This is the best of American innovation and cause for celebration. But breakthroughs like these won’t fix the fact that despite spending the highest percentage of its G.D.P. on health care among O.E.C.D. nations, the United States has a life expectancy years lower than comparable nations—the U.K. and Canada— and a rate of preventable death far higher .

The solution to that problem is messy, incremental, protean and inglorious. It requires massive investment in housing, addiction treatment, free and low-barrier health care and social services. It calls for just as much innovation in the social realm as in the biomedical, for acknowledgment that inequities — based on race, class, primary language and other categories — mediate how disease becomes embodied. If health care is interpreted in the truest sense of caring for people’s health, it must be a practice that extends well beyond the boundaries of hospitals and clinics.

Meanwhile, on the ground, we make do. Though the social workers are excellent and try valiantly, there are too few of them , both in my hospital and throughout a country that devalues and underfunds their profession. And so the medical student spends hours helping the family of a newly arrived Filipino immigrant navigate the health insurance system. Without her efforts, he wouldn’t get treatment for acute hepatitis C. Another patient, who is in her 20s, can’t afford rent after losing her job because of repeated hospitalizations for pancreatitis — but she can’t get the pancreatic operation she needs without a home in which to recuperate. I phone an eviction defense lawyer friend; the young woman eventually gets surgery.

Sorting out housing and insurance isn’t the best use of my skill set or that of the medical students and residents, but our efforts can be rewarding. The internet turned up the work email of the daughter of the retired man. Her house was a little cramped with his grandchildren, she said, but she would make room. The medical student came in beaming.

In these cases we succeeded; in many others we don’t. Safety-net hospitals can feel like the rapids foreshadowing a waterfall, the final common destination to which people facing inequities are swept by forces beyond their control. We try our hardest to fish them out, but sometimes we can’t do much more than toss them a life jacket or maybe a barrel and hope for the best.

I used to teach residents about the principles of internal medicine — sodium disturbances, delirium management, antibiotics. I still do, but these days I also teach about other topics — tapping community resources, thinking creatively about barriers and troubleshooting how our patients can continue to get better after leaving the supports of the hospital.

When we debrief, residents tell me how much they struggle with the moral dissonance of working in a system in which the best medicine they can provide often falls short. They’re right about how much it hurts, so I don’t know exactly what to say to them. Perhaps I never will.

Francis Collins: Why I’m going public with my prostate cancer diagnosis

I served medical research. now it’s serving me. and i don’t want to waste time..

Over my 40 years as a physician-scientist, I’ve had the privilege of advising many patients facing serious medical diagnoses. I’ve seen them go through the excruciating experience of waiting for the results of a critical blood test, biopsy or scan that could dramatically affect their future hopes and dreams.

But this time, I was the one lying in the PET scanner as it searched for possible evidence of spread of my aggressive prostate cancer . I spent those 30 minutes in quiet prayer. If that cancer had already spread to my lymph nodes, bones, lungs or brain, it could still be treated — but it would no longer be curable.

Why am I going public about this cancer that many men are uncomfortable talking about? Because I want to lift the veil and share lifesaving information, and I want all men to benefit from the medical research to which I’ve devoted my career and that is now guiding my care.

Five years before that fateful PET scan, my doctor had noted a slow rise in my PSA, the blood test for prostate-specific antigen. To contribute to knowledge and receive expert care, I enrolled in a clinical trial at the National Institutes of Health, the agency I led from 2009 through late 2021.

At first, there wasn’t much to worry about — targeted biopsies identified a slow-growing grade of prostate cancer that doesn’t require treatment and can be tracked via regular checkups, referred to as “active surveillance.” This initial diagnosis was not particularly surprising. Prostate cancer is the most commonly diagnosed cancer in men in the United States, and about 40 percent of men over age 65 — I’m 73 — have low-grade prostate cancer . Many of them never know it, and very few of them develop advanced disease.

Why am I going public about this cancer that many men are uncomfortable talking about? Because I want to lift the veil and share lifesaving information.

But in my case, things took a turn about a month ago when my PSA rose sharply to 22 — normal at my age is less than 5. An MRI scan showed that the tumor had significantly enlarged and might have even breached the capsule that surrounds the prostate, posing a significant risk that the cancer cells might have spread to other parts of the body.

New biopsies taken from the mass showed transformation into a much more aggressive cancer. When I heard the diagnosis was now a 9 on a cancer-grading scale that goes only to 10, I knew that everything had changed.

Thus, that PET scan, which was ordered to determine if the cancer had spread beyond the prostate, carried high significance. Would a cure still be possible, or would it be time to get my affairs in order? A few hours later, when my doctors showed me the scan results, I felt a rush of profound relief and gratitude. There was no detectable evidence of cancer outside of the primary tumor.

Later this month, I will undergo a radical prostatectomy — a procedure that will remove my entire prostate gland. This will be part of the same NIH research protocol — I want as much information as possible to be learned from my case, to help others in the future.

While there are no guarantees, my doctors believe I have a high likelihood of being cured by the surgery.

My situation is far better than my father’s when he was diagnosed with prostate cancer four decades ago. He was about the same age that I am now, but it wasn’t possible back then to assess how advanced the cancer might be. He was treated with a hormonal therapy that might not have been necessary and had a significant negative impact on his quality of life.

Because of research supported by NIH, along with highly effective collaborations with the private sector, prostate cancer can now be treated with individualized precision and improved outcomes.

As in my case, high-resolution MRI scans can now be used to delineate the precise location of a tumor. When combined with real-time ultrasound, this allows pinpoint targeting of the prostate biopsies. My surgeon will be assisted by a sophisticated robot named for Leonardo da Vinci that employs a less invasive surgical approach than previous techniques, requiring just a few small incisions.

Advances in clinical treatments have been informed by large-scale, rigorously designed trials that have assessed the risks and benefits and were possible because of the willingness of cancer patients to enroll in such trials.

I feel compelled to tell this story openly. I hope it helps someone. I don’t want to waste time.

If my cancer recurs, the DNA analysis that has been carried out on my tumor will guide the precise choice of therapies. As a researcher who had the privilege of leading the Human Genome Project , it is truly gratifying to see how these advances in genomics have transformed the diagnosis and treatment of cancer.

I want all men to have the same opportunity that I did. Prostate cancer is still the No. 2 cancer killer among men. I want the goals of the Cancer Moonshot to be met — to end cancer as we know it. Early detection really matters, and when combined with active surveillance can identify the risky cancers like mine, and leave the rest alone. The five-year relative survival rate for prostate cancer is 97 percent, according to the American Cancer Society , but it’s only 34 percent if the cancer has spread to distant areas of the body.

But lack of information and confusion about the best approach to prostate cancer screening have impeded progress. Currently, the U.S. Preventive Services Task Force recommends that all men age 55 to 69 discuss PSA screening with their primary-care physician, but it recommends against starting PSA screening after age 70.

Other groups, like the American Urological Association , suggest that screening should start earlier, especially for men with a family history — like me — and for African American men, who have a higher risk of prostate cancer. But these recommendations are not consistently being followed.

Our health-care system is afflicted with health inequities. For example, the image-guided biopsies are not available everywhere and to everyone. Finally, many men are fearful of the surgical approach to prostate cancer because of the risk of incontinence and impotence, but advances in surgical techniques have made those outcomes considerably less troublesome than in the past. Similarly, the alternative therapeutic approaches of radiation and hormonal therapy have seen significant advances.

A little over a year ago, while I was praying for a dying friend, I had the experience of receiving a clear and unmistakable message. This has almost never happened to me. It was just this: “Don’t waste your time, you may not have much left.” Gulp.

Having now received a diagnosis of aggressive prostate cancer and feeling grateful for all the ways I have benefited from research advances, I feel compelled to tell this story openly. I hope it helps someone. I don’t want to waste time.
