Medical Research: Essential To The Future Of A Healthy Kenya

This afternoon, Helene Gayle, Congressman Keith Ellison and the rest of their team are headed out to the Kenya Medical Research Institute in Kisumu.

KEMRI is one of Africa's top medical research centers, and has been key to Kenya's efforts to control HIV, malaria and other diseases. Here the team is looking to examine the work of the institute, the potential to enhance current research partnerships with the American government, the status of malaria diagnosis and treatment, and the overall state of medical research in Kenya.

Many observers acknowledge that programs like KEMRI are critical for improving health in developing countries, particularly by building up local capacities for medical research and surveillance, as well as providing essential laboratory services for the region.

The Kenya Medical Research Institute (KEMRI) was established in 1979 under the Science and Technology (Amendment) Act of that year to represent the national body responsible for carrying out health science research in Kenya. Prior to the establishment of KEMRI, health research in Kenya was conducted under the auspices of the East African Medical Research Council which had been established in 1957 to serve the countries of the East African Community. Following the break-up of the East African Community in 1977, the Kenyan Parliament passed the Science and Technology Act in 1977 and amended it in 1979 to provide for the establishment of research institutes.

The Research Office is established in the Academic Department and operates under the office of a Registrar who assists the Deputy Director in charge of Academics (DDA) to manage this area.

Strategic objectives

KMTC aims to enhance institutional research capacity through the following strategies:

  • Building capacity of staff to carry out research
  • Disseminating research findings to stakeholders
  • Mobilizing resources for research
  • Establishing Research and Scientific Committees in all campuses
  • Developing a Monitoring and Evaluation framework for research activities

Research Policy

KMTC has a Research Policy that guides its research activities and ensures that the College upholds the highest ethical standards in a research community that is committed the principals of integrity, trust, justice and collegiality. The Policy has encouraged and empowered staff and students to conduct, disseminate and report results effectively through the creation of an enabling environment and the development of a research culture. KMTC has developed Standard Operating Procedures (SOPs) to operationalize the Research Policy. The Research Policy can be accessed  here .

Science, Innovation and Technology Strategy

KMTC aligns its research priorities to the national strategies for Science, Innovation and Technology (STI), geared towards realization of the country’s long-term development goals. More information on this can be found  here

The College allocates funds for research activities every financial year to fund research studies, attend symposia, workshops, seminars and conferences both nationally and internationally. It encourages studies that stimulate new innovative approaches and ideas that contribute to KMTC’s strategic priorities and general public health concerns in areas such as, training, student welfare, healthcare access, health finance and other emerging issues. The College also provides training opportunities to staff in all areas of research; proposal writing, publishing, review and evaluation of research documents and assessment.

Besides, internal funding, KMTC actively applies for external research funding to complement internal funding.

Scientific Conference and Symposia

Kenya Medical Training College (KMTC) has established a bi-ennial Scientific Conference geared towards enhancing collaboration and exchanging ideas. KMTC conference brings together scientists from different fields of specialization to learn about recent developments, present new data and critically discuss advances in the scientific world. KMTC attaches great importance to the Conference, as it is a culmination of one of its core mandates. It envisages bringing together experts/senior scientists, policy makers and young scientists to deliberate on topical issues affecting health delivery in Kenya.

The College also holds symposia every year aimed at addressing emerging issues in the healthcare sector.

The KMTC Research Webinar Series is a monthly online interactive event that aims at improving research skills among KMTC faculty and the public. The series began in January 2021 and so far, 11 online webinars on various research topics have been held. The organizers of the series have been keen to invite various experienced professional from different organizations to share their experiences in various aspects of research. MOMBASA, Kenya, May 16 (Xinhua) -- More than 100 health, finance and trade experts from across Africa convened in Mombasa, Kenya, for a three-day meeting aimed at enhancing the affordability, availability and equitable access to essential medical supplies.

In her opening remarks, Susan Nakhumicha Wafula, Kenyan cabinet secretary in the Ministry of Health, said that an African Pooled Procurement Mechanism (APPM) presents an opportunity for the continent to collectively harmonize regulations that will enhance the supply of quality and safe medical supplies for the continent.

"The approach will help enhance supplies of quality and safe medical supplies as well as lower the cost of pharmaceutical products," Nakhumicha told the participants.

She noted that Africa continues to face the challenges of the delivery of quality and affordable products, an obstacle that makes healthcare inaccessible for millions across the continent.

The meeting, organized by the United Nations Economic Commission for Africa (UNECA), Africa Centers for Disease Control and Prevention, and African Union Development Agency (AUDA-NEPAD), aims to shape the market, promote local production, and transform African economies.

Hanan Morsy, deputy executive secretary and chief economist at UNECA, said it is time African nations learned from the vulnerabilities exposed by recent health emergencies and the evolving burden of pandemics.

Morsy urged policymakers to address the challenges that prevent the attainment of sustainable and inclusive development in the continent.

Abebe Genetu Bayih, acting coordinator of the Partnership for African Vaccine Manufacturing, said the APPM has a unique value proposition of locally manufactured health products and technologies that meet global standards.

Bayih noted that collective bargaining power for this value proposition would help African nations achieve supply security and reduced costs.

Chimwemwe Chamdimba, head of Health at AUDA-NEPAD, said that the lessons from implementing the pharmaceutical for the past five years call for facilitating and advocating for local production for resilient, inclusive, and sustainable health systems in Africa.

Chamdimba said that the private sector should be included to fill the gaps and contribute to improved health outcomes on the continent.

Two decades of studies suggest health benefits associated with plant-based diets

Vegetarian and vegan diets are generally associated with better status on various medical factors linked to cardiovascular health and cancer risk, as well as lower risk of cardiovascular diseases, cancer, and death, according to a new review of 48 previously published papers. Angelo Capodici and colleagues present these findings in the open-access journal PLOS ONE on May 15, 2024.

Prior studies have linked certain diets with increased risk of cardiovascular disease and cancer. A diet that is poor in plant products and rich in meat, refined grains, sugar, and salt is associated with higher risk of death. Reducing consumption of animal-based products in favor of plant-based products has been suggested to lower the risk of cardiovascular disease and cancer. However, the overall benefits of such diets remain unclear.

To deepen an understanding of the potential benefits of plant-based diets, Capodici and colleagues reviewed 48 papers published between January 2000 and June 2023 that had compiled evidence from multiple prior studies. Following an "umbrella" review approach, they extracted and analyzed data from the 48 papers on links between plant-based diets, cardiovascular health , and cancer risk.

Their analysis showed that, overall, vegetarian and vegan diets have a robust statistical association with better health status on a number of risk factors associated with cardiometabolic diseases, cancer, and mortality, such as blood pressure , management of blood sugar, and body mass index. Such diets are associated with reduced risk of ischemic heart disease, gastrointestinal and prostate cancer , and death from cardiovascular disease.

However, among pregnant women specifically, those with vegetarian diets faced no difference in their risk of gestational diabetes and hypertension compared to those on non-plant-based diets.

Overall, these findings suggest that plant-based diets are associated with significant health benefits. However, the researchers note, the statistical strength of this association is significantly limited by the many differences between past studies in terms of the specific diet regimens followed, patient demographics, study duration, and other factors.

Moreover, some plant-based diets may introduce vitamin and mineral deficiencies for some people. Thus, the researchers caution against large-scale recommendation of plant-based diets until more research is completed.

The authors add, "Our study evaluates the different impacts of animal-free diets for cardiovascular health and cancer risk showing how a vegetarian diet can be beneficial to human health and be one of the effective preventive strategies for the two most impactful chronic diseases on human health in the 21st century."

Dr. Linnet Ongeri is a researcher in the field of mental health working at the Centre for Clinical Research in the Kenya Medical Research Institute (KEMRI) and a current postdoctoral fellow in the Global Initiative for Neuropsychiatric Genetics Education in Research (GINGER) program, hosted jointly by the Harvard T.H. Chan School of Public Health and the Stanley Center for Psychiatric Research at the Broad Institute of MIT and Harvard.

Having worked in Kenya for the last 10 years as a medical doctor and later as a psychiatrist, Dr. Ongeri has gained meaningful experience and understanding of the dire needs of the mentally ill in Kenya. She informs that a key challenge that stands out in both her clinical and research experience is the growing burden of mental illnesses in the country. Mental disorders are not only highly prevalent but also highly disabling if left untreated. Despite the existence of effective treatment, 75 percent of people with serious mental illness in the region do not receive any treatment. Inaccessibility of mental health treatment is a key contributor to the large treatment gap. The country and region grapple with inadequate financial and human resource allocation to mental health. Innovative programs that tap into already existing physical and human resources can go a long way to addressing this gap. It is for this same reason that she has been drawn to research approaches that strive to design culturally and regionally appropriate models of integrating mental health care in primary health care at community level.

ACHIEVEMENTS. /MILESTONES

Following the malaria, mental health and Immunity household survey, Dr. Ongeri has collaborated closely with Prof. Rachel Jenkins, formerly at WHO and now emeritus lecturer at King’s College London. Together, they continue to publish papers and apply for funding in collaboration with the team in the UK. She has also established strong networks with the University of California San Francisco (UCSF) through Prof. Susan Meffert her primary mentor for the GloCal (Fogarty) funding and the PI for the RCT focusing on interpersonal psychotherapy. They have recently been awarded an NIH grant to scale up the RCT work for IPT further strengthening this collaboration. In addition, her recent postdoc fellowship, she established strong networks with a number of researchers/faculty from Harvard University in Boston as well as the Broad Institute that focuses on genetics research work. This postdoc fellowship furthermore brings together 17 fellows from around Africa (Uganda, Ethiopia, South Africa and Kenya). They have penned a paper in the  Lancet  describing this strong collaboration that seeks to capacity build in the areas of psychiatry genetics in Africa. The Neurogap a research project looking at the genetics research in which Dr. Ongeri is  collaborating with Wellcome Trust as a co-investigator in the project. She is currently pursuing a PhD on Suicidal behavior as a sub-study of the NeuroGAP project, the PhD is through additional networks with Vrije University in Netherlands. Lastly, as an elected member (Treasurer) of the national executive committee for the Kenya Psychiatric Association, Dr. Ongeri is strategically positioned to foster numerous networks with national, regional and international partners interested in mental health related collaborations.

Title:  Neuropsychiatric Genetics of African Populations – Psychosis (NeuroGAP-Psychosis)

Goal : This study involves genetic analysis of saliva samples from 4,000 Kenyan participants (2,000 adults with psychotic disorder, including schizophrenia and bipolar disorder, (N=2000) of controls comprising those without psychotic disorders). Kenya is contributing to a total of 14,600 samples being recruited for NeuroGAP, with other sites in South Africa and Uganda. The study aims at estimating the fraction of psychosis and bipolar disorder attributable to inherited genetic differences, determining the distribution of genetic Copy Number Variants (CNV) in those with these disorders, and utilizing genome-wide association studies (GWAS) to look for new genetic variants associated with psychotic disorders in Africa.

Goal:  The study uses a mixed method approach to determine the prevalence, pattern and associated comorbidities related to drug and substance use among 1,691 participants in Kiambu and Nakuru Counties. The team explore stakeholders’ perceptions on the disease burden as well as best preventive and care treatment strategies. By conducting a pilot nested quasi experimental study on a sub sample of alcohol users, the team will generate evidence on integrated provision of mental health services by comparing the outcome of two recommended alcohol use disorder therapy modalities in health centres in Kiambu County.
