The background to your study, sometimes called the ‘state of the art’ (especially in grant writing), sets the scene for a paper. This section shows readers why your research is important, relevant, and why they should continue reading. You must hook them in with a great background to your study, which is part of the overall introduction to your research paper.

In higher impact articles, such as those published in Nature or Science (which is what we are all aiming for, after all …), the study background is t he middle section of an essentially three-part introduction . This section is framed by a presentation of ‘the question’ (first part of the introduction) and a quick explanation of ‘what this paper will do’ (the third part of the introduction).

The introduction of a research paper should be “shaped” like an upside down triangle: 

Start broad. Set the scene with a large-scale general research area [e.g., why doing a PhD erases your writing skills (ha ha) or mental health in teenagers and why this is such a widespread global issue] and then focus down to the question your research addresses (e.g., how can writing skills be improved in PhD students, or brain scans and how these can be used in treatment).

What is the background of a study?

The background to a study sets the scene . It lays out the “state of the art”. It tells your reader about other research done on the topic in question, via useful review papers and other summaries of the literature. 

A background is not a literature review: No one wants to read endless citations back-to-back in this section. You don’t need to list all the papers you’ve read, or all the work done in the past on this topic. 

Set the scene and frame your question in the context of the literature. Seek out review articles in particular. The aim of this section is to build on what has come before so your reader will be armed with all the information they need to understand the remainder of your article, and why - in context - the aims of your study are important.

How to write the background to your research paper

Cater to your audience.

It’s important to frame your background to the right audience.

The background of your study needs to be pitched differently depending on your target journal. A more subject-area specific journal (e.g. Journal of Brain Studies ) will be read by specialists in your field. Generally, less information to set up the paper in a wider context and less background information will be required. Your readers are already experts on the topic in question .

However, if you are aiming your paper at a more general audience (a journal like Nature or Science , for example) then you're going to need to explain more in your background. A reader of a specialized journal will know about the neocortex within the brain and where this is located, but a general reader will need you to set things up more.

Readers are always the most important people in research publishing, after all: If you want your work to be read, used, and cited (and therefore drive up your H-index as well as your institution’s ranking) you’ll need a well-pitched background of your study.

What is included in the background of a study?

Remember this section sits in the middle of the introduction. Here’s a handy template for what to include:

  • Existing research on the area of study (not everything, but a broad overview. Aim to cite review papers if you can). Start this section with preliminary data and then build it out;
  • Mention any controversies around your topic (either that you’ve identified, or that have been picked up by earlier work. Check the discussion sections of recent articles for pointers here);
  • Any gaps in existing research?, and;
  • How will your study fill these gaps? State your research methodologies. Any further research that needs to be done?

Aim for one paragraph , or a series of short paragraphs within one section. The last two of the topics outlined above can be short, just one or two sentences. These are there to hook the reader in and to frame your background so that the text leads into the final section of the introduction where you explain ‘What your paper is going to do’.

And finally…some thoughts

I used to get really bogged down with article writing, especially the shape of the introduction.

Here’s a trick to keep in mind: Remember that the average length of an academic research paper published in a peer reviewed journal is around 4,000 - 5,000 words - not too long. 

This means that you're likely going to be aiming for an article of about this length the next time you sit down to write: Not too many words for an effective and well-structured introduction. You’ve got about 1,500 - 2,000 words maximum. And aim to keep it short (this will be enforced by word count limits, especially in higher impact journals like Nature and Science ). Editors at these journals are trained to cut down your writing to make sure your research fits in.

Less is more, in other words.

Keeping tight word count limits in mind means you can’t write an expansive, flowing background to your study that goes off in all directions and covers a huge amount of ground. Keep an eye on our tips for what to include, cite review papers, and keep your readers interested in the question your paper seeks to address.

A well written background to your study will ensure your paper gets read all the way through to the end. Can’t ask for more than that!

What is the Background of the Study in Research? 

The background of the study is the first section of a research paper and gives context surrounding the research topic. The background explains to the reader where your research journey started, why you got interested in the topic, and how you developed the research question that you will later specify. That means that you first establish the context of the research you did with a general overview of the field or topic and then present the key issues that drove your decision to study the specific problem you chose.

Once the reader understands where you are coming from and why there was indeed a need for the research you are going to present in the following—because there was a gap in the current research, or because there is an obvious problem with a currently used process or technology—you can proceed with the formulation of your research question and summarize how you are going to address it in the rest of your manuscript.

Why is the Background of the Study Important?

No matter how surprising and important the findings of your study are, if you do not provide the reader with the necessary background information and context, they will not be able to understand your reasons for studying the specific problem you chose and why you think your study is relevant. And more importantly, an editor who does not share your enthusiasm for your work (because you did not fill them in on all the important details) will very probably not even consider your manuscript worthy of their and the reviewers’ time and will immediately send it back to you.

To avoid such desk rejections , you need to make sure you pique the reader’s interest and help them understand the contribution of your work to the specific field you study, the more general research community, or the public. Introducing the study background is crucial to setting the scene for your readers.

Background of the Study Structure

Before writing your study background, it is essential to understand what to include. The following elements should all be included in the background and are presented in greater detail in the next section:

  • A general overview of the topic and why it is important (overlaps with establishing the “importance of the topic” in the Introduction)
  • The current state of the research on the topic or on related topics in the field
  • Controversies about current knowledge or specific past studies that undergird your research methodology
  • Any claims or assumptions that have been made by researchers, institutions, or politicians that might need to be clarified
  • Methods and techniques used in the study or from which your study deviated in some way

Presenting the Study Background

As you begin introducing your background, you first need to provide a general overview and include the main issues concerning the topic. Depending on whether you do “basic” (with the aim of providing further knowledge) or “applied” research (to establish new techniques, processes, or products), this is either a literature review that summarizes all relevant earlier studies in the field or a description of the process (e.g., vote counting) or practice (e.g., diagnosis of a specific disease) that you think is problematic or lacking and needs a solution.

Example s of a general overview

If you study the function of a Drosophila gene, for example, you can explain to the reader why and for whom the study of fly genetics is relevant, what is already known and established, and where you see gaps in the existing literature. If you investigated how the way universities have transitioned into online teaching since the beginning of the Covid-19 pandemic has affected students’ learning progress, then you need to present a summary of what changes have happened around the world, what the effects of those changes have been so far, and where you see problems that need to be addressed. Note that you need to provide sources for every statement and every claim you make here, to establish a solid foundation of knowledge for your own study. 

Describing the current state of knowledge

When the reader understands the main issue(s), you need to fill them in more specifically on the current state of the field (in basic research) or the process/practice/product use you describe (in practical/applied research). Cite all relevant studies that have already reported on the Drosophila gene you are interested in, have failed to reveal certain functions of it, or have suggested that it might be involved in more processes than we know so far. Or list the reports from the education ministries of the countries you are interested in and highlight the data that shows the need for research into the effects of the Corona-19 pandemic on teaching and learning.

Discussing controversies, claims, and assumptions

Are there controversies regarding your topic of interest that need to be mentioned and/or addressed? For example, if your research topic involves an issue that is politically hot, you can acknowledge this here. Have any earlier claims or assumptions been made, by other researchers, institutions, or politicians, that you think need to be clarified?

Mentioning methodologies and approaches

While putting together these details, you also need to mention methodologies : What methods/techniques have been used so far to study what you studied and why are you going to either use the same or a different approach? Are any of the methods included in the literature review flawed in such a way that your study takes specific measures to correct or update? While you shouldn’t spend too much time here justifying your methods (this can be summarized briefly in the rationale of the study at the end of the Introduction and later in the Discussion section), you can engage with the crucial methods applied in previous studies here first.

When you have established the background of the study of your research paper in such a logical way, then the reader should have had no problem following you from the more general information you introduced first to the specific details you added later. You can now easily lead over to the relevance of your research, explain how your work fits into the bigger picture, and specify the aims and objectives of your study. This latter part is usually considered the “ statement of the problem ” of your study. Without a solid research paper background, this statement will come out of nowhere for the reader and very probably raise more questions than you were planning to answer.   

Where does the study background section go in a paper?

Unless you write a research proposal or some kind of report that has a specific “Background” chapter, the background of your study is the first part of your introduction section . This is where you put your work in context and provide all the relevant information the reader needs to follow your rationale. Make sure your background has a logical structure and naturally leads into the statement of the problem at the very end of the introduction so that you bring everything together for the reader to judge the relevance of your work and the validity of your approach before they dig deeper into the details of your study in the methods section .

Background information identifies and describes the history and nature of a well-defined research problem with reference to the existing literature.   • Political -- concerns the environment in which something is produced indicating it's public purpose or agenda.
  • Social -- the environment of people that surrounds something's creation or intended audience, reflecting how the people around something use and interpret it.
  • Temporal -- reflects issues or events of, relating to, or limited by time.

Background information can also include summaries of important, relevant research studies . The key is to summarize for the reader what is known about the specific research problem before you conducted your analysis. This is accomplished with a general review of the foundational research literature (with citations) that report findings that inform your study's aims and objectives.

NOTE : Research studies cited as part of the background information of your introduction should not include very specific, lengthy explanations. This should be discussed in greater detail in your literature review section.

Providing background information in the Introduction of a research paper serves as a bridge that links the reader to the topic of your study . But precisely how long and in-depth this bridge should be is largely dependent upon how much information you think the reader will need in order to understand the research problem being discussed and to appreciate why the issues you are investigating are important.

From another perspective, the length and detail of background information also depends on the degree to which you need to demonstrate to your professor how much you understand the topic. Keep this in mind because providing succinct background information can be an effective way to show that you have a clear grasp of key issues and concepts underpinning your overall study. Don't try to show off, though!

Given that the structure and writing style of your background information can vary depending upon the complexity of your research and/or the nature of the assignment, here are some questions to consider while writing :

  • Are there concepts, terms, theories, or ideas that may be unfamiliar to the reader and, thus, require additional explanation?
  • Are there historical elements that need to be explored in order to add needed context, to highlight specific people, issues, or events, or to lay a foundation for understanding the emergence of a current issue or event?
  • Is the research study unusual in some way that requires additional explanation, such as, a) your study uses a method never applied before to the research problem you are investigating; b) your study investigates a very esoteric or complex research problem; or, c) your study relies upon analyzing unique texts or documents, such as archival materials or primary documents like diaries or personal letters, that do not represent the established body of source literature on the topic.

Although in social sciences research introductory background information can often blend into the literature review portion of the paper, basic background information should not be considered a substitute for a comprehensive review and synthesis of relevant research literature.

The introduction leads the reader from a general subject area to a particular topic of inquiry. It establishes the scope, context, and significance of the research being conducted by summarizing current understanding and background information about the topic, stating the purpose of the work in the form of the research problem supported by a hypothesis or a set of questions, explaining briefly the methodological approach used to examine the research problem, highlighting the potential outcomes your study can reveal, and outlining the remaining structure and organization of the paper.

Importance of a Good Introduction

Think of the introduction as a mental road map that must answer for the reader these four questions:

  • What was I studying?
  • Why was this topic important to investigate?
  • What did we know about this topic before I did this study?
  • How will this study advance new knowledge or new ways of understanding?

According to Reyes, there are three overarching goals of a good introduction: 1) ensure that you summarize prior studies about the topic in a manner that lays a foundation for understanding the research problem; 2) explain how your study specifically addresses gaps in the literature, insufficient consideration of the topic, or other deficiency in the literature; and, 3) note the broader theoretical, empirical, and/or policy contributions and implications of your research.

A well-written introduction is important because, quite simply, you never get a second chance to make a good first impression. The opening paragraphs of your paper will provide your readers with their initial impressions about the logic of your argument, your writing style, the overall quality of your research, and, ultimately, the validity of your findings and conclusions. A vague, disorganized, or error-filled introduction will create a negative impression, whereas, a concise, engaging, and well-written introduction will lead your readers to think highly of your analytical skills, your writing style, and your research approach. All introductions should conclude with a brief paragraph that describes the organization of the rest of the paper.

Structure and Writing Style

I.  Structure and Approach

The introduction is the broad beginning of the paper that answers three important questions for the reader:

  • What is this?
  • Why should I read it?
  • What do you want me to think about / consider doing / react to?

Think of the structure of the introduction as an inverted triangle of information that lays a foundation for understanding the research problem. Organize the information so as to present the more general aspects of the topic early in the introduction, then narrow your analysis to more specific topical information that provides context, finally arriving at your research problem and the rationale for studying it [often written as a series of key questions to be addressed or framed as a hypothesis or set of assumptions to be tested] and, whenever possible, a description of the potential outcomes your study can reveal.

These are general phases associated with writing an introduction: 1.  Establish an area to research by:

  • Highlighting the importance of the topic, and/or
  • Making general statements about the topic, and/or
  • Presenting an overview on current research on the subject.

2.  Identify a research niche by:

  • Opposing an existing assumption, and/or
  • Revealing a gap in existing research, and/or
  • Formulating a research question or problem, and/or
  • Continuing a disciplinary tradition.

3.  Place your research within the research niche by:

  • Stating the intent of your study,
  • Outlining the key characteristics of your study,
  • Describing important results, and
  • Giving a brief overview of the structure of the paper.

NOTE:   It is often useful to review the introduction late in the writing process. This is appropriate because outcomes are unknown until you've completed the study. After you complete writing the body of the paper, go back and review introductory descriptions of the structure of the paper, the method of data gathering, the reporting and analysis of results, and the conclusion. Reviewing and, if necessary, rewriting the introduction ensures that it correctly matches the overall structure of your final paper.

II.  Delimitations of the Study

Delimitations refer to those characteristics that limit the scope and define the conceptual boundaries of your research . This is determined by the conscious exclusionary and inclusionary decisions you make about how to investigate the research problem. In other words, not only should you tell the reader what it is you are studying and why, but you must also acknowledge why you rejected alternative approaches that could have been used to examine the topic.

Obviously, the first limiting step was the choice of research problem itself. However, implicit are other, related problems that could have been chosen but were rejected. These should be noted in the conclusion of your introduction. For example, a delimitating statement could read, "Although many factors can be understood to impact the likelihood young people will vote, this study will focus on socioeconomic factors related to the need to work full-time while in school." The point is not to document every possible delimiting factor, but to highlight why previously researched issues related to the topic were not addressed.

Examples of delimitating choices would be:

  • The key aims and objectives of your study,
  • The research questions that you address,
  • The variables of interest [i.e., the various factors and features of the phenomenon being studied],
  • The method(s) of investigation,
  • The time period your study covers, and
  • Any relevant alternative theoretical frameworks that could have been adopted.

Review each of these decisions. Not only do you clearly establish what you intend to accomplish in your research, but you should also include a declaration of what the study does not intend to cover. In the latter case, your exclusionary decisions should be based upon criteria understood as, "not interesting"; "not directly relevant"; “too problematic because..."; "not feasible," and the like. Make this reasoning explicit!

NOTE:   Delimitations refer to the initial choices made about the broader, overall design of your study and should not be confused with documenting the limitations of your study discovered after the research has been completed.

ANOTHER NOTE: Do not view delimitating statements as admitting to an inherent failing or shortcoming in your research. They are an accepted element of academic writing intended to keep the reader focused on the research problem by explicitly defining the conceptual boundaries and scope of your study. It addresses any critical questions in the reader's mind of, "Why the hell didn't the author examine this?"

III.  The Narrative Flow

Issues to keep in mind that will help the narrative flow in your introduction :

  • Your introduction should clearly identify the subject area of interest . A simple strategy to follow is to use key words from your title in the first few sentences of the introduction. This will help focus the introduction on the topic at the appropriate level and ensures that you get to the subject matter quickly without losing focus, or discussing information that is too general.
  • Establish context by providing a brief and balanced review of the pertinent published literature that is available on the subject. The key is to summarize for the reader what is known about the specific research problem before you did your analysis. This part of your introduction should not represent a comprehensive literature review--that comes next. It consists of a general review of the important, foundational research literature [with citations] that establishes a foundation for understanding key elements of the research problem. See the drop-down menu under this tab for " Background Information " regarding types of contexts.
  • Clearly state the hypothesis that you investigated . When you are first learning to write in this format it is okay, and actually preferable, to use a past statement like, "The purpose of this study was to...." or "We investigated three possible mechanisms to explain the...."
  • Why did you choose this kind of research study or design? Provide a clear statement of the rationale for your approach to the problem studied. This will usually follow your statement of purpose in the last paragraph of the introduction.

IV.  Engaging the Reader

A research problem in the social sciences can come across as dry and uninteresting to anyone unfamiliar with the topic . Therefore, one of the goals of your introduction is to make readers want to read your paper. Here are several strategies you can use to grab the reader's attention:

  • Open with a compelling story . Almost all research problems in the social sciences, no matter how obscure or esoteric , are really about the lives of people. Telling a story that humanizes an issue can help illuminate the significance of the problem and help the reader empathize with those affected by the condition being studied.
  • Include a strong quotation or a vivid, perhaps unexpected, anecdote . During your review of the literature, make note of any quotes or anecdotes that grab your attention because they can used in your introduction to highlight the research problem in a captivating way.
  • Pose a provocative or thought-provoking question . Your research problem should be framed by a set of questions to be addressed or hypotheses to be tested. However, a provocative question can be presented in the beginning of your introduction that challenges an existing assumption or compels the reader to consider an alternative viewpoint that helps establish the significance of your study. 
  • Describe a puzzling scenario or incongruity . This involves highlighting an interesting quandary concerning the research problem or describing contradictory findings from prior studies about a topic. Posing what is essentially an unresolved intellectual riddle about the problem can engage the reader's interest in the study.
  • Cite a stirring example or case study that illustrates why the research problem is important . Draw upon the findings of others to demonstrate the significance of the problem and to describe how your study builds upon or offers alternatives ways of investigating this prior research.

NOTE:   It is important that you choose only one of the suggested strategies for engaging your readers. This avoids giving an impression that your paper is more flash than substance and does not distract from the substance of your study.

Avoid the "Dictionary" Introduction

Giving the dictionary definition of words related to the research problem may appear appropriate because it is important to define specific terminology that readers may be unfamiliar with. However, anyone can look a word up in the dictionary and a general dictionary is not a particularly authoritative source because it doesn't take into account the context of your topic and doesn't offer particularly detailed information. Also, placed in the context of a particular discipline, a term or concept may have a different meaning than what is found in a general dictionary. If you feel that you must seek out an authoritative definition, use a subject specific dictionary or encyclopedia [e.g., if you are a sociology student, search for dictionaries of sociology]. A good database for obtaining definitive definitions of concepts or terms is Credo Reference .

When Do I Begin?

A common question asked at the start of any paper is, "Where should I begin?" An equally important question to ask yourself is, "When do I begin?" Research problems in the social sciences rarely rest in isolation from history. Therefore, it is important to lay a foundation for understanding the historical context underpinning the research problem. However, this information should be brief and succinct and begin at a point in time that illustrates the study's overall importance. For example, a study that investigates coffee cultivation and export in West Africa as a key stimulus for local economic growth needs to describe the beginning of exporting coffee in the region and establishing why economic growth is important. You do not need to give a long historical explanation about coffee exports in Africa. If a research problem requires a substantial exploration of the historical context, do this in the literature review section. In your introduction, make note of this as part of the "roadmap" [see below] that you use to describe the organization of your paper.

Always End with a Roadmap

The final paragraph or sentences of your introduction should forecast your main arguments and conclusions and provide a brief description of the rest of the paper [the "roadmap"] that let's the reader know where you are going and what to expect. A roadmap is important because it helps the reader place the research problem within the context of their own perspectives about the topic. In addition, concluding your introduction with an explicit roadmap tells the reader that you have a clear understanding of the structural purpose of your paper. In this way, the roadmap acts as a type of promise to yourself and to your readers that you will follow a consistent and coherent approach to addressing the topic of inquiry. Refer to it often to help keep your writing focused and organized.

A Practical Guide to Writing Quantitative and Qualitative Research Questions and Hypotheses in Scholarly Articles

The development of research questions and the subsequent hypotheses are prerequisites to defining the main research purpose and specific objectives of a study. Consequently, these objectives determine the study design and research outcome. The development of research questions is a process based on knowledge of current trends, cutting-edge studies, and technological advances in the research field. Excellent research questions are focused and require a comprehensive literature search and in-depth understanding of the problem being investigated. Initially, research questions may be written as descriptive questions which could be developed into inferential questions. These questions must be specific and concise to provide a clear foundation for developing hypotheses. Hypotheses are more formal predictions about the research outcomes. These specify the possible results that may or may not be expected regarding the relationship between groups. Thus, research questions and hypotheses clarify the main purpose and specific objectives of the study, which in turn dictate the design of the study, its direction, and outcome. Studies developed from good research questions and hypotheses will have trustworthy outcomes with wide-ranging social and health implications.

INTRODUCTION

Scientific research is usually initiated by posing evidenced-based research questions which are then explicitly restated as hypotheses. 1 , 2 The hypotheses provide directions to guide the study, solutions, explanations, and expected results. 3 , 4 Both research questions and hypotheses are essentially formulated based on conventional theories and real-world processes, which allow the inception of novel studies and the ethical testing of ideas. 5 , 6

It is crucial to have knowledge of both quantitative and qualitative research 2 as both types of research involve writing research questions and hypotheses. 7 However, these crucial elements of research are sometimes overlooked; if not overlooked, then framed without the forethought and meticulous attention it needs. Planning and careful consideration are needed when developing quantitative or qualitative research, particularly when conceptualizing research questions and hypotheses. 4

There is a continuing need to support researchers in the creation of innovative research questions and hypotheses, as well as for journal articles that carefully review these elements. 1 When research questions and hypotheses are not carefully thought of, unethical studies and poor outcomes usually ensue. Carefully formulated research questions and hypotheses define well-founded objectives, which in turn determine the appropriate design, course, and outcome of the study. This article then aims to discuss in detail the various aspects of crafting research questions and hypotheses, with the goal of guiding researchers as they develop their own. Examples from the authors and peer-reviewed scientific articles in the healthcare field are provided to illustrate key points.

DEFINITIONS AND RELATIONSHIP OF RESEARCH QUESTIONS AND HYPOTHESES

A research question is what a study aims to answer after data analysis and interpretation. The answer is written in length in the discussion section of the paper. Thus, the research question gives a preview of the different parts and variables of the study meant to address the problem posed in the research question. 1 An excellent research question clarifies the research writing while facilitating understanding of the research topic, objective, scope, and limitations of the study. 5

On the other hand, a research hypothesis is an educated statement of an expected outcome. This statement is based on background research and current knowledge. 8 , 9 The research hypothesis makes a specific prediction about a new phenomenon 10 or a formal statement on the expected relationship between an independent variable and a dependent variable. 3 , 11 It provides a tentative answer to the research question to be tested or explored. 4

Hypotheses employ reasoning to predict a theory-based outcome. 10 These can also be developed from theories by focusing on components of theories that have not yet been observed. 10 The validity of hypotheses is often based on the testability of the prediction made in a reproducible experiment. 8

Conversely, hypotheses can also be rephrased as research questions. Several hypotheses based on existing theories and knowledge may be needed to answer a research question. Developing ethical research questions and hypotheses creates a research design that has logical relationships among variables. These relationships serve as a solid foundation for the conduct of the study. 4 , 11 Haphazardly constructed research questions can result in poorly formulated hypotheses and improper study designs, leading to unreliable results. Thus, the formulations of relevant research questions and verifiable hypotheses are crucial when beginning research. 12

CHARACTERISTICS OF GOOD RESEARCH QUESTIONS AND HYPOTHESES

Excellent research questions are specific and focused. These integrate collective data and observations to confirm or refute the subsequent hypotheses. Well-constructed hypotheses are based on previous reports and verify the research context. These are realistic, in-depth, sufficiently complex, and reproducible. More importantly, these hypotheses can be addressed and tested. 13

There are several characteristics of well-developed hypotheses. Good hypotheses are 1) empirically testable 7 , 10 , 11 , 13 ; 2) backed by preliminary evidence 9 ; 3) testable by ethical research 7 , 9 ; 4) based on original ideas 9 ; 5) have evidenced-based logical reasoning 10 ; and 6) can be predicted. 11 Good hypotheses can infer ethical and positive implications, indicating the presence of a relationship or effect relevant to the research theme. 7 , 11 These are initially developed from a general theory and branch into specific hypotheses by deductive reasoning. In the absence of a theory to base the hypotheses, inductive reasoning based on specific observations or findings form more general hypotheses. 10

TYPES OF RESEARCH QUESTIONS AND HYPOTHESES

Research questions and hypotheses are developed according to the type of research, which can be broadly classified into quantitative and qualitative research. We provide a summary of the types of research questions and hypotheses under quantitative and qualitative research categories in Table 1 .

Research questions in quantitative research

In quantitative research, research questions inquire about the relationships among variables being investigated and are usually framed at the start of the study. These are precise and typically linked to the subject population, dependent and independent variables, and research design. 1 Research questions may also attempt to describe the behavior of a population in relation to one or more variables, or describe the characteristics of variables to be measured ( descriptive research questions ). 1 , 5 , 14 These questions may also aim to discover differences between groups within the context of an outcome variable ( comparative research questions ), 1 , 5 , 14 or elucidate trends and interactions among variables ( relationship research questions ). 1 , 5 We provide examples of descriptive, comparative, and relationship research questions in quantitative research in Table 2 .

Hypotheses in quantitative research

In quantitative research, hypotheses predict the expected relationships among variables. 15 Relationships among variables that can be predicted include 1) between a single dependent variable and a single independent variable ( simple hypothesis ) or 2) between two or more independent and dependent variables ( complex hypothesis ). 4 , 11 Hypotheses may also specify the expected direction to be followed and imply an intellectual commitment to a particular outcome ( directional hypothesis ) 4 . On the other hand, hypotheses may not predict the exact direction and are used in the absence of a theory, or when findings contradict previous studies ( non-directional hypothesis ). 4 In addition, hypotheses can 1) define interdependency between variables ( associative hypothesis ), 4 2) propose an effect on the dependent variable from manipulation of the independent variable ( causal hypothesis ), 4 3) state a negative relationship between two variables ( null hypothesis ), 4 , 11 , 15 4) replace the working hypothesis if rejected ( alternative hypothesis ), 15 explain the relationship of phenomena to possibly generate a theory ( working hypothesis ), 11 5) involve quantifiable variables that can be tested statistically ( statistical hypothesis ), 11 6) or express a relationship whose interlinks can be verified logically ( logical hypothesis ). 11 We provide examples of simple, complex, directional, non-directional, associative, causal, null, alternative, working, statistical, and logical hypotheses in quantitative research, as well as the definition of quantitative hypothesis-testing research in Table 3 .

Research questions in qualitative research

Unlike research questions in quantitative research, research questions in qualitative research are usually continuously reviewed and reformulated. The central question and associated subquestions are stated more than the hypotheses. 15 The central question broadly explores a complex set of factors surrounding the central phenomenon, aiming to present the varied perspectives of participants. 15

There are varied goals for which qualitative research questions are developed. These questions can function in several ways, such as to 1) identify and describe existing conditions ( contextual research question s); 2) describe a phenomenon ( descriptive research questions ); 3) assess the effectiveness of existing methods, protocols, theories, or procedures ( evaluation research questions ); 4) examine a phenomenon or analyze the reasons or relationships between subjects or phenomena ( explanatory research questions ); or 5) focus on unknown aspects of a particular topic ( exploratory research questions ). 5 In addition, some qualitative research questions provide new ideas for the development of theories and actions ( generative research questions ) or advance specific ideologies of a position ( ideological research questions ). 1 Other qualitative research questions may build on a body of existing literature and become working guidelines ( ethnographic research questions ). Research questions may also be broadly stated without specific reference to the existing literature or a typology of questions ( phenomenological research questions ), may be directed towards generating a theory of some process ( grounded theory questions ), or may address a description of the case and the emerging themes ( qualitative case study questions ). 15 We provide examples of contextual, descriptive, evaluation, explanatory, exploratory, generative, ideological, ethnographic, phenomenological, grounded theory, and qualitative case study research questions in qualitative research in Table 4 , and the definition of qualitative hypothesis-generating research in Table 5 .

Qualitative studies usually pose at least one central research question and several subquestions starting with How or What . These research questions use exploratory verbs such as explore or describe . These also focus on one central phenomenon of interest, and may mention the participants and research site. 15

Hypotheses in qualitative research

Hypotheses in qualitative research are stated in the form of a clear statement concerning the problem to be investigated. Unlike in quantitative research where hypotheses are usually developed to be tested, qualitative research can lead to both hypothesis-testing and hypothesis-generating outcomes. 2 When studies require both quantitative and qualitative research questions, this suggests an integrative process between both research methods wherein a single mixed-methods research question can be developed. 1

FRAMEWORKS FOR DEVELOPING RESEARCH QUESTIONS AND HYPOTHESES

Research questions followed by hypotheses should be developed before the start of the study. 1 , 12 , 14 It is crucial to develop feasible research questions on a topic that is interesting to both the researcher and the scientific community. This can be achieved by a meticulous review of previous and current studies to establish a novel topic. Specific areas are subsequently focused on to generate ethical research questions. The relevance of the research questions is evaluated in terms of clarity of the resulting data, specificity of the methodology, objectivity of the outcome, depth of the research, and impact of the study. 1 , 5 These aspects constitute the FINER criteria (i.e., Feasible, Interesting, Novel, Ethical, and Relevant). 1 Clarity and effectiveness are achieved if research questions meet the FINER criteria. In addition to the FINER criteria, Ratan et al. described focus, complexity, novelty, feasibility, and measurability for evaluating the effectiveness of research questions. 14

The PICOT and PEO frameworks are also used when developing research questions. 1 The following elements are addressed in these frameworks, PICOT: P-population/patients/problem, I-intervention or indicator being studied, C-comparison group, O-outcome of interest, and T-timeframe of the study; PEO: P-population being studied, E-exposure to preexisting conditions, and O-outcome of interest. 1 Research questions are also considered good if these meet the “FINERMAPS” framework: Feasible, Interesting, Novel, Ethical, Relevant, Manageable, Appropriate, Potential value/publishable, and Systematic. 14

As we indicated earlier, research questions and hypotheses that are not carefully formulated result in unethical studies or poor outcomes. To illustrate this, we provide some examples of ambiguous research question and hypotheses that result in unclear and weak research objectives in quantitative research ( Table 6 ) 16 and qualitative research ( Table 7 ) 17 , and how to transform these ambiguous research question(s) and hypothesis(es) into clear and good statements.

CONSTRUCTING RESEARCH QUESTIONS AND HYPOTHESES

To construct effective research questions and hypotheses, it is very important to 1) clarify the background and 2) identify the research problem at the outset of the research, within a specific timeframe. 9 Then, 3) review or conduct preliminary research to collect all available knowledge about the possible research questions by studying theories and previous studies. 18 Afterwards, 4) construct research questions to investigate the research problem. Identify variables to be accessed from the research questions 4 and make operational definitions of constructs from the research problem and questions. Thereafter, 5) construct specific deductive or inductive predictions in the form of hypotheses. 4 Finally, 6) state the study aims . This general flow for constructing effective research questions and hypotheses prior to conducting research is shown in Fig. 1 .

Research questions are used more frequently in qualitative research than objectives or hypotheses. 3 These questions seek to discover, understand, explore or describe experiences by asking “What” or “How.” The questions are open-ended to elicit a description rather than to relate variables or compare groups. The questions are continually reviewed, reformulated, and changed during the qualitative study. 3 Research questions are also used more frequently in survey projects than hypotheses in experiments in quantitative research to compare variables and their relationships.

Hypotheses are constructed based on the variables identified and as an if-then statement, following the template, ‘If a specific action is taken, then a certain outcome is expected.’ At this stage, some ideas regarding expectations from the research to be conducted must be drawn. 18 Then, the variables to be manipulated (independent) and influenced (dependent) are defined. 4 Thereafter, the hypothesis is stated and refined, and reproducible data tailored to the hypothesis are identified, collected, and analyzed. 4 The hypotheses must be testable and specific, 18 and should describe the variables and their relationships, the specific group being studied, and the predicted research outcome. 18 Hypotheses construction involves a testable proposition to be deduced from theory, and independent and dependent variables to be separated and measured separately. 3 Therefore, good hypotheses must be based on good research questions constructed at the start of a study or trial. 12

In summary, research questions are constructed after establishing the background of the study. Hypotheses are then developed based on the research questions. Thus, it is crucial to have excellent research questions to generate superior hypotheses. In turn, these would determine the research objectives and the design of the study, and ultimately, the outcome of the research. 12 Algorithms for building research questions and hypotheses are shown in Fig. 2 for quantitative research and in Fig. 3 for qualitative research.

Research questions and hypotheses are crucial components to any type of research, whether quantitative or qualitative. These questions should be developed at the very beginning of the study. Excellent research questions lead to superior hypotheses, which, like a compass, set the direction of research, and can often determine the successful conduct of the study. Many research studies have floundered because the development of research questions and subsequent hypotheses was not given the thought and meticulous attention needed. The development of research questions and hypotheses is an iterative process based on extensive knowledge of the literature and insightful grasp of the knowledge gap. Focused, concise, and specific research questions provide a strong foundation for constructing hypotheses which serve as formal predictions about the research outcomes. Research questions and hypotheses are crucial elements of research that should not be overlooked. They should be carefully thought of and constructed when planning research. This avoids unethical studies and poor outcomes by defining well-founded objectives that determine the design, course, and outcome of the study.

Specifically, when writing research background you can discuss major theories and models related to your research problem in a chronological order to outline historical developments in the research area.  When writing research background, you also need to demonstrate how your research relates to what has been done so far in the research area.

Research background is written after the literature review. Therefore, literature review has to be the first and the longest stage in the research process, even before the formulation of research aims and objectives, right after the selection of the research area. Once the research area is selected, the literature review is commenced in order to identify gaps in the research area.

Research aims and objectives need to be closely associated with the elimination of this gap in the literature. Background/Aims: Physical inactivity is common in patients with chronic kidney disease (CKD) and is an important modifiable risk factor for mortality, morbidity, and reduced quality of life. The present single-centre pilot study evaluated the possibility of performing structured physical exercise using a specific walking model, Fitwalking, in a population of patients with CKD and, according to the American College of Rheumatology guidelines, also in a population with immuno-rheumatological disease. Methods: Patients were recruited from the nephrology, haemodialysis, peritoneal dialysis, transplantation, and immuno-rheumatology outpatient clinics. After general and functional clinical evaluation and exercise prescription at the Department of Sports Medicine, we performed scientifically proven tests on CKD (Six-minute walk test and Sit to stand test), before and after the Fitwalking technique training course, and again after 6 and 12 months, to evaluate its effectiveness and identify any critical issues. Results: We enrolled 80 patients, (41 males, 51.2%), mean age 53±12 years; the clinical data showed statistically significant improvements in systolic, average, and differential blood pressure, average speed, and physical strength. Participants also adapted to muscle fatigue, experienced a reduction in BMI with stable lean mass and reduced fat mass, and reported improved perceptions of physical and mental health, and quality of life. Conclusion: All enrolled patients successfully completed the process. A specific prescription was used that provided health education and allowed for the implementation of structured physical activity that could be performed safely and independently even after the training period. The activity was sustainable thanks to the training of in-house medical and nursing staff, demonstrating that it is possible to overcome this type of barrier to physical activity in CKD and in immuno-rheumatological patients.

Health Care Professionals’ Experiences With Using Information and Communication Technologies in Patient Care During the COVID-19 Pandemic: Qualitative Study

Background: The COVID-19 pandemic acted as a catalyst for the use of information and communication technology (ICT) in inpatient and outpatient health care settings. Digital tools were used to connect patients, families, and providers amid visitor restrictions, while web-based platforms were used to continue care amid COVID-19 lockdowns. What we have yet to learn is the experiences of health care providers (HCPs) regarding the use of ICT that supported changes to clinical care during the COVID-19 pandemic.

Objective: The aim of this paper was to describe the experiences of HCPs in using ICT to support clinical care changes during the COVID-19 pandemic. This paper is reporting on a subset of a larger body of data that examined changes to models of care during the pandemic.

Methods: This study used a qualitative, descriptive study design. In total, 30 HCPs were recruited from 3 hospitals in Canada. One-on-one semistructured interviews were conducted between December 2022 and June 2023. Qualitative data were analyzed using an inductive thematic approach to identify themes across participants.

Results: A total of 30 interviews with HCPs revealed 3 themes related to their experiences using ICT to support changes to clinical care during the COVID-19 pandemic. These included the use of ICT (1) to support in-person communication with patients, (2) to facilitate connection between provider to patient and patient to family, and (3) to provide continuity of care.

Conclusions: HCP narratives revealed the benefits of digital tools to support in-person communication between patient and provider, the need for thoughtful consideration for the use of ICT at end-of-life care, and the decision-making that is needed when choosing service delivery modality (eg, web based or in person). Moving forward, organizations are encouraged to provide education and training on how to support patient-provider communication, find ways to meet patient and family wishes at end-of-life care, and continue to give autonomy to HCPs in their clinical decision-making regarding service delivery modality.

Introduction

The health care workforce had to quickly adapt to the COVID-19 pandemic, with health systems grappling with the provision of COVID-19 care at the same time as non-COVID-19 care. Restrictions to reduce the spread of COVID-19 put an additional strain on the health care system. Health care providers (HCPs) were left to problem-solve how to continue providing compassionate, connected care among layers of personal protective equipment and visitor restrictions. Fortunately, the COVID-19 pandemic was a catalyst for digital health to support the ongoing response to the COVID-19 pandemic, with web-based care emerging as the primary innovation of information and communication technology (ICT) used in medical care [ 1 , 2 ]. Uses of ICT in medical care include remote consultations, digital noninvasive care, and digital platforms for data sharing [ 3 ].

ICT played an important role in supporting changes to clinical care within inpatient and outpatient health care settings. Within inpatient settings, ICT was integral in maintaining connectivity between patients, families, and providers when changes to visitor policies were implemented [ 4 ]. For example, the use of mobile devices and tablets allowed for connection between patient and family and supported knowledge transfer between provider and family [ 5 ]. Within outpatient settings, ICT was integral in continuing care when COVID-19 lockdown restrictions limited in-person visits [ 1 ]. For example, videoconference and telemedicine services (ie, web-based care) emerged as a platform for providers to use to allow for remote care [ 1 ]. In both facets, ICT facilitated connection, acting as an essential link between patients, families, and providers. However, we have yet to learn of HCPs’ experiences in using ICT to support clinical care.

Learning from the experiences of HCPs’ use of ICT will offer valuable insights into how innovative uses of ICT might continue to be used in inpatient and outpatient health care settings moving forward. From here, uses of ICT can inform organizational leadership of the systems or processes that may require further investigation to support ICT use in clinical care in a postpandemic world. The main objective of the study was to examine changes to models of care during the pandemic from the perspectives of HCPs, implementation team members, and leaders across 3 Canadian hospitals. For this paper, we report on a storyline that emerged from this work to describe the experiences of HCPs’ use of ICT that supported changes to clinical care during the COVID-19 pandemic.

Study Design

This qualitative descriptive study was undertaken from March 2022 to June 2023 to understand changes to models of care during the COVID-19 pandemic through the experiences of HCPs, implementation team members, and leaders across 3 hospitals in Canada. This paper is reporting on a subset of data related to HCPs’ experiences of using ICT in supporting changes to clinical care, drawn from the larger study that explored changes to models of care that took place during the COVID-19 pandemic. The reporting of this study was guided by the Standards for Reporting Qualitative Research [ 6 ].

Sampling and Participant Recruitment

In total, 30 HCPs were recruited from critical care, inpatient, and ambulatory services across 3 hospitals in Canada. A purposeful sampling strategy was used where recruiting took place in organizations that were known to have been affected by COVID-19 restrictions and policies. Site leads at participating institutions disseminated study information to HCPs (eg, nurses, physicians, and allied health disciplines) working within their respective health care organizations. From here, interview participants self-referred to this study. Inclusion criteria included current employment as an HCP working at the health care organization over the course of the pandemic and postpandemic recovery.

Data Collection

One-on-one, semistructured interviews were conducted by members of the research team (Kang Kang Margolese, Marina Morris, Lily Zeng, Marie Oliveira, Adebisi Akande, HR, Frances Bruno, or CAC) between December 2022 and June 2023. Demographic information, including age, gender, ethnicity, health discipline, time in profession, time in organization, and time in current role, was collected from all participants before the interview to ensure diversity within the sample. An interview guide was developed by the research team that explored the following five areas: (1) changes to care (eg, “What was your role like before the pandemic? How did care change over the course of the last 3 years?”), (2) provisions of care (eg, “What did you/your team start/stop doing? How did you prioritize care?”), (3) emotions (eg, “How did care change feel for you/your team? What supports were available to you?”), (4) implementation and evaluation (eg, “How were changes implemented and evaluated?”), and (5) lessons that were learned or future recommendations.

Data collection was completed by nonclinical research staff (Kang Kang Margolese, Marina Morris, Lily Zeng, Adebisi Akande, and HR) and clinical research staff (Marie Oliveira, Frances Bruno, and CAC). Data collection was concluded when saturation of themes was reached, meaning that limited new insights emerged from existing themes with the collected data sample [ 7 ]. The interviews were conducted via either a videoconferencing platform or in person and were approximately 45 to 60 minutes in length.

Ethical Considerations

Ethics was formally reviewed and approved by Sinai Health’s Research Ethics Board (REB# 22-0153-E), as well as at each participating site: Sunnybrook Health Sciences Centre (REB# 5571) and Providence Health Care (REB# H22-02792). Participants were informed that participation in this study was completely voluntary and that they could withdraw from the study at any time without penalty. Verbal informed consent was obtained before the start of the interviews, and participants were given an electronic gift card in recognition of their time. The honorarium for participants was CAD $20 (US $26.4). Demographic information was collected from all participants before the interview. These data were anonymized and stored separately from the transcripts, which were deidentified and stored on a secure server.

Data Analysis

The research design was conceived within an interpretivist paradigm, where the researchers’ purpose was to gather insight into how clinical care changed during the COVID-19 pandemic through the learning of the experiences of participants [ 8 ]. Interviews were analyzed using an inductive thematic analysis approach, which included openly coding line by line to organize data in a meaningful, systematic way; examining the codes to identify themes; and reviewing the themes [ 9 ]. Specifically, the entire research team openly coded a small group of interviews (n=3) independently, line by line, and then met as a group to review codes, discuss themes, and develop an initial codebook through consensus. From here, the research team coded the bulk of the interviews in pairs, meeting as needed to ensure the reliability of coding, using the primary investigator (LJ) to triangulate and resolve any discrepancies as needed.

Reflexivity was demonstrated through regular debriefs of interviews and a review of the codebook at 1- to 2-week intervals during the coding process. Primary adjustments were additions of new codes as interviews were collected from new participant subgroups. For example, the initial codebook was derived from nurse interviews, and new codes were required as the project expanded into allied health disciplines. Codes that related to HCPs’ experiences of ICT included disciplinary changes, technical changes and innovations, improvisation, problem-solving, tools, and technology recommendations. NVivo software (QSR International) was used to facilitate the cross-synthesis analysis. As a final step of analysis to ensure saturation and methodological rigor, the primary investigator for the study (LJ) reviewed the emergent coding schema with the original transcripts.

Participant Characteristics

A total of 30 participants (site A: n=4, site B: n=14, and site C: n=12) described their experiences of how ICT supported changes to clinical care. Table 1 presents the demographic characteristics. Themes generated from participants included the use of ICT (1) for supporting in-person communication with patients; (2) for enabling connection between patients, providers, and families; and (3) for providing continuity of care amid COVID-19 restrictions.

Supporting In-Person Communication With Patients

Participants described how tablets supported in-person communication to mitigate the impact that personal protective equipment (PPE) had on verbal interactions with patients. PPE such as masks, Plexiglas, and visors posed challenges in communicating effectively, particularly for patients who were hard of hearing or who had difficulties with comprehension. Efforts to support communication were essential as communication breakdowns created confusion for the patients with detrimental consequences:

And so when talking to elderly people, when they can’t read your lips or when they can’t really hear you through three layers of protective equipment, they get very confused and multiple confusing events leads to possible more agitation and agitation leads to an automatic write-off from a lot of health care providers as to a reason why not to provide a certain person with care. [Site B, 01, physiotherapist]

Participants described coming up with innovative ways to facilitate communication amid the layers of PPE, with tablets and phones used to break down communication barriers. Applications such as speech to text allowed live transcription of providers’ speech, which can be used as a tool to support comprehension for patients who were hearing impaired. Further, speech-to-text applications provided patients and families a model of how this tool can be used to support communication outside of the hospital setting:

And so, this [iPads] has been a huge help...it helps people, patients who haven’t heard of this...they go home with a brand-new strategy that makes their daily life so much easier. [Site C, 08, social worker]

In addition to using tablets to support communication with patients who were hard of hearing, participants also expressed the value of using tablets for translation services for patients who did not speak English. Benefits included the convenience of dialing translation services from an iPad:

We have translation services on them [iPads]...which has been so, so wonderful to have to just go into someone’s room who doesn’t speak English...And just call up this interpretation service, have a human being there and that was really a key. [Site C, 29, spiritual health practitioner]

Challenges surfaced when both a videoconferencing platform and translation services were required—specifically, the difficulties in handling 2 ICT tools simultaneously and the need to prioritize videoconferencing all the while hoping that family members were relaying information correctly:

...you can’t hold a Zoom, you know, iPad and then hold a translator phone to it, you know what I mean? So then it became family trying to find someone at their end who could relay information. [Site B, 13, occupational therapist]

Enabling Connection Between Patients, Providers, and Families

Participants described how digital devices facilitated the connection between provider to family and provider to patient during visitor restrictions. This included using phones and iPads to connect families to their loved ones in hospitals, especially at end-of-life care. Participants also described that providing a digital connection to families at end-of-life care was a service that could help families move through the grief process.

...we facilitated a FaceTime and all kinds of video calls for people to be able to talk to their loved ones. And even to their religious leaders in certain cases...Families were not able to be with a loved one when they were dying…we were a bridge between them. [Site B, 07, spiritual care]
...we recorded a memorial service that was generic and was put up on YouTube and we could send the link...And so many people just didn’t have the needed ritual to move through grief. And that was something that we could give them and that was—we received so much good feedback and gratitude for that. [Site C, 29, spiritual health practitioner]

While there were benefits of tablet use to connect families to patients at end-of-life care, a digital connection created an internal struggle for HCPs as they witnessed the lack of physical touch and difficulties in accommodating end-of-life rituals:

I feel like I struggled when I had to use an iPad to connect patients to family members and it could be in a very vulnerable situation, like a patient was dying, he doesn’t speak English, the daughter’s on the iPad, she’s crying, she can’t hold her dad, can’t hold his hand...I think we have to recognize that...there is a rite of passage before somebody dies. There are certain steps for religious people and families that need to happen to honour a dying body for them to move on to wherever that place is...So anointing, communion, confession. Those are not things that are amenable to a Zoom method. [Site B, 12, nursing]

Further, participants expressed the challenges with navigating the frequency of communication between patient and family, such as balancing family requests with staffing resources within the hospital:

...when you had multiple family members who each wanted their turn to visit once a week. Well, you know, you don’t have staff to be able to support five Facetimes per resident. So, we started to have to limit it and say...like two Facetimes a week for a family, or for a resident...So, that was a challenge. [Site B, 05, social worker]

Providing Continuity of Care

Participants described how the use of videoconferencing platforms such as Zoom (Zoom Video Communications) enhanced communication between providers and families, such as when needing to provide medical updates or discharge recommendations. Zoom provided accessible options for patients with hearing or comprehension challenges using closed captioning. Furthermore, Zoom enabled more efficient and faster communication between the care team and family, rather than being faced with the complexities of coordinating schedules of team members and families who may be coming in from out of town:

It [Zoom] optimized our efficiency for delivering family meetings...the specialist physicians were able to attend more of these family meetings than in the past, because of the ability to attend virtually. And then, more family were able to attend than...in the past. And it was able to happen faster because we could do it virtually versus waiting several days for a family member to arrive from another city. [Site B, 13, occupational therapist]

Participants also expressed the benefits of web-based care for patient access, particularly for patients with mobility challenges or lack of transportation:

I can actually say that shift [to virtual] was very positive because...it actually eliminated some of the concerns my patients have about transportation, or ways that they’re able to get out there, be it because of their physical impairment post-operation. Or simply just because they don’t have the resources to get transit for whatever reason. [Site A, 23, social worker]

Further, some participants expressed how web-based care positively changed clinical practice for counseling services:

And from all the patients I’ve intervened with...I’d say .01% want to come in person...I find that on Zoom you can sort of see the environment they’re in...I think that COVID has revolutionized social work intervention...I only have good things to say about it. COVID has opened up a whole new world for counseling. [Site B, 15, social worker]

Web-based care was not without its challenges. Clinicians described that greater access to care increased referrals from patients who would historically not come for in-person treatment, particularly for mental health services:

...we found that we were getting more referrals from ... all these different patients who would have not been able to come to hospital to do in-person groups...people with anxiety disorders, like agoraphobia. People who had not seen—have difficulty going outside the house. [Site A, 16, nursing]
...the workload increased enormously, and was impossible to keep up with because before people had to come in to [the hospital] to see me so that actually restricted the number of people that I could see to people who lived in [the city], or in some neighbouring communities. At times, people would come in and come drive like 90 to 120 minutes to come and see me but due to Covid, when we shifted to online therapy...now, everybody in [the province] had access to me who were part of these programs...many people wanted to see the psychologist because they wouldn’t have to drive in. [Site C, 16, psychologist]

Consequently, participants described that more visits over Zoom led to greater fatigue as a result of having to simultaneously navigate Zoom and in-person teaching, resulting in a reduction in group therapy frequency:

We noticed for us clinicians we were just getting so fatigued that it was just too much. Because running a group in-person, and running it over Zoom is very, very different. You’re staring at a screen, you’re looking at all the faces in the room. You’re trying to navigate the PowerPoint, there’s a lot of things happening simultaneously, that when we were doing four groups a week we just noticed this is not sustainable for us. So we had to shift it to three groups. So one less group a week. So I think that’s a huge change in terms of provision of care. [Site A, 16, nursing]

In terms of providing clinical care, clinicians described the challenges of conducting a physical assessment or providing counseling treatment via Zoom or by phone:

We do some physical examination. So it’s hard just to understand the status just by phone, even if you ask them “Any swelling?” Then they say no but actually they have, so the knowledge may not be there. [Site C, 10, registered dietician]
...in Zoom it’s very limited and you mostly see the face. Right? You don’t see what the person is doing with their hands, arms, with their legs, with their feet. [Site B, 07, spiritual care practitioner]
It’s just something about being in the same room with someone when their emotions are high that you don’t actually have to do anything in particular, but just the calming presence makes a difference. I think that people get some of that on Zoom...I don’t know how similar or different, but I’m just assuming that it’s probably a bit watered down...Whereas if I was just in the room, I think just being quiet with the person would be enough and might be even better at times. [Site C, 16, psychologist]

Finally, clinicians described the challenges of using web-based care when working with older patients due to limited experiences with technology or cognitive impairments. Interestingly, some participants felt that the reliance on web-based care reduced the attendance of older populations who were not familiar with the technology.

...our average age is 97, they’re not tech savvy, they’re not necessarily understanding, comprehending, you know, that, you know, as we would understand that you can actually talk to someone who’s not present here, but it’s in the same time...So, I would call it, you’re having a video call. I try and explain it’s that, you’re having a video telephone call. And then, they just think they’re looking at a television, you know, and they’re just watching kind of a show and stuff. [Site B, 10, recreation therapist]
Some of our clients—some people with dementia don’t understand...either they don’t recognize themselves, or they get agitated by the sight of themselves—so having the person facilitating the Zoom understand how to turn off the view that you can see yourself, was important...I think I lost a number of older spouses that used to come to the group, because they...had difficulty understanding the technology, or just their digital literacy, or access to technology wasn’t that great. So currently...and interestingly, that has changed the demographic of people who are coming in my Caregiver Group. [Site B, 08, social worker]

Principal Findings

The aim of this qualitative descriptive study was to describe the experiences of HCPs in how ICT supported changes to clinical care during the COVID-19 pandemic. Participant narratives revealed 3 key findings: the benefits of digital tools to support in-person communication between patient and provider, the need for thoughtful consideration for the use of ICT at end-of-life care, and the support for the continued use of web-based care, when appropriate. We discuss HCPs’ experiences as they relate to the literature and provide recommendations for health care organizations that can make use of ICT in a more collaborative way while reflecting on patient and family values.

Communication between patients and providers is essential for quality care and for reducing preventable adverse medical events [ 10 ]. Patients who have been appropriately supported in their communication have reported to be more satisfied in their hospital stay [ 11 ]. Devices to assist with communication, more commonly referred to as alternative augmentative communication (AAC), have existed in health care for decades. AAC is an intervention approach for individuals who require added support (augmentative) or a replacement (alternative) for their communication [ 12 ]. AAC can be low technology such as communication boards or pictures or high technology such as communication systems on iPads and speech-generation devices and can be used for a short or long period of time depending on the individual’s communication needs [ 12 ].

The COVID-19 pandemic spawned a rapid adoption of digital tools such as tablets, which became an available tool to reduce communication barriers experienced with mask-wearing when speaking to patients and families and allow for participation in conversation. Additionally, tablets enabled access to video language interpretation for patients who were mechanically ventilated and awake [ 13 ], a unique example of reducing language barriers when families were not able to be present for interpretation. However, participant narratives using digital tools within acute care and rehabilitation contrast the literature describing the experiences of patients and families in the intensive care unit. In the intensive care unit, HCPs and families reported barriers to the implementation of communication supports, particularly for patients who were mechanically ventilated and awake [ 14 ]. Nurses reported feeling inadequate and frustrated in trying to support patients [ 14 ], whereas families reported frustration with communication breakdowns, inconsistent availability of tools, and insufficient training by the HCP [ 15 ]. Patients described being mechanically ventilated as a vulnerable, lonely, and fearful experience [ 15 ], particularly as verbal communication was not an option.

The collective experiences of nurses, families, and patients emphasize the impact that a lack of communication supports can have at the bedside. Further, the experiences of nurses, families, and patients shed light on the education and training that is needed for successful patient-provider communication to support participation in conversation, particularly for patients on mechanical ventilation. Reports from speech-language pathologists working with patients who are critically ill revealed positive patient-provider communication outcomes when there was nurse collaboration and readily available communication supports at the bedside [ 13 ]. Thus, the experiences of patients, families, and HCPs highlight the integral role that leadership and hospital policies play in prioritizing communication access, tool availability, and organizational-wide training [ 13 , 16 ]. For system-level change, it is recommended that hospital leaders develop regular staff training on communication supports led by professionals with expertise in this area such as speech-language pathologists [ 14 ]. For increased awareness on the importance of communication supports in health care, it is recommended that education on patient-provider communication starts as early as the undergraduate and postgraduate level for health discipline (ie, clinical) programs [ 14 ].

Videoconferencing tools have been used to connect loved ones for over a decade and have been shown to have positive psychosocial outcomes for nursing home residents when used as an addition to in-person family visits [ 17 ]. Specifically, older residents in nursing homes who received videoconferencing visits with family in addition to in-person family visits had a greater mean change in baseline depressive symptoms and feelings of loneliness when compared to older residents who had in-person visits only [ 17 ]. During the pandemic, however, videoconferencing tools and digital devices were used as a substitute for in-person visits due to visitor restrictions imposed by the COVID-19 pandemic. Although this enabled a connection between patient and family, the reduced frequency of family connections created tensions between both HCPs and family members.

Similar tensions were described by HCPs in the United Kingdom including communicating devastating news to relatives without having ever met them in person and the moral dilemma of what is “best” end-of-life care versus what could be offered given the COVID-19 restrictions [ 18 ]. Further, clinicians in Canada reported that web-based visits at end-of-life care prevented meaningful conversations typically had between family members at the bedside [ 19 ]. One physician described the importance of family connection in end-of-life care: “I’m now convinced that family members at the bedside improves patients’ ability to get better” [ 19 ]. The experiences of bereaved relatives aligned with the internal conflicts of HCPs in the United Kingdom: families wanted frequent communication that was easy to understand, one last chance to say goodbye through physical touch, and speaking to their loved one at bedside [ 20 ]. Similarly in Canada, HCPs, patients, and families all felt that restrictive acute care visitor policies impacted the safety and quality of care, mental health of everyone involved, families as partners in care, and communication and advocacy [ 4 ].

Although COVID-19 visitor restrictions have lifted, the experiences described by clinicians and families highlight the considerations needed for a positive, meaningful, end-of-life experience. One example of an organizational-wide intervention for end-of-life care includes the 3 Wishes Project (3WP), an intervention that gathers 3 wishes from the patient and family to help personalize and humanize end-of-life care [ 21 ]. The 3WP has demonstrated a positive impact on families and clinicians; families had a significantly higher rating of emotional and spiritual support than families who did not receive the 3WP [ 22 ], while clinicians reported greater morale and collaboration in helping families move toward acceptance [ 23 ]. Further, the 3WP has shown to build capacity for compassion at the organization level by facilitating collective noticing, feeling, and responding [ 24 ]. In other words, the implementation of 3WP creates system-level processes and structures to facilitate compassionate care while promoting the connection between patients, families, and HCPs [ 24 ]. Thus, while the use of digital devices will likely continue to be a complement to care [ 25 ], it is important that organizations encourage collective, compassionate care to meet the wishes of patients and families.

Literature describing the benefits and challenges of web-based care aligned with participant narratives. Benefits included faster access to care, greater efficiency, and improved convenience for patients [ 26 ]; challenges included conducting assessments without the ability to complete in-person physical examinations [ 26 ] and offering web-based care to patients with poor digital literacy [ 27 - 29 ]. What was unique to this study’s findings was the increase in referral rates with the implementation of web-based care. Two reasons for an increase in referrals as described by participants included greater access for patients with significant mental health needs who otherwise would not come in for services and greater access for patients living far away from the hospital. Consequently, more referrals increased the workload of HCPs, demonstrating the dichotomy between patient access to care and provider workload. This emphasizes the considerations needed to balance clinician workload with patient preference of service modality as organizations move toward hybrid models of care [ 25 ].

A recent US study examined patient preference for service modality for nonurgent care and found that when out-of-pocket costs were not a factor, slightly more than half of the sample (53%) preferred in-person visits to web-based care, while one-fifth (21%) preferred web-based and one-quarter (26%) had no preference or did not know what they preferred [ 30 ]. For individuals who had video visit experience, this was associated with their preference for video visits [ 30 ]. A closer look at demographic factors revealed that those who did not feel that video calls had a role in their medical care were generally older people, who lived rurally, and who had a lower income and educational level [ 30 ]. Conversely, patients who were younger and had a higher income and education were more likely to choose a video visit over in-person care [ 30 ]. While choice of service modality may be an option for nonurgent care moving forward, some populations may not have the same ability to choose. Rather, it is up to the HCP to decide whether web-based care is appropriate.

HCPs, such as psychiatrists, who work with patients with significant mental health disorders have described the role that contextual factors contribute to decision-making of service modality [ 31 ]. Contextual factors in decision-making included if an in-person visit provided greater therapeutic benefit than a web-based visit, if a general examination was needed, if there were caregivers nearby who could provide information, if insight into the living environment was necessary, and if safety resources were required for in-person visits [ 31 ]. There was no consensus among psychiatrists on the mental health conditions that would best be served, as some respondents felt web-based care offered unique benefits such as improved patient safety and reduced likelihood of escalation [ 31 ]. Taken together, a combination of factors will need to continue to be considered for service delivery modality moving forward, such as patient preference, nature of service provided, and technology literacy. Furthermore, thoughtful planning for the accessibility of technology use for underserved populations will likely be an element of consideration for the field of health care [ 32 ].

Limitations

First, this study is limited to the experiences of the HCP from urban hospitals in Ontario and British Columbia and may not be transferable to the full scope of pandemic hospital worker experiences across the globe. Consequently, there may have been uses of ICT that happened during the pandemic that were particularly novel or interesting but may not have been captured due to the nature of this qualitative study. Second, participants were given an electronic gift card after the interview in recognition of their time, which may have impacted self-referral into the study. Third, there were several research team members involved in interviews, which may have impacted the depth of information provided by the participants across interviews.

Conclusions

Experiences from HCP highlight the uses of ICT to support changes to clinical care during the pandemic. The use of digital tools supported patient-provider communication, enabled a connection between patients and families at end-of-life care, and provided continuity of care amid COVID-19 lockdowns. Moving forward, organizations are encouraged to provide education and training on how to support patient-provider communication in clinical care; find ways to implement collaborative, compassionate, end-of-life care; and continue to give autonomy to HCPs in their clinical decision-making regarding service delivery modality.

In healthcare, regulation of professions is an important tool to protect the public. With increasing regulation however, professions find themselves under increasing scrutiny. Recently there has also been considerable concern with regulator performance, with high profile reports pointing to cases of inefficiency and bias. Whilst reports have often focused on large staff groups, such as doctors, in the literature there is a dearth of data on the experiences of smaller professional groups such Clinical Scientists with their regulator, the Health and Care Professions Council.

This article reports the findings of a survey from Clinical Scientists (Physical Sciences modality) about their experiences with their regulator, and their perception of the quality and safety of that regulation.

Between July–October 2022, a survey was conducted via the Medical Physics and Engineering mail-base, open to all medical physicists & engineers. Questions covered typical topics of registration, communication, audit and fitness to practice. The questionnaire consisted of open and closed questions. Likert scoring, and thematic analysis were used to assess the quantitative and qualitative data.

Of 146 responses recorded, analysis was based on 143 respondents. Overall survey sentiment was significantly more negative than positive, in terms of regulator performance (negative responses 159; positive 106; significant at p  < 0.001). Continuous Professional Development audit was rated median 4; other topics were rated as neutral (fitness to practice, policies & procedures); and some as poor (value).

Conclusions

The Clinical Scientist (Physical Sciences) professional registrants rated the performance of their regulator more negatively than other reported assessments (by the Professional Standards Authority). Survey respondents suggested a variety of performance aspects, such as communication and fitness to practice, would benefit from improvement. Indications from this small dataset, suggest a larger survey of HCPC registrants would be useful.

Peer Review reports

In Healthcare, protection of patients and the public is a core principle. Part the framework of protections, includes regulation of professions [ 1 ]. This aims to mitigate risks such as the risk from bogus practitioners – insufficiently trained people acting as fully-trained professional practitioners, see Fig.  1 .

Regulation of professions ensures that titles (e.g. Doctor, Dentist, Clinical Scientist) are protected in law. The protected title means someone may only use that title, if they are on the national register, managed by the regulator – the Health and Care Professions Council (HCPC). It is a criminal offence to use a protected title if you are not entitled to do so [ 3 ]. There are a large number of regulators in healthcare – see Table  1 . Most of the regulators manage a register for one profession, except the HCPC which regulates 15 professions.

To be included on the register, a candidate must meet the regulators criteria for knowledge and training, and a key element to remain, is to show evidence of continuous professional development (CPD). Being on the register ensures that a practitioner has met the appropriate level of competence and professional practice.

For many healthcare workers, being on the HCPC register is a compulsory requirement to be appointable to a post. They must pay the necessary annual fees, and abide by the policies drawn-up by the regulator, and generally professions have no choice of regulator – these are statutory bodies, setup by government.

Recently, there has been considerable public dissatisfaction with the activity & performance of some regulators, notably Ofwat [ 4 ], and Ofgem [ 5 ]. Healthcare workers should expect a high level of professionalism, efficiency, and integrity from a regulator, as the regulator’s performance directly affects staff and public safety.

In terms of the regulation of UK Clinical Scientists, there is a dearth of data regarding experiences with the HCPC and views on the quality of regulation provided.

Findings are reported here from a 2022 survey of Medical Physicists and Engineers (one of the 16 job roles or ‘modalities’ under the umbrella of Clinical Scientist). The research aim was to assess experiences, and the level of ‘satisfaction’ with the regulator. For the remainder of this report, the term Clinical Scientist will be taken to mean Clinical Scientist (Medical Physicist/Engineer). The survey was designed to gather & explore data about opinions and experiences regarding several key aspects of how the HCPC performs its role, and perception of the quality & safety of regulation delivered.

A short survey questionnaire was developed, with questions aimed to cover the main regulatory processes, including registration & renewal, CPD audit, and fitness-to-practice. There were also questions relating more generally to HCPC’s performance as an organisation, e.g. handling of personal data. Finally, participants were asked to rate the HCPC’s overall performance and what they felt was the ‘value’ of regulation. The survey questions are listed in the Supplementary file along with this article.

Questions were carefully worded and there was a balance of open and closed questions. A five-point Likert score was used to rate closed questions. The survey was anonymous, and the questions were not compulsory, allowing the responders to skip irrelevant or difficult questions. The survey also aimed to be as short & concise as possible, to be a minimal burden to busy clinical staff & hopefully maximise response rate. There were a small number of questions at the start of the survey, to collect basic demographics on the respondents (role, grade, UK nation etc.).

The survey was advertised on the online JISC-hosted UK Medical Physics and Engineering (UKMPE) mail-base. This offered convenient access for the majority of Clinical Scientists. The survey was advertised twice, to allow for potential work absence, holiday/illness etc. It was active from the end of July 2002 until October 2022, when responses appeared to saturate.

The data is a combination of quantitative rating scores, and qualitative text responses. This allows a mixed-methods approach to data analysis, combining quantitative assessment of the Likert scoring, and (recursive) thematic analysis of the free-text answers [ 6 ]. Thematic analysis is a standard tool, and has been reported as a useful & appropriate for assessing experiences, thoughts, or behaviours in a dataset [ 7 ]. The survey questions addressed the main themes, but further themes were identified using an inductive, data-driven approach. Qualitative data analysis (QDA) was performed using NVivo (QSR International).

Two survey questions attempted to obtain an overall perception of HCPC’s performance: the direct one (Q12), and a further question’Would you recommend HCPC as a regulator…?’. This latter question doesn’t perhaps add anything more, and in fact a few respondents suggested it was a slightly awkward question, given professions do not have a choice of regulator – so that has been excluded from the analysis.

Study conduct was performed in accordance with relevant guidelines and regulations [ 8 , 9 ]. Before conducting the survey of Clinical Scientists, the survey was sent to their professional body, the Institute of Physics and Engineering in Medicine (IPEM). The IPEM Professional Standards Committee reviewed the survey questions [ 10 ]. Written informed consent was obtained from participants.

Data analysis

Data was collected via an MS form, in a single excel sheet and stored on a secure network drive. The respondents were anonymised, and the data checked for errors. The data was then imported into NVivo v12.

Qualitative data was manually coded for themes, and auto-coded for sentiment. An inductive approach was used to develop themes.

The sample size of responses allowed the use of simple parametric tests to establish the level of statistical significance.

Survey demographics

A total of 146 responses were collected. Two respondents noted that they worked as an HCPC Partner (a paid role). They were excluded from the analysis due to potential conflict of interest. One respondent’s responses were all blank aside from the demographic data, so they were also excluded from further analysis.

Analysis is based on 143 responses, which represents ~ 6% of the UK profession [ 11 ]. It is arguable whether it is representative of the profession at this proportion of response – but these responses do offer the only sizeable pool of data currently available. The survey was aimed at those who are on the statutory register as they are most likely to have relevant interactions & experiences of the HCPC, but a small number of responses were also received from Clinical Technologists (Medical Technical Officers-MTOs) and Engineers (CEs) and these have been included in the analysis. Figure  2 shows the breakdown in respondents, by nation.

Of the respondents, 91% are registered Clinical Scientists, and would therefore have a broad range of experience with HCPC and its processes. Mean time on the register was 12 yrs. Respondents show a large range in seniority, and their roles are shown in Fig.  3 (CS-Clinical Scientist; CE-Clinical Engineer; MTO-Medical Technical Officer/Technician; CS-P are those working in private healthcare settings, so not on Agenda for Change (AfC) pay bands).

The perception of overall regulator performance, can be assessed in two ways – one interview question directly asked for a rating score, and the overall survey sentiment also offers additional insight.

The score for overall performance was a median of 3 (mean 2.7; response rate 90%) which suggests neutral satisfaction.

Respondents were not asked directly to explain this overall performance rating – themes were extracted from the questionnaire as a whole.

The auto-coded sentiment scores generated in the NVivo software are shown in Table  2 . There is a significantly stronger negative sentiment than positive for HCPC performance – moderate, strong and total sentiment scores are all higher for negative sentiment. The normal test for a single proportion (109), shows the negative and positive sentiment differences have statistical significance with p  < 0.001. Whilst the PSA assessment of HCPC performance in 2022–23 shows 100% performance for 4 out of 5 assessment areas, survey data here from regulated professionals suggests considerably less satisfaction with HCPC. This raises associated questions about the relevance and validity of PSA assessment.

A large number of respondents seem to question the value of regulation. Whilst many accepted the value for it in terms of protecting the safety of the public, many questioned its relevance & benefit to themselves. Many respondents also queried the payment model where although the main beneficiaries of regulation are the public & the employer, it is the registrants actually pay the fees for registration. There was very little mention in survey responses, of benefit in terms of protected-title. These issues were amalgamated into Theme 1— Value of regulation , with the two sub-themes Value in monetary terms (value-for-money) and Value in professional terms (benefit and relevance to the individual professional) (see Table  3 ).

In the survey, several aspects of HCPC organisational performance were scored – handling of personal data, registration and renewal, engagement with the profession, audit, and the quality and usefulness of HCPC policies. These formed Theme 2 and its sub-themes.

A third theme Registrant competence and vulnerability , was developed to focus on responses to questions related to the assessment of registrant competence and Fitness To Practice (FTP) processes.

Finally, the survey also directly asked respondents if they could suggest improvements which would have resulted in higher scoring for regulation quality and performance. These were grouped into Theme 4.

Theme 1 – Value of regulation

Value in monetary terms.

The Likert score for value-for-money was a median of 2 (mean 2.3; response rate 100%) which suggests dissatisfaction. This is one of the few survey questions to elicit a 100% response rate – a clear signal of its importance for registrants.

There was a high number of responses suggesting fees are too expensive (and a significantly smaller number suggesting good value). This ties in with some respondents explaining that the ‘benefit’ from registration is mainly for the employer (an assurance of high quality, well-trained staff). Several respondents point to little ‘tangible’ benefit for registrants and query whether the payment model is fair and if the employer should pay registrant fees.

Several respondents suggested that as registration was mandated by the employer, it was in essence an additional ‘tax’ on their employment, which was highlighted previously by Unison [ 12 ]. A comparator for payment model, are the checks preformed on potential staff who will be working with children and vulnerable adults. In general, these ‘disclosure’ checks are paid for by the employer, however the checks are not recurrent cost for each individual, but done once at recruitment.

Value in professional terms & relevance

This was not a direct question on the questionnaire, but emerged consistently in survey responses. Aside from value-for-money, the value of regulation can also refer to more general benefit and relevance for a professional, for example in protecting a professional title or emphasising the importance of a role. Many respondents commented, in relation to the ‘value’ of regulation, about the relevance of the HCPC to them and their job/role.

The largest number of responses highlighted the lack of clarity about HCPC’s role, and also to note its lack of relevance felt by a significant proportion of respondents.

Theme 2 – HCPC performance

Communication & handling data.

The survey questionnaire did not have a specific question relating to communication, therefore no specific Likert scores are available. Rather, communication was a sub-theme which emerged in survey responses. The response numbers related to positive (1) and negative experiences (50) clearly suggest an overall experience of poor communication processes (and statistically significant at p  < 0.001 for a normal proportion test).

One respondent noted they had ‘given up’ trying to communicate with HCPC electronically. Several respondents also noted issues with conventional communication—letters from HCPC going to old addresses, or being very slow to arrive.

When trying to renew their registration, communication with HCPC was so difficult that two respondents noted they raised a formal complaint.

A number of respondents noted that when they eventually got through to the HCPC, staff were helpful, so the main communication issue may relate to insufficiently resourced lines of communication (phones & email) or the need for a more focussed first point of contact e.g. some form of helpdesk or triaging system.

This topic overlaps with the next (Processing Registration & renewals) in that both involve online logins, website use etc.

Security & data handling was rated as neutral (median 3, mean 3.4; response rate 91%). Although responses were balanced in terms of satisfaction, a significant number noted a lack of knowledge about HCPC processes. There are almost equal proportions of respondents reporting no issues, some problems with handling of personal data, or insufficient knowledge to express an opinion.

Registration and renewal

The score for processing registrations & renewals, was a median of 4 (mean 3.5; response rate 92%) which suggests modest satisfaction.

The overall rating also suggests that the issues may have been experienced by a comparative minority of registrants and that for most, renewal was straightforward.

“They expected people to call their phone number, which then wasn’t picked up. They didn’t reply to emails except after repeated attempts and finally having to resort to raising a complaint.” Resp023

Although the Likert score is positive, the themes in responses explaining the rating, are more mixed. Many respondents mentioned either having or knowing others who had issues with registration renewal, and its online processes including payments. A few respondents mentioned that the process was unforgiving of small errors. One respondent, for example, missed ticking a box on the renewal form, was removed from the register and experienced significant difficulties (poor communication with HCPC) getting the issue resolved.

Some respondents noted issues related to a long absence from work (e.g. maternity/illness etc.) causing them to miss registration deadlines – for some, this seems to have resulted in additional fees to renew registration. It seems rather easy for small errors (on either side) to result in registrants being removed from the register. For registrants, this can have very serious consequences and it can then be difficult and slow to resolve this, sometimes whilst on no pay. There have also been other reported instances of renewal payment collection errors [ 13 ].

“I had been off work… and had missed their renewal emails…I was told that there would be no allowances for this situation, and I would have to pay an additional fee to re-register…” Resp139.

Some respondents raised the issue of exclusion – certain staff groups not being included on the register—such as Clinical Technologists and Clinical Engineers. This desire for inclusion, also points to a perception of value in being on the register. One respondent raised an issue of very difficult and slow processing of registration for a candidate from outside the UK.

“Staff member who qualified as medical physicist abroad…has had a dreadful, drawn out and fruitless experience.” Resp135

Overall, many respondents noted difficulties in renewing registration and issues with HCPC’s online processes. Some of these issues (e.g. website renewal problems) may have been temporary and are now resolved, but others (e.g. available routes for registration) remain to be resolved.

Audit process & policies

In the survey, 12% respondents reported having been audited by HCPC regarding their CPD (response rate 97%). This is well above the level of 2.5% of each profession, which HCPC aims to review at each renewal [ 14 ], and similar values reported by some professional bodies [ 15 ]. The participants seem representative, although two respondents mentioned their perception of low audit rates. Respondents rated the process of being audited as a median of 4 (mean 3.7), which is the joint highest score on the survey, pointing to satisfaction with the process. From the responses, the overall perception could be summed up as straight-forward, but time-consuming. Without regular record-keeping, unfortunately most audits will be time-consuming – the HCPC more so, as it is not an annual audit, but covers the two preceding years.

Some respondents did find the process not only straight-forward, but also useful (related to feedback received). However, responses regarding feedback were mixed, with comments on both good, and poor feedback from HCPC.

The HCPC’s policies & procedures were rated as a median of 3 (mean 3.2; response rate 98%). This neutral score could suggest a mixture of confidence in HCPC practise. This score may also reflect the fact that the majority of respondents had either not read, or felt they had no need to read the policies, and so are largely unfamiliar with them.

The reasons for this lack of familiarity are also explained by some respondents – four commented that the policies & procedures are rather too generic/vague. Three respondents noted that they felt the policies were not sufficiently relevant to their clinical roles to be useful. This may be due to the policies being written at a level to be applicable to registrants from all 16 modalities – and perhaps a limitation of the nature of HCPC as a very large regulator. Familiarity seemed mainly to be restricted to policies around registration, and CPD. There were slightly lower response levels for positive sentiment (6), than negative sentiment (9).

One respondent noted not only that the policies were ‘as you might expect’, but felt the policies were less demanding than those from other similar bodies such as the CQC ( https://www.cqc.org.uk/publications ).

“…Other regulatory bodies (such as the CQC for example) have policies and procedures that are a lot more challenging to comply with.” Resp022

Theme 3 – Registrant competence and vulnerability

In this survey, 3.5% (5/143) of respondents noted some involvement with the HCPC’s Fitness to Practice service. These interactions were rated at a median of 3 (mean 2.8) suggesting neutral sentiment.

Firstly, we can immediately see the level of interaction with the FTP team is very small. CS registrants represent approx. 2% of HCPC registrants, and the level of CS referrals to FTP in 2020–21 was 0.2% [ 16 ].

The data is a very small sample, but responses vary strongly, so it is worth digging a little further into the granularity of individual responses. Response scores were 1, 1, 2, 5, 5 – which are mainly at the extremes of the rating spectrum. The majority of respondents described poor experiences with the FTP team: errors, a process which was ‘extremely prolonged’, involved slow/poor communication, and processes which were ‘entirely opaque’.

Some respondents commented that the team were not only difficult to contact, but also didn’t seem to listen. At the end of a process which involved errors from HCPC, one respondent noted were ‘no explanation, apologies or assurance that it would not happen again’, leaving the registrant ‘disillusioned’. These experiences do not fit with the HCPC’s stated goal to be a compassionate regulator, see Fig.  4 . Arguably it is more difficult to change a culture of behaviour and beliefs, than to publish a corporate goal or statement of vision.

Some survey respondents have noted the necessity of regulation for our profession.

Theme 4 – Suggestions for improved regulation

Following the question relating to overall performance, respondents were invited to suggest things which might improve their rating for HCPC’s performance and value. These suggestions were also combined with those which appeared in earlier survey responses.

Although we are in a current cost-of-living crisis, responses did not query simply high absolute cost of fees, but also queried the value/benefit of HCPC regulation for registrants. Many responses expressed doubt as to the added value & relevance of HCPC registration for them. They seem to point to a desire for more tangible benefit from their fees. Perhaps, given the costs and levels of scrutiny, registrants want some definite benefit to balance the scales .

A significant number of responses pointed out that the main benefits of registration are for the public, and for employers – but that it is the registrants who pay for registration. Many queries why this should be, and whether there should be a different payment model, where for example employers pay.

Similarly, some respondents felt that the HCPC’s unusual position of regulating a large swathe of healthcare professions was not necessarily helpful for their profession or others.

Communication and response times are obviously an issue of concern for registrants, and improvements are needed based on the low satisfaction levels reported here. This is also linked to a wish for increased engagement with the CS profession.

Some responses suggested they would be comforted by increased accountability / governance of HCPC including improved FTP efficiency.

Finally, improvement in terms of additional registration routes for Engineers & Technical staff were also suggested. It may be damaging to work-place moral, if two professionals doing roles of a similar nature are not being governanced is the same way and if there is not parity of their gross salary due to mandatory professional fees & reductions.

Value-for-money : This will vary between individuals depending on many variables, such as upbringing & environment, salary, lifestyle priorities, political persuasion, and so on. However, many of these factors should balance in a large sample. In general, it can be suggestive of satisfaction (or lack of) with a service. The score here suggesting dissatisfaction, echoes with other reports on HCPC’s spending, and financial irregularities [ 18 , 19 ].

In the survey findings, respondents have voiced dissatisfaction with registration value for money. In fact, HCPC’s registration fees are not high when compared to the other healthcare professions regulators. Table 1 shows data from 2021–22 for regulator annual registration fees. However, the HCPC has risen from having the lowest regulator fees in 2014–5, to its current position (9 th of 13) slightly higher in the table. Perhaps more concerning than the absolute level of fees, are when large increases are proposed [ 12 , 20 , 21 , 22 ].

However, fees have regularly increased to current figure of £196.48 for a two-year cycle. During a consultation process in 2018, the Academy for Healthcare Clinical Scientists (AHCS) wrote an open letter to the HCPC, disputing what they felt was a disproportionate fee increase [ 23 ]. Further fee rises have also been well above the level of inflation at the time.

HCPC expenditure (which is linked to registration fees) has arguably been even more controversial than fee increases – noted by several respondents. A freedom of information (FOI) request in 2016 showed HCPC’s spending of £17,000 for their Christmas party [ 18 ] – which amounts to just over £76 per person. This cost was close to the annual registration fee (at that time) for registrants.

In 2019, regulation of social workers in England moved from HCPC, to Social Work England. This resulted in a loss of over 100,000 registrants, and a loss in registration fee income. HCPC raised fees to compensate, but a freedom of information (FoI) request in 2020 [ 18 ] showed that even though there was an associated lowering in workload associated with the loss of 100 k registrants, the HCPC had no redundancies, suggesting the loss of income was compensated mainly by the fees increase.

Inherent value & relevance

One of HCPC’s aims is to promote ‘the value of regulation’ [ 24 ]. However, not only is there dissatisfaction with value-for-money, the second highest response suggests a lack of inherent value (or benefit) from regulation to the individual registrant. In some ways, there is a lack of balance – registrants are under increasing scrutiny, but feel there is little direct benefit, to provide balance.

This also suggests that HCPC’s aim or message is not getting through to the CS profession. It’s not clear what the HCPC 2021–22 achieved milestone – ‘Embedded our registrant experiences research into employee learning and development and inductions’ has actually achieved.

A large number of responses pointed to the lack of clarity about HCPC’s role, and also to note its lack of relevance for respondents. Some of this is understandable – until recently, many CS registrants will have little interaction with HCPC. They would typically get one email reminder each year to renew their registration and pay those fees, and hear little else from the HCPC. That is beginning to change, and HCPC have recently begun to send more regular, direct emails/updates to registrants.

However, for many registrants, the HCPC appears not to be clearly communicating its role, or the relevance/importance of regulation. As mentioned above, this also links in to previous mentions of the lack of any tangible benefit for registrants. Some note little more relevance other than the mandatory aspects of regulation.

Finally, relevance is also queried in relation to the limited access for some professional groups to a professional register. The current situation of gaps in registration for some groups, results in two situations – firstly, for Clinical Scientists and Clinical Engineers/Technologists, one group has to compulsorily pay a fee to be allowed/approved to do their job and the other does not; also, the public are routinely helped and assisted by Clinical Scientists and Clinical Engineers/Technologists – but only one group is regulated to ensure public safety.

HCPC Communication

This was highlighted by respondents as often poor. Recently in the media, there has been a concern raised by The College of Paramedics (CoP) about communications issues with HCPC—changes to the HCPC policy on the use of social media [ 25 ]. They raised particular concerns about the use of social media content and ‘historical content’ in the context of investigations of fitness-to practice.

There have previously been some concerns raised on the UKMPE mail-base regarding handling of personal data, and lack of efficiency in addressing the issue [ 26 ]. Several messages detailed HCPC communicating unencrypted registrant passwords in emails and sending personal data to the incorrect registrant. Some on the forum noted that they had reported this problem over a period of several years to HCPC, suggesting HCPC’s response to these serious issues was extremely slow. Several responses noted these previous issues.

Registration processes

Although responses here show some satisfaction, there have been reports in the media of significant issues with registration (such as removing registrants from the register in error) with associated impact for patients and the public [ 27 , 28 ]. Similarly, there were reports on the UKMPE mail-base of significant issues with registration renewals being problematic [ 26 ]. In Scotland, NHS.net email accounts ceased to be supported in July-Sept 2020 and the associated lack of access to email accounts and messages used for HCPC communication and registration, caused a major issue in registration renewal. This coincided with COVID lockdowns and a period of unusually difficult communication with HCPC. If NHS staff lose registration (irrespective of the reason), respondents noted that some Human Resources (HR) departments were quick to suspend staff from work, and in some cases withhold pay. That spike in difficulties is likely the cause of the most common responses suggesting issues with a complicated process.

In safe-guarding public safety, a key task for a healthcare regulator is assessing the competence of registrants. This is done via a small set of related activities. Registrants must return regular evidence of CPD, and these are audited for 2.5% registrants. This process is simple and routine, and as seen in Theme 2 responses here suggest registrants are reasonably satisfied with this process.

More formal and in-depth competence assessment happens when a complaint is raised against a registrant, either by a work colleague/management, a member of the public or occasionally by the HCPC itself. The process is complex, lengthy and can end in a registrant attending a court hearing [ 29 ].

It is usual for registrants to continue in their normal job during FTP investigations – effectively the public remains at risk from a registrant if their competence is eventually proven to be below the regulators standards, so there is a need for investigations to be efficient both in timeliness, and outcome.

Obviously, being under investigation can be highly stressful, and has the potential for the registrant to be ‘struck off’ the register, and lose their job if registration is mandated (e.g. NHS posts). There are many reports of the process & experience either provoking or increasing underlying mental health challenges [ 30 , 31 , 32 ]. Along with efficiency, a regulator needs to behave compassionately. Investigations of highly-skilled professionals engaging in complex work activities, is also necessarily complex and requires a high degree of knowledge and experience from the regulator’s investigational panel.

The Professional Standards Authority (PSA) regulate the HCPC, and publish annual reviews of their performance ( https://www.professionalstandards.org.uk/publications/performance-reviews ) (see Table  4 ). HCPC performance as reported by PSA, seems to be generally higher than noted by survey respondents here. For 2022–23, aside from one area, the HCPC has scored 100% for performance, which seems at odds with these survey responses [ 33 ]. The FTP team is notable in repeatedly performing very poorly compared to most other sections of the HCPC (even though the majority of the HCPC budget goes to FTP activity, see Fig.  4 ). The HCPC Annual Report 2018–9 [ 34 ] highlighted the completion of the first phase of the Fitness-To-Practice Improvement Plan. This delivered “A root and branch review of this regulatory function… a restructure, tightened roles and processes and the introduction of a new Threshold Policy”, but this seems to have no impact on the performance reported by the PSA for the next few years shown in Table  4 . However, the most recent data does suggest improvement, and HCPC continues to develop FTP team practice [ 17 ].

There are other reports of poor experiences with this team [ 35 , 36 ], and in one report the FTP team’s processes have been noted as being rather inhumane [ 35 ].

Regulation is an important part of public protection, but how effectively it is managed & enforced is also a concern, given it involves increased scrutiny of registrants. A topical comparator is the current dissatisfaction by a large section of the public about several other government regulators allowing seemingly poor performance to go unchecked [ 4 , 5 ].

It is arguable, that registrants remain on the register as long as the HCPC allows them. Several respondents in this survey noted being removed from the register through HCPC administrative error. Removal could also happen through poor judgement/decision-making – the FTP team handle large numbers of very complex investigational cases – 1603 concluded cases for the year 2021–22 and 1024 hearings [ 16 ]. Every justice system is subject to a level of error – guilty parties can be erroneously ‘cleared’, and vice-versa. It is essential therefore, that policies & procedures relating to FTP are fit for purpose—that the FTP team work effectively and humanely, and that there is genuine & effective governance of HCPC to ensure accountability. In this survey, some respondents seem to be saying that currently this seems not to be the case.

It might have been anticipated that the greatest concern is costs, especially in the current cost-of-living crisis. The recent HCPC consultation to increase fees [ 37 ] seems particularly tone-deaf and has caused concern across the professions [ 21 , 22 ].

Above findings show respondents are interested in lower fees, but also increased benefit for their fees. Some respondents pointed out that whilst registrants pay for registration, benefit is mainly for the public and employers. The HCPC is a statutory body, its funding model will have been designed/decided upon by government, and may be unlikely to change. However, there are a variety of potential regulation models [ 38 ], and so change is possible. A review of the financial model for regulation may be welcome.

Regulator size

Some aspects of HCPC performance, policies, and distribution of spending, is related to the nature of it being the largest and only multi-professional regulator in the healthcare sector. Data from the HCPC suggests (see Fig.  5 ) that the majority of spending relates to FTP activity. Data also points to Clinical Scientists having very low levels of FTP investigation compared to others in HCPC [ 16 ]. This suggests that a significant proportion of CS registrant fees are used to investigate other professions. It’s possible (perhaps simplistically) that if, like many other healthcare professions such as doctors & dentists who’s regulator is concerned only with that single profession, if CSs were regulated separately, their registrant fees may be reduced. This model of single-profession regulation may also mitigate against other disadvantages of the HCPC’s practice, such as the ‘generic’ policies aiming to apply to a pool of 15 professions.

Although there is a very low level of data for this topic, the concerned raised by registrants are serious in nature. There also seems to be issues in handling of complaints related to this service and advocacy for registrants. Certainly, there is a clear governance path via PSA, to the Health Secretary. However, this does not offer a route for individual complaints to be raised and addressed. Unlike complaints from the public in other areas, there is no recourse to an ombudsman for registrants. The only option for individual registrants, is the submission of a formal complaint to the HCPC itself, which is dealt with internally. Comments from survey respondents suggest this process does not guarantee satisfaction. Indeed, one of the respondents who mentioned submitting a complaint, made it clear they remained unhappy with HCPC’s response. Overall, there seems to be a lack of clear & effective advocacy for registrants.

FTP processes affect a comparatively small number of CS registrants, compared to other professions. However, it seems clear that the majority of those who have interacted with the FTP team have had poor experiences, and respondents have suggested improvements are needed. The reason for FTP investigations, is protection of staff and the public. If processes are slow, and investigations prolonged, or decisions flawed, the public may be exposed to increased levels of risk, as healthcare practitioners who may be lacking in competence continue to practice. The data in Table  4 shows concerning but improving trends in FTP performance levels.

Limitations

There are two main limitations to this work. Firstly, due to time constraints, there was no pilot work done when designing the survey questionnaire. This may have helped, as noted earlier, a few responses pointed to some awkwardness with one survey question. Although no pilot work was done, the questionnaire was reviewed by the IPEM Professional Standards Committee, as noted in the Acknowledgements section.

The other obvious limitation is the low response rate (~ 6% of UK Medical Physicists). Circulation of the survey was performed via the only online forum for the profession currently available. The survey was advertised multiple times to ensure visibility to staff who may have missed it initially due to leave etc. However, the forum does reach 100% of the profession, and some addressees may have filters set to send specific posts to junk folders etc. The professional body IPEM declined to offer support in circulating the survey (believing the issues involved would affect/be of interest only to a small minority of members.)

The low response rate also has a particular impact on the pool of responses relating to FTP issues, which inherently affect low numbers of registrants.

However, the importance of some of the findings here (e.g. expressed dissatisfaction with regulation in terms of value; the poor experience of some members with the Registration, Communication and FTP teams) and the low sample surveyed, both justify the need for a larger follow-on survey, across all of Clinical Science.

In Healthcare, regulation of professions is a key aspect of protecting the public. However, to be effective, regulation must be performed professionally, impartially, and associated concerns or complaints investigated efficiently and respectfully.

This report presents findings from a survey aimed at collecting a snap-shot of the experiences of Clinical Scientists with their regulator, and their perception of the quality and safety of that regulation performance.

Overall survey sentiment scores showed a significantly more negative responses than positive. Survey comments relate not only to current issues, but to previous problems and controversial issues [ 18 , 26 ]. It seems that some respondents have at some point lost confidence and trust in the HCPC, and survey responses suggest there has not been enough engagement and work done by HCPC to repair and rebuild this trust.

In the midst of a cost of living crisis, costs are a large concern for many. The HCPC fees are neither the highest not lowest amongst the healthcare regulators. Spending is transparent, and details can be found in any of the HCPC’s annual reports.

A repeating sub-theme in responses, was a lack of tangible value for the registrant, and that the employer should pay the costs of registration, where registration is mandated by the job.

Many respondents have suggested that they feel there should be more proactive engagement from HCPC with the profession. Most respondents were not familiar with or felt the HCPC policies are relevant/important to them.

Survey data showed moderate satisfaction with registration processes for the majority of respondents. Some respondents also noted a lack of registration route for engineering & technical healthcare staff. CPD processes also achieved a score indicating registrant satisfaction. This generated the highest ratings in the survey. Communication scored poorly and many respondents suggests there needs to be improved levels of communication in terms of response times and access to support.

The CS profession experiences low levels of interaction with the FTP service. However, those interactions which were recorded in the survey, show some poor experiences for registrants. There also seems to be a lack of advocacy/route for complaints about HCPC from individual registrants. There may need to be more engagement between registrants and their professional body regarding HCPC performance, and more proactivity from the stake-holder, IPEM.

Some of the findings reported here relate to important issues, but the survey data are based on a low response rate. A larger survey across all of Clinical Science is being planned.

Mechanism of thermal compressive strength evolution of carbon-bearing iron ore pellet without binders during reduction process

Against the background of “carbon peak and carbon neutrality,” it is of great practical significance to develop non-blast furnace ironmaking technology for the sustainable development of steel industry. Carbon-bearing iron ore pellet is an innovative burden of direct reduction ironmaking due to its excellent self-reducing property, and the thermal strength of pellet is a crucial metallurgical property that affects its wide application. The carbon-bearing iron ore pellet without binders (CIPWB) was prepared using iron concentrate and anthracite, and the effects of reducing agent addition amount, size of pellet, reduction temperature and time on the thermal compressive strength of CIPWB during the reduction process were studied. Simultaneously, the mechanism of the thermal strength evolution of CIPWB was revealed. The results showed that during the low-temperature reduction process (300–500 °C), the thermal compressive strength of CIPWB linearly increases with increasing the size of pellet, while it gradually decreases with increasing the anthracite ratio. When the CIPWB with 8% anthracite is reduced at 300 °C for 60 min, the thermal strength of pellet is enhanced from 13.24 to 31.88 N as the size of pellet increases from 8.04 to 12.78 mm. Meanwhile, as the temperature is 500 °C, with increasing the anthracite ratio from 2% to 8%, the thermal compressive strength of pellet under reduction for 60 min remarkably decreases from 41.47 to 8.94 N. Furthermore, in the high-temperature reduction process (600–1150 °C), the thermal compressive strength of CIPWB firstly increases and then reduces with increasing the temperature, while it as well as the temperature corresponding to the maximum strength decreases with increasing the anthracite ratio. With adding 18% anthracite, the thermal compressive strength of pellet reaches the maximum value at 800 °C, namely 35.00 N, and obtains the minimum value at 1050 °C, namely 8.60 N. The thermal compressive strength of CIPWB significantly depends on the temperature, reducing agent dosage, and pellet size.

Is Fluoridated Drinking Water Safe for Pregnant Women?

New research suggests a link between prenatal fluoride levels and behavioral issues in children. Experts are divided on the study’s significance.

A small study published Monday suggested that higher levels of fluoride consumed during the third trimester of pregnancy were associated with a greater risk of behavioral problems in the mothers’ children at 3 years old. The authors of the study, which was funded in part by the National Institutes of Health and the Environmental Protection Agency and published in the journal JAMA Network Open, believe it is the first to examine links between prenatal fluoride exposure and child development among families living in the United States, where fluoride is often added to community water supplies to prevent dental cavities.

The study’s authors and some outside researchers said that the findings should prompt policymakers to evaluate the safety of fluoride consumption during pregnancy.

“I think it’s a warning sign,” said Dr. Beate Ritz, an environmental epidemiologist at the U.C.L.A. Fielding School of Public Health.

But other experts cautioned that the study had several important limitations that made it difficult to assess the potential effects of fluoride consumption during pregnancy.

“There is nothing about this study that alarms me or would make me recommend that pregnant women stop drinking tap water,” said Dr. Patricia Braun, a professor of pediatrics at the University of Colorado School of Medicine and a spokesperson for the American Academy of Pediatrics.

The Background

Fluoride strengthens tooth enamel, and research suggests that drinking water with added fluoride can reduce cavities by up to 25 percent . Many communities in the United States have added fluoride to their water for this reason since the 1940s, a practice widely celebrated as a major public health achievement . In 2020, 63 percent of people in the United States lived in areas with at least 0.7 milligrams per liter of fluoride in the water — considered optimal for cavity prevention — though some areas have levels that are higher , in part because of naturally high fluoride in the groundwater.

In the last few years, several studies from Mexico and Canada have suggested that fluoride exposure during pregnancy is linked to slightly lower scores on intelligence tests and other measures of cognitive function in children.

But recent studies from Spain and Denmark have found no such link.

There is a “contentious debate” about water fluoridation, acknowledged Ashley Malin, an assistant professor of epidemiology in the College of Public Health and Health Professions at the University of Florida and the lead author of the new study. The issue is currently the subject of a lawsuit filed by the nonprofit Food and Water Watch and other groups against the Environmental Protection Agency. The nonprofit claims that water fluoridation poses a risk to children’s health.

The Research

The study looked at a group of 229 predominantly low-income Hispanic pregnant women in Los Angeles who were already being followed in other research. Most of the women lived in areas with fluoridated water. The researchers measured the fluoride levels in their urine in a single test during the third trimester. Then, when their children were 3 years old, the mothers filled out the Preschool Child Behavior Checklist , a measure used to detect emotional, behavioral and social problems.

Overall, 14 percent of the children had a total score in the “borderline clinical” or “clinical” range, meaning that a doctor may want to watch or evaluate them, or provide additional support, Dr. Malin said. And on average, higher fluoride levels in the mothers’ urine were correlated with a greater risk of behavioral problems in the children. The researchers found that women with urine fluoride levels at the 75th percentile were 83 percent more likely to have children with borderline or clinically significant behavioral problems than women with levels at the 25th percentile.

The main problems reported by the mothers were emotional reactivity, which is the tendency to overreact; somatic complaints, such as headaches and stomachaches; anxiety; and symptoms linked to autism (though those symptoms alone would not be enough for an autism diagnosis).

The researchers did not find an association with other behavioral symptoms like aggression or issues with concentration.

The findings are important and add to evidence suggesting prenatal fluoride consumption may affect the developing brain, said Joseph Braun, a professor of epidemiology and the director of the Center for Children’s Environmental Health at Brown University, who was not involved in the research. That said, the increases in behavioral scores were relatively small — about two points on a scale from 28 to 100 for overall behavioral problems. It’s hard to say whether such a difference might be noticeable in an individual child, he said.

But given how widespread water fluoridation is, he added, even minor behavioral changes in individual children could have a meaningful impact on the overall population.

The Limitations

The study was relatively small and didn’t include a diverse group of women. It didn’t account for many factors that can affect child development, including genetics , maternal nutrition, home environment and community support, several experts not involved in the study said.

The data would also have been stronger if the researchers had measured fluoride in urine samples from several points of time during pregnancy and collected information on tap water, bottled water and tea consumption to better understand how each contributed to the women’s fluoride levels, experts said. Black and green teas can contain high levels of fluoride.

The Preschool Child Behavior Checklist that was used to evaluate the 3-year-olds is considered a reliable measure of child behavior. But it did not take into account the fact that symptoms can change in frequency and intensity during early childhood, said Catherine Lord, an expert on autism and related disorders at the University of California, Los Angeles medical school.

Dr. Lord, who was not involved in the fluoride research, added that the checklist is not considered a reliable way to diagnose autism.

It would also be helpful to follow the children to see if the problematic behaviors persisted beyond age 3, said Melissa Melough, an assistant professor of nutrition sciences at the University of Delaware, who was not involved in the research.

What’s Next

While the experts agreed that more robust research was needed to untangle the potential effects of prenatal fluoride exposure, they had differing opinions about the study’s bottom line.

Dr. Malin said that, based on her findings and the evidence from previous studies, it might be a good idea for women to limit fluoride intake during pregnancy, a view that was echoed by Dr. Ritz and others.

“For me, the takeaway is: Protect pregnancy,” said Marcela Tamayo-Ortiz, an environmental epidemiologist at the Columbia University Mailman School of Public Health who has studied prenatal exposures for more than two decades.

But the American Dental Association said in a statement that the organization stands by its recommendation to “brush twice a day with fluoride toothpaste and drink optimally fluoridated water.”

And Dr. Nathaniel DeNicola, an OB-GYN and environmental health expert in Orange County, Calif., said he wouldn’t advise his pregnant patients to avoid fluoridated water based on the study, because “it’s not conclusive.”

Dr. Melough said she didn’t think women should be alarmed by the findings. But, she added, while it’s clear that fluoride helps to reduce cavities, it’s possible that adding it to the water “could have some unintended consequences,” and policymakers should continually evaluate the practice as new science emerges.

You can find out what the fluoride levels are in your local water by contacting your water utility or checking the C.D.C.’s My Water’s Fluoride website . If you want to reduce your fluoride consumption, experts said, limit how much black or green tea you drink. You can also purchase certain water filters that remove some fluoride. There’s no reason to stop brushing your teeth with fluoride toothpaste — just don’t swallow it.
