2021 Research Highlights — Human Health Advances

Disease prevention, diagnosis, and treatment.

With NIH support, scientists across the United States and around the world conduct wide-ranging research to discover ways to enhance health, lengthen life, and reduce illness and disability. Groundbreaking NIH-funded research often receives top scientific honors. In 2021, these honors included Nobel Prizes to five NIH-supported scientists . Here’s just a small sample of the NIH-supported research accomplishments in 2021.

COVID-19 spread and vaccines

NIH researchers continued to make scientific breakthroughs to help control the COVID-19 pandemic. Studies of spread suggested strategies for controlling infections . Research also revealed who was most at risk of becoming severely ill from COVID-19: nearly two-thirds of COVID-19 hospitalizations in the U.S. were due to obesity, diabetes, hypertension, and heart failure . Wide-spread vaccine rollouts slowed the spread of SARS-CoV-2, the virus that causes COVID-19. The Moderna COVID-19 vaccine, developed with NIH, proved to be 94% effective against symptomatic COVID-19 . Six months later, people who had been vaccinated still showed signs of immunity . COVID-19 vaccines saved an estimated 140,000 lives through May 2021, and hundreds of thousands more have been saved since. As SARS-CoV-2 mutated and new variants became common, scientists conducted studies on booster doses of vaccine. These suggested that COVID-19 boosters not only lengthen immunity but help broaden and strengthen the immune response to protect against a wide variety of variants.

Drug delays type 1 diabetes onset

In type 1 diabetes, the immune system’s T cells attack the insulin-producing beta cells in the pancreas. Those affected need insulin treatment to survive. In a clinical trial of people at risk of developing type 1 diabetes (with a median age of 13), the drug teplizumab delayed disease onset and improved insulin production. The findings support the use of the drug for delaying or preventing type 1 diabetes.

Gene therapy effective in human trials

Advances in gene therapy are showing promise for people with life-threatening conditions. Using gene therapy, researchers were able to introduce a healthy copy of a disease-causing gene and restore immune system function in children with a rare genetic disorder called severe combined immunodeficiency, or SCID. In another study, researchers safely delivered gene therapy to the brain to treat a debilitating neurological disease for which there are no effective treatments.

Low-fat diet compared to low-carb diet

There has been a great deal of debate over what aspects of our diets affect weight control. A carefully controlled study found that people ate fewer calories per day and lost more weight on a plant-based, low-fat diet compared to an animal-based, low-carb diet. However, the low-fat diet led to higher insulin and blood sugar levels, which can be risk factors for heart disease. The findings reveal how restricting dietary carbohydrates or fats may impact health.

Supplement targets gut microbes to boost growth in malnourished children

The effects of childhood malnutrition can cause lifelong health problems. Researchers found that a supplement designed to repair the gut microbiome—the bacteria, viruses, and fungi in the digestive system—helped malnourished children. Those given the supplement gained more weight than children on a standard nutritional supplement. The experimental supplement also raised levels of proteins in the blood associated with bone, cartilage, and brain health.

Test may help reduce racial disparities in kidney disease

A common blood test for kidney function measures a protein called creatinine. But Black Americans generally have higher amounts of creatinine. As a result, doctors take race into account when testing for kidney disease. A study showed that measuring levels of another protein called cystatin C can accurately estimate kidney function without needing to take race into account. A race-blind method for estimating kidney function could more effectively identify chronic kidney disease.

Malaria vaccines provide strong and lasting immunity

Malaria is caused by parasites transmitted by infected mosquitoes. Researchers developed a vaccine approach that uses live sporozoites, the infectious form of the malaria parasite, along with a drug that kills the parasite. In a small trial, the approach led to broad, long-lasting protection against malaria. The strategy is now being tested in real-world conditions in a Phase 2 clinical trial in Mali.

Devices in the brain help paralyzed people communicate

Technological advances are enabling people who have lost the ability to move or speak to communicate again. Scientists developed a device to decode brain activity into words in real time , allowing a person with paralysis to communicate in complete sentences. Another system quickly translates brain signals for imagined handwriting into text . These systems could make communication for people with severe paralysis more natural and efficient.

First-generation medical students face unique challenges and need more targeted support

Medical research is increasingly informed by recognition of diversity’s key role in addressing health equity. But when it comes to medical education, there’s a group that has remained not just underrepresented but also under-researched: first-generation (first-gen) medical students — those whose parents have not earned bachelor’s degrees. These students are more likely to be older, identify as racial or ethnic minorities, be immigrants or children of immigrants, or come from low-income families. Along with anecdotal evidence, the minimal previous research indicates that these students face some unique struggles on top of the common challenges most medical students encounter.

“It became clear to me that schools — even the great ones that are intentional and diligent about building diverse classes — are not truly ready to receive first-gen students,” said Catherine Havemann, MD, an emergency medicine chief resident at UChicago Medicine. “Admission isn’t the same as full access to the institution. Sometimes support doesn’t exist, and other times it’s off-target.”

To increase understanding of the first-gen experience and identify opportunities for educators and administrators to provide the most meaningful support, Havemann helped lead a team of researchers to perform an in-depth qualitative study. They analyzed data collected in interviews with a diverse group of medical students recruited from 27 medical schools across the U.S. The results, published in JAMA , have the potential to inform efforts at increasing educational equity at both the institutional and individual level.

Struggles shared amongst a diverse first-gen population

Overall, the study confirmed that first-gen medical students feel that they face disproportionate adversity throughout their education and do not receive the support they need to compensate for that. Participants identified 4 main themes: feelings of isolation and exclusion; difficulties accessing basic resources such as food, rent, transportation and textbooks; a general lack of institutional support; and pressure to rely on personal “grit” and resilience for survival.

Some issues highlighted in the data were relatively unsurprising, such as financial difficulties.

“No matter which subset of first-gen students we talk to, money is a foundational part of the challenges they face — even if they're not technically low-income,” said Havemann, the paper’s first author. “Within the medical community, we need to talk more about the discomfort of disadvantaged students entering incredibly wealthy institutions with mostly wealthy peers. What does it mean to create some basic degree of equity?”

Other issues emerged as more persistent than the researchers had anticipated. For example, interviewees frequently mentioned transportation problems, such as situations where student loans don’t cover the cost of having a car but medical school necessitates one. An especially striking theme was that many students reported being overtly discouraged by mentors or teachers during their education.

“People who meet the criteria for medical school admission are being told ‘This is not for you,’” Havemann said. “It’s disheartening to see, and it makes me think differently about my career as an aspiring educator. Saying ‘Yes’ — even in a small way — to someone who has heard a lifetime of ‘No’ can make all the difference. To think there are people out there discouraging others is frankly appalling.”

Responding to the findings

Havemann said the paper resonated strongly with student communities online following its publication.

“Responses ranged from ‘This is obvious’ and ‘Water is wet’ to ‘Why doesn't my school understand this?’ or "We knew this already — where are the solutions?’” she said.

As a former first-gen student herself, she was struck by the consistency of experience revealed by the study’s results. “It was validating as a researcher but also profoundly validating as a person.”

But while the student response online served as important confirmation that the study’s findings are representative, the real target audience is the educators who have the power to make a difference.

“I would love for them to read this paper and feel what a powerful position they're in to make a more equitable world,” Havemann said. “Even the little things matter a lot.”

Even as she and others conduct more research on this topic, Havemann said institutions can and should already be taking steps to provide better support for first-generation medical students.

“People like to talk about using holistic review in admissions to look at the whole student — we also have to look at holistic support once they enroll,” she said.

Future studies will dive deeper into themes like professional identity formation, financial challenges, burnout and sense of belonging. Now that the qualitative groundwork has been laid, researchers can design more nuanced quantitative and mixed-method studies.

“For example, I’d like to quantify the percentage of first-gen students who are not only trying to support themselves but also keeping the lights on for their parents,” Havemann said. “I think the answers would be gutting.”

New cardiac research will save women's lives by improving detection of heart failure

An important new study has advanced how heart failure is detected in women -- meaning more female patients can be diagnosed and at an earlier stage.

Researchers led by teams from the Universities of East Anglia (UEA), Sheffield and Leeds, have been able to fine-tune how magnetic resonance imaging (MRI) is used to detect heart failure in women's hearts, making it more accurate.

Lead author Dr Pankaj Garg, of the University of East Anglia's Norwich Medical School and a consultant cardiologist at the Norfolk and Norwich University Hospital, said: "By refining the method for women specifically, we were able to diagnose 16.5pc more females with heart failure.

"This could have huge impact in the NHS, which diagnoses around 200,000 patients with heart failure each year.

"This improved method will increase early detection, meaning more women can get life-saving treatment sooner."

In 2022, UEA and the University of Sheffield published research which showed how using MRI scans could be used to detect heart failure and which led to this technique being widely employed by medics.

When a heart starts to fail, it is unable to pump blood out effectively, and so the pressure in the heart rises.

Co-author Dr Gareth Matthews, of the University of East Anglia's Norwich Medical School, said: "Currently one of the best ways of diagnosing heart failure is to measure pressures inside the heart with a tube called a catheter.

"While this is very accurate, it is an invasive procedure, and therefore carries risks for patients, which limits its use.

"For this reason, doctors tend to use echocardiograms, which are based on ultrasound, to assess heart function, but this is inaccurate in up to 50 per cent of cases. Using MRI, we can get much more accurate images of how the heart is working."

The team was able to create an equation which allowed them to non-invasively derive the pressure in the heart using an MRI scanner.

However, previous use of this method wasn't as accurate as the researchers would have liked in diagnosing heart failure in women, especially in early or borderline disease.

Co-author Professor Andy Swift, of the University of Sheffield's School of Medicine and Population Health, said: "Women's hearts are biologically different to men's.

"Our work suggests that in heart failure women's hearts may respond differently in response to increases in pressure."

Heart failure can be classed differently, depending on the amount of blood squeezed out of the main chamber of the heart with every beat, known as the heart's ejection fraction.

Women suffer disproportionately from a type of heart failure where the pumping function of the heart is preserved but the ability of the heart to relax and fill with blood is impaired.

Echocardiography really struggles to diagnose this type of heart failure. The improvements in diagnosis from this new work will enable more of this particular cohort to be diagnosed more accurately and hopefully drive better treatments.

Co-author Dr Peter Swoboda, of the University of Leeds' Faculty of Medicine and Health, said: "The symptoms of heart failure, like breathlessness and fatigue, can have a devastating effect on patients' quality of life.

"We are increasingly recognising the importance of early diagnosis and, early treatment can improve symptoms and life expectancy.

"This research will help diagnose heart failure in women more quickly and get them established on life-saving treatments sooner."

The Government's Health and Social Care Secretary, Victoria Atkins, said: "Heart failure is a devastating condition affecting hundreds of thousands of women in the UK, so this research is a hugely positive development that could make it possible for thousands of people to get diagnosed and treated at an earlier stage.

"For the second year of our Women's Health Strategy for England, I have been clear that we need more research to look at the differences between how conditions affect men and women.

"I am delighted that this government-backed research has met this challenge so that we can get life-saving treatment to women faster."

The research was a collaboration between the University of East Anglia, the University of Leeds, the University of Sheffield, the Norfolk and Norwich University Hospital NHS Foundation Trust, the National Heart Research Institute Singapore, Duke-NUS Medical School in Singapore, Queen Mary University of London, the National Institute for Health and Care Research's Sheffield Biomedical Research Centre, the University of Amsterdam and Kocaeli City Hospital in Turkey.

It was funded by the National Institute for Health and Care Research (NIHR) Sheffield Biomedical Research Centre, the Wellcome Trust, and the National Medical Research Council (NMRC).

ChatGPT can accurately analyze medical charts for clinical research, other applications

ChatGPT, the artificial intelligence (AI) chatbot designed to assist with language-based tasks, can effectively extract data for research purposes from physicians' clinical notes, UT Southwestern Medical Center researchers report in a new study.

Their findings , published in npj Digital Medicine , could significantly accelerate clinical research and lead to new innovations in computerized clinical decision-making aids.

"By transforming oceans of free-text health care data into structured knowledge, this work paves the way for leveraging artificial intelligence to derive insights, improve clinical decision-making, and ultimately enhance patient outcomes ," said study leader Yang Xie, Ph.D., Professor in the Peter O'Donnell Jr. School of Public Health and the Lyda Hill Department of Bioinformatics at UT Southwestern.

Dr. Xie is also Associate Dean of Data Sciences at UT Southwestern Medical School, Director of the Quantitative Biomedical Research Center, and a member of the Harold C. Simmons Comprehensive Cancer Center.

Much of the research in the Xie Lab focuses on developing and using data science and AI tools to improve biomedical research and health care. She and her colleagues wondered whether ChatGPT might speed the process of analyzing clinical notes—the memos physicians write to document patients' visits, diagnoses, and statuses as part of their medical record—to find relevant data for clinical research and other uses.

Clinical notes are a treasure trove of information, Dr. Xie explained; however, because they are written in free text, extracting structured data typically involves having a trained medical professional read and annotate them. This process requires a huge investment of time and often resources—and can also introduce human bias.

Existing programs that use natural language processing require extensive human annotation and model training. As a result, clinical notes are largely underused for research purposes.

To determine whether ChatGPT could convert clinical notes to structured data, Dr. Xie and her colleagues had it analyze more than 700 sets of pathology notes for lung cancer patients to find the major features of primary tumors, whether lymph nodes were involved, and the cancer stage and subtype.

Overall, Dr. Xie said, the average accuracy of ChatGPT to make these determinations was 89%, based on reviews by human readers.

Their analysis took several weeks of full-time work compared with the few days it took to fine-tune data extraction from the ChatGPT model. This accuracy was significantly better than other traditional natural language processing methods tested for this use.

To test whether this approach is applicable to other diseases, Dr. Xie and her colleagues used ChatGPT to extract information about cancer grade and margin status from 191 clinical notes on patients from Children's Health with osteosarcoma, the most common type of bone cancer in children and adolescents. Here, ChatGPT returned information with nearly 99% accuracy on grade and 100% accuracy on margin status.

Dr. Xie noted that the results were strongly influenced by what prompts ChatGPT was given to perform each task—a phenomenon called prompt engineering. Providing multiple options to choose from, giving examples of appropriate responses, and directing ChatGPT to rely on evidence to draw conclusions improved its performance.

She added that using ChatGPT or other large language models to extract structured data from clinical notes could not only speed clinical research but also help clinical trial enrollment by matching patients' information to clinical trial protocols. However, she said, ChatGPT won't replace the need for human physicians.

"Even though this technology is an extremely promising way to save time and effort, we should always use it with caution. Rigorous and continuous evaluation is very important," Dr. Xie said.

Adding polymerized ionic liquid improves performance of perovskite solar cells

Perovskite solar cells, which use materials with the same crystal structure as perovskite, are lightweight, flexible, easy to manufacture, and inexpensive. They can be attached to many different surfaces and are a promising technology. However, current perovskite solar cells are not durable, and they tend to be inefficient.

Research shows how additive engineering with a polymerized ionic liquid to the metal halide perovskite material can improve the solar cell's function, helping to pave the way for the future wide adoption of perovskite solar cells .

The research was published in Energy Materials and Devices on March 29.

"The commonly employed solution processing method for fabricating perovskite layers introduces many defects in both the bulk and surface of the perovskite layer. These intrinsic defects within the perovskite absorption layer pose a significant constraint on the overall performance of the devices.

"Additive engineering has been demonstrated to be effective as a strategy for defect passivation and performance enhancement in perovskite solar cells," said Qi Cao, a researcher at Northwestern Polytechnical University in Xi'an, China.

To further improve the ionic liquids added to perovskite solar cells, researchers can create polymerized or poly ionic liquids. Polymers are added to ionic liquids, enhancing the properties of the ionic liquid. In this study, researchers synthesized a poly ionic liquid called poly4-styrenesulfonyl(trifluoremethylsulfonyl)imidepyridine, or PSTSIPPyri for short.

The addition of PSTSIPPyri to the metal halide perovskite solar cell has many benefits. It can prevent halide ion migration, which helps maintain the crystal structure , and facilitate the fixation of organic and halide ions, which improves the solar cell's stability.

"To date, researchers have devoted considerable attention to the meticulous selection of additives that enhance the performance of perovskite solar cells. Among these, ionic liquids have received widespread attention. Ionic bonds in ionic liquids tend to be stronger and more stable, and they offer various tunable properties, including viscosity, polarity, and conductivity," said Xuanhua Li, a researcher at Northwestern Polytechnical University.

"This tunability makes it possible to fine-tune the ionic liquid properties to meet the specific requirements of the perovskite film, thereby optimizing device performance."

To test the success of the addition of PSTSIPPyri, researchers conducted comprehensive studies of the perovskite solar cells with the poly ionic liquid, especially for factors that are essential for the performance of the solar cells. The perovskite films were aged for 300 hours at 85°C and 60% relative humidity . The enhanced perovskite film had a slower rate of change than the perovskite film with the poly ionic liquid.

In a high humidity , high heat environment, it also maintained 84.5% of its efficiency after 1000 hours, while the control perovskite solar cell only maintained 43.6% of its initial efficiency.

Since perovskite solar cells are often less durable than alternatives, it was important to test the long-term durability of the perovskite solar cell with the addition of PSTSIPPyri. With the poly ionic liquid, the perovskite solar cell maintained 87.6% of its power conversion efficiency after 1,500 hours of continuous light, compared to the control which only maintained 61.1% of its power conversion efficiency.

"Incorporating PSTSIPPyri as an additive leads to a significant enhancement in the power conversion efficiency of inverted perovskite solar cells from 22.06% to 24.62%. They also demonstrate excellent long-term operational stability," said Cao. "This strategy illustrates the potential of poly ionic liquids as a promising additive for perovskite solar cells, offering both high performance and stability."
