Most children are defenseless against abuse, are dependent on their caretakers, and are unable to protect themselves from these acts.

Childhood serves as the basis for growth, development, and socialization. Throughout adolescence, children are taught how to become productive and positive, functioning members of society. Much of the socializing of children, particularly in their very earliest years, comes at the hands of family members. Unfortunately, the messages conveyed to and the actions against children by their families are not always the positive building blocks for which one would hope.

In 2008, the Children’s Defense Fund reported that each day in America, 2,421 children are confirmed as abused or neglected, 4 children are killed by abuse or neglect, and 78 babies die before their first birthday. Several issues arise when considering the amount of child abuse that occurs annually in the United States. Child abuse is very hard to estimate because much (or most) of it is not reported. Children who are abused are unlikely to report their victimization because they may not know any better, they still love their abusers and do not want to see them taken away (or do not themselves want to be taken away from their abusers), they have been threatened into not reporting, or they do not know to whom they should report their victimizations. Still further, children may report their abuse only to find the person to whom they report does not believe them or take any action on their behalf. Continuing to muddy the waters, child abuse can be disguised as legitimate injury, particularly because young children are often somewhat uncoordinated and are still learning to accomplish physical tasks, may not know their physical limitations, and are often legitimately injured during regular play. In the end, children rarely report child abuse; most often it is an adult who makes a report based on suspicion (e.g., teacher, counselor, doctor, etc.).

Even when child abuse is reported, social service agents and investigators may not follow up or substantiate reports for a variety of reasons. Parents can pretend, lie, or cover up injuries or stories of how injuries occurred when social service agents come to investigate. While most laws about child abuse and neglect fall at the state levels, federal legislation provides a foundation for states by identifying a minimum set of acts and behaviors that define child abuse and neglect. The Federal Child Abuse Prevention and Treatment Act (CAPTA), which stems from the Keeping Children and Families Safe Act of 2003, defines child abuse and neglect as, at minimum, “(1) any recent act or failure to act on the part of a parent or caretaker which results in death, serious physical or emotional harm, sexual abuse, or exploitation; or (2) an act or failure to act which presents an imminent risk or serious harm.”

Using these minimum standards, each state is responsible for providing its own definition of maltreatment within civil and criminal statutes. When defining types of child abuse, many states incorporate similar elements and definitions into their legal statutes. For example, neglect is often defined as failure to provide for a child’s basic needs. Neglect can encompass physical elements (e.g., failure to provide necessary food or shelter, or lack of appropriate supervision), medical elements (e.g., failure to provide necessary medical or mental health treatment), educational elements (e.g., failure to educate a child or attend to special educational needs), and emotional elements (e.g., inattention to a child’s emotional needs, failure to provide psychological care, or permitting the child to use alcohol or other drugs). Failure to meet needs does not always mean a child is neglected, as situations such as poverty, cultural values, and community standards can influence the application of legal statutes. In addition, several states distinguish between failure to provide based on financial inability and failure to provide for no apparent financial reason.

Statutes on physical abuse typically include elements of physical injury (ranging from minor bruises to severe fractures or death) as a result of punching, beating, kicking, biting, shaking, throwing, stabbing, choking, hitting (with a hand, stick, strap, or other object), burning, or otherwise harming a child. Such injury is considered abuse regardless of the intention of the caretaker. In addition, many state statutes include allowing or encouraging another person to physically harm a child (such as noted above) as another form of physical abuse in and of itself. Sexual abuse usually includes activities by a parent or caretaker such as fondling a child’s genitals, penetration, incest, rape, sodomy, indecent exposure, and exploitation through prostitution or the production of pornographic materials.

Finally, emotional or psychological abuse typically is defined as a pattern of behavior that impairs a child’s emotional development or sense of self-worth. This may include constant criticism, threats, or rejection, as well as withholding love, support, or guidance. Emotional abuse is often the most difficult to prove and, therefore, child protective services may not be able to intervene without evidence of harm to the child. Some states suggest that harm may be evidenced by an observable or substantial change in behavior, emotional response, or cognition, or by anxiety, depression, withdrawal, or aggressive behavior. At a practical level, emotional abuse is almost always present when other types of abuse are identified.

Some states include an element of substance abuse in their statutes on child abuse. Circumstances that can be considered substance abuse include (a) the manufacture of a controlled substance in the presence of a child or on the premises occupied by a child (Colorado, Indiana, Iowa, Montana, South Dakota, Tennessee, and Virginia); (b) allowing a child to be present where the chemicals or equipment for the manufacture of controlled substances are used (Arizona, New Mexico); (c) selling, distributing, or giving drugs or alcohol to a child (Florida, Hawaii, Illinois, Minnesota, and Texas); (d) use of a controlled substance by a caregiver that impairs the caregiver’s ability to adequately care for the child (Kentucky, New York, Rhode Island, and Texas); and (e) exposure of the child to drug paraphernalia (North Dakota), the criminal sale or distribution of drugs (Montana, Virginia), or drug-related activity (District of Columbia).

One of the most difficult issues with which the U.S. legal system must contend is that of allowing parents the right to use corporal punishment when disciplining a child, while not letting them cross over the line into the realm of child abuse. Some parents may abuse their children under the guise of discipline, and many instances of child abuse arise from angry parents who go too far when disciplining their children with physical punishment. Generally, state statutes use terms such as “reasonable discipline of a minor,” “causes only temporary, short-term pain,” and may cause “the potential for bruising” but not “permanent damage, disability, disfigurement or injury” to the child as ways of indicating the types of discipline behaviors that are legal. However, corporal punishment that is “excessive,” “malicious,” “endangers the bodily safety of,” or is “an intentional infliction of injury” is not allowed under most state statutes (e.g., state of Florida child abuse statute).

Most research finds that the use of physical punishment (most often spanking) is not an effective method of discipline. The literature on this issue tends to find that spanking stops misbehavior, but no more effectively than other firm measures. Further, it seems to hinder rather than improve general compliance/obedience (particularly when the child is not in the presence of the punisher). Researchers have also explained why physical punishment is not any more effective at gaining child compliance than nonviolent forms of discipline. Some of the problems that arise when parents use spanking or other forms of physical punishment include the fact that spanking does not teach what children should do, nor does it provide them with alternative behavior options should the circumstance arise again. Spanking also undermines reasoning, explanation, or other forms of parental instruction because children cannot learn, reason, or problem solve well while experiencing threat, pain, fear, or anger. Further, the use of physical punishment is inconsistent with nonviolent principles, or parental modeling. In addition, the use of spanking chips away at the bonds of affection between parents and children, and tends to induce resentment and fear. Finally, it hinders the development of empathy and compassion in children, and they do not learn to take responsibility for their own behavior (Pitzer, 1997).

One of the biggest problems with the use of corporal punishment is that it can escalate into much more severe forms of violence. Usually, parents spank because they are angry (and somewhat out of control) and they can’t think of other ways to discipline. When parents are acting as a result of emotional triggers, the notion of discipline is lost while punishment and pain become the foci.

In 2006, of the children who were found to be victims of child abuse, nearly 75% of them were first-time victims (or had not come to the attention of authorities prior). A slight majority of child abuse victims were girls—51.5%, compared to 48% of abuse victims being boys. The younger the child, the more at risk he or she is for child abuse and neglect victimization. Specifically, the rate for infants (birth to 1 year old) was approximately 24 per 1,000 children of the same age group. The victimization rate for children 1–3 years old was 14 per 1,000 children of the same age group. The abuse rate for children aged 4– 7 years old declined further to 13 per 1,000 children of the same age group. African American, American Indian, and Alaska Native children, as well as children of multiple races, had the highest rates of victimization. White and Latino children had lower rates, and Asian children had the lowest rates of child abuse and neglect victimization. Regarding living arrangements, nearly 27% of victims were living with a single mother, 20% were living with married parents, while 22% were living with both parents but the marital status was unknown. (This reporting element had nearly 40% missing data, however.) Regarding disability, nearly 8% of child abuse victims had some degree of mental retardation, emotional disturbance, visual or hearing impairment, learning disability, physical disability, behavioral problems, or other medical problems. Unfortunately, data indicate that for many victims, the efforts of the child protection services system were not successful in preventing subsequent victimization. Children who had been prior victims of maltreatment were 96% more likely to experience another occurrence than those who were not prior victims. Further, child victims who were reported to have a disability were 52% more likely to experience recurrence than children without a disability. Finally, the oldest victims (16–21 years of age) were the least likely to experience a recurrence, and were 51% less likely to be victimized again than were infants (younger than age 1) (National Child Abuse and Neglect Data System, 2006).

Child fatalities are the most tragic consequence of maltreatment. Yet, each year, children die from abuse and neglect. In 2006, an estimated 1,530 children in the United States died due to abuse or neglect. The overall rate of child fatalities was 2 deaths per 100,000 children. More than 40% of child fatalities were attributed to neglect, but physical abuse also was a major contributor. Approximately 78% of the children who died due to child abuse and neglect were younger than 4 years old, and infant boys (younger than 1) had the highest rate of fatalities at 18.5 deaths per 100,000 boys of the same age in the national population. Infant girls had a rate of 14.7 deaths per 100,000 girls of the same age (National Child Abuse and Neglect Data System, 2006).

One question to be addressed regarding child fatalities is why infants have such a high rate of death when compared to toddlers and adolescents. Children under 1 year old pose an immense amount of responsibility for their caretakers: they are completely dependent and need constant attention. Children this age are needy, impulsive, and not amenable to verbal control or effective communication. This can easily overwhelm vulnerable parents. Another difficulty associated with infants is that they are physically weak and small. Injuries to infants can be fatal, while similar injuries to older children might not be. The most common cause of death in children less than 1 year is cerebral trauma (often the result of shaken-baby syndrome). Exasperated parents can deliver shakes or blows without realizing how little it takes to cause irreparable or fatal damage to an infant. Research informs us that two of the most common triggers for fatal child abuse are crying that will not cease and toileting accidents. Both of these circumstances are common in infants and toddlers whose only means of communication often is crying, and who are limited in mobility and cannot use the toilet. Finally, very young children cannot assist in injury diagnoses. Children who have been injured due to abuse or neglect often cannot communicate to medical professionals about where it hurts, how it hurts, and so forth. Also, nonfatal injuries can turn fatal in the absence of care by neglectful parents or parents who do not want medical professionals to possibly identify an injury as being the result of abuse.

Estimates reveal that nearly 80% of perpetrators of child abuse were parents of the victim. Other relatives accounted for nearly 7%, and unmarried partners of parents made up 4% of perpetrators. Of those perpetrators that were parents, over 90% were biological parents, 4% were stepparents, and 0.7% were adoptive parents. Of this group, approximately 58% of perpetrators were women and 42% were men. Women perpetrators are typically younger than men. The average age for women abusers was 31 years old, while for men the average was 34 years old. Forty percent of women who abused were younger than 30 years of age, compared with 33% of men being under 30. The racial distribution of perpetrators is similar to that of victims. Fifty-four percent were white, 21% were African American, and 20% were Hispanic/Latino (National Child Abuse and Neglect Data System, 2006).

There are many factors that are associated with child abuse. Some of the more common/well-accepted explanations are individual pathology, parent–child interaction, past abuse in the family (or social learning), situational factors, and cultural support for physical punishment along with a lack of cultural support for helping parents here in the United States.

The first explanation centers on the individual pathology of a parent or caretaker who is abusive. This theory focuses on the idea that people who abuse their children have something wrong with their individual personality or biological makeup. Such psychological pathologies may include having anger control problems; being depressed or having post-partum depression; having a low tolerance for frustration (e.g., children can be extremely frustrating: they don’t always listen; they constantly push the line of how far they can go; and once the line has been established, they are constantly treading on it to make sure it hasn’t moved. They are dependent and self-centered, so caretakers have very little privacy or time to themselves); being rigid (e.g., having no tolerance for differences—for example, what if your son wanted to play with dolls? A rigid father would not let him, laugh at him for wanting to, punish him when he does, etc.); having deficits in empathy (parents who cannot put themselves in the shoes of their children cannot fully understand what their children need emotionally); or being disorganized, inefficient, and ineffectual. (Parents who are unable to manage their own lives are unlikely to be successful at managing the lives of their children, and since many children want and need limits, these parents are unable to set them or adhere to them.)

Biological pathologies that may increase the likelihood of someone becoming a child abuser include having substance abuse or dependence problems, or having persistent or reoccurring physical health problems (especially health problems that can be extremely painful and can cause a person to become more self-absorbed, both qualities that can give rise to a lack of patience, lower frustration tolerance, and increased stress).

The second explanation for child abuse centers on the interaction between the parent and the child, noting that certain types of parents are more likely to abuse, and certain types of children are more likely to be abused, and when these less-skilled parents are coupled with these more difficult children, child abuse is the most likely to occur. Discussion here focuses on what makes a parent less skilled, and what makes a child more difficult. Characteristics of unskilled parents are likely to include such traits as only pointing out what children do wrong and never giving any encouragement for good behavior, and failing to be sensitive to the emotional needs of children. Less skilled parents tend to have unrealistic expectations of children. They may engage in role reversal— where the parents make the child take care of them—and view the parent’s happiness and well-being as the responsibility of the child. Some parents view the parental role as extremely stressful and experience little enjoyment from being a parent. Finally, less-skilled parents tend to have more negative perceptions regarding their child(ren). For example, perhaps the child has a different shade of skin than they expected and this may disappoint or anger them, they may feel the child is being manipulative (long before children have this capability), or they may view the child as the scapegoat for all the parents’ or family’s problems. Theoretically, parents with these characteristics would be more likely to abuse their children, but if they are coupled with having a difficult child, they would be especially likely to be abusive. So, what makes a child more difficult? Certainly, through no fault of their own, children may have characteristics that are associated with child care that is more demanding and difficult than in the “normal” or “average” situation. Such characteristics can include having physical and mental disabilities (autism, attention deficit hyperactivity disorder [ADHD], hyperactivity, etc.); the child may be colicky, frequently sick, be particularly needy, or cry more often. In addition, some babies are simply unhappier than other babies for reasons that cannot be known. Further, infants are difficult even in the best of circumstances. They are unable to communicate effectively, and they are completely dependent on their caretakers for everything, including eating, diaper changing, moving around, entertainment, and emotional bonding. Again, these types of children, being more difficult, are more likely to be victims of child abuse.

Nonetheless, each of these types of parents and children alone cannot explain the abuse of children, but it is the interaction between them that becomes the key. Unskilled parents may produce children that are happy and not as needy, and even though they are unskilled, they do not abuse because the child takes less effort. At the same time, children who are more difficult may have parents who are skilled and are able to handle and manage the extra effort these children take with aplomb. However, risks for child abuse increase when unskilled parents must contend with difficult children.

Social learning or past abuse in the family is a third common explanation for child abuse. Here, the theory concentrates not only on what children learn when they see or experience violence in their homes, but additionally on what they do not learn as a result of these experiences. Social learning theory in the context of family violence stresses that if children are abused or see abuse (toward siblings or a parent), those interactions and violent family members become the representations and role models for their future familial interactions. In this way, what children learn is just as important as what they do not learn. Children who witness or experience violence may learn that this is the way parents deal with children, or that violence is an acceptable method of child rearing and discipline. They may think when they become parents that “violence worked on me when I was a child, and I turned out fine.” They may learn unhealthy relationship interaction patterns; children may witness the negative interactions of parents and they may learn the maladaptive or violent methods of expressing anger, reacting to stress, or coping with conflict.

What is equally as important, though, is that they are unlikely to learn more acceptable and nonviolent ways of rearing children, interacting with family members, and working out conflict. Here it may happen that an adult who was abused as a child would like to be nonviolent toward his or her own children, but when the chips are down and the child is misbehaving, this abused-child-turned-adult does not have a repertoire of nonviolent strategies to try. This parent is more likely to fall back on what he or she knows as methods of discipline.

Something important to note here is that not all abused children grow up to become abusive adults. Children who break the cycle were often able to establish and maintain one healthy emotional relationship with someone during their childhoods (or period of young adulthood). For instance, they may have received emotional support from a nonabusing parent, or they received social support and had a positive relationship with another adult during their childhood (e.g., teacher, coach, minister, neighbor, etc.). Abused children who participate in therapy during some period of their lives can often break the cycle of violence. In addition, adults who were abused but are able to form an emotionally supportive and satisfying relationship with a mate can make the transition to being nonviolent in their family interactions.

Moving on to a fourth familiar explanation for child abuse, there are some common situational factors that influence families and parents and increase the risks for child abuse. Typically, these are factors that increase family stress or social isolation. Specifically, such factors may include receiving public assistance or having low socioeconomic status (a combination of low income and low education). Other factors include having family members who are unemployed, underemployed (working in a job that requires lower qualifications than an individual possesses), or employed only part time. These financial difficulties cause great stress for families in meeting the needs of the individual members. Other stress-inducing familial characteristics are single-parent households and larger family size. Finally, social isolation can be devastating for families and family members. Having friends to talk to, who can be relied upon, and with whom kids can be dropped off occasionally is tremendously important for personal growth and satisfaction in life. In addition, social isolation and stress can cause individuals to be quick to lose their tempers, as well as cause people to be less rational in their decision making and to make mountains out of mole hills. These situations can lead families to be at greater risk for child abuse.

Finally, cultural views and supports (or lack thereof) can lead to greater amounts of child abuse in a society such as the United States. One such cultural view is that of societal support for physical punishment. This is problematic because there are similarities between the way criminals are dealt with and the way errant children are handled. The use of capital punishment is advocated for seriously violent criminals, and people are quick to use such idioms as “spare the rod and spoil the child” when it comes to the discipline or punishment of children. In fact, it was not until quite recently that parenting books began to encourage parents to use other strategies than spanking or other forms of corporal punishment in the discipline of their children. Only recently, the American Academy of Pediatrics has come out and recommended that parents do not spank or use other forms of violence on their children because of the deleterious effects such methods have on youngsters and their bonds with their parents. Nevertheless, regardless of recommendations, the culture of corporal punishment persists.

Another cultural view in the United States that can give rise to greater incidents of child abuse is the belief that after getting married, couples of course should want and have children. Culturally, Americans consider that children are a blessing, raising kids is the most wonderful thing a person can do, and everyone should have children. Along with this notion is the idea that motherhood is always wonderful; it is the most fulfilling thing a woman can do; and the bond between a mother and her child is strong, glorious, and automatic—all women love being mothers. Thus, culturally (and theoretically), society nearly insists that married couples have children and that they will love having children. But, after children are born, there is not much support for couples who have trouble adjusting to parenthood, or who do not absolutely love their new roles as parents. People look askance at parents who need help, and cannot believe parents who say anything negative about parenthood. As such, theoretically, society has set up a situation where couples are strongly encouraged to have kids, are told they will love kids, but then society turns a blind or disdainful eye when these same parents need emotional, financial, or other forms of help or support. It is these types of cultural viewpoints that increase the risks for child abuse in society.

The consequences of child abuse are tremendous and long lasting. Research has shown that the traumatic experience of childhood abuse is life changing. These costs may surface during adolescence, or they may not become evident until abused children have grown up and become abusing parents or abused spouses. Early identification and treatment is important to minimize these potential long-term effects. Whenever children say they have been abused, it is imperative that they be taken seriously and their abuse be reported. Suspicions of child abuse must be reported as well. If there is a possibility that a child is or has been abused, an investigation must be conducted.

Children who have been abused may exhibit traits such as the inability to love or have faith in others. This often translates into adults who are unable to establish lasting and stable personal relationships. These individuals have trouble with physical closeness and touching as well as emotional intimacy and trust. Further, these qualities tend to cause a fear of entering into new relationships, as well as the sabotaging of any current ones.

Psychologically, children who have been abused tend to have poor self-images or are passive, withdrawn, or clingy. They may be angry individuals who are filled with rage, anxiety, and a variety of fears. They are often aggressive, disruptive, and depressed. Many abused children have flashbacks and nightmares about the abuse they have experienced, and this may cause sleep problems as well as drug and alcohol problems. Posttraumatic stress disorder (PTSD) and antisocial personality disorder are both typical among maltreated children. Research has also shown that most abused children fail to reach “successful psychosocial functioning,” and are thus not resilient and do not resume a “normal life” after the abuse has ended.

Socially (and likely because of these psychological injuries), abused children have trouble in school, will have difficulty getting and remaining employed, and may commit a variety of illegal or socially inappropriate behaviors. Many studies have shown that victims of child abuse are likely to participate in high-risk behaviors such as alcohol or drug abuse, the use of tobacco, and high-risk sexual behaviors (e.g., unprotected sex, large numbers of sexual partners). Later in life, abused children are more likely to have been arrested and homeless. They are also less able to defend themselves in conflict situations and guard themselves against repeated victimizations.

Medically, abused children likely will experience health problems due to the high frequency of physical injuries they receive. In addition, abused children experience a great deal of emotional turmoil and stress, which can also have a significant impact on their physical condition. These health problems are likely to continue occurring into adulthood. Some of these longer-lasting health problems include headaches; eating problems; problems with toileting; and chronic pain in the back, stomach, chest, and genital areas. Some researchers have noted that abused children may experience neurological impairment and problems with intellectual functioning, while others have found a correlation between abuse and heart, lung, and liver disease, as well as cancer (Thomas, 2004).

Victims of sexual abuse show an alarming number of disturbances as adults. Some dislike and avoid sex, or experience sexual problems or disorders, while other victims appear to enjoy sexual activities that are self-defeating or maladaptive—normally called “dysfunctional sexual behavior”—and have many sexual partners.

Abused children also experience a wide variety of developmental delays. Many do not reach physical, cognitive, or emotional developmental milestones at the typical time, and some never accomplish what they are supposed to during childhood socialization. In the next section, these developmental delays are discussed as a means of identifying children who may be abused.

There are two primary ways of identifying children who are abused: spotting and evaluating physical injuries, and detecting and appraising developmental delays. Distinguishing physical injuries due to abuse can be difficult, particularly among younger children who are likely to get hurt or receive injuries while they are playing and learning to become ambulatory. Nonetheless, there are several types of wounds that children are unlikely to give themselves during their normal course of play and exploration. These less likely injuries may signal instances of child abuse.

While it is true that children are likely to get bruises, particularly when they are learning to walk or crawl, bruises on infants are not normal. Also, the back of the legs, upper arms, or on the chest, neck, head, or genitals are also locations where bruises are unlikely to occur during normal childhood activity. Further, bruises with clean patterns, like hand prints, buckle prints, or hangers (to name a few), are good examples of the types of bruises children do not give themselves.

Another area of physical injury where the source of the injury can be difficult to detect is fractures. Again, children fall out of trees, or crash their bikes, and can break limbs. These can be normal parts of growing up. However, fractures in infants less than 12 months old are particularly suspect, as infants are unlikely to be able to accomplish the types of movement necessary to actually break a leg or an arm. Further, multiple fractures, particularly more than one on a bone, should be examined more closely. Spiral or torsion fractures (when the bone is broken by twisting) are suspect because when children break their bones due to play injuries, the fractures are usually some other type (e.g., linear, oblique, compacted). In addition, when parents don’t know about the fracture(s) or how it occurred, abuse should be considered, because when children get these types of injuries, they need comfort and attention.

Head and internal injuries are also those that may signal abuse. Serious blows to the head cause internal head injuries, and this is very different from the injuries that result from bumping into things. Abused children are also likely to experience internal injuries like those to the abdomen, liver, kidney, and bladder. They may suffer a ruptured spleen, or intestinal perforation. These types of damages rarely happen by accident.

Burns are another type of physical injury that can happen by accident or by abuse. Nevertheless, there are ways to tell these types of burn injuries apart. The types of burns that should be examined and investigated are those where the burns are in particular locations. Burns to the bottom of the feet, genitals, abdomen, or other inaccessible spots should be closely considered. Burns of the whole hand or those to the buttocks are also unlikely to happen as a result of an accident.

Turning to the detection and appraisal of developmental delays, one can more readily assess possible abuse by considering what children of various ages should be able to accomplish, than by noting when children are delayed and how many milestones on which they are behind schedule. Importantly, a few delays in reaching milestones can be expected, since children develop individually and not always according to the norm. Nonetheless, when children are abused, their development is likely to be delayed in numerous areas and across many milestones.

As children develop and grow, they should be able to crawl, walk, run, talk, control going to the bathroom, write, set priorities, plan ahead, trust others, make friends, develop a good self-image, differentiate between feeling and behavior, and get their needs met in appropriate ways. As such, when children do not accomplish these feats, their circumstances should be examined.

Infants who are abused or neglected typically develop what is termed failure to thrive syndrome. This syndrome is characterized by slow, inadequate growth, or not “filling out” physically. They have a pale, colorless complexion and dull eyes. They are not likely to spend much time looking around, and nothing catches their eyes. They may show other signs of lack of nutrition such as cuts, bruises that do not heal in a timely way, and discolored fingernails. They are also not trusting and may not cry much, as they are not expecting to have their needs met. Older infants may not have developed any language skills, or these developments are quite slow. This includes both verbal and nonverbal means of communication.

Toddlers who are abused often become hypervigilant about their environments and others’ moods. They are more outwardly focused than a typical toddler (who is quite self-centered) and may be unable to separate themselves as individuals, or consider themselves as distinct beings. In this way, abused toddlers cannot focus on tasks at hand because they are too concerned about others’ reactions. They don’t play with toys, have no interest in exploration, and seem unable to enjoy life. They are likely to accept losses with little reaction, and may have age-inappropriate knowledge of sex and sexual relations. Finally, toddlers, whether they are abused or not, begin to mirror their parents’ behaviors. Thus, toddlers who are abused may mimic the abuse when they are playing with dolls or “playing house.”

Developmental delays can also be detected among abused young adolescents. Some signs include the failure to learn cause and effect, since their parents are so inconsistent. They have no energy for learning and have not developed beyond one- or two-word commands. They probably cannot follow complicated directions (such as two to three tasks per instruction), and they are unlikely to be able to think for themselves. Typically, they have learned that failure is totally unacceptable, but they are more concerned with the teacher’s mood than with learning and listening to instruction. Finally, they are apt to have been inadequately toilet trained and thus may be unable to control their bladders.

Older adolescents, because they are likely to have been abused for a longer period of time, continue to get further and further behind in their developmental achievements. Abused children this age become family nurturers. They take care of their parents and cater to their parents’ needs, rather than the other way around. In addition, they probably take care of any younger siblings and do the household chores. Because of these default responsibilities, they usually do not participate in school activities; they frequently miss days at school; and they have few, if any, friends. Because they have become so hypervigilant and have increasingly delayed development, they lose interest in and become disillusioned with education. They develop low self-esteem and little confidence, but seem old for their years. Children this age who are abused are still likely to be unable to control their bladders and may have frequent toileting accidents.

Other developmental delays can occur and be observed in abused and neglected children of any age. For example, malnutrition and withdrawal can be noticed in infants through teenagers. Maltreated children frequently have persistent or untreated illnesses, and these can become permanent disabilities if medical conditions go untreated for a long enough time. Another example can be the consequences of neurological damage. Beyond being a medical issue, this type of damage can cause problems with social behavior and impulse control, which, again, can be discerned in various ages of children.

Once child abuse is suspected, law enforcement officers, child protection workers, or various other practitioners may need to interview the child about the abuse or neglect he or she may have suffered. Interviewing children can be extremely difficult because children at various stages of development can remember only certain parts or aspects of the events in their lives. Also, interviewers must be careful that they do not put ideas or answers into the heads of the children they are interviewing. There are several general recommendations when interviewing children about the abuse they may have experienced. First, interviewers must acknowledge that even when children are abused, they likely still love their parents. They do not want to be taken away from their parents, nor do they want to see their parents get into trouble. Interviewers must not blame the parents or be judgmental about them or the child’s family. Beyond that, interviews should take place in a safe, neutral location. Interviewers can use dolls and role-play to help children express the types of abuse of which they may be victims.

Finally, interviewers must ask age-appropriate questions. For example, 3-year-olds can probably only answer questions about what happened and who was involved. Four- to five-year-olds can also discuss where the incidents occurred. Along with what, who, and where, 6- to 8-year-olds can talk about the element of time, or when the abuse occurred. Nine- to 10-year-olds are able to add commentary about the number of times the abuse occurred. Finally, 11-year-olds and older children can additionally inform interviewers about the circumstances of abusive instances.

A conclusion is not a summary of what a writer has already mentioned. On the contrary, it is the last point made. Taking every detail of the investigation, the researcher makes the concluding point. In this part of a paper, you need to put a full stop in your research. You need to persuade the reader in your opinion.

One common strategy advocated is the use of public service announcements that encourage individuals to report any suspected child abuse. Currently, many mandatory reporters (those required by law to report abuse such as teachers, doctors, and social service agency employees) and members of communities feel that child abuse should not be reported unless there is substantial evidence that abuse is indeed occurring. Child advocates stress that this notion should be changed, and that people should report child abuse even if it is only suspected. Public service announcements should stress that if people report suspected child abuse, the worst that can happen is that they might be wrong, but in the grander scheme of things that is really not so bad.

Child advocates also stress that greater interagency cooperation is needed. This cooperation should be evident between women’s shelters, child protection agencies, programs for at-risk children, medical agencies, and law enforcement officers. These agencies typically do not share information, and if they did, more instances of child abuse would come to the attention of various authorities and could be investigated and managed. Along these lines, child protection agencies and programs should receive more funding. When budgets are cut, social services are often the first things to go or to get less financial support. Child advocates insist that with more resources, child protection agencies could hire more workers, handle more cases, conduct more investigations, and follow up with more children and families.

Continuing, more educational efforts must be initiated about issues such as punishment and discipline styles and strategies; having greater respect for children; as well as informing the community about what child abuse is, and how to recognize it. In addition, Americans must alter the cultural orientation about child bearing and child rearing. Couples who wish to remain child-free must be allowed to do so without disdain. And, it must be acknowledged that raising children is very difficult, is not always gloriously wonderful, and that parents who seek help should be lauded and not criticized. These kinds of efforts can help more children to be raised in nonviolent, emotionally satisfying families, and thus become better adults.

How to Make a Successful Research Presentation

Turning a research paper into a visual presentation is difficult; there are pitfalls, and navigating the path to a brief, informative presentation takes time and practice. As a TA for  GEO/WRI 201: Methods in Data Analysis & Scientific Writing this past fall, I saw how this process works from an instructor’s standpoint. I’ve presented my own research before, but helping others present theirs taught me a bit more about the process. Here are some tips I learned that may help you with your next research presentation:

More is more

In general, your presentation will always benefit from more practice, more feedback, and more revision. By practicing in front of friends, you can get comfortable with presenting your work while receiving feedback. It is hard to know how to revise your presentation if you never practice. If you are presenting to a general audience, getting feedback from someone outside of your discipline is crucial. Terms and ideas that seem intuitive to you may be completely foreign to someone else, and your well-crafted presentation could fall flat.

Less is more

Limit the scope of your presentation, the number of slides, and the text on each slide. In my experience, text works well for organizing slides, orienting the audience to key terms, and annotating important figures–not for explaining complex ideas. Having fewer slides is usually better as well. In general, about one slide per minute of presentation is an appropriate budget. Too many slides is usually a sign that your topic is too broad.

Limit the scope of your presentation

Don’t present your paper. Presentations are usually around 10 min long. You will not have time to explain all of the research you did in a semester (or a year!) in such a short span of time. Instead, focus on the highlight(s). Identify a single compelling research question which your work addressed, and craft a succinct but complete narrative around it.

Craft a compelling research narrative

After identifying the focused research question, walk your audience through your research as if it were a story. Presentations with strong narrative arcs are clear, captivating, and compelling.

  • Introduction (exposition — rising action)

Orient the audience and draw them in by demonstrating the relevance and importance of your research story with strong global motive. Provide them with the necessary vocabulary and background knowledge to understand the plot of your story. Introduce the key studies (characters) relevant in your story and build tension and conflict with scholarly and data motive. By the end of your introduction, your audience should clearly understand your research question and be dying to know how you resolve the tension built through motive.

  • Methods (rising action)

The methods section should transition smoothly and logically from the introduction. Beware of presenting your methods in a boring, arc-killing, ‘this is what I did.’ Focus on the details that set your story apart from the stories other people have already told. Keep the audience interested by clearly motivating your decisions based on your original research question or the tension built in your introduction.

  • Results (climax)

Less is usually more here. Only present results which are clearly related to the focused research question you are presenting. Make sure you explain the results clearly so that your audience understands what your research found. This is the peak of tension in your narrative arc, so don’t undercut it by quickly clicking through to your discussion.

  • Discussion (falling action)

By now your audience should be dying for a satisfying resolution. Here is where you contextualize your results and begin resolving the tension between past research. Be thorough. If you have too many conflicts left unresolved, or you don’t have enough time to present all of the resolutions, you probably need to further narrow the scope of your presentation.

  • Conclusion (denouement)

Return back to your initial research question and motive, resolving any final conflicts and tying up loose ends. Leave the audience with a clear resolution of your focus research question, and use unresolved tension to set up potential sequels (i.e. further research).

Use your medium to enhance the narrative

Visual presentations should be dominated by clear, intentional graphics. Subtle animation in key moments (usually during the results or discussion) can add drama to the narrative arc and make conflict resolutions more satisfying. You are narrating a story written in images, videos, cartoons, and graphs. While your paper is mostly text, with graphics to highlight crucial points, your slides should be the opposite. Adapting to the new medium may require you to create or acquire far more graphics than you included in your paper, but it is necessary to create an engaging presentation.

The most important thing you can do for your presentation is to practice and revise. Bother your friends, your roommates, TAs–anybody who will sit down and listen to your work. Beyond that, think about presentations you have found compelling and try to incorporate some of those elements into your own. Remember you want your work to be comprehensible; you aren’t creating experts in 10 minutes. Above all, try to stay passionate about what you did and why. You put the time in, so show your audience that it’s worth it.

Market research definition

At the risk of stating the slightly obvious, market research is the gathering and analyzing of data on consumers, competitors, distributors, and markets. As such it’s not quite the same as consumer research , but there’s significant overlap.

Market research matters because it can help you take the guesswork out of getting through to audiences. By studying consumers and gathering information on their likes, dislikes, and so on, brands can make evidence-based decisions instead of relying on instinct or experience.

What is market research?

Market research is the organized gathering of information about target markets and consumers’ needs and preferences. It’s an important component of business strategy and a major factor in maintaining competitiveness.

If a business wants to know – really know – what sort of products or services consumers want to buy, along with where, when, and how those products and services should be marketed, it just makes sense to ask the prospective audience. 

Without the certainty that market research brings, a business is basically hoping for the best. And while we salute their optimism, that’s not exactly a reliable strategy for success.

What are the types of market research?

Primary research .

Primary research is a type of market research you either conduct yourself or hire someone to do on your behalf.

A classic example of primary research involves going directly to a source – typically customers or prospective customers in your target market – to ask questions and gather information about a product or service. Interviewing methods include in-person, online surveys, phone calls, and focus groups.

The big advantage of primary research is that it’s directly focused on your objectives, so the outcome will be conclusive, detailed insights – particularly into customer views – making it the gold standard.

The disadvantages are it can be time-consuming and potentially costly, plus there’s a risk of survey bias creeping in, in the sense that research samples may not be representative of the wider group.

Secondary research 

Primary market research means you collect the data your business needs, whereas the types of market research known as secondary market research use information that’s already been gathered for other purposes but can still be valuable. Examples include published market studies, white papers, analyst reports, customer emails, and customer surveys/feedback.

For many small businesses with limited budgets, secondary market research is their first choice because it’s easier to acquire and far more affordable than primary research.

Secondary research can still answer specific business questions, but with limitations. The data collected from that audience may not match your targeted audience exactly, resulting in skewed outcomes. 

A big benefit of secondary market research is helping lay the groundwork and get you ready to carry out primary market research by making sure you’re focused on what matters most.

Qualitative research

Qualitative research is one of the two fundamental types of market research. Qualitative research is about people and their opinions. Typically conducted by asking questions either one-on-one or in groups, qualitative research can help you define problems and learn about customers’ opinions, values, and beliefs.

Classic examples of qualitative research are long-answer questions like “Why do you think this product is better than competitive products? Why do you think it’s not?”, or “How would you improve this new service to make it more appealing?”

Because qualitative research generally involves smaller sample sizes than its close cousin quantitative research, it gives you an anecdotal overview of your subject, rather than highly detailed information that can help predict future performance.

Qualitative research is particularly useful if you’re developing a new product, service, website or ad campaign and want to get some feedback before you commit a large budget to it.

Quantitative research

If qualitative research is all about opinions, quantitative research is all about numbers, using math to uncover insights about your audience. 

Typical quantitative research questions are things like, “What’s the market size for this product?” or “How long are visitors staying on this website?”. Clearly the answers to both will be numerical.

Quantitative research usually involves questionnaires. Respondents are asked to complete the survey, which marketers use to understand consumer needs, and create strategies and marketing plans.

Importantly, because quantitative research is math-based, it’s statistically valid, which means you’re in a good position to use it to predict the future direction of your business.

Consumer research 

As its name implies, consumer research gathers information about consumers’ lifestyles, behaviors, needs and preferences, usually in relation to a particular product or service. It can include both quantitative and qualitative studies.

Examples of consumer research in action include finding ways to improve consumer perception of a product, or creating buyer personas and market segments, which help you successfully market your product to different types of customers.

Understanding consumer trends , driven by consumer research, helps businesses understand customer psychology and create detailed purchasing behavior profiles. The result helps brands improve their products and services by making them more customer-centric, increasing customer satisfaction, and boosting bottom line in the process.

Product research 

Product research gives a new product (or indeed service, we don’t judge) its best chance of success, or helps an existing product improve or increase market share.

It’s common sense: by finding out what consumers want and adjusting your offering accordingly, you gain a competitive edge. It can be the difference between a product being a roaring success or an abject failure.

Examples of product research include finding ways to develop goods with a higher value, or identifying exactly where innovation effort should be focused. 

Product research goes hand-in-hand with other strands of market research, helping you make informed decisions about what consumers want, and what you can offer them.

Brand research  

Brand research is the process of gathering feedback from your current, prospective, and even past customers to understand how your brand is perceived by the market.

It covers things like brand awareness, brand perceptions, customer advocacy, advertising effectiveness, purchase channels, audience profiling, and whether or not the brand is a top consideration for consumers.

The result helps take the guesswork out of your messaging and brand strategy. Like all types of market research, it gives marketing leaders the data they need to make better choices based on fact rather than opinion or intuition.

Market research methods 

So far we’ve reviewed various different types of market research, now let’s look at market research methods, in other words the practical ways you can uncover those all-important insights.

Face-to-face interviews 

Despite the rise in popularity of online surveys , face-to-face survey interviewing – using mobile devices or even the classic paper survey – is still a popular data collection method.

In terms of advantages, face-to-face interviews help with accurate screening, in the sense the interviewee can’t easily give misleading answers about, say, their age. The interviewer can also make a note of emotions and non-verbal cues. 

On the other hand, face-to-face interviews can be costly, while the quality of data you get back often depends on the ability of the interviewer. Also, the size of the sample is limited to the size of your interviewing staff, the area in which the interviews are conducted, and the number of qualified respondents within that area.

Social listening 

Social listening is a powerful solution for brands who want to keep an ear to the ground, gathering unfiltered thoughts and opinions from consumers who are posting on social media. 

Many social listening tools store data for up to a couple of years, great for trend analysis that needs to compare current and past conversations.

Social listening isn’t limited to text. Images, videos, and emojis often help us better understand what consumers are thinking, saying, and doing better than more traditional research methods. 

Perhaps the biggest downside is there are no guarantees with social listening, and you never know what you will (or won’t) find. It can also be tricky to gauge sentiment accurately if the language used is open to misinterpretation, for example if a social media user describes something as “sick”.

There’s also a potential problem around what people say vs. what they actually do. Tweeting about the gym is a good deal easier than actually going. The wider problem – and this may shock you – is that not every single thing people write on social media is necessarily true, which means social listening can easily deliver unreliable results.

Public domain data 

Public domain data comes from think tanks and government statistics or research centers like the UK’s National Office for Statistics or the United States Census Bureau and the National Institute of Statistical Sciences. Other sources are things like research journals, news media, and academic material.

Its advantages for market research are it’s cheap (or even free), quick to access, and easily available. Public domain datasets can be huge, so potentially very rich.

On the flip side, the data can be out of date, it certainly isn’t exclusive to you, and the collection methodology can leave much to be desired. But used carefully, public domain data can be a useful source of secondary market research.

Telephone interviews 

You know the drill – you get a call from a researcher who asks you questions about a particular topic and wants to hear your opinions. Some even pay or offer other rewards for your time.

Telephone surveys are great for reaching niche groups of consumers within a specific geographic area or connected to a particular brand, or who aren’t very active in online channels. They’re not well-suited for gathering data from broad population groups, simply because of the time and labor involved.

How to use market research 

Data isn’t an end in itself; instead it’s a springboard to make other stuff happen. So once you’ve drawn conclusions from your research, it’s time to think of what you’ll actually do based on your findings.

While it’s impossible for us to give a definitive list (every use case is different), here are some suggestions to get you started.

Leverage it . Think about ways to expand the use – and value – of research data and insights, for example by using research to support business goals and functions, like sales, market share or product design.

Integrate it . Expand the value of your research data by integrating it with other data sources, internal and external. Integrating data like this can broaden your perspective and help you draw deeper insights for more confident decision-making.

Justify it . Enlist colleagues from areas that’ll benefit from the insights that research provides – that could be product management, product development, customer service, marketing, sales or many others – and build a business case for using research.

How to choose the right type of market research 

Broadly speaking, choosing the right research method depends on knowing the type of data you need to collect. To dig into ideas and opinions, choose qualitative; to do some testing, it’s quantitative you want.

There are also a bunch of practical considerations, not least cost. If a particular approach sounds great but costs the earth then clearly it’s not ideal for any brand on a budget.

Then there’s how you intend to use the actual research, your level of expertise with research data, whether you need access to historical data or just a snapshot of today, and so on.

The point is, different methods suit different situations. When choosing, you’ll want to consider what you want to achieve, what data you’ll need, the pros and cons of each method, the costs of conducting the research, and the cost of analyzing the results.

What are publications on a resume?

Publications on a resume include written and scholarly works published in reputable journals, websites, or other distinguished platforms. Blog posts on sites like WordPress, Medium, or Substack do not often qualify for the publications section of a resume and should often not be linked on your resume .

Examples of publications worthy of a resume include:

  • Book title , if you’re an author, researcher, or academic
  • Peer-reviewed journal articles or papers , if you’re a professor, researcher, or in academia
  • Research papers , if you’re in law, science, medicine, etc. 
  • Article title , if you’ve written articles for a well-known journal, website, or magazine (e.g. Journal of the American Medical Association , The New York Times, The Los Angeles Times, The Washington Post, National Geographic, TIME)
  • Conference papers , if you’re a researcher or academic 

The publications and presentations on your resume should be related to the industry you’re in and the open position in question.

Should you include publications on your resume?

You may have an impressive list of publications. But now that you’re on the job search, you’re not sure about adding publications on a resume. 

To help you decide, ask yourself: Is this related to the job? Could it help me land a job interview? If so, your potential employers want to see them. 

If the answer is no, skip it. Bryan Berthot , a project manager, scrum master, and Doctor of Business Administration (DBA) candidate at Univ. of South Florida shares his strategy. 

“My practice is to add publications when they’re germane to the job in question and when they maximize the chance that my resume will be noticed. I walk the line between academia and industry, as I’m both a project manager practitioner and an adjunct faculty member who teaches.”

That means including published works for academic positions and leaving them off other positions. 

“Even though most adjunct faculty positions involve teaching and not research, for those jobs I always send my entire CV, which includes my academic publications. It sets me apart from many instructional faculty because by including my publications, it demonstrates that I can bring aspects of research into the classroom to give students real-world examples."

Finally, Berthot shared examples of when not to include publications on your resume.

For project manager jobs, I typically omit my academic publications and books from my resume—unless I suspect that it will set me apart from other candidates.”

Sometimes, the answer is obvious. If you match one of the following descriptions, it makes sense to add research publications on your resume.

  • Author. If you’ve published a book, it establishes your credibility in a certain field. If you’re applying for a professorship or a writing-heavy role, include the book title. 
  • Researcher. If you’ve published your research and are applying for an academic position, research-based role, or as a speaker for a conference, it makes sense to include your research paper on your resume. 
  • Student. If you’re a full-time student, research and writing might be your full-time job right now. In lieu of current working experience, listing publications on a resume can show your areas of expertise and highlight your writing skills.
  • Professor. Getting published is an important step on your journey to becoming a professor. If you’re applying for a teaching or research role, include all of your publications on your resume.

How to list publications on your resume

If you’ve determined your published works are relevant to the job you’re applying for, you can add them to your resume. 

Here’s how to list publications on your resume:

  • Add a Publications section. You should create a dedicated resume section to list publications. In most cases, your Publications section will follow your Education section. 
  • List each publication in a bullet point. Include the publication title, the name of the publisher, and the date (month and year).
  • Choose a style. Depending on your field, there might be a specific citation style you should use. Some examples include MLA format, APA style, AMA style, or IEEE style. 
  • Start with your most recent publications. List your publications on your resume in reverse chronological order. That means starting with your most recent publication and listing your older work in descending order.
  •  Refine your list. If you’re including a co-authored piece, make that clear and list out your role in the piece, such as “Lead author.” You can also include pending pieces by including the article title and noting it’s “Under review” or “Submitted for publication.” If any publications aren’t relevant to the job, delete them from the list.

While there are nuances to every field and industry, these are best practices for how to show publications on your resume. Consult peers in your field and the job description instructions to properly format publications on your resume.

How to list scientific publications on resume

If you’re in the sciences and have published papers, you’ll likely use American Psychological Association (APA) or the Council of Science Editors (CSE) style when citing your work. 

There are also slight differentiations on how to cite the publication based on the type of work. For instance, listing a book is different from listing a journal on your resume. 

CSE also has different citation variations:

  • Citation-sequence
  • Citation-name

Choose a style and cite your work using the appropriate format. According to Boston University Libraries , the general format for citing a journal using CSE is:

Author. Year (or Date). Title of article. Title of journal. Volume number and issue number. Page numbers. URL in angle brackets. Date accessed.

The key is to use the same style for each publication listed on your resume. Not only is this more visually appealing, it’s also easier to understand. 

How to list academic publications on a resume

Academic publications typically use either MLA or APA style. MLA refers to the Modern Language Association and APA is the American Psychological Association. Which one you should use may depend on your field. MLA is generally used in the humanities while APA is generally used in the sciences. 

According to the Library of Congress , article citations using MLA style include:

Last name, First name. Title. Title of the Website, Version or edition, Publisher. Day Month Year of publication, URL. Day Month Year of access.

You can use a tool like Citation Machine to easily cite journal articles in APA style. The basic APA journal citation formula is:

Last Name, First Initial. Middle Initial. (The year it was published). Article title. Publisher title, Volume or Issue , page range.

How to list research publications on a resume

Many research publications use MLA or APA style for citations. You can choose which style best fits and use the formula when putting publications on your resume. 

If you’re a research assistant and don’t have publications to list quite yet, you can still add your skills to your resume. You can create a research section on your resume if you have a lot to add. If it’s only a couple of items, include it in your achievements section.

Cultural Relativity and Acceptance of Embryonic Stem Cell Research

There is a debate about the ethical implications of using human embryos in stem cell research, which can be influenced by cultural, moral, and social values. This paper argues for an adaptable framework to accommodate diverse cultural and religious perspectives. By using an adaptive ethics model, research protections can reflect various populations and foster growth in stem cell research possibilities.

INTRODUCTION

Stem cell research combines biology, medicine, and technology, promising to alter health care and the understanding of human development. Yet, ethical contention exists because of individuals’ perceptions of using human embryos based on their various cultural, moral, and social values. While these disagreements concerning policy, use, and general acceptance have prompted the development of an international ethics policy, such a uniform approach can overlook the nuanced ethical landscapes between cultures. With diverse viewpoints in public health, a single global policy, especially one reflecting Western ethics or the ethics prevalent in high-income countries, is impractical. This paper argues for a culturally sensitive, adaptable framework for the use of embryonic stem cells. Stem cell policy should accommodate varying ethical viewpoints and promote an effective global dialogue. With an extension of an ethics model that can adapt to various cultures, we recommend localized guidelines that reflect the moral views of the people those guidelines serve.

Stem cells, characterized by their unique ability to differentiate into various cell types, enable the repair or replacement of damaged tissues. Two primary types of stem cells are somatic stem cells (adult stem cells) and embryonic stem cells. Adult stem cells exist in developed tissues and maintain the body’s repair processes. [1] Embryonic stem cells (ESC) are remarkably pluripotent or versatile, making them valuable in research. [2] However, the use of ESCs has sparked ethics debates. Considering the potential of embryonic stem cells, research guidelines are essential. The International Society for Stem Cell Research (ISSCR) provides international stem cell research guidelines. They call for “public conversations touching on the scientific significance as well as the societal and ethical issues raised by ESC research.” [3] The ISSCR also publishes updates about culturing human embryos 14 days post fertilization, suggesting local policies and regulations should continue to evolve as ESC research develops. [4]  Like the ISSCR, which calls for local law and policy to adapt to developing stem cell research given cultural acceptance, this paper highlights the importance of local social factors such as religion and culture.

I.     Global Cultural Perspective of Embryonic Stem Cells

Views on ESCs vary throughout the world. Some countries readily embrace stem cell research and therapies, while others have stricter regulations due to ethical concerns surrounding embryonic stem cells and when an embryo becomes entitled to moral consideration. The philosophical issue of when the “someone” begins to be a human after fertilization, in the morally relevant sense, [5] impacts when an embryo becomes not just worthy of protection but morally entitled to it. The process of creating embryonic stem cell lines involves the destruction of the embryos for research. [6] Consequently, global engagement in ESC research depends on social-cultural acceptability.

a.     US and Rights-Based Cultures

In the United States, attitudes toward stem cell therapies are diverse. The ethics and social approaches, which value individualism, [7] trigger debates regarding the destruction of human embryos, creating a complex regulatory environment. For example, the 1996 Dickey-Wicker Amendment prohibited federal funding for the creation of embryos for research and the destruction of embryos for “more than allowed for research on fetuses in utero.” [8] Following suit, in 2001, the Bush Administration heavily restricted stem cell lines for research. However, the Stem Cell Research Enhancement Act of 2005 was proposed to help develop ESC research but was ultimately vetoed. [9] Under the Obama administration, in 2009, an executive order lifted restrictions allowing for more development in this field. [10] The flux of research capacity and funding parallels the different cultural perceptions of human dignity of the embryo and how it is socially presented within the country’s research culture. [11]

b.     Ubuntu and Collective Cultures

African bioethics differs from Western individualism because of the different traditions and values. African traditions, as described by individuals from South Africa and supported by some studies in other African countries, including Ghana and Kenya, follow the African moral philosophies of Ubuntu or Botho and Ukama , which “advocates for a form of wholeness that comes through one’s relationship and connectedness with other people in the society,” [12] making autonomy a socially collective concept. In this context, for the community to act autonomously, individuals would come together to decide what is best for the collective. Thus, stem cell research would require examining the value of the research to society as a whole and the use of the embryos as a collective societal resource. If society views the source as part of the collective whole, and opposes using stem cells, compromising the cultural values to pursue research may cause social detachment and stunt research growth. [13] Based on local culture and moral philosophy, the permissibility of stem cell research depends on how embryo, stem cell, and cell line therapies relate to the community as a whole. Ubuntu is the expression of humanness, with the person’s identity drawn from the “’I am because we are’” value. [14] The decision in a collectivistic culture becomes one born of cultural context, and individual decisions give deference to others in the society.

Consent differs in cultures where thought and moral philosophy are based on a collective paradigm. So, applying Western bioethical concepts is unrealistic. For one, Africa is a diverse continent with many countries with different belief systems, access to health care, and reliance on traditional or Western medicines. Where traditional medicine is the primary treatment, the “’restrictive focus on biomedically-related bioethics’” [is] problematic in African contexts because it neglects bioethical issues raised by traditional systems.” [15] No single approach applies in all areas or contexts. Rather than evaluating the permissibility of ESC research according to Western concepts such as the four principles approach, different ethics approaches should prevail.

Another consideration is the socio-economic standing of countries. In parts of South Africa, researchers have not focused heavily on contributing to the stem cell discourse, either because it is not considered health care or a health science priority or because resources are unavailable. [16] Each country’s priorities differ given different social, political, and economic factors. In South Africa, for instance, areas such as maternal mortality, non-communicable diseases, telemedicine, and the strength of health systems need improvement and require more focus [17] Stem cell research could benefit the population, but it also could divert resources from basic medical care. Researchers in South Africa adhere to the National Health Act and Medicines Control Act in South Africa and international guidelines; however, the Act is not strictly enforced, and there is no clear legislation for research conduct or ethical guidelines. [18]

Some parts of Africa condemn stem cell research. For example, 98.2 percent of the Tunisian population is Muslim. [19] Tunisia does not permit stem cell research because of moral conflict with a Fatwa. Religion heavily saturates the regulation and direction of research. [20] Stem cell use became permissible for reproductive purposes only recently, with tight restrictions preventing cells from being used in any research other than procedures concerning ART/IVF.  Their use is conditioned on consent, and available only to married couples. [21] The community's receptiveness to stem cell research depends on including communitarian African ethics.

c.     Asia

Some Asian countries also have a collective model of ethics and decision making. [22] In China, the ethics model promotes a sincere respect for life or human dignity, [23] based on protective medicine. This model, influenced by Traditional Chinese Medicine (TCM), [24] recognizes Qi as the vital energy delivered via the meridians of the body; it connects illness to body systems, the body’s entire constitution, and the universe for a holistic bond of nature, health, and quality of life. [25] Following a protective ethics model, and traditional customs of wholeness, investment in stem cell research is heavily desired for its applications in regenerative therapies, disease modeling, and protective medicines. In a survey of medical students and healthcare practitioners, 30.8 percent considered stem cell research morally unacceptable while 63.5 percent accepted medical research using human embryonic stem cells. Of these individuals, 89.9 percent supported increased funding for stem cell research. [26] The scientific community might not reflect the overall population. From 1997 to 2019, China spent a total of $576 million (USD) on stem cell research at 8,050 stem cell programs, increased published presence from 0.6 percent to 14.01 percent of total global stem cell publications as of 2014, and made significant strides in cell-based therapies for various medical conditions. [27] However, while China has made substantial investments in stem cell research and achieved notable progress in clinical applications, concerns linger regarding ethical oversight and transparency. [28] For example, the China Biosecurity Law, promoted by the National Health Commission and China Hospital Association, attempted to mitigate risks by introducing an institutional review board (IRB) in the regulatory bodies. 5800 IRBs registered with the Chinese Clinical Trial Registry since 2021. [29] However, issues still need to be addressed in implementing effective IRB review and approval procedures.

The substantial government funding and focus on scientific advancement have sometimes overshadowed considerations of regional cultures, ethnic minorities, and individual perspectives, particularly evident during the one-child policy era. As government policy adapts to promote public stability, such as the change from the one-child to the two-child policy, [30] research ethics should also adapt to ensure respect for the values of its represented peoples.

Japan is also relatively supportive of stem cell research and therapies. Japan has a more transparent regulatory framework, allowing for faster approval of regenerative medicine products, which has led to several advanced clinical trials and therapies. [31] South Korea is also actively engaged in stem cell research and has a history of breakthroughs in cloning and embryonic stem cells. [32] However, the field is controversial, and there are issues of scientific integrity. For example, the Korean FDA fast-tracked products for approval, [33] and in another instance, the oocyte source was unclear and possibly violated ethical standards. [34] Trust is important in research, as it builds collaborative foundations between colleagues, trial participant comfort, open-mindedness for complicated and sensitive discussions, and supports regulatory procedures for stakeholders. There is a need to respect the culture’s interest, engagement, and for research and clinical trials to be transparent and have ethical oversight to promote global research discourse and trust.

d.     Middle East

Countries in the Middle East have varying degrees of acceptance of or restrictions to policies related to using embryonic stem cells due to cultural and religious influences. Saudi Arabia has made significant contributions to stem cell research, and conducts research based on international guidelines for ethical conduct and under strict adherence to guidelines in accordance with Islamic principles. Specifically, the Saudi government and people require ESC research to adhere to Sharia law. In addition to umbilical and placental stem cells, [35] Saudi Arabia permits the use of embryonic stem cells as long as they come from miscarriages, therapeutic abortions permissible by Sharia law, or are left over from in vitro fertilization and donated to research. [36] Laws and ethical guidelines for stem cell research allow the development of research institutions such as the King Abdullah International Medical Research Center, which has a cord blood bank and a stem cell registry with nearly 10,000 donors. [37] Such volume and acceptance are due to the ethical ‘permissibility’ of the donor sources, which do not conflict with religious pillars. However, some researchers err on the side of caution, choosing not to use embryos or fetal tissue as they feel it is unethical to do so. [38]

Jordan has a positive research ethics culture. [39] However, there is a significant issue of lack of trust in researchers, with 45.23 percent (38.66 percent agreeing and 6.57 percent strongly agreeing) of Jordanians holding a low level of trust in researchers, compared to 81.34 percent of Jordanians agreeing that they feel safe to participate in a research trial. [40] Safety testifies to the feeling of confidence that adequate measures are in place to protect participants from harm, whereas trust in researchers could represent the confidence in researchers to act in the participants’ best interests, adhere to ethical guidelines, provide accurate information, and respect participants’ rights and dignity. One method to improve trust would be to address communication issues relevant to ESC. Legislation surrounding stem cell research has adopted specific language, especially concerning clarification “between ‘stem cells’ and ‘embryonic stem cells’” in translation. [41] Furthermore, legislation “mandates the creation of a national committee… laying out specific regulations for stem-cell banking in accordance with international standards.” [42] This broad regulation opens the door for future global engagement and maintains transparency. However, these regulations may also constrain the influence of research direction, pace, and accessibility of research outcomes.

e.     Europe

In the European Union (EU), ethics is also principle-based, but the principles of autonomy, dignity, integrity, and vulnerability are interconnected. [43] As such, the opportunity for cohesion and concessions between individuals’ thoughts and ideals allows for a more adaptable ethics model due to the flexible principles that relate to the human experience The EU has put forth a framework in its Convention for the Protection of Human Rights and Dignity of the Human Being allowing member states to take different approaches. Each European state applies these principles to its specific conventions, leading to or reflecting different acceptance levels of stem cell research. [44]

For example, in Germany, Lebenzusammenhang , or the coherence of life, references integrity in the unity of human culture. Namely, the personal sphere “should not be subject to external intervention.” [45]  Stem cell interventions could affect this concept of bodily completeness, leading to heavy restrictions. Under the Grundgesetz, human dignity and the right to life with physical integrity are paramount. [46] The Embryo Protection Act of 1991 made producing cell lines illegal. Cell lines can be imported if approved by the Central Ethics Commission for Stem Cell Research only if they were derived before May 2007. [47] Stem cell research respects the integrity of life for the embryo with heavy specifications and intense oversight. This is vastly different in Finland, where the regulatory bodies find research more permissible in IVF excess, but only up to 14 days after fertilization. [48] Spain’s approach differs still, with a comprehensive regulatory framework. [49] Thus, research regulation can be culture-specific due to variations in applied principles. Diverse cultures call for various approaches to ethical permissibility. [50] Only an adaptive-deliberative model can address the cultural constructions of self and achieve positive, culturally sensitive stem cell research practices. [51]

II.     Religious Perspectives on ESC

Embryonic stem cell sources are the main consideration within religious contexts. While individuals may not regard their own religious texts as authoritative or factual, religion can shape their foundations or perspectives.

The Qur'an states:

“And indeed We created man from a quintessence of clay. Then We placed within him a small quantity of nutfa (sperm to fertilize) in a safe place. Then We have fashioned the nutfa into an ‘alaqa (clinging clot or cell cluster), then We developed the ‘alaqa into mudgha (a lump of flesh), and We made mudgha into bones, and clothed the bones with flesh, then We brought it into being as a new creation. So Blessed is Allah, the Best of Creators.” [52]

Many scholars of Islam estimate the time of soul installment, marked by the angel breathing in the soul to bring the individual into creation, as 120 days from conception. [53] Personhood begins at this point, and the value of life would prohibit research or experimentation that could harm the individual. If the fetus is more than 120 days old, the time ensoulment is interpreted to occur according to Islamic law, abortion is no longer permissible. [54] There are a few opposing opinions about early embryos in Islamic traditions. According to some Islamic theologians, there is no ensoulment of the early embryo, which is the source of stem cells for ESC research. [55]

In Buddhism, the stance on stem cell research is not settled. The main tenets, the prohibition against harming or destroying others (ahimsa) and the pursuit of knowledge (prajña) and compassion (karuna), leave Buddhist scholars and communities divided. [56] Some scholars argue stem cell research is in accordance with the Buddhist tenet of seeking knowledge and ending human suffering. Others feel it violates the principle of not harming others. Finding the balance between these two points relies on the karmic burden of Buddhist morality. In trying to prevent ahimsa towards the embryo, Buddhist scholars suggest that to comply with Buddhist tenets, research cannot be done as the embryo has personhood at the moment of conception and would reincarnate immediately, harming the individual's ability to build their karmic burden. [57] On the other hand, the Bodhisattvas, those considered to be on the path to enlightenment or Nirvana, have given organs and flesh to others to help alleviate grieving and to benefit all. [58] Acceptance varies on applied beliefs and interpretations.

Catholicism does not support embryonic stem cell research, as it entails creation or destruction of human embryos. This destruction conflicts with the belief in the sanctity of life. For example, in the Old Testament, Genesis describes humanity as being created in God’s image and multiplying on the Earth, referencing the sacred rights to human conception and the purpose of development and life. In the Ten Commandments, the tenet that one should not kill has numerous interpretations where killing could mean murder or shedding of the sanctity of life, demonstrating the high value of human personhood. In other books, the theological conception of when life begins is interpreted as in utero, [59] highlighting the inviolability of life and its formation in vivo to make a religious point for accepting such research as relatively limited, if at all. [60] The Vatican has released ethical directives to help apply a theological basis to modern-day conflicts. The Magisterium of the Church states that “unless there is a moral certainty of not causing harm,” experimentation on fetuses, fertilized cells, stem cells, or embryos constitutes a crime. [61] Such procedures would not respect the human person who exists at these stages, according to Catholicism. Damages to the embryo are considered gravely immoral and illicit. [62] Although the Catholic Church officially opposes abortion, surveys demonstrate that many Catholic people hold pro-choice views, whether due to the context of conception, stage of pregnancy, threat to the mother’s life, or for other reasons, demonstrating that practicing members can also accept some but not all tenets. [63]

Some major Jewish denominations, such as the Reform, Conservative, and Reconstructionist movements, are open to supporting ESC use or research as long as it is for saving a life. [64] Within Judaism, the Talmud, or study, gives personhood to the child at birth and emphasizes that life does not begin at conception: [65]

“If she is found pregnant, until the fortieth day it is mere fluid,” [66]

Whereas most religions prioritize the status of human embryos, the Halakah (Jewish religious law) states that to save one life, most other religious laws can be ignored because it is in pursuit of preservation. [67] Stem cell research is accepted due to application of these religious laws.

We recognize that all religions contain subsets and sects. The variety of environmental and cultural differences within religious groups requires further analysis to respect the flexibility of religious thoughts and practices. We make no presumptions that all cultures require notions of autonomy or morality as under the common morality theory , which asserts a set of universal moral norms that all individuals share provides moral reasoning and guides ethical decisions. [68] We only wish to show that the interaction with morality varies between cultures and countries.

III.     A Flexible Ethical Approach

The plurality of different moral approaches described above demonstrates that there can be no universally acceptable uniform law for ESC on a global scale. Instead of developing one standard, flexible ethical applications must be continued. We recommend local guidelines that incorporate important cultural and ethical priorities.

While the Declaration of Helsinki is more relevant to people in clinical trials receiving ESC products, in keeping with the tradition of protections for research subjects, consent of the donor is an ethical requirement for ESC donation in many jurisdictions including the US, Canada, and Europe. [69] The Declaration of Helsinki provides a reference point for regulatory standards and could potentially be used as a universal baseline for obtaining consent prior to gamete or embryo donation.

For instance, in Columbia University’s egg donor program for stem cell research, donors followed standard screening protocols and “underwent counseling sessions that included information as to the purpose of oocyte donation for research, what the oocytes would be used for, the risks and benefits of donation, and process of oocyte stimulation” to ensure transparency for consent. [70] The program helped advance stem cell research and provided clear and safe research methods with paid participants. Though paid participation or covering costs of incidental expenses may not be socially acceptable in every culture or context, [71] and creating embryos for ESC research is illegal in many jurisdictions, Columbia’s program was effective because of the clear and honest communications with donors, IRBs, and related stakeholders.  This example demonstrates that cultural acceptance of scientific research and of the idea that an egg or embryo does not have personhood is likely behind societal acceptance of donating eggs for ESC research. As noted, many countries do not permit the creation of embryos for research.

Proper communication and education regarding the process and purpose of stem cell research may bolster comprehension and garner more acceptance. “Given the sensitive subject material, a complete consent process can support voluntary participation through trust, understanding, and ethical norms from the cultures and morals participants value. This can be hard for researchers entering countries of different socioeconomic stability, with different languages and different societal values. [72]

An adequate moral foundation in medical ethics is derived from the cultural and religious basis that informs knowledge and actions. [73] Understanding local cultural and religious values and their impact on research could help researchers develop humility and promote inclusion.

IV.     Concerns

Some may argue that if researchers all adhere to one ethics standard, protection will be satisfied across all borders, and the global public will trust researchers. However, defining what needs to be protected and how to define such research standards is very specific to the people to which standards are applied. We suggest that applying one uniform guide cannot accurately protect each individual because we all possess our own perceptions and interpretations of social values. [74] Therefore, the issue of not adjusting to the moral pluralism between peoples in applying one standard of ethics can be resolved by building out ethics models that can be adapted to different cultures and religions.

Other concerns include medical tourism, which may promote health inequities. [75] Some countries may develop and approve products derived from ESC research before others, compromising research ethics or drug approval processes. There are also concerns about the sale of unauthorized stem cell treatments, for example, those without FDA approval in the United States. Countries with robust research infrastructures may be tempted to attract medical tourists, and some customers will have false hopes based on aggressive publicity of unproven treatments. [76]

For example, in China, stem cell clinics can market to foreign clients who are not protected under the regulatory regimes. Companies employ a marketing strategy of “ethically friendly” therapies. Specifically, in the case of Beike, China’s leading stem cell tourism company and sprouting network, ethical oversight of administrators or health bureaus at one site has “the unintended consequence of shifting questionable activities to another node in Beike's diffuse network.” [77] In contrast, Jordan is aware of stem cell research’s potential abuse and its own status as a “health-care hub.” Jordan’s expanded regulations include preserving the interests of individuals in clinical trials and banning private companies from ESC research to preserve transparency and the integrity of research practices. [78]

The social priorities of the community are also a concern. The ISSCR explicitly states that guidelines “should be periodically revised to accommodate scientific advances, new challenges, and evolving social priorities.” [79] The adaptable ethics model extends this consideration further by addressing whether research is warranted given the varying degrees of socioeconomic conditions, political stability, and healthcare accessibilities and limitations. An ethical approach would require discussion about resource allocation and appropriate distribution of funds. [80]

While some religions emphasize the sanctity of life from conception, which may lead to public opposition to ESC research, others encourage ESC research due to its potential for healing and alleviating human pain. Many countries have special regulations that balance local views on embryonic personhood, the benefits of research as individual or societal goods, and the protection of human research subjects. To foster understanding and constructive dialogue, global policy frameworks should prioritize the protection of universal human rights, transparency, and informed consent. In addition to these foundational global policies, we recommend tailoring local guidelines to reflect the diverse cultural and religious perspectives of the populations they govern. Ethics models should be adapted to local populations to effectively establish research protections, growth, and possibilities of stem cell research.

For example, in countries with strong beliefs in the moral sanctity of embryos or heavy religious restrictions, an adaptive model can allow for discussion instead of immediate rejection. In countries with limited individual rights and voice in science policy, an adaptive model ensures cultural, moral, and religious views are taken into consideration, thereby building social inclusion. While this ethical consideration by the government may not give a complete voice to every individual, it will help balance policies and maintain the diverse perspectives of those it affects. Embracing an adaptive ethics model of ESC research promotes open-minded dialogue and respect for the importance of human belief and tradition. By actively engaging with cultural and religious values, researchers can better handle disagreements and promote ethical research practices that benefit each society.

This brief exploration of the religious and cultural differences that impact ESC research reveals the nuances of relative ethics and highlights a need for local policymakers to apply a more intense adaptive model.

Female labor force participation

Across the globe, women face inferior income opportunities compared with men. Women are less likely to work for income or actively seek work. The global labor force participation rate for women is just over 50% compared to 80% for men. Women are less likely to work in formal employment and have fewer opportunities for business expansion or career progression. When women do work, they earn less. Emerging evidence from recent household survey data suggests that these gender gaps are heightened due to the COVID-19 pandemic.

Women’s work and GDP

Women’s work is posited to be related to development through the process of economic transformation.

Levels of female labor force participation are high for the poorest economies generally, where agriculture is the dominant sector and women often participate in small-holder agricultural work. Women’s participation in the workforce is lower in middle-income economies which have much smaller shares of agricultural activities. Finally, among high-income economies, female labor force participation is again higher, accompanied by a shift towards a service sector-based economy and higher education levels among women.

This describes the posited  U-shaped relationship  between development (proxied by GDP per capita) and female labor force participation where women’s work participation is high for the poorest economies, lower for middle income economies, and then rises again among high income economies.

This theory of the U-shape is observed globally across economies of different income levels. But this global picture may be misleading. As more recent studies have found, this pattern does not hold within regions or when looking within a specific economy over time as their income levels rise.

In no region do we observe a U-shape pattern in female participation and GDP per capita over the past three decades.

Structural transformation, declining fertility, and increasing female education in many parts of the world have not resulted in significant increases in women’s participation as was theorized. Rather, rigid historic, economic, and social structures and norms factor into stagnant female labor force participation.

Historical view of women’s participation and GDP

Taking a historical view of female participation and GDP, we ask another question: Do lower income economies today have levels of participation that mirror levels that high-income economies had decades earlier?

The answer is no.

This suggests that the relationship of female labor force participation to GDP for lower-income economies today is different than was the case decades past. This could be driven by numerous factors -- changing social norms, demographics, technology, urbanization, to name a few possible drivers.

Gendered patterns in type of employment

Gender equality is not just about equal access to jobs but also equal access for men and women to good jobs. The type of work that women do can be very different from the type of work that men do. Here we divide work into two broad categories: vulnerable work and wage work.

The Gender gap in vulnerable and wage work by GDP per capita

Vulnerable employment is closely related to GDP per capita. Economies with high rates of vulnerable employment are low-income contexts with a large agricultural sector. In these economies, women tend to make up the higher share of the vulnerably employed. As economy income levels rise, the gender gap also flips, with men being more likely to be in vulnerable work when they have a job than women.

From COVID-19 crisis to recovery

The COVID-19 crisis has exacerbated these gender gaps in employment. Although comprehensive official statistics from labor force surveys are not yet available for all economies,  emerging studies  have consistently documented that working women are taking a harder hit from the crisis. Different patterns by sector and vulnerable work do not explain this. That is, this result is not driven by the sectors in which women work or their higher rates of vulnerable work—within specific work categories, women fared worse than men in terms of COVID-19 impacts on jobs.

Among other explanations is that women have borne the brunt of the increase in the demand for care work (especially for children). A strong and inclusive recovery will require efforts which address this and other underlying drivers of gender gaps in employment opportunities.

Research Methodology for Investigating the Effectiveness of Cognitive Behavioral Therapy in Reducing Symptoms of Depression in Adults

Introduction:

The aim of this research is to investigate the effectiveness of cognitive-behavioral therapy (CBT) in reducing symptoms of depression in adults. To achieve this objective, a randomized controlled trial (RCT) will be conducted using a mixed-methods approach.

Research Design:

The study will follow a pre-test and post-test design with two groups: an experimental group receiving CBT and a control group receiving no intervention. The study will also include a qualitative component, in which semi-structured interviews will be conducted with a subset of participants to explore their experiences of receiving CBT.

Participants:

Participants will be recruited from community mental health clinics in the local area. The sample will consist of 100 adults aged 18-65 years old who meet the diagnostic criteria for major depressive disorder. There is a growing emphasis on prevention to reconcile demographic pressures, resource scarcity and expectations of better quality care and support. The Care Act 2014 placed a statutory duty on English local authorities to prevent and delay the development of needs for care and support. However, evidence suggests that the prevention approach has secured less impact than intended. Given that existing approaches have achieved such limited results, new ways of addressing this apparently intractable challenge should be considered. We argue here that theory-based models that support the understanding of, and responses to, implementation barriers and facilitators can provide tools to support the development of more successful implementation. Drawing on in-depth interviews ( n  = 20) in selected English councils and analyses of their policy documents, we explore the ‘Ready, Willing and Able’ (RWA) model, which posits that those three preconditions must be satisfied before new practices can be implemented sustainably. We argue that RWA can provide a straightforward and parsimonious framework for identifying implementation barriers and facilitators. Using the model to identify potential bottlenecks prior to the implementation can help local actors clarify baseline barriers to progress. RWA could help to inform opportunities to target identified problems, by reinforcing facilitators and moderating barriers.

Faced with population ageing, predicted increases in the number of frail older people with long-term care needs, rising expectations for the quality of care and most recently coronavirus disease 2019 (COVID-19), the sustainability of care systems has been increasingly questioned. More attention has been focused within the UK and internationally on how, and how far, such challenges might be met through prevention strategies that aim to keep people healthier and living more independently for longer ( Meijer et al. , 2017 ; Cylus et al. , 2019 ; Read et al. , 2023 ; Rostgaard et al. , 2023 , Tew et al. 2023 ). The European Commission (2021) , OECD (2017) and World Health Organization ( Marczak et al. , 2019a ) have all advocated the development of prevention and provided examples of services that have adopted this approach.

Within the UK, social care policy has increasingly focused on prevention as a means of promoting well-being, multi-agency working and simultaneously, constraining demand for resources ( Marczak et al. , 2017 ; Read et al. , 2023 ). For example, Social Services and Well-being (Wales) Act 2014 highlights prevention and early intervention as one of the five core principles at the heart of the Welsh programme of change for social services. However, the programme’s government-funded evaluation found that the funding of preventative initiatives was hampered by the necessity of funding services in crisis ( Llewellyn et al. , 2023 ).

Prevention became a statutory requirement in England with the implementation of the Care Act 2014, though many English councils had funded such services since the1990s ( Wistow and Lewis, 1997 ). The 2014 Act placed a new duty on English local authorities (LAs) with responsibility for adult social care (ASC) to prevent and delay the development of (higher level) needs for care and support. It also included a requirement to consider whether individuals might benefit from preventative services, before assessing their eligibility for statutory support ( HM Government, 2014 ). Implementation guidance recognised that effective prevention depended on involving many agencies, including the National Health Service (NHS), Public Health, housing and voluntary sectors ( Department of Health, 2014 ). The emphasis on partnerships to promote preventative approaches was reinforced by expectations of their systems’ wide role in helping to mitigate the impact of fiscal austerity and population ageing by reducing demand for more intensive care ( Miller, 2014 ). Such expectations recognised the potentially complex causal relationships between investment and return, typified by the belief that social care investments could reduce demand on the NHS.

The introduction of statutory duties does not guarantee their adoption or intended results. There is little evidence about how councils translated their prevention duties into practice, existing evidence suggests the Act’s vision is not being fully realised (for exception see Carers Trust, 2015 ; Tew et al. , 2019 ). Field (2017) found that most councils further developed prevention strategies and procedures after 2014 but identified few improvements in provision, whilst collaboration with health partners remained limited. Declining LA resources were identified as a common obstacle in implementing Care Act responsibilities, including duties towards prevention ( Hastings et al. , 2015 ; NAO, 2015 ; Tew et al. , 2019 ). In addition, the breadth and diversity of meanings attached to the concept of prevention—within and across agencies—and a related lack of consensus over service models and investment strategies also posed implementation challenges ( Starfield et al. , 2008 ; Lombard, 2013 ; Marczak et al. , 2019b ).

Whilst evidence about the implementation of the Care Act’s provisions for extending prevention is limited, there is considerable evidence about factors that generally constrain the adoption of new ideas and practices into organisations. For example, failure to adopt new practices at all or to do so in ways that produce expected and sustainable results is often attributed to limitations in stakeholders’ capacity and capability that have been identified through the analysis of factors, which impede and facilitate the adoption of new policies and programmes ( Fullan, 2003 ; Flaspohler et al. , 2012 ; Tew et al. , 2019 ). However, the adoption of theory-based analyses of barriers and opportunities for the successful implementation of prevention policies in local councils is in its early stages.

Research objectives

This article draws on case studies of selected English councils to better understand how councils may translate prevention objectives and policies into effective practice. We employ the ‘Ready, Willing and Able’ (RWA) model ( Coale, 1973 ; Lesthaeghe and Vanderhoeft, 2001 ) as a theoretically grounded, conceptually straightforward and accessible tool for policymakers, analysts and practitioners to identify potential obstacles and facilitators in order to understand necessary preconditions for implementing new practices.

Ready, willing and able

We employ the RWA approach as an organising framework for identifying and potentially modifying barriers and facilitators to the sustained adoption of new practices. RWA has been employed in demography since the 1970s to understand the adoption of new practices and behaviour within a defined population ( Coale, 1973 ; Lesthaeghe and Vanderhoeft, 2001 ). More recently, the model has been applied to analyse the adoption of new practices in different healthcare and national contexts ( Shultz and Jimbo, 2015 ; Bunzli et al. , 2017 ; Dearing and Cox, 2017 ). As illustrated in Figure 1 , the RWA model specifies three preconditions that need to be satisfied before a new practice can be adopted successfully:

Readiness (R): the perceived utility of a given/new practice, that is the overall perceived benefit of the new practice must outweigh the perceived disutility/cost of securing its adoption.

Willingness (W): the perceived legitimacy of the new practice, the question here is to what extent new practices run counter to established traditional practices and codes of conduct, and to what extent there is a willingness to modify them.

Ability (A): capabilities for adopting new practices given available technical, legal or financial means.

An important component of the RWA model is the bottleneck principle (weakest link); namely, for a new practice to be adopted in a sustained way, all three components (R, W, A) must be present: missing elements act as ‘bottlenecks’ limiting the effective and sustained implementation of a new practice.

The use of the RWA has the benefit of linking insights from economics focused on the R condition and other disciplines that pay more attention to normative and cultural aspects vital for the W condition ( Coale, 1973 ; Lesthaeghe and Vanderhoeft, 2001 ). Furthermore, whilst conceptually concise, the model is sufficiently broad to permit critical analysis from a systems-based approach at the organisational level ( Shultz and Jimbo, 2015 ).

RWA and other conceptual models

Implementation research has increased its use of theoretical approaches to provide a better understanding and explanation of how and why implementation succeeds or fails. Many frameworks exist to describe the diffusion and implementation of new policies and interventions in health care and public sector settings more generally ( Rogers, 1995 ; Hartley, 2005 ; Osborne and Brown, 2011 ). For example, Greenhalgh et al. ’s (2004) model identified eleven key attributes of innovations in health care to explain variations in their adoption, including relative advantage; compatibility; complexity; trialability; observability; reinvention; fuzzy boundaries; risk; task issues; knowledge; augmentation/support. Whilst this influential model assesses implementation from a comprehensive perspective, it is not particularly user friendly and its complexity can itself be a barrier to executing an effective implementation strategy ( Fuller et al. , 2020 ). In contrast, RWA is a conceptually simple and intuitive tool that enables those implementing a new policy to assess the necessary preconditions for its adoption ( Shultz and Jimbo, 2015 ). Current funding and demand pressures on local services also support the value of an easily understood and applied model.

The Production of Welfare (POW) model has been used specifically in social care prevention ( Marczak et al. , 2019b ) and establishes theoretical expectations about the relationship between needs, services and outcomes at the individual level. It also emphasises the need to understand local processes and structures explaining why and how change takes place ( Davies and Knapp, 1981 ; Knapp, 1984 ). POW can underpin estimations of cost–effectiveness for specific prevention interventions but is silent on the willingness and ability dimensions, which may be vital in determining how far implementation will succeed. Given the potential challenges of implementing prevention policy and interventions ( Greenhalgh and Papoutsi, 2019 ; Tew et al. , 2019 ), RWA has a particular utility in identifying implementation-related bottlenecks and in exploring whether and how these could be addressed.

This article draws on findings from a broader study, funded by the NIHR School for Social Care Research, which aimed to develop an evaluation framework for English councils to review the prevention effects of local services. The study employed qualitative case study and economic evaluation methods, we report here on the former and present qualitative data from in-depth, semi-structured interviews ( n  = 20) with stakeholders in two English councils, and detailed analysis of policy documents in six councils.

Interview sample

Invitations to participate were sent to Councils identified through the research team’s knowledge of authorities’ work in prevention, including an earlier exploratory study ( Marczak et al. , 2019b ). Six local areas expressed an interest in participating. Following initial discussions with all of them, and based on the local capacity to provide resources for the study, we secured an agreement for two to be full participants. Key informants within the two case study authorities were selected purposively to provide a heterogeneous set of contexts for policy implementation and practice development. Twenty key informants were interviewed in total. Interviewees in each area included personnel with policy and project implementation responsibilities (see Table 1 for councils’ and informants’ characteristics).

Interviews focused on exploring barriers and facilitators to implementing and evaluating social care prevention policies and interventions ( Lincoln and Denzin, 2000 ; Patton, 2002 ; Ritchie and Lewis, 2003 ). Separate interview guides were developed for conducting interviews with senior managers, data analysts and those engaged in individual projects (question guides are available from the authors). Respondents were asked to elaborate on local understandings of prevention goals and local prevention policies, commissioning strategies, implementation partnerships, barriers encountered and delivery successes. We also asked about ongoing evaluations of prevention, including services covered, outcomes and indicators prioritised, cross-agency collaboration, etc. Interviews were conducted by the authors in 2016 and 2017. Interviews were recorded, transcribed verbatim, and material was entered into the qualitative data management software: NVivo 11 ( QSR International Pty Ltd, 2015 ).

Data analysis of interview material

Thematic analysis was used to organise and interpret interview data by systematically focusing on the identification and reporting of response patterns and themes across the whole data-set ( Boyatzis, 1998 ; Braun and Clarke, 2006 ). Initial codes were generated by breaking the transcript contents down into smaller components, coding them in a systematic manner and collating passages relevant to each code. Coded data were used to develop themes and sub-themes amongst implementation facilitators and barriers and then grouped within the ready, willing and able framework categories (see Table 3 for details). These resultant themes and sub-themes were checked and revised to ensure that passages gathered under a theme formed a coherent pattern. Extracts that did not fit were reviewed and either re-assigned to different themes or to newly generated themes ( Boyatzis, 1998 ).

Documentary analyses

An analysis of local policy documents was conducted in the six authorities expressing initial interest in the study to understand approaches to prevention policy locally and contexts for their implementation (see Table 2 ).

Documents were analysed individually using the steps highlighted by Ritchie and Spencer (1994) : familiarisation; identification of thematic framework; indexing of evidence in line with identified themes; charting of the themed evidence; and mapping and interpretation of the evidence emerging. Documentary data from the six areas were amalgamated, synthesised and categorised into themes and sub-themes and then grouped within the RWA framework categories (see Table 3 , illustrating the themes and sub-themes).

Ethical consideration

The project obtained ethical approval from The Social Care Research Ethics Committee (REC reference: 15/IEC08/0052) and was endorsed by ADASS. Informed consent to participate in and to record the interviews was obtained from all respondents. Participants were provided information verbally and in writing about their rights and the obligations of the researcher and given an opportunity to ask questions before signing the written consent form. LAs and interviewees were anonymised to protect confidentiality.

To our knowledge, this study is the first which used RWA as an organizing framework to identify facilitators and potential barriers (bottlenecks) in the implementation of prevention policies and schemes within English local authority settings. Such barriers and facilitators to the implementation were grouped by the research team into salient themes and sub-themes using the RWA model as shown in Table 3 .

Readiness to adopt prevention initiatives includes perceived contributions to managing demand and promoting population health and well-being.

The reduction of health inequalities was another advantage of prevention identified in documents and interviews.

Were they ‘willing’?

Three themes were identified during data analysis for the willing criterion: (i) understanding of prevention within and across agencies; (ii) available evidence to invest in prevention; and (iii) individuals’ commitment to prevention.

Conceptualising prevention

Comprehensive prevention strategies were often absent from the documents that we analysed and approaches to prevention tended to be ‘scattered’ across different strategy documents, for example for older people, housing, ageing well, etc. ( Tew et al. , 2019 , see also Tew et al. , 2023 ). This could lead, in turn, to difficulties in developing a coherent approach based on shared understandings and spending priorities.

Some interviewees identified services as preventative where there was a direct link to the problem and/or evidence that they could reduce or delay levels or severity of need (e.g. reablement, falls prevention and home adaptations). Others defined prevention as a broad term to describe a wide range of interventions that had the potential to enhance independence and well-being, such as information and advice, home care, housing, fire safety checks for older people, assistive technology including telecare, handyperson services, meals on wheels, enhanced care provided by social care staff (e.g. in care homes), together with activities intended to increase social participation and reduce loneliness.

Documentary analyses also indicated a wider disjunction between ASC and health agencies’ prevention goals. Whereas ASC prevention activities frequently related to interventions maximising independence for individuals, NHS agencies’ prevention agendas often focused on individual health conditions, such as cardiovascular diseases or smoking. Analyses of STPs similarly found them focused around conditions such as cancer, cardiac problems and stroke, with some also covering hypertension, substance abuse, smoking and obesity ( Ham et al. , 2017 ). Only three of the six STPs covering our authorities referred to falls prevention, increasing independence or reablement and contained limited reference to social determinants.

… a lot of my NHS colleagues would look at the preventative work … being the work you would do once somebody’s had a fall… rather than the work you might have done earlier on in the continuum to try and prevent the fall in the first place. [LA1, R5]

Availability of evidence to demonstrate prevention effects

Even where the potential utility of prevention investments was recognised, the lack of evidence about which interventions worked best and for whom continued to be an obstacle. As in our earlier study ( Marczak et al. , 2019b ), respondents reported that commissioners favoured immediate evaluations to demonstrate rapid results rather than more robust evaluations capable of capturing cumulative, longer term effects. Moreover, we found different understandings of what constituted sufficiently ‘robust’ evidence to justify spending. NHS respondents generally highlighted the ‘weak’ quantitative evidence base for social care interventions whilst social care respondents tended to emphasise that such evidence was often unattainable for low-level, prevention services (e.g. information and advice, support groups), because benefits of such interventions are often hard to quantify.

Individuals’ attitudes

…we’ve benefited from having a good leader who is very committed, very passionate, very vocal and that has really helped to drive prevention forward… we had [different]challenges …you name it, but at the core of it is that passion, that commitment to [prevention]. [LA2, R19]

Individuals are not passive recipients of new policies and practices, they find meaning in them (or not), challenge them, complain about them, and modify them—often through dialogue with others. This highlights the complexities of jointly implementing policies and practices where agencies and individuals operate in contexts where differences in interpretation and meaning are active elements in local organisational cultures ( Greenhalgh et al. , 2005 ).

Were councils ‘able’ to deliver prevention agendas?

For the ability criterion, the most prevalent themes identified during data analyses were (i) Care Act 2014, (ii) financial resources and (iii) resources to conduct evaluations.

The Care Act 2014

Policies and legislation often constitute an enabling mechanism in RWA models because they establish infrastructures to underpin implementation ( Lesthaeghe and Vanderhoeft, 2001 ; Bunzli et al. , 2017 ). The Care Act 2014 obligations were often mentioned in ASC documents as drivers for investing in prevention. Informants reported that the Act’s emphasis on prevention enabled some longer term investments for previously short-term initiatives. However, some respondents perceived the Act as achieving a little more than the retrospective legitimation of preventative activity, which had existed locally prior to the statutory obligations.

Financial resources

… as resources get even more tight, to make that case that we should free up some more very scarce resource and put it (in)to this thing [prevention], it is hard… currently… (we are) just dealing with what’s hitting our doorstep. [LA2, R12]

Consequently, as local budgets became tighter, the tension between prioritising statutory duties to meet the highest needs and the duty to invest in services that prevented and reduced future needs was becoming more difficult to manage and was creating an implementation bottleneck (see also Tew et al. , 2019 , 2023 ).

Having to rely on short-term funding was reported to be a considerable barrier to sustainability because it discouraged recruitment and retention. However, it was also noted that such funding, including that provided through the Better Care Fund (BCF) at least provided the opportunity to develop, learn from and make a stronger case for longer term funding for schemes that might not otherwise have been able to get underway.

Resources to conduct evaluations

Notwithstanding the demand for local evidence to justify prevention investments, respondents often reported that councils had limited abilities to evaluate existing prevention schemes due to inadequate manpower, including shortages of analysts with advanced statistical capabilities. They often highlighted the limitations of evaluation tools and the lack of resources to develop them. Furthermore, evaluations across ASC and health services faced challenges in accessing data due to, for example, confidentiality issues and different patient IDs or IT systems. Consequently, it was often impossible to analyse the impact of schemes across sectors and authoritatively identify the different points where costs and benefits might fall within care systems.

Investment in prevention has been advocated in English ASC for some time as a means of mitigating the challenges of population ageing. The Care Act 2014 embedded prevention as a statutory duty of such services. Despite this, knowledge about how far English local councils have implemented prevention remains limited. This study employed the RWA framework to identify facilitators, barriers and, thus, potential bottlenecks (weakest links) to implementing prevention locally. By helping to identify the potential bottlenecks, we anticipated that RWA might extend understandings of implementation deficits and provide focal points for mitigating actions.

We identified several potential bottlenecks in the willing and able categories. Conversely, the readiness criterion appeared to have been met as the hopes for prevention to manage demand and improve population health and well-being were perceived as having great value and there was a broad consensus amongst interviewees that investing in prevention was worthwhile, even if current pressures on services meant that meeting current demand took priority over proactive investment to reduce or delay future needs.

The willingness criterion, which related to the perceived legitimacy of developing and implementing local prevention approaches and programmes, was significantly underdeveloped, particularly in relation to definitions of prevention and availability of evidence. Definitions and understandings of legitimate prevention activities were contested and not supported by beliefs and codes of conduct prevalent in different agencies. Over the years, the term prevention has been found to describe a wide range of objectives and interventions ( Godfrey, 2001 ; Starfield et al. , 2008 ; Lombard, 2013 ) and our findings suggest that shared meanings remain elusive. The Care Act’s Statutory Guidance notes that there is no single definition of prevention and that different local approaches to prevention may be developed ( Department of Health and Social Care, 2018 ). Our study indicates that, in so far, as the meaning of prevention is ambiguous or open to conflicting interpretations in particular cases and contexts, what is to be implemented and evaluated may also remain contested.

The validity of evidence to justify investment in prevention was disputed by social and healthcare agencies because of differences in conceptualising prevention and the appropriateness of different evaluation methodologies. Because economic evidence was often required to secure procurement, interventions where qualitative prevention effects are more observable and feasible to gather may receive less support. When the desired standard for evidence-based commissioning in the health sector is quantifiable clinical and financial data as well as local evaluations ( Glasby, 2012 ; Wye et al. , 2015 ), social care commissioners may find it challenging to convince healthcare partners to invest in jointly funded prevention schemes for which quantifiable data are lacking and/or may be inappropriate. Individuals’ attitudes were the third theme vital for the willingness criterion. Individuals carry cultural, professional and individual beliefs, mindsets, norms and interests; they are not passive recipients of policies, but they have agency and can have significant impacts on implementation outcomes as facilitators or obstructors ( Greenhalgh et al. , 2004 ; Greenhalgh et al. , 2005 ; Damschroder et al. , 2009 ).

RWA could help to identify strategies to overcome frequently occurring bottlenecks by reinforcing facilitators and moderating barriers and we use our findings to illustrate how some of the facilitators could be employed to overcome barriers. For example, as Figure 2 indicates some of the barriers in the willingness criterion (e.g. the lack of evidence) may be overcome by strong leaders and their commitment to prevention. Key commissioning professionals can influence the utilisation and interpretation of evidence by making common sense or expert opinion judgements about its application and applicability ( Orton et al. , 2011 ). The RWA framework can also be valuable in helping structure the identification of factors that reinforce facilitators and undermine barriers, thereby highlighting areas where the strengthening of implementation capacities and capabilities should be prioritised. However, further research is needed to establish specific interventions and implementation programmes to put such insights into effect.

The ability criterion, which relates to the capability to adopt a practice given legal and financial means, was partly met. Whilst the Care Act 2014 appeared to facilitate and/or legitimise investment in prevention, the reported lack of financial resources and evidence for investments in prevention have been found to be significant barriers ( Tew et al. , 2019 ). Over the last decade, managing financial scarcity has become perhaps the overriding task for local government, posing major—and sometimes conflicting—dilemmas and challenges for councils and their partners. In this study, some of the same interviewees who identified financial resources as a barrier to implementing prevention interventions also considered that investment in prevention would pay off long term as it could reduce demand in the future. This apparent paradox between the perceived utility of investing in prevention, and the apparent lack of resources to do so, reflects the complexity of prevention investment decisions and not least the tension between immediate costs and longer term benefits, especially where financial benefits fall to budgets, which have not incurred commensurate costs. However, as Figure 2 suggests, beliefs that investing in prevention could reduce demand and long-term costs, combined with strong leadership and commitment to prevention, could help to secure investment in prevention.

The RWA model enables critical analysis from a systems-based, multi-levelled perspective. For example, at the individual level, the framework can be applied to professionals, providers and service users; at the organisational level, it can be applied to local councils, care systems, and voluntary sector organisations; whilst at the local and national policy levels, it can be applied to care policy, the economics of social care or the legal umbrella under which care is managed. The implementation of prevention strategies and services at the local level requires multiple stakeholder interests to be aligned across different agencies and administrative levels and each stakeholder needs to have a degree of readiness, willingness and ability to facilitate the necessary change. The implementation process is dynamic and iterative, with influences from one level shaping other levels. Consequently, some factors influencing prevention implementation may apply to more than one of the criteria (R, W, A) or to more than one level (individual, organisational, policy) concurrently ( Shultz and Jimbo, 2015 ).

Notwithstanding our findings suggesting the value of RWA as a heuristic framework, the study’s limitations must be recognised when interpreting its results. First, it is important to note that distinctions between the three core elements may, at times, appear somewhat contrived, since a given item may sometimes be categorised under more than one heading. For example, although we have coded the availability of evidence to justify prevention investment, as part of the willingness criterion, because it is strongly related to stakeholders’ willingness to invest in prevention, it could also be seen as part of readiness as it impacts perceptions of the utility of prevention. The purpose, however, of representing a given component of the model, is not to exhaust every possible contingency, but to summarise findings and create a useful heuristic that can inform critical thinking about potential obstacles to implementation.

Secondly, some authors have argued for a conceptualisation of social care prevention that recognises the role of structural or systemic influences on outcomes ( Verity et al. , 2020 ). The breadth and parsimony of the RWA model may limit in-depth analyses of such complexities as ‘the causes of the causes’ of barriers and facilitators to developing and implementing policies and implementation programmes (see also Marmot et al. , 2010 ). Although such factors may be explored in RWA through the ability component, a more fundamental analysis than the model allows may be required to identify the underlying nature of presenting barriers and opportunities.

Finally, whilst effort was made to capture a diverse range of local views, opinions and practices related to the prevention agenda in a variety of contexts, the exploratory nature of the study and the small sample size may be thought to limit the generalisability of its findings. The sampled councils where interviews were conducted are amongst the less deprived based on the English indices of deprivation, even though they have pockets of deprivation. However, the per capita level of local authority spending on ASC varied and was below average in one and above average in the other council ( MHCLG, 2019 ; NHS Digital, 2017 ). As this research was based on a small sample, the strength of specific barriers may have been different in another sample, for example, the impact of financial austerity on prevention may have been more prominent in more deprived areas. A detailed analysis of the impact of financial austerity on prevention activity is of critical importance, and merits separate research, which is beyond the scope of this article. The purpose of this article was not to record in exhaustive detail every barrier or facilitator but to identify overarching themes and assess their applicability to the RWA model as a tool for critical analysis and a potential guide for future research.

We found English local councils were experiencing continuing difficulties in the design and delivery of sustainable prevention strategies and interventions. Additions to their implementation toolkits might helpfully reinforce their capabilities to address those challenges. We used the RWA framework to expose both the range of implementation difficulties that councils were facing and to identify the features of individual challenges and inter-relationships between them, especially where they create specific bottlenecks to effective implementation. The framework’s strength is in identifying and categorising facilitators, barriers and their interrelationships. Thus, RWA can provide a straightforward and parsimonious framework for identifying implementation difficulties singly and in combination. However, it is less helpful as a tool for constructing routes through the bottlenecks that it identifies. Success in implementing prevention may thus require a bigger toolkit, of which RWA may be just one element. For example, and like Tew et al. (2019) , we would point to the potential utility of the theory of change approach and associated logic models, which enable implementors to map and help influence causal pathways, which may link specific contextual influences, selected mechanisms and desired outcomes to support the introduction and operation of prevention strategies.

In summary, therefore, RWA may provide a helpful mapping tool for gaining a high-level appreciation of current barriers and bottlenecks. As such, it might be adopted to support problem analysis and help to pinpoint areas where targeted improvement initiatives might be particularly beneficial. In an ASC context where we found opportunities and capabilities for analysis and reflection remarkably scarce, it may be short-sighted to ignore a relatively parsimonious approach that can help local actors begin to clarify and understand the dimensions of obstacles to and opportunities for progress with prevention. However, further studies, which explore the application of the model to social care settings, would help to establish both the feasibility of its adoption and the extent to which the limitations we have anticipated here might need to be met by other items in a well-stocked implementation toolbox.
