PROGRAM OF THE CHEMISTS LICENSURE EXAMINATION IN NCR, CDO, BAGUIO AND CEBU   ON OCTOBER 27 and 28, 2021

PROGRAM OF THE CHEMICAL TECHNICIANS LICENSURE EXAMINATION IN NCR, CDO, BAGUIO AND CEBU   ON OCTOBER 29, 2021

THIS EXAMINATION WILL BE COMPUTERIZED

• Check or verify your school/building assignments at the PRC official website (www.prc.gov.ph) or through the Notices/Announcements to be posted at the PRC premises two (2) to three (3) working days before the examination. Bring your NOTICE OF ADMISSION when you verify your school/building assignment. Visit your school/building assignment prior to the day of the examination.
• Report to your school/building assignment before 6:00 A.M. on the first day of the examination to verify your room and seat number. Be punctual. Late examinees will not be admitted.
• Examinees are required to wear the following attire every examination day: Male Examinees – White polo or shirts with collar, tucked-in (without any seal, logo or mark) Decent pants or slacks Female Examinees – White blouses or shirt with collar (without any seal, logo or mark), Decent pants or slacks/skirts
• Bring the following on the days of the examination: a. Notice of Admission b. Official Receipt c. Two or more pencils (No. 2) d. Ballpen with BLACK ink only e. One (1) piece Long Brown Envelope f. One (1) piece Long Transparent (non-colored) Plastic Envelope (for keeping your valuables and other allowed items) g.  Calculators with the following features only – addition, subtraction, multiplication, division, trigonometric function, logarithm, and x y . h. Health Forms (Pursuant to Memorandum No. 68, series of 2020) i. Negative RT-PCR Test Results, if applicable, or Certificate of Quarantine (Pursuant to Memorandum No. 68, series of 2020)

NOTE: ONLY ONE CALCULATOR SHALL BE ALLOWED, SUBJECT TO THOROUGH INSPECTION BY THE EXAMINATION PERSONNEL.

• The following items are strictly prohibited inside the examination premises/rooms: 1. Calculators which are programmable or with embedded functions especially CASIO FX991ES and CASIO FX-991ES plus; 2. Books, notes, review materials, and other printed materials; 3. Apple, Samsung and other smart watches, cellular phones, bluetooth, ear plugs, transmitters, portable computers and other electronic gadgets/devices;  4. Bags of any kind (ladies bag, shoulder bags, attache’ case, backpacks, etc.); 5. Other examination aids not stated in the program.
• Read carefully and follow the instructions on your NOTICE OF ADMISSION and EXAMINEES GUIDE .
• Per Memorandum No. 24 (s.2020), the following precautionary measures and protocols shall be observed and implemented during the conduct of licensure examination such as, but not limited to: i. All examinees shall be required to wear and bring their own protective face mask and alcohol-based sanitizers. For identity verification purposes and pre-examination checking of examination paraphernalia, the face mask shall be required to be properly taken off temporarily to show the examinees’ face and the inner part of the facemask. Face masks during the progress of the exam shall not be allowed to be taken off. ii. All examination personnel shall be provided with extra protective face masks or face shield and latex gloves. iii. All examinees and examination personnel shall be required to queue upon entry with a one-meter distance radius from other examinees. iv. Body temperature shall be taken through thermal scanners prior entry to the testing venue premises. Those with fever, cough or colds shall not be allowed entry to the testing venues. Affected examinees shall be allowed, however, to take the next licensure exam without additional cost. v. Physical/social distancing among and between examinees and examination personnel during the entirety of the exam and during their entire stay at the testing venue shall be observed. Congregation, gathering or assembly within the examination rooms, hallways or any area within the testing venue shall be strictly prohibited. vi. All examinees and examination personnel shall not be allowed to go outside the examination room during break time and lunch time. They shall be advised to bring their own packed lunch and snacks with them to be placed under their seats, and to be taken out only during break time. vii. Proper hand hygiene shall be observed particularly every after use of the restrooms. viii. Prior to entry to the examination rooms, examinees shall be required to sanitize their hands and their plastic envelopes with their sanitizers or alcohol. ix. Prior to the distribution of the Test Questionnaires/Booklet, examinees shall again be required to sanitize their hands using their alcohol or hand sanitizers. Hand sanitizers thereafter shall be placed in their transparent envelopes and shall be deposited or placed at the designated area of the examination room. The same process shall be done every start of each examination subject. x. The testing venues shall be required to conduct thermal scanning procedure including the provision of sanitizing footbath and for their security personnel to implement proper COVID-19 precautionary measures.
• In addition to the above, pursuant to Memorandum No. 68 (s. 2020), the examinees are required to submit RT-PCR Test Results, if falling under the groups stated in DOH Memorandum 2020-258A, or Certificate of Quarantine. ROOM ASSIGNMENTS: December 2023 Chemical Technician Licensure Exam (CTLE)

The Professional Regulation Commission (PRC) releases the room assignments for the December 2023 Chemical Technician Licensure Exam (CTLE)  a few days before the examination.

This year’s Chemical Technician Licensure Exam (CTLE) will be conducted on December 15, 2023.

Room Assignments

Below are room assignments for the December 2023 Chemical Technician Licensure Exam (CTLE) :

• Cagayan De Oro
• Cebu – PWD
• Rosales, Pangasinan

Examinees shall report before 6:30 in the morning on the said date because latecomers will not be admitted.

Things to Bring Examination day

Here are the things to bring during the examination proper:

• Notice of Admission
• Official Receipt
• One (1) piece of metered-stamped window mailing envelope
• Two (2) or more pencils (NO. 2)
• Ball pens (black ink only)
• One (1) piece long brown envelope
• One (1) piece long transparent/plastic envelope (for keeping your valuables and other allowed items)
• Health Forms (Pursuant to Memorandum No. 68, series of 2020)
• Negative RT-PCR Test Results, if applicable, or Certificate of Quarantine (Pursuant to Memorandum No. 68, series of 2020)

Dress Code on Examination Day

Here are the specified dress codes from PRC:

• For male examinees, tucked-in white polo shirt with collar (without any seal, logo, or mark); decent pants or slacks
• For female examinees, tucked-in white blouse or shirt with collar (without any seal, logo, or mark); decent pants or slacks

Prohibited Items Inside the Exam Rooms:

• Books, notes, review materials, and other printed materials containing coded information or formulas
• Calculators which are programmable or with embedded functions, especially CASIO FX991ES and CASIO FX-991ES plus
• Apple, Samsung and other smart watches, cellular phones, ear plugs, transmitters, portable computers, Bluetooth and other electronic devices which may be used for communication purposes;
• Bags of any kind (place your documents inside the transparent/plastic envelope)
• Other examination aides not stated in this program

All personal belongings of the examinees shall be surrendered to the proctor and shall be placed in a secured space within the examination room.

The rules on the conduct of bodily search, an inspection of personal belongings of the examinees, and the seizure/confiscation of prohibited items during the licensure exam (Annex A of PRC Memorandum Order No. 57, s. 2020) shall be strictly observed.

The exams will be conducted in different parts of the Philippines, including NCR, Baguio, Cagayan de Oro, Cebu, Davao, Iloilo, Koronadal, Legazpi, Lucena, Pampanga, Tacloban, and Zamboanga. The exam will take place on October 17 to 18, 2022 for chemists examinees and October 19, 2022 for chemical technicians examinees.

The exam is administered by the Board of Chemists headed by the chairman ADORACION P. RESURRECCION, and members SOLEDAD S. CASTAÑEDA and MA. THERESA C. CAYTON.

• Calculators with the following features only – addition, subtraction, multiplication, division, trigonometric function, logarithm and x y .

NOTE: ONLY ONE CALCULATOR WITH THE AFOREMENTIONED FEATURES SHALL BE ALLOWED INSIDE THE EXAMINATION ROOM. ALL CALCULATORS SHALL BE THOROUGHLY INSPECTED BY THE EXAMINATION PERSONNEL. 

• Books, notes, review materials, and other printed materials containing coded data/information/formula 
• PROGRAMMABLE CALCULATORS, especially CASIO FX991ES and CASIO FX-991ES plus
• CELLULAR PHONES, EARPLUGS, TRANSMITTERS, PORTABLE COMPUTERS, SMART WATCHES, BLUETOOTH and other electronic gadgets/devices which may be used for communication purposes.
• Any type of bag
• Other examination aides not stated
• For male examinees, a tucked-in white polo shirt with collar (without any seal, logo, or mark) paired with decent pants or slacks.
• For female examinees, a tucked-in white blouse or shirt with collar (without any seal, logo, or mark) paired with decent pants or slacks.

During the PRC licensure exams, the following precautionary health and safety rules must be followed at all times:

• Observe physical distancing of at least two (2) meters between examinees.
• Examinees shall be restricted to their assigned seat;
• Wear face mask (at least 3-ply surgical mask, preferably N95 mask) and face shield at all times; 
• Bring 70% ethyl alcohol for hand disinfection;
• Avoid close contacts like “beso-beso”, hugging, handshake, and directly touching other persons;
• Avoid touching one’s eyes, nose, and mouth;
• When sneezing and/or coughing, facial tissues must be used to wipe the nose and mouth areas. Dispose of used facial tissues properly; and
• Avoid spitting in public, on floors, and along corridors.

PRC Room Assignments for October 2022 Chemical Technician Licensure Examination

Manila, Philippines – The Professional Regulation Commission (PRC) has announced the room assignments for the October 19, 2022 Chemical Technicians Licensure Examination for testing locations in Manila/National Capital Region (NCR), Baguio, Cagayan de Oro, Cebu, Davao, Iloilo, Legazpi, Lucena, Koronadal, Pampanga, Tacloban, and Zamboanga.

The licensure exams for Chemists were administered by PRC and the Board of Chemistry headed by its Chairman, Ms. Adoracion P. Resurreccion, and its members, Ms. Soledad S. Castañeda and Ms. Ma. Theresa C. Cayton.

Chemical Technicians Licensure Exam Coverage

Examination attire.

Male examinees are required to wear a white polo shirt with a collar (without any seal, logo, or mark) and decent pants or slacks.

For female examinees, a white blouse or shirt with collar (without any seal, logo, or mark), decent skirt or slacks, and pony-tailed long hair.

What to bring?

Don’t forget the following on examination day:

• Notice of Admission (NOA)
• Official Receipt
• Two (2) or more pencils (No. 1 or No. 2)
• Ball pens with BLACK ink only
• One (1) piece long brown envelope
• One (1) piece long transparent/plastic envelope (for keeping your valuables and other allowed items)
• Health forms
• Negative RT-PCR Test Results, if applicable, or Certificate of Quarantine

Examinees are not allowed to bring books, notes, or similar items in the examination room. Electronic gadgets such as Apple and Samsung products are not allowed either. Bags of any kind are also not allowed inside the examination premises.

Precautionary Measures

The following precautionary measures shall be observed during the conduct of the October 2022 Chemical Technicians Licensure Exam:

• Wearing of face mask and face shield
• Sanitizing using alcohol and hand-based sanitizers whenever necessary
• Physical Distancing of one (1) meter at all times
• Those with fever and cold are not allowed to take the exam
• Observe proper hygiene at all times

ROOM ASSIGNMENTS: December 2021 RadTech and X-Ray Tech Licensure Exams

MANILA, Philippines – The Professional Regulation Commission (PRC) Board of Radiologic Technology releases the list of room assignments for the December 2021 RadTech and X-Ray Tech licensure exams few days before the exams.

The December 2021 RadTech board exams will be conducted on December 20-21, 2021 at PRC testing centers located at Metro Manila, Baguio, Cagayan de Oro, Cebu, Davao, Iloilo, Legaspi, Lucena, Rosales, Tacloban, Tuguegarao and Zamboanga.

The board of Radiologic Technology is headed by the Officer-in-Charge, Mr. Reynaldo Apolonio S. Tisado and its members, Dr. Orestes P. Monzon, Mr. Bayani C. San Juan and Ms. Ma. Jesette B. Canales.

Room Assignments for December 2021 Radiologic Technologists Board Exams

Examinees are advised to verify the room assignments to the links below or through the notices and announcements to be posted at the PRC official website. Here is the list of room assignments for the December 2021 RadTech and X-Ray Tech licensure exams.

Examinees shall report before 5:30 in the morning on the said date because late comers will not be admitted.

What to bring on the day of exams?

Here are the things to bring during the examination proper:

• Notice of Admission
• Official Receipt
• One (1) piece of metered-stamped window mailing envelope
• Two (2) or more pencils (No. 2)
• Ball pens (black ink only)
• One (1) piece long brown envelope
• One (1) piece long transparent/plastic envelope (for keeping your valuables and other allowed items)
• Any licensed Physician (government or private physician)
• Municipal Health Officer, Provincial Health Officer or City Health Officer
• Barangay Officer visiting the examinee’s residence attesting that he/she has been in quarantine for 14 days;
• Informed Consent Form and Health Declaration Form
• Face mask preferably N95 and Face Shield (to be worn at all times)
• 70% ethyl alcohol for regular hand disinfection
• Pre-packed meal/snacks and drinks

PRC said that examinees shall submit the Post Examination Health Surveillance Form to the concerned Regional Office 14 days after the conduct of exams.

What if I failed to take the exams?

In the event that the examinee failed to take the exams due to below reasons, examinee will be allowed to take on the next schedule subject to the following requirements:

• Health Related Reasons: The examinee must submit a letter to the commission or respective board with notarized Medical Certificate as attachment
• Travel Restriction Reasons: The examinee must submit a letter to the commission or respective board with Official letter or restriction from the concerned LGU

What to wear on examination day?

Here are the specified dress codes from PRC:

• For male examinees, white polo shirt with collar (without any seal, logo, or mark); decent pants or slacks
• For female examinees, white blouse or shirt with collar (without any seal, logo, or mark); decent pants or slacks

Not allowed during exams

• Books, notes, review materials, and other printed materials containing coded information or formulas
• Calculators which are programmable or with embedded functions especially CASIO FX991ES and CASIO FX-991ES plus
• Apple, Samsung and other smart watches, cellular phones, ear plugs, transmitters, portable computers, Bluetooth and other electronic devices which may be used for communication purposes
• Bags of any kind

Board exams during the COVID-19 pandemic

The following precautionary health and safety guidelines shall be observed at all times during the conduct of the PRC licensure exams:

• Wearing of face mask and face shield. Examinees are also required to bring alcohol-based sanitizers.
• Examination personnel shall be provided with face mask, face shield and latex gloves.
• Observe physical distancing by maintaining 1-meter distance from one another.
• Body temperature will be checked using thermal scanner prior to entry at examination premises. Those with fever, colds or cough are not allowed to take the exams. Their payment will not be forfeited and can be used in the next licensure exam.
• Examinees are advised to bring their food as they will not be allowed to exit the examination room during break time and lunch time.
• Examinees shall sanitize before entering the examination room, after using the rest room and before distribution of test questionnaires.

PRC added that they will not be responsible of any lost personal belongings.
