Nutrition generally refers to the macro- and micro-nutrients essential for survival, but we do not simply eat nutrition. Instead, we eat animal- and plant-based foods without always being conscious of its nutritional value. Furthermore, various cultural factors influence and shape our taste, preferences, taboos and practices towards preparing and consuming food as a meal and diet. Biogerontological understanding of ageing has identified food as one of the three foundational pillars of health and survival. Here we address the issues of nutrition, food and diet by analyzing the biological importance of macro- and micro-nutrients including hormetins, discussing the health claims for various types of food, and by reviewing the general principles of healthy dietary patterns, including meal timing, caloric restriction, and intermittent fasting. We also present our views about the need for refining our approaches and strategies for future research on nutrition, food and diet by incorporating the molecular, physiological, cultural and personal aspects of this crucial pillar of health, healthy ageing and longevity.

1. Introduction

The terms nutrition, food and diet are often used interchangeably. However, whereas nutrition generally refers to the macro- and micro-nutrients essential for survival, we do not simply eat nutrition, which could, in principle, be done in the form of a pill. Instead, we eat food which normally originates from animal- and plant-based sources, without us being aware of or conscious of its nutritional value. Even more importantly, various cultural factors influence and shape our taste, preferences, taboos and practices towards preparing and consuming food as a meal and diet [ 1 ]. Furthermore, geo-political-economic factors, such as governmental policies that oversee the production and consumption of genetically modified foods, geological/climatic challenges of growing such crops in different countries, and the economic affordability of different populations for such foods, also influence dietary habits and practices [ 2 , 3 ]. On top of all this lurks the social evolutionary history of our species, previously moving towards agriculture-based societies from the hunter-gatherer lifestyle, now becoming the consumers of industrially processed food products that affect our general state of health, the emergence of diseases, and overall lifespan [ 1 , 4 ]. The aim of this article is to provide a commentary and perspective on nutrition, food and diet in the context of health, healthy ageing and longevity.

Biogerontological understanding of ageing has identified food as one of the three foundational pillars of health and survival. The other two pillars, especially in the case of human beings, are physical exercise and socio-mental engagement [ 5 , 6 , 7 ]. A huge body of scientific and evidence-based information has been amassed with respect to the qualitative and quantitative nature of optimal nutrition for human health and survival. Furthermore, a lot more knowledge has developed regarding how different types of foods provide different kinds of nutrition to different extents, and how different dietary practices have either health-beneficial or health-harming effects.

Here we endeavor to address these issues of nutrition, food and diet by analyzing the biological importance of macro- and micro-nutrients, and by discussing the health-claims about animal-based versus plant-based foods, fermented foods, anti-inflammatory foods, functional foods, foods for brain health, and so on. Finally, we discuss the general principles of healthy dietary patterns, including the importance of circadian rhythms, meal timing, chronic caloric restriction (CR), and intermittent fasting for healthy ageing and extended lifespan [ 8 , 9 ]. We also present our views about the need for refining our approaches and strategies for future research on nutrition, food and diet by incorporating the molecular, physiological, cultural and personal aspects of this crucial pillar of health, healthy ageing and longevity.

2. Nutrition for Healthy Ageing

The science of nutrition or the “nutritional science” is a highly advanced field of study, and numerous excellent books, journals and other resources are available for fundamental information about all nutritional components [ 10 ]. Briefly, the three essential macronutrients which provide the basic materials for building biological structures and for producing energy required for all physiological and biochemical processes are proteins, carbohydrates and lipids. Additionally, about 18 micronutrients, comprised of minerals and vitamins, facilitate the optimal utilization of macronutrients via their role in the catalysis of numerous biochemical processes, in the enhancement of their bioavailability and absorption, and in the balancing of the microbiome. Scientific literature is full of information about almost all nutritional components with respect to their importance and role in basic metabolism for survival and health throughout one’s life [ 10 ].

In the context of ageing, a major challenge to maintain health in old age is the imbalanced nutritional intake resulting into nutritional deficiency or malnutrition [ 11 , 12 ]. Among the various reasons for such a condition is the age-related decline in the digestive and metabolic activities, exacerbated by a reduced sense of taste and smell and worsening oral health, including the ability to chew and swallow [ 13 , 14 ]. Furthermore, an increased dependency of the older persons on medications for the management or treatment of various chronic conditions can be antagonistic to certain essential nutrients. For example, long term use of metformin, which is the most frequently prescribed drug against Type 2 diabetes, reduces the levels of vitamin B12 and folate in the body [ 15 , 16 ]. Some other well-known examples of the drugs used for the management or treatment of age-related conditions are cholesterol-lowering medicine statin which can cause coenzyme Q10 levels to be too low; various diuretics (water pills) can cause potassium levels to be too low; and antacids can decrease the levels of vitamin B12, calcium, magnesium and other minerals [ 15 , 16 ]. Thus, medications used in the treatment of chronic diseases in old age can also be “nutrient wasting” or “anti-nutrient” and may cause a decrease in the absorption, bioavailability and utilization of essential micronutrients and may have deleterious effects to health [ 11 ]. In contrast, many nutritional components have the potential to interact with various drugs leading to reduced therapeutic efficacy of the drug or increased adverse effects of the drug, which can have serious health consequences. For example, calcium in dairy products like milk, cheese and yoghurt can inhibit the absorption of antibiotics in the tetracycline and quinolone class, thus compromising their ability to treat infection effectively. Some other well-known examples of food sources which can alter the pharmacokinetics and pharmacodynamics of various drugs are grape fruits, bananas, apple juice, orange juice, soybean flour, walnuts and high-fiber foods (see: https://www.aarp.org/health/drugs-supplements/info-2022/food-medication-interaction.html (accessed on 13 November 2022)).

It is also known that the nutritional requirements of older persons differ both qualitatively and quantitatively from young adults [ 11 ]. This is mainly attributed to the age-related decline in the bioavailability of nutrients, reduced appetite, also known as ‘anorexia of ageing,’ as well as energy expenditure [ 12 , 17 , 18 ]. Therefore, in order to maintain a healthy energy balance, the daily uptake of total calories may need to be curtailed without adversely affecting the nutritional balance. This may be achieved by using nutritional supplements with various vitamins, minerals and other micronutrients, without adding to the burden of total calories [ 12 , 17 , 18 ]. More recently, the science of nutrigenomics (how various nutrients affect gene expression), and the science of nutrigenetics (how individual genetic variations respond to different nutrients) are generating novel and important information on the role of nutrients in health, survival and longevity.

3. Food for Healthy Ageing

The concept of healthy ageing is still being debated among biogerontologists, social-gerontologists and medical practioners. It is generally agreed that an adequate physical and mental independence in the activities of daily living can be a pragmatic definition of health in old age [ 7 ]. Thus, healthy ageing can be understood as a state of maintaining, recovering and enhancing health in old age, and the foods and dietary practices which facilitate achieving this state can be termed as healthy foods and diets.

From this perspective, although nutritional requirements for a healthy and long life could be, in principle, fulfilled by simply taking macro- and micro-nutrients in their pure chemical forms, that is not realistic, practical, attractive or acceptable to most people. In practice, nutrition is obtained by consuming animals and plants as sources of proteins, carbohydrates, fats and micronutrients. There is a plethora of tested and reliable information available about various food sources with respect to the types and proportion of various nutrients present in them. However, there are still ongoing discussions and debates as to what food sources are best for human health and longevity [ 19 , 20 ]. Often such discussions are emotionally highly charged with arguments based on faith, traditions, economy and, more recently, on political views with respect to the present global climate crisis and sustainability.

Scientifically, there is no ideal food for health and longevity. Varying agricultural and food production practices affect the nutritional composition, durability and health beneficial values of various foods. Furthermore, the highly complex “science of cooking” [ 21 ], evolved globally during thousands of years of human cultural evolution, has discovered the pros and cons of food preparation methods such as soaking, boiling, frying, roasting, fermenting and other modes of extracting, all with respect to how best to use these food sources for increasing the digestibility and bioavailability of various nutrients, as well as how to eliminate the dangers and toxic effects of other chemicals present in the food.

The science of food preparation and utilization has also discovered some paradoxical uses of natural compounds, especially the phytochemicals such as polyphenols, flavonoids, terpenoids and others. Most of these compounds are produced by plants as toxins in response to various stresses, and as defenses against microbial infections [ 22 , 23 ]. However, humans have discovered, mostly by trial and error, that numerous such toxic compounds present in algae, fungi, herbs and other sources can be used in small doses as spices and condiments with potential benefits of food preservation, taste enhancement and health promotion [ 23 ].

The phenomenon of “physiological hormesis” [ 24 ] is a special example of the health beneficial effects of phytotoxins. According to the concept of hormesis, a deliberate and repeated use of low doses of natural or synthetic toxins in the food can induce one or more stress responses in cells and tissues, followed by the stimulation of numerous defensive repair and maintenance processes [ 25 , 26 ]. Such hormesis-inducing compounds and other conditions are known as hormetins, categorized as nutritional, physical, biological and mental hormetins [ 27 , 28 , 29 ]. Of these, nutritional hormetins, present naturally in the food or as synthetic hormetins to be used as food supplements, are attracting great attention from food-researchers and the nutraceutical and cosmeceutical industry [ 27 , 30 ]. Other food supplements being tested and promoted for health and longevity are various prebiotics and probiotics strengthening and balancing our gut microbiota [ 31 , 32 , 33 ].

Recently, food corporations in pursuit of both exploiting and creating a market for healthy ageing products, have taken many initiatives in producing new products under the flagship of nutraceuticals, super-foods, functional foods, etc. Such products are claimed and marketed not only for their nutritional value, but also for their therapeutic potentials [ 10 ]. Often the claims for such foods are hyped and endorsed as, for example, anti-inflammatory foods, food for the brain, food for physical endurance, complete foods, anti-ageing foods and so on [ 34 , 35 , 36 ]. Traditional foods enriched with a variety of minerals, vitamins and hormetins are generally promoted as “functional foods” [ 37 ]. Even in the case of milk and dairy products, novel and innovative formulations are claimed to improve their functionality and health promotional abilities [ 38 ]. However, there is yet a lot to be discovered and understood about such reformulated, fortified and redesigned foods with respect to their short- and long-term effects on physiology, microbiota balance and metabolic disorders in the context of health and longevity.

4. Diet and Culture for Healthy and Long Life

What elevates food to become diet and a meal is the manner and the context in which that food is consumed [ 4 ]. Numerous traditional and socio-cultural facets of dietary habits can be even more significant than their molecular, biochemical, and physiological concerns regarding their nutritional ingredients and composition. For example, various well-known diets, such as the paleo, the ketogenic, the Chinese, the Ayurvedic, the Mediterranean, the kosher, the halal, the vegetarian, and more recently, the vegan diet, are some of the diverse expressions of such cultural, social, and political practices [ 1 ]. The consequent health-related claims of such varied dietary patterns have influenced their acceptance and adaptation globally and cross-culturally.

Furthermore, our rapidly developing understanding about how biological daily rhythms affect and regulate nutritional needs, termed “chrono-nutrition”, has become a crucial aspect of optimal and healthy eating habits [ 39 , 40 ]. A similar situation is the so-called “nutrient timing” that involves consuming food at strategic times for achieving certain specific outcomes, such as weight reduction, muscle strength, and athletic performance. The meal-timing and dietary patterns are more anticipatory of health-related outcomes than any specific foods or nutrients by themselves [ 41 , 42 , 43 , 44 ]. However, encouraging people to adopt healthy dietary patterns and meal-timing requires both the availability, accessibility and affordability of food, and the intentional, cultural and behavioral preferences of the people.

Looking back at the widely varying and constantly changing cultural history of human dietary practices, one realizes that elaborate social practices, rituals and normative behaviors for obtaining, preparing and consuming food, are often more critical aspects of health-preservation and health-promotion than just the right combination of nutrients. Therefore, one cannot decide on a universal food composition and consumption pattern ignoring the history and the cultural practices and preferences of the consumers. After all, “we eat what we are”, and not, as the old adage says, “we are what we eat”.

5. Conclusions and Perspectives

Food is certainly one of the foundational pillars of good and sustained health. Directed and selective evolution through agricultural practices and experimental manipulation and modification of food components have been among the primary targets for improving food quality. This is further authenticated by extensive research performed, mainly on experimental animal and cell culture model systems, demonstrating the health-promoting effects of individual nutritional components and biological extracts in the regulation, inhibition or stimulation of different molecular pathways with reference to healthy ageing and longevity [ 45 ]. Similarly, individual nutrients or a combination of a few nutrients are being tested for their potential use as calorie restriction mimetics, hormetins and senolytics [ 46 , 47 , 48 ]. However, most commonly, these therapeutic strategies follow the traditional “one target, one missile” pharmaceutical-like approach, and consider ageing as a treatable disease. Based on the results obtained from such experimental studies, the claims and promises made which can often be either naïve extrapolations from experimental model systems to human applications, or exaggerated claims and even false promises [ 49 ].

Other innovative, and possibly holistic, food- and diet-based interventional strategies for healthy ageing are adopting regimens such as caloric- and dietary-restriction, as well as time-restricted eating (TRE). Intermittent fasting (IF), the regimen based on manipulating the eating/fasting timing, is another promising interventional strategy for healthy ageing. Chrono-nutrition, which denotes the link between circadian rhythms and nutrient-sensing pathways, is a novel concept illustrating how meal timings alignment with the inherent molecular clocks of the cells functions to preserve metabolic health. TRE, which is a variant of the IF regimen, claims that food intake timing in alignment with the circadian rhythm is more beneficial for health and longevity [ 39 , 40 , 41 , 50 ]. Moreover, TRE has translational benefits and is easy to complete in the long term as it only requires limiting the eating time to 8–10 h during the day and the fasting window of 12–16 h without restricting the amount of calories consumed. Some pilot studies on the TRE regimen have reported improvement in glucose tolerance and the management of body weight and blood pressure in obese adults as well as men at risk of T2D. Meta-analyses of several pilot scale studies in human subjects suggest and support the beneficial effects of a TRE regimen on several health indicators [ 39 , 50 ]. Several other practical recommendations, based on human clinical trials have also been recommended for meeting the optimal requirements of nutrition in old age, and for preventing or slowing down the progression of metabolic syndromes [ 39 , 40 , 41 , 50 ].

What we have earlier discussed in detail [ 4 ] is supported by the following quote: “…food is more than just being one of the three pillars of health. Food is both the foundation and the scaffolding for the building and survival of an organism on a daily basis. Scientific research on the macro- and micro-nutrient components of food has developed deep understanding of their molecular, biochemical and physiological roles and modes of action. Various recommendations are repeatedly made and modified for some optimal daily requirements of nutrients for maintaining and enhancing health, and for the prevention and treatment of diseases. Can we envisage developing a “nutrition pill” for perfect health, which could be used globally, across cultures, and at all ages? We don’t think so” [ 4 ].

Our present knowledge about the need and significance of nutrients is mostly gathered from the experimental studies using individual active components isolated from various food sources. In reality, however, these nutritional components co-exist interactively with numerous other compounds, and often become chemically modified through the process of cooking and preservation, affecting their stability and bioavailability. There is still a lot to be understood about how the combination of foods, cooking methods and dietary practices affect health-related outcomes, especially with respect to ageing and healthspan.

An abundance of folk knowledge in all cultures about food-related ‘dos and don’ts’ requires scientific verification and validation. We also need to reconsider and change our present scientific protocols for nutritional research, which seem to be impractical for food and dietary research at the level of the population. It is a great scientific achievement that we have amassed a body of information with respect to the nature of nutritional components required for health and survival, the foods which can provide those nutritional components and the variety of dietary and eating practices which seem to be optimal for healthy survival and longevity.

Finally, whereas abundant availability of and accessibility to food in some parts of the world has led to over-consumption and consequent life-style-induced metabolic diseases and obesity, in many other parts of the world food scarcity and economic disparity continue to perpetuate starvation, malnutrition, poor health and shortened lifespan. Often, it is not a lack of knowledge about the optimal nutrition, food and diet that leads to making bad choices; rather, it is either our inability to access and afford healthy foods or our gullibility to fall prey to the exaggerated claims in the commercial interests of food producing and marketing companies. We must continue to gather more scientific information and knowledge about the biochemical, physiological and cultural aspects of nutrition, food and diet, which should then be recommended and applied wisely and globally, incorporating the social, cultural and environmental needs of all. After all, “we eat what we are”, and not merely “we are what we eat”!

Researchers want a ‘nutrition label’ for academic-paper facts

Inspired by the nutrition-facts labels that have been displayed on US food packaging since the 1990s, John Willinsky wants to see academic publishing take a similar approach to help to inform readers on how strictly a paper meets scholarly standards.

A team at the Public Knowledge Project, a non-profit organization run by Willinsky and his colleagues at Simon Fraser University in Burnaby, Canada, has been investigating how such a label might be standardized in academic publishing 1 .

Willinsky spoke to Nature Index about what he hopes to achieve with the initiative.

Why should academic papers have publication-facts labels?

I, like many others, have grown concerned about research integrity . Through transparency, we want to show how closely journals and authors are adhering to the scholarly standards of publishing. We want to help readers, including researchers, the media and the public, to decide whether an article is worth reporting on or citing.

The facts that we have selected for the label include publisher and funder names, the journal’s acceptance rate and the number of peer reviewers. The label also shows whether the paper includes a competing-interests statement and an editor list, where the journal is indexed and whether the data have been made publicly available. Averages for other participating journals are listed, for comparison.

It’s important that such information is readily available. When we conducted an exercise with secondary-school students, asking them to find these facts for a single academic article online, many of them took 30 minutes to do so. Some couldn’t find the information. This finding justifies the need for the label: it shouldn’t take half an hour to establish that a journal adheres to scholarly standards.

How did you create the label?

The US nutrition-facts label has been proved to change people’s behaviour, specifically their food-purchasing habits 2 . Given that so much work went into the label’s development, I thought it would be wise to build on its design.

On the basis of our early consultations with researchers, editors, science journalists, primary-school teachers and others, we created a prototype with eight elements that reflect scholarly publishing standards. We’re now gathering feedback, and might decide to change some of the facts, or to add others. Some people, for example, suggested that we include the number of days that the peer-review process took to complete.

Is AI ready to mass-produce lay summaries of research articles?

We’ve built in ways to automatically generate the label, to ensure that the format is standardized across journals and articles and to make the label available in several languages. We have created a third-party verification system, too, to ensure that authors’ identities are not revealed to peer reviewers and vice versa. This relies on authors, reviewers and editors using ORCID, the service that provides unique indicators with which to identify researchers.

The label will be displayed on the article landing page of the journal website and will be included in the article PDF.

How are you trialling the label’s use?

We’ve completed work with ten focus groups involving journal editors and authors in the United States and Latin America. We also interviewed 15 science journalists about what kinds of fact they’d want to see at a glance.

We built the label specifically for journals using the scholarly publishing workflow system Open Journal System (OJS), run by the Public Knowledge Project. By the middle of the year, we hope to launch a pilot programme involving more than 100 journals using the OJS. The goal is to explore the prospects of industry-wide implementation of the label by next year.

How could journals be compelled to display such a label?

Unlike the nutrition-facts label, which was mandated by the US government, the publication-facts label is the result of voluntary concern about research integrity in the publishing industry.

Although many groups, such as the International Association of Scientific, Technical and Medical Publishers and the Committee on Publication Ethics, manage concerns about research integrity by releasing guidelines on best practices and accumulating tools to flag suspicious activity, we feel that they have not addressed the fact that open access is public access . We need to adapt our practices to cater to the needs of different audiences, not just those in academia.

Although we’re initially building the label for OJS journals, it is an open-source plug-in that other publishing platforms will easily be able to adapt. The software is currently listed as being ‘under development’ on GitHub and will be shared there on release.

We want to show the publishing industry that we’ve piloted this in our own environment and that it is readily adaptable. We want to show that, although you could build your own label, for the sake of comprehensibility, it’s better to have a common format.

How can I plan what to eat or drink when I have diabetes?

How can physical activity help manage my diabetes, what can i do to reach or maintain a healthy weight, should i quit smoking, how can i take care of my mental health, clinical trials for healthy living with diabetes.

Healthy living is a way to manage diabetes . To have a healthy lifestyle, take steps now to plan healthy meals and snacks, do physical activities, get enough sleep, and quit smoking or using tobacco products.

Healthy living may help keep your body’s blood pressure , cholesterol , and blood glucose level, also called blood sugar level, in the range your primary health care professional recommends. Your primary health care professional may be a doctor, a physician assistant, or a nurse practitioner. Healthy living may also help prevent or delay health problems  from diabetes that can affect your heart, kidneys, eyes, brain, and other parts of your body.

Making lifestyle changes can be hard, but starting with small changes and building from there may benefit your health. You may want to get help from family, loved ones, friends, and other trusted people in your community. You can also get information from your health care professionals.

What you choose to eat, how much you eat, and when you eat are parts of a meal plan. Having healthy foods and drinks can help keep your blood glucose, blood pressure, and cholesterol levels in the ranges your health care professional recommends. If you have overweight or obesity, a healthy meal plan—along with regular physical activity, getting enough sleep, and other healthy behaviors—may help you reach and maintain a healthy weight. In some cases, health care professionals may also recommend diabetes medicines that may help you lose weight, or weight-loss surgery, also called metabolic and bariatric surgery.

Choose healthy foods and drinks

There is no right or wrong way to choose healthy foods and drinks that may help manage your diabetes. Healthy meal plans for people who have diabetes may include

  • dairy or plant-based dairy products
  • nonstarchy vegetables
  • protein foods
  • whole grains

Try to choose foods that include nutrients such as vitamins, calcium , fiber , and healthy fats . Also try to choose drinks with little or no added sugar , such as tap or bottled water, low-fat or non-fat milk, and unsweetened tea, coffee, or sparkling water.

Try to plan meals and snacks that have fewer

  • foods high in saturated fat
  • foods high in sodium, a mineral found in salt
  • sugary foods , such as cookies and cakes, and sweet drinks, such as soda, juice, flavored coffee, and sports drinks

Your body turns carbohydrates , or carbs, from food into glucose, which can raise your blood glucose level. Some fruits, beans, and starchy vegetables—such as potatoes and corn—have more carbs than other foods. Keep carbs in mind when planning your meals.

You should also limit how much alcohol you drink. If you take insulin  or certain diabetes medicines , drinking alcohol can make your blood glucose level drop too low, which is called hypoglycemia . If you do drink alcohol, be sure to eat food when you drink and remember to check your blood glucose level after drinking. Talk with your health care team about your alcohol-drinking habits.

Find the best times to eat or drink

Talk with your health care professional or health care team about when you should eat or drink. The best time to have meals and snacks may depend on

  • what medicines you take for diabetes
  • what your level of physical activity or your work schedule is
  • whether you have other health conditions or diseases

Ask your health care team if you should eat before, during, or after physical activity. Some diabetes medicines, such as sulfonylureas  or insulin, may make your blood glucose level drop too low during exercise or if you skip or delay a meal.

Plan how much to eat or drink

You may worry that having diabetes means giving up foods and drinks you enjoy. The good news is you can still have your favorite foods and drinks, but you might need to have them in smaller portions  or enjoy them less often.

For people who have diabetes, carb counting and the plate method are two common ways to plan how much to eat or drink. Talk with your health care professional or health care team to find a method that works for you.

Carb counting

Carbohydrate counting , or carb counting, means planning and keeping track of the amount of carbs you eat and drink in each meal or snack. Not all people with diabetes need to count carbs. However, if you take insulin, counting carbs can help you know how much insulin to take.

Plate method

The plate method helps you control portion sizes  without counting and measuring. This method divides a 9-inch plate into the following three sections to help you choose the types and amounts of foods to eat for each meal.

  • Nonstarchy vegetables—such as leafy greens, peppers, carrots, or green beans—should make up half of your plate.
  • Carb foods that are high in fiber—such as brown rice, whole grains, beans, or fruits—should make up one-quarter of your plate.
  • Protein foods—such as lean meats, fish, dairy, or tofu or other soy products—should make up one quarter of your plate.

If you are not taking insulin, you may not need to count carbs when using the plate method.

Work with your health care team to create a meal plan that works for you. You may want to have a diabetes educator  or a registered dietitian  on your team. A registered dietitian can provide medical nutrition therapy , which includes counseling to help you create and follow a meal plan. Your health care team may be able to recommend other resources, such as a healthy lifestyle coach, to help you with making changes. Ask your health care team or your insurance company if your benefits include medical nutrition therapy or other diabetes care resources.

Talk with your health care professional before taking dietary supplements

There is no clear proof that specific foods, herbs, spices, or dietary supplements —such as vitamins or minerals—can help manage diabetes. Your health care professional may ask you to take vitamins or minerals if you can’t get enough from foods. Talk with your health care professional before you take any supplements, because some may cause side effects or affect how well your diabetes medicines work.

Research shows that regular physical activity helps people manage their diabetes and stay healthy. Benefits of physical activity may include

  • lower blood glucose, blood pressure, and cholesterol levels
  • better heart health
  • healthier weight
  • better mood and sleep
  • better balance and memory

Talk with your health care professional before starting a new physical activity or changing how much physical activity you do. They may suggest types of activities based on your ability, schedule, meal plan, interests, and diabetes medicines. Your health care professional may also tell you the best times of day to be active or what to do if your blood glucose level goes out of the range recommended for you.

Do different types of physical activity

People with diabetes can be active, even if they take insulin or use technology such as insulin pumps .

Try to do different kinds of activities . While being more active may have more health benefits, any physical activity is better than none. Start slowly with activities you enjoy. You may be able to change your level of effort and try other activities over time. Having a friend or family member join you may help you stick to your routine.

The physical activities you do may need to be different if you are age 65 or older , are pregnant , or have a disability or health condition . Physical activities may also need to be different for children and teens . Ask your health care professional or health care team about activities that are safe for you.

Aerobic activities

Aerobic activities make you breathe harder and make your heart beat faster. You can try walking, dancing, wheelchair rolling, or swimming. Most adults should try to get at least 150 minutes of moderate-intensity physical activity each week. Aim to do 30 minutes a day on most days of the week. You don’t have to do all 30 minutes at one time. You can break up physical activity into small amounts during your day and still get the benefit. 1

Strength training or resistance training

Strength training or resistance training may make your muscles and bones stronger. You can try lifting weights or doing other exercises such as wall pushups or arm raises. Try to do this kind of training two times a week. 1

Balance and stretching activities

Balance and stretching activities may help you move better and have stronger muscles and bones. You may want to try standing on one leg or stretching your legs when sitting on the floor. Try to do these kinds of activities two or three times a week. 1

Some activities that need balance may be unsafe for people with nerve damage or vision problems caused by diabetes. Ask your health care professional or health care team about activities that are safe for you.

Stay safe during physical activity

Staying safe during physical activity is important. Here are some tips to keep in mind.

Drink liquids

Drinking liquids helps prevent dehydration , or the loss of too much water in your body. Drinking water is a way to stay hydrated. Sports drinks often have a lot of sugar and calories , and you don’t need them for most moderate physical activities.

Avoid low blood glucose

Check your blood glucose level before, during, and right after physical activity. Physical activity often lowers the level of glucose in your blood. Low blood glucose levels may last for hours or days after physical activity. You are most likely to have low blood glucose if you take insulin or some other diabetes medicines, such as sulfonylureas.

Ask your health care professional if you should take less insulin or eat carbs before, during, or after physical activity. Low blood glucose can be a serious medical emergency that must be treated right away. Take steps to protect yourself. You can learn how to treat low blood glucose , let other people know what to do if you need help, and use a medical alert bracelet.

Avoid high blood glucose and ketoacidosis

Taking less insulin before physical activity may help prevent low blood glucose, but it may also make you more likely to have high blood glucose. If your body does not have enough insulin, it can’t use glucose as a source of energy and will use fat instead. When your body uses fat for energy, your body makes chemicals called ketones .

High levels of ketones in your blood can lead to a condition called diabetic ketoacidosis (DKA) . DKA is a medical emergency that should be treated right away. DKA is most common in people with type 1 diabetes . Occasionally, DKA may affect people with type 2 diabetes  who have lost their ability to produce insulin. Ask your health care professional how much insulin you should take before physical activity, whether you need to test your urine for ketones, and what level of ketones is dangerous for you.

Take care of your feet

People with diabetes may have problems with their feet because high blood glucose levels can damage blood vessels and nerves. To help prevent foot problems, wear comfortable and supportive shoes and take care of your feet  before, during, and after physical activity.

If you have diabetes, managing your weight  may bring you several health benefits. Ask your health care professional or health care team if you are at a healthy weight  or if you should try to lose weight.

If you are an adult with overweight or obesity, work with your health care team to create a weight-loss plan. Losing 5% to 7% of your current weight may help you prevent or improve some health problems  and manage your blood glucose, cholesterol, and blood pressure levels. 2 If you are worried about your child’s weight  and they have diabetes, talk with their health care professional before your child starts a new weight-loss plan.

You may be able to reach and maintain a healthy weight by

  • following a healthy meal plan
  • consuming fewer calories
  • being physically active
  • getting 7 to 8 hours of sleep each night 3

If you have type 2 diabetes, your health care professional may recommend diabetes medicines that may help you lose weight.

Online tools such as the Body Weight Planner  may help you create eating and physical activity plans. You may want to talk with your health care professional about other options for managing your weight, including joining a weight-loss program  that can provide helpful information, support, and behavioral or lifestyle counseling. These options may have a cost, so make sure to check the details of the programs.

Your health care professional may recommend weight-loss surgery  if you aren’t able to reach a healthy weight with meal planning, physical activity, and taking diabetes medicines that help with weight loss.

If you are pregnant , trying to lose weight may not be healthy. However, you should ask your health care professional whether it makes sense to monitor or limit your weight gain during pregnancy.

Both diabetes and smoking —including using tobacco products and e-cigarettes—cause your blood vessels to narrow. Both diabetes and smoking increase your risk of having a heart attack or stroke , nerve damage , kidney disease , eye disease , or amputation . Secondhand smoke can also affect the health of your family or others who live with you.

If you smoke or use other tobacco products, stop. Ask for help . You don’t have to do it alone.

Feeling stressed, sad, or angry can be common for people with diabetes. Managing diabetes or learning to cope with new information about your health can be hard. People with chronic illnesses such as diabetes may develop anxiety or other mental health conditions .

Learn healthy ways to lower your stress , and ask for help from your health care team or a mental health professional. While it may be uncomfortable to talk about your feelings, finding a health care professional whom you trust and want to talk with may help you

  • lower your feelings of stress, depression, or anxiety
  • manage problems sleeping or remembering things
  • see how diabetes affects your family, school, work, or financial situation

Ask your health care team for mental health resources for people with diabetes.

Sleeping too much or too little may raise your blood glucose levels. Your sleep habits may also affect your mental health and vice versa. People with diabetes and overweight or obesity can also have other health conditions that affect sleep, such as sleep apnea , which can raise your blood pressure and risk of heart disease.

NIDDK conducts and supports clinical trials in many diseases and conditions, including diabetes. The trials look to find new ways to prevent, detect, or treat disease and improve quality of life.

What are clinical trials for healthy living with diabetes?

Clinical trials—and other types of clinical studies —are part of medical research and involve people like you. When you volunteer to take part in a clinical study, you help health care professionals and researchers learn more about disease and improve health care for people in the future.

Researchers are studying many aspects of healthy living for people with diabetes, such as

  • how changing when you eat may affect body weight and metabolism
  • how less access to healthy foods may affect diabetes management, other health problems, and risk of dying
  • whether low-carbohydrate meal plans can help lower blood glucose levels
  • which diabetes medicines are more likely to help people lose weight

Take it from the rats: A junk food diet can cause long-term damage to adolescent brains

A new USC-led study on rats that feasted on a high-fat, sugary diet raises the possibility that a junk food-filled diet in teens may disrupt their brains' memory ability for a long time.

"What we see not just in this paper, but in some of our other recent work, is that if these rats grew up on this junk food diet, then they have these memory impairments that don't go away," said Scott Kanoski, a professor of biological sciences at the USC Dornsife College of Letters, Arts and Sciences. "If you just simply put them on a healthy diet, these effects unfortunately last well into adulthood."

The study appears in the May issue of the journal Brain, Behavior, and Immunity .

In developing the study, Kanoski and lead author and postdoctoral research fellow Anna Hayes considered that prior research has shown a link between poor diet and Alzheimer's disease. People who suffer from Alzheimer's disease tend to have lower levels of a neurotransmitter called acetylcholine in the brain that is essential for memory and functions such as learning, attention, arousal and involuntary muscle movement.

The team wondered what this could mean for younger people who may be on a similar fat-filled, sugary Western diet, particularly during adolescence when their brain is undergoing significant development. By tracking the impact of the diet on the rats' levels of acetylcholine, and running the rats through some memory testing, they could learn more about the important relationship between diet and memory.

The researchers tracked the acetylcholine levels of a group of rats on a fatty, sugary diet and in a control group of rats by analyzing their brain responses to certain tasks designed to test their memory. The team examined the rats' brains post-mortem for signs of disrupted acetylcholine levels.

The memory test involved letting the rats explore new objects in different locations. Days later, the researchers reintroduced the rats to the scene that was nearly identical except for the addition of one new object. Rats on the junk food diet showed signs they could not remember which object they had previously seen, and where, while those in the control group showed familiarity.

"Acetylcholine signaling is a mechanism to help them encode and remember those events, analogous to 'episodic memory' in humans that allows us to remember events from our past," lead author Hayes explained. "That signal appears to not be happening in the animals that grew up eating the fatty, sugary diet."

Kanoski emphasized that adolescence is a very sensitive period for the brain when important changes are occurring in development. "I don't know how to say this without sounding like Cassandra and doom and gloom," he said, "but unfortunately, some things that may be more easily reversible during adulthood are less reversible when they are occurring during childhood."

There is at least some hope for intervention. Kanoski said that in another round of the study, the research team examined whether the memory damage in rats raised on the junk food diet could be reversed with medication that induces the release of acetylcholine. They used two drugs, PNU-282987 and carbachol, and found that with those treatments given directly to the hippocampus, a brain region that controls memory and is disrupted in Alzheimer's disease, the rats' memory ability was restored.

But without that special medical intervention, Kanoski said more research is needed to know how memory problems from a junk food diet during adolescence can be reversed.

Cracking the calcium code to understand its role in health

People seeking straightforward nutrition advice might have a bone to pick with calcium, a building block of health that can start to seem like a piece from a complicated puzzle.

Luckily, the basics—such as understanding why you need it and how to make sure you're getting enough—don't have to be puzzling at all.

"Calcium is important for overall health," said Dr. Kristina Petersen, an associate professor of nutritional sciences at Penn State University in University Park, Pennsylvania. "And we know inadequate intake or deficiency causes health problems."

Calcium is best known as the building material for your skeleton. More than 99% of it is in bones and teeth. But calcium also plays several roles related to heart health. It may help regulate blood clotting , heart rhythms and how blood vessels dilate and contract.

People whose diets are deficient in calcium have a higher risk of high blood pressure, said Dr. Martha Gulati, director of preventive cardiology in the Smidt Heart Institute at Cedars-Sinai in Los Angeles. But exactly how that works isn't fully understood, said Gulati, who also is president of the American Society for Preventive Cardiology.

Dietary calcium can be tricky to study, Petersen said, because it's difficult to discern its effects from other nutrients in calcium-rich foods.

The recommended daily allowance for calcium is 1,000 to 1,200 milligrams a day for adults, depending on age; teens and pre-teens need slightly more. But this is where conversations about calcium can start to sound like a puzzle, or the seating arrangements at a high school cafeteria—because it's all about who calcium is fitting in with at mealtime.

Without adequate vitamin D, for example, your body can't absorb much calcium from food. That's particularly important for bone health, Petersen said. (Few foods contain vitamin D naturally, but your skin makes it when exposed to sunlight, and it's routinely fortified into milk and milk substitutes such as oat milk.)

Meanwhile, some foods contain compounds that bind with calcium and prevent the body from using it. For example, spinach has a fair amount of calcium, but only about 5% ends up being bioavailable—that is, absorbed into the body, according to a study published in The Journal of Nutrition . By comparison, about 32% of the calcium in milk is bioavailable.

People who don't want to fetch a calculator or open a spreadsheet to plan each meal might be tempted to reach for a calcium supplement to boost their levels. Not so fast.

First, you should consult with your primary care physician before taking any supplement, Petersen said. And although some people might have a valid reason to take one, in general, most can get the calcium they need in their diet.

Some studies, Petersen said, have found that high levels of calcium from supplements actually can be bad for heart health. Gulati, who co-wrote the National Osteoporosis Foundation and American Society for Preventive Cardiology guidelines about calcium supplements in 2016, said that studies overall have been contradictory about calcium supplements' safety and benefits.

"Ultimately, I think that the best way to get nutrients is through our food," Gulati said. "I don't advocate for a lot of supplements unless somebody truly can't get it from food."

Happily, experts have an easy answer to the question of how to get the calcium you need without keeping a nutrition textbook handy: Just eat a range of healthy foods throughout the day.

"That should be sufficient," Gulati said. Sure, you could schedule your meals to avoid calcium-binding foods at the same time you're eating those that are calcium-rich, she said. "But I think that's a very difficult way to eat."

By eating a variety of healthy foods throughout your day, chances are you're not always eating something that's calcium-binding, she said, and you're probably getting something else that's beneficial.

"And the thing about calcium is that there are actually lots of sources out there," she said.

Most bioavailable calcium is going to come from dairy products such as low-fat milk, yogurt and cheese, Petersen said. Federal dietary guidelines recommend that adults have three one-cup servings of dairy a day. The DASH (Dietary Approaches to Stop Hypertension) eating plan also includes two to three servings of low-fat or fat-free dairy. (Butter is technically dairy but is not a good source of calcium, she said.)

Plant-based milks can be a good dairy alternative for people who are lactose intolerant or who don't consume animal-based products. "However, you do need to make sure that you are choosing a calcium-fortified one," Petersen said.

Gulati happens to be vegan and draws on a long list of non-dairy options to meet her needs. "Edamame is one great source of calcium," she said. "Tofu also is a great source. Nuts, particularly almonds, are another good source," as are leafy greens such as bok choy and collard greens, especially when cooked.

Other sources include beans, winter squash, sardines and canned salmon.

Petersen said she finds dairy to be a convenient way to get her calcium. She likes milk in her coffee and is a big fan of yogurt and its many portable forms. "You can just put one in your bag and take it to work, and that's your snack."

Combining Childhood Education and Nutrition to Cultivate Bright Futures in Senegal

Addressing the multifaceted issues of poverty and inequality in Senegal, as in West Africa as a whole, represents a critical development challenge. Senegal currently ranks in the lowest quintile of the Human Development Index (HDI), ranked 170 of the 190 countries surveyed 1 , with 9.2 percent of the population living on less than $2.15 a day. While poverty figures are lower than some neighboring countries, such as Mali (15 percent), Guinea (14 percent), and Guinea Bissau (22 percent), they underscore the substantial hurdles facing the region. Rural areas in Senegal bear a disproportionate burden: they contain just over more than half the population (53 percent) but are home to 80 percent of chronically poor and vulnerable households. The impact is particularly severe on young children, as 85 percent of the poorest households have children under the age of five, and poverty adversely affects their nutrition, health, and education 2 . 

Regarding nutrition, a significant proportion of infants, 64 percent, are not exclusively breastfed, and 93 percent of young children do not receive a minimum acceptable diet, thus hampering their growth and educational prospects. The gravity of the situation poses a threat to Senegal's achievement of key Sustainable Development Goals (SDGs).

In the education sector, despite a significant increase of over ten percentage points between 2000 and 2020, the proportion of children attending pre-school is still low compared to the regional average, with a rate of 18 percent for girls and 16 percent for boys in Senegal. This compares to an average of 24 percent for both sexes in the West and Central Africa region 3 . Thus, pre-school education reaches a small proportion of the country's school-age population; it is primarily a concern of the middle- and upper-income classes, who live in cities and see the benefits of pre-schooling for their children's success. Additionally, in both rural and urban areas, many parents prefer to give their children a religious education from an early age.

Despite Senegal's robust economic growth—consistently surpassing 6 percent annually from 2014 to 2020—inclusive development remains a challenge. Although government spending on social sectors averages 30 percent, there is a discernible gap in translating economic growth into widespread prosperity. The presence of a fragmented institutional framework and the lack of strong leadership compounds the complexities faced in achieving sustainable and inclusive development in Senegal. Difficulties in making progress spurred engagement with multiple actors and cross-sectoral coordination to tackle underlying challenges.

Aligned with the government’s Plan Senegal Emergent, and its Second Priority Action Plan (2019-2023), the World Bank's strategic focus emphasizes early childhood interventions, expanding access to services in marginalized rural and peri-urban areas, and enhancing the efficiency and transparency of governance institutions and social protection systems. The Investing in Early Years for Human Development Project support to this plan is twofold, involving support to both the education and nutrition sectors.

The Investing in Early Years for Human Development Project builds on the Investing in Maternal, Child, and Adolescent Health (ISMEA) Project, a World Bank initiative supporting maternal and child health services. Both projects have divided geographic regions during the same time period to ensure widespread coverage, implementing consistent early nutrition and stimulation activities nationwide. This collaborative approach aims to enhance the impact and reach of crucial initiatives to support early childhood development.

The World Bank's support for Senegal’s National Nutrition Enhancement Program (CNDN) is a pivotal initiative in the battle against poverty and malnutrition . As such, the Investing in Early Years for Human Development Project incorporates support for the CNDN, which is designed to dismantle the numerous barriers that families, especially women, encounter in accessing quality nutrition and resources for their households. The CNDN works with local communities and religious leaders to provide a stronger foundation for Senegalese children. Notably, the engagement of over 900 religious leaders plays a crucial role in advocating for increased investments and parental involvement in early childhood development. 

The CNDN has embraced a multifaceted approach over the years, implementing cross-sectoral and innovative practices to empower women in rural and semi-urban areas grappling with high rates of food insecurity and acute malnutrition in infants. These practices encompass child nutrition and early stimulation in the crucial first 1,000 days by leveraging new technologies to strengthen existing systems, as well as promoting essential family health and nutrition practices. The success of this novel approach hinges on the empowerment of women and the prioritization of community assets. 

The Investing in Early Years for Human Development Project also aims to support quality education. Thus, it undertakes innovative partnerships with Koranic schools, known as Daaras in Senegal, an approach that has proven to be highly effective. Performance-based contracts with Daaras ensure the integration of at least 10 hours per week of quality, play-based early learning, thereby fostering cognitive and social development, as well as early literacy and math skills that can further advance children’s educational development. The project also supports the creation of community pre-school classes (CPCs) and the launching of the Read@Home initiative. By distributing story books in Arabic, French, and seven Senegalese languages, the Read@Home initiative aims to reach 50 percent of all children under the age of six in seven regions, particularly those with the lowest early childhood development outcomes.

Despite daunting challenges posed by the COVID-19 pandemic and government restructuring, since 2018 the Project has displayed remarkable progress and tangible achievements. 

A standout accomplishment is the transformative impact for over 10 million children , 52 percent of those aged 0-23 months in targeted regions, who benefited from vital nutrition and early stimulation interventions . This represents a remarkable surge from a mere 19 percent in 2018, underscoring the Investing in Early Years for Human Development Project’s prowess in reaching and positively impacting a significant segment of the intended demographic.

Beyond education, the initiative has made substantial strides in improving birth registration rates for children aged 0-5 years, surging from 43 percent to a 73 percent . This achievement is of particular importance because birth registration is a prerequisite to enrollment in schools at an appropriate age and grade level, as well as to better targeted nutritional support. 

The Investing in Early Years for Human Development Project’s focus on gender inclusivity is evident, particularly its emphasis on girls' access to quality early learning services. Direct interventions have resulted in over 92,000 girls and over 94,000 boys benefiting from improved early learning services .

The Investing in Early Years for Human Development Project, funded by a $75 million grant from the International Development Association (IDA), unfolds as a comprehensive initiative. This endeavor allocates $37 million to sow the seeds of health and development in the first 1,000 days. It also dedicates an additional $22 million to fostering quality early learning. Additional components build on this base, with an allocation of $11 million toward child protection and system strengthening to ensure resilience and safety for the most vulnerable. 

Partners 

The Investing in Early Years for Human Development Project's multi-sectoral approach necessitates collaboration with key stakeholders directly engaged in its implementation. This includes the National Council for Nutrition Development (CNDN); the Ministry of National Education (MEN); the Ministry of Family, Women and Child Protection (MFFPE); and the National Agency for Early Childhood Development (ANPECTP). To facilitate coordination, a dynamic multi-sectoral Technical Working Group has been established. It is chaired by the Secretary-General of the MFFPE and comprised of technical officers from the three implementing structures and the Project Implementation Unit (PIU). Building on a longstanding partnership dating back to the 2000s, the World Bank collaborates closely with the CNDN, resulting in a substantial expansion of early year services encompassing nutrition, stimulation, and protection within the Project. Implementation is further enriched by the involvement of entities, such as the Birth Registration Agency, as well as technical partners including the United Nations Children’s Fund (UNICEF), the United States Agency for International Development (USAID), Save The Children, ChildFund, the Associates in Research and Education for Development (ARED), La Lumiere, and Tostan, alongside various national non-governmental organizations (NGOs). 

The World Bank’s partnership with UNICEF illustrates the broad range of activities carried out in partnership with other organizations. For example, early stimulation activities developed as part of the Investing in Early Years for Human Development Project were based on a pilot program financed by UNICEF. Similarly, the Project has drawn on UNICEF's work with the Ministry of National Education to develop a curriculum and pedagogical tools for Koranic schools. Additionally, regular consultations have been held with the UNICEF team since the design and implementation of the Project began. This seamless, cross-sectoral collaboration, bridging national agencies with local and regional actors, exemplifies effective project implementation, and provides a replicable model for similar settings in other countries and regions.

Looking Ahead

Senegal's government is forging ahead in its collaboration with the World Bank, demonstrating its dedication to constructing resilient communities. The ongoing Investing in Early Years for Human Development Project seeks to enhance management and accessibility to fundamental services across the remaining seven regions of Senegal, impacting nearly 4 million additional individuals. In addition, it will continue to refine strategies with a focus on improving childcare activities, education, as well as social protection in higher education and health initiatives. 

The valuable lesson learned from this endeavor is the effective cross-sectorial approach, exemplified by the geographic distribution across two World Bank projects (namely, the Investing in Early Years for Human Development Project and the Investing in Maternal, Child and Adolescent Health Project) to ensure comprehensive coverage. Drawing from this experience, new higher education, and health projects in the region (including the Senegal Higher Education Project, Enseignement Supérieur Professionnel Orienté Insertion et Réussite des Jeunes (Espoir-Jeunes) Project in Senegal, effective in January 2024, and the Sahel Women’s Empowerment and Demographic Dividend (SWEDD) Project, effective in September 2023) integrate childcare activities. Social protection teams are also exploring innovative ways to utilize the social registry and cash transfers, while also working closely with mother leaders’ groups to bolster Early Childhood Development (ECD) initiatives. 

Extending this cross-sectoral paradigm, other countries in West Africa are embracing the opportunity to reinforce developmental initiatives by fully incorporating diverse ministries into collaborative projects. In Benin, for instance, the Gbessoke Program (effective in 2023) is an integrated initiative that links people with a range of social services to foster productive livelihoods, resilience, and improved education and health outcomes. It is expected that 150,000 individuals, representing 61 percent of extremely poor households in the social registry, will benefit from the program, which will run until the end of 2027.
